Tuberculosis is a very important problem for public health all around the world. In this study, 168 patients with TB who were treated in Batman Tuberculosis Control Dispensary in 2003 were retrospectively assessed. Ninety six of our cases were male, 72 were female. Eighty one patients who underwent for the tuberculous treatment were with pulmonary tuberculosis and 87 cases were with extrapulmonary tuberculosis. Acid fast bacilli (AFB) at sputum was positive in 66.17% of the cavitary pulmonary TB cases before treatment and only one patient’s sputum was positive in pulmonary tuberculosis without cavity. Results of the tuberculosis treatment of cases revealed that one case left the treatment, 3 patients were inharmonious, 3 patients were dead, 161 patients were cured. Success rate of treatment was 95,83% (5 of these cases were cured and 156 patients completed the treatment). These data suggest that in order to increase the rate of the cure, bacteriological examination of the sputum should be considered at the end of the treatment like as the beginning

The association between TAPSE and right atrial contractile strain

    Get PDF
BACKGROUND: In the descending arm of tricuspid annular plane systolic excursion (TAPSE) there is a notch formation which corresponds to the contractile phase of the atrial strain curve. Theoretically, this notch formation stands for atrial contraction. AIMS: We aim to characterize the notch formation on the TAPSE, the predictors of its existence, its relationship with the right ventricle and right atrial strain (RAS) parameters. METHODS: Retrospectively selected 240 patients were investigated for the determinants of the notch formation on TAPSE and the relation between RAS and TAPSE. RAS was analyzed using 2D speckle tracking in a dedicated mode for atrial analysis and reported separately for the reservoir, conduit, and contractile phases. RESULTS: 71.7% (n = 172) of patients had the notch formation on the TAPSE and 70.4% (n = 169) had a normal value of right atrial contractile strain (RASct). Most of the patients with a notch formation also had preserved RASct (95.9%; P <0.001). In multivariable analysis, RASct (odds ratio [OR], 1.45; 95% confidence interval [CI]: 1.13‒1.77; P = 0.020) remained significant with the notch formation. Receiver operator characteristic (ROC) analysis demonstrated that a RASct of ‒19% was found as a cut-off for presence of notch formation. ROC area was 0.897 (95% CI 0.844–0.951; P <0.001). CONCLUSIONS: The changes in TAPSE configuration represents the changes in atrial contractile phase. The descending arm of the TAPSE indicates the RASct as whether it is preserved or not. The notch formation persists if the RASct is above ‒19%. So, an easier, more applicable, and more effortless tool TAPSE can be used as an indicator of atrial contractile phase by its configuration in daily routine

OBJECTIVE: Differential diagnosis of idiopathic pulmonary fibrosis (IPF) is important among fibrotic interstitial lung diseases (ILD). This study aimed to evaluate the rate of IPF in patients with fibrotic ILD and to determine the clinical-laboratory features of patients with and without IPF that would provide the differential diagnosis of IPF. MATERIAL AND METHODS: The study included the patients with the usual interstitial pneumonia (UIP) pattern or possible UIP pattern on thorax high-resolution computed tomography, and/or UIP pattern, probable UIP or possible UIP pattern at lung biopsy according to the 2011 ATS/ERSARS/ALAT guidelines. Demographics and clinical and radiological data of the patients were recorded. All data recorded by researchers was evaluated by radiology and the clinical decision board. RESULTS: A total of 336 patients (253 men, 83 women, age 65.8 +/- 9.0 years) were evaluated. Of the patients with sufficient data for diag-nosis (n=300), the diagnosis was IPF in 121 (40.3%), unclassified idiopathic interstitial pneumonia in 50 (16.7%), combined pulmonary fibrosis and emphysema (CPFE) in 40 (13.3%), and lung involvement of connective tissue disease (CTD) in 16 (5.3%). When 29 patients with definite IPF features were added to the patients with CPFE, the total number of IPF patients reached 150 (50%). Rate of male sex (p<0.001), smoking history (p<0.001), and the presence of clubbing (p=0.001) were significantly high in patients with IPE None of the women <50 years and none of the men <50 years of age without a smoking history were diagnosed with IPE Presence of at least 1 of the symptoms suggestive of CTD, erythrocyte sedimentation rate (ESR), and antinuclear antibody (FANA) positivity rates were significantly higher in the non-IPF group (p<0.001, p=0.029, p=0.009, respectively). CONCLUSION: The rate of IPF among patients with fibrotic ILD was 50%. In the differential diagnosis of IPF, sex, smoking habits, and the presence of clubbing are important. The presence of symptoms related to CTD, ESR elevation, and EANA positivity reduce the likelihood of IPF

An appeal to the global health community for a tripartite innovation: an ‘‘Essential Diagnostics List,’’ ‘‘Health in All Policies,’’ and ‘‘See-Through 21st Century Science and Ethics"

Diagnostics spanning a wide range of new biotechnologies, including proteomics, metabolomics