    Tumor testicular de saco vitelino con metástasis a pleura: primer caso reportado en la literatura

    ResumenEl cáncer testicular es el tumor urológico maligno más común en hombres jóvenes. Más del 90% corresponden a tumores de células germinales. El objetivo del estudio es el reporte del primer caso de cáncer testicular de tumores de células germinales con metástasis a pleura.Paciente masculino de 21 años, inicia su padecimiento con dolor en hemitórax derecho; la radiografía de tórax presenta una radioopacidad en lóbulo medio de pulmón derecho y la tomografía de tórax detecta una masa intratorácica y extrapulmonar. La biopsia torácica guiada por ultrasonido muestra una masa sólida adosada a la pared torácica, con efecto de desplazamiento sobre la pleura, conocido como «efecto en cola de cometa». La biopsia fue positiva para lesión neoplásica maligna poco diferenciada de la pleura. A la exploración encontramos testículo derecho con tumoración en polo inferior el cual se corrobora por ultrasonido. Los marcadores tumorales solo con elevación de alfa-fetoproteína 295.87ng/dL y deshidrogenasa láctica en 2,380UI/L. En la tomografía de abdomen se identificó actividad ganglionar interaortocava. Realizamos orquiectomía radical derecha. Reporte patológico: tumor testicular de saco vitelino, pT1N2M1S2.En este caso se pudo comprobar que las metástasis fueron a nivel pleural, que lo ubica como una forma de diseminación excepcional.AbstractTesticular cancer is the most common malignant urologic tumor in young men. More than 90% are germ cell tumors. The aim of this study was to report the first case of germ cell tumor testicular cancer with metastasis to the pleura.A 21-year-old man had disease onset with pain in the right hemithorax. The chest x-ray showed radio-opacity in the middle lobe of the right lung. A chest tomography scan detected an intrathoracic and extrapulmonary mass. Ultrasound-guided thoracic biopsy revealed a solid mass attached to the thoracic wall, displaced over the pleura, known as the “comet tail effect”. The biopsy was positive for a poorly differentiated malignant neoplastic lesion of the pleura. Upon physical examination we encountered a right testis with a tumor at the lower pole; this was corroborated through ultrasound. In relation to tumor markers, only alpha-fetoprotein was elevated at 295.87ng/dl and lactate dehydrogenase at 2,380 IU/l. An abdominal tomography scan identified interaortocaval lymph node activity. We performed a right radical orchiectomy and the pathology report was testicular yolk sac tumor, pT1N2M1S2.In our patient, metastasis at the level of the pleura was confirmed, making this a case with an exceptional form of spread

    Polymorphisms in XPC, XPD, XRCC1, and XRCC3 DNA repair genes and lung cancer risk in a population of Northern Spain

    <p>Abstract</p> <p>Background</p> <p>Polymorphisms in DNA repair genes have been associated to repair DNA lesions, and might contribute to the individual susceptibility to develop different types of cancer. Nucleotide excision repair (NER), base excision repair (BER), and double-strand break repair (DSBR) are the main DNA repair pathways. We investigated the relationship between polymorphisms in two NER genes, <it>XPC </it>(poly (AT) insertion/deletion: PAT-/+) and <it>XPD </it>(Asp312Asn and Lys751Gln), the BER gene <it>XRCC1 </it>(Arg399Gln), and the DSBR gene <it>XRCC3 </it>(Thr241Met) and the risk of developing lung cancer.</p> <p>Methods</p> <p>A hospital-based case-control study was designed with 516 lung cancer patients and 533 control subjects, matched on ethnicity, age, and gender. Genotypes were determined by PCR-RFLP and the results were analysed using multivariate unconditional logistic regression, adjusting for age, gender and pack-years.</p> <p>Results</p> <p>Borderline association was found for <it>XPC </it>and <it>XPD </it>NER genes polymorphisms, while no association was observed for polymorphisms in BER and DSBR genes. <it>XPC PAT+/+ </it>genotype was associated with no statistically significant increased risk among ever smokers (OR = 1.40; 95%CI = 0.94–2.08), squamous cell carcinoma (OR = 1.44; 95%CI = 0.85–2.44), and adenocarcinoma (OR = 1.72; 95%CI = 0.97–3.04). <it>XPD </it>variant genotypes (<it>312Asn/Asn </it>and <it>751Gln/Gln</it>) presented a not statistically significant risk of developing lung cancer (OR = 1.52; 95%CI = 0.91–2.51; OR = 1.38; 95%CI = 0.85–2.25, respectively), especially among ever smokers (OR = 1.58; 95%CI = 0.96–2.60), heavy smokers (OR = 2.07; 95%CI = 0.74–5.75), and adenocarcinoma (OR = 1.88; 95%CI = 0.97–3.63). On the other hand, individuals homozygous for the XRCC1 <it>399Gln </it>allele presented no risk of developing lung cancer (OR = 0.87; 95%CI = 0.57–1.31) except for individuals carriers of <it>399Gln/Gln </it>genotype and without family history of cancer (OR = 0.57; 95%CI = 0.33–0.98) and no association was found between <it>XRCC3 </it>Thr241Met polymorphism and lung cancer risk (OR = 0.92; 95%CI = 0.56–1.50), except for the <it>241Met/Met </it>genotype and squamous cell carcinoma risk (OR = 0.47; 95%CI = 0.23–1.00).</p> <p>Conclusion</p> <p>In conclusion, we analysed the association between <it>XPC</it>, <it>XPD</it>, <it>XRCC1</it>, and <it>XRCC3 </it>polymorphisms and the individual susceptibility to develop lung cancer in the Spanish population, specifically with a highly tobacco exposed population. We attempt to contribute to the discovery of which biomarkers of DNA repair capacity are useful for screening this high-risk population for primary preventing and early detection of lung cancer.</p

    Enzybiotic-mediated antimicrobial functionalization of polyhydroxyalkanoates

    Polymeric nanoparticles (NPs) present some ideal properties as biomedical nanocarriers for targeted drug delivery such as enhanced translocation through body barriers. Biopolymers, such as polyhydroxyalkanoates (PHAs) are gaining attention as nanocarrier biomaterials due to their inherent biocompatibility, biodegradability, and ability to be vehiculized through hydrophobic media, such as the lung surfactant (LS). Upon colonization of the lung alveoli, below the LS layer, Streptococcus pneumoniae, causes community-acquired pneumonia, a severe respiratory condition. In this work, we convert PHA NPs into an antimicrobial material by the immobilization of an enzybiotic, an antimicrobial enzyme, via a minimal PHA affinity tag. We first produced the fusion protein M711, comprising the minimized PHA affinity tag, MinP, and the enzybiotic Cpl-711, which specifically targets S. pneumoniae. Then, a PHA nanoparticulate suspension with adequate physicochemical properties for pulmonary delivery was formulated, and NPs were decorated with M711. Finally, we assessed the antipneumococcal activity of the nanosystem against planktonic and biofilm forms of S. pneumoniae. The resulting system displayed sustained antimicrobial activity against both, free and sessile cells, confirming that tag-mediated immobilization of enzybiotics on PHAs is a promising platform for bioactive antimicrobial functionalization

    Predictors of enhancing human physical attractiveness: Data from 93 countries

    People across the world and throughout history have gone to great lengths to enhance their physical appearance. Evolutionary psychologists and ethologists have largely attempted to explain this phenomenon via mating preferences and strategies. Here, we test one of the most popular evolutionary hypotheses for beauty-enhancing behaviors, drawn from mating market and parasite stress perspectives, in a large cross-cultural sample. We also test hypotheses drawn from other influential and non-mutually exclusive theoretical frameworks, from biosocial role theory to a cultural media perspective. Survey data from 93,158 human participants across 93 countries provide evidence that behaviors such as applying makeup or using other cosmetics, hair grooming, clothing style, caring for body hygiene, and exercising or following a specific diet for the specific purpose of improving one physical attractiveness, are universal. Indeed, 99% of participants reported spending >10 min a day performing beauty-enhancing behaviors. The results largely support evolutionary hypotheses: more time was spent enhancing beauty by women (almost 4 h a day, on average) than by men (3.6 h a day), by the youngest participants (and contrary to predictions, also the oldest), by those with a relatively more severe history of infectious diseases, and by participants currently dating compared to those in established relationships. The strongest predictor of attractiveness-enhancing behaviors was social media usage. Other predictors, in order of effect size, included adhering to traditional gender roles, residing in countries with less gender equality, considering oneself as highly attractive or, conversely, highly unattractive, TV watching time, higher socioeconomic status, right-wing political beliefs, a lower level of education, and personal individualistic attitudes.     The aim of the study was to assess the effect of partial replacement of cracked maize ears with ground maize (GM) or sugar cane molasses (SCM) in supplements for dual purpose cows. Eighteen (18) multiparous cows (414 ± 13 kg of body weight and 106 ± 32 d in milk) were randomly assigned to the treatments. Treatments were as follows: 1) Control supplement (CS) which consisted of 87% of cracked maize ears (CME), 11% soybean meal, and 2% urea; 2) Ground maize replacing 20% of CME in CS (GMS); 3) Sugar cane molasses replacing 18% of CME in the CS (MOS). Each cow received 5 kg/d of supplement DM, whereas their calves received 1.8 kg/d DM of the CS. The experiment lasted eleven weeks, and data were recorded once at the end of every week. Data were analysed using a linear mixed model as a completely randomized design. Net profit from milk and beef due to supplements were estimated using the partial budget approach. There were no differences (P>0.05) between treatments on milk composition, body conditions score, nor daily weight gain of cows and calves. However, compared to GM, CS shown greater (9.0 %, P0.05) entre los tratamientos sobre la composición de la leche, la condición corporal ni el aumento de peso diario de las vacas y terneros. Sin embargo, en comparación con el MM, el ST mostró mayor (9.0 %, P0.05) en las demás variables de respuesta. Los márgenes combinados de ganancias netas (ventas de leche y terneros), fueron 9 % mayores para el tratamiento con MCA en comparación con el resto de los suplementos

    Background: Parkinson?s disease (PD) is a progressive neurodegenerative disorder causing motor and non-motor symptoms that can affect independence, social adjustment and the quality of life (QoL) of both patients and caregivers. Studies designed to find diagnostic and/or progression biomarkers of PD are needed. We describe here the study protocol of COPPADIS-2015 (COhort of Patients with PArkinson?s DIsease in Spain, 2015), an integral PD project based on four aspects/concepts: 1) PD as a global disease (motor and non-motor symptoms); 2) QoL and caregiver issues; 3) Biomarkers; 4) Disease progression.Methods/design: Observational, descriptive, non-interventional, 5-year follow-up, national (Spain), multicenter (45 centers from 15 autonomous communities), evaluation study. Specific goals: (1) detailed study (clinical evaluations, serum biomarkers, genetic studies and neuroimaging) of a population of PD patients from different areas of Spain, (2) comparison with a control group and (3) follow-up for 5 years. COPPADIS-2015 has been specifically designed to assess 17 proposed objectives. Study population: approximately 800 non-dementia PD patients, 600 principal caregivers and 400 control subjects. Study evaluations: (1) baseline includes motor assessment (e.g., Unified Parkinson?s Disease Rating Scale part III), non-motor symptoms (e.g., Non-Motor Symptoms Scale), cognition (e.g., Parkinson?s Disease Cognitive Rating Scale), mood and neuropsychiatric symptoms (e.g., Neuropsychiatric Inventory), disability, QoL (e.g., 39-item Parkinson?s disease Quality of Life Questionnaire Summary-Index) and caregiver status (e.g., Zarit Caregiver Burden Inventory); (2) follow-up includes annual (patients) or biannual (caregivers and controls) evaluations. Serum biomarkers (S-100b protein, TNF-?, IL-1, IL-2, IL-6, vitamin B12, methylmalonic acid, homocysteine, uric acid, C-reactive protein, ferritin, iron) and brain MRI (volumetry, tractography and MTAi [Medial Temporal Atrophy Index]), at baseline and at the end of follow-up, and genetic studies (DNA and RNA) at baseline will be performed in a subgroup of subjects (300 PD patients and 100 control subjects). Study periods: (1) recruitment period, from November, 2015 to February, 2017 (basal assessment); (2) follow-up period, 5 years; (3) closing date of clinical follow-up, May, 2022.     Influence of the length of hospitalisation in post-discharge outcomes in patients with acute heart failure: Results of the LOHRCA study

    Objective: To investigate the relationship between length of hospitalisation (LOH) and post-discharge outcomes in acute heart failure (AHF) patients and to ascertain whether there are different patterns according to department of initial hospitalisation. Methods: Consecutive AHF patients hospitalised in 41 Spanish centres were grouped based on the LOH (15 days). Outcomes were defined as 90-day post-discharge all-cause mortality, AHF readmissions, and the combination of both. Hazard ratios (HRs), adjusted by chronic conditions and severity of decompensation, were calculated for groups with LOH >6 days vs. LOH <6 days (reference), and stratified by hospitalisation in cardiology, internal medicine, geriatrics, or short-stay units. Results: We included 8563 patients (mean age: 80 (SD = 10) years, 55.5% women), with a median LOH of 7 days (IQR 4–11): 2934 (34.3%) had a LOH 15 days. The 90-day post-discharge mortality was 11.4%, readmission 32.2%, and combined endpoint 37.4%. Mortality was increased by 36.5% (95%CI = 13.0–64.9) when LOH was 11–15 days, and by 72.0% (95%CI = 42.6–107.5) when >15 days. Conversely, no differences were found in readmission risk, and the combined endpoint only increased 21.6% (95%CI = 8.4–36.4) for LOH >15 days. Stratified analysis by hospitalisation departments rendered similar post-discharge outcomes, with all exhibiting increased mortality for LOH >15 days and no significant increments in readmission risk. Conclusions: Short hospitalisations are not associated with worse outcomes. While post-discharge readmissions are not affected by LOH, mortality risk increases as the LOH lengthens. These findings were similar across hospitalisation departments

    Activation of the Innate Immune Response against DENV in Normal Non-Transformed Human Fibroblasts

    In this work, we demonstrate that that both human whole skin and freshly isolated skin fibroblasts are productively infected with Dengue virus (DENV). In addition, primary skin fibroblast cultures were established and subsequently infected with DENV-2; we showed in these cells the presence of the viral antigen NS3, and we found productive viral infection by a conventional plaque assay. Of note, the infectivity rate was almost the same in all the primary cultures analyzed from different donors. The skin fibroblasts infected with DENV-2 underwent signaling through both TLR3 and RIG-1, but not Mda5, triggering up-regulation of IFNβ, TNFα, defensin 5 (HB5) and β defensin 2 (HβD2). In addition, DENV infected fibroblasts showed increased nuclear translocation of interferon (IFN) regulatory factor 3 (IRF3), but not interferon regulatory factor 7 IRF7, when compared with mock-infected fibroblasts. Our data suggest that fibroblasts might even participate producing mediators involved in innate immunity that activate and contribute to the orchestration of the local innate responses. This work is the first evaluating primary skin fibroblast cultures obtained from different humans, assessing both their susceptibility to DENV infection as well as their ability to produce molecules crucial for innate immunity
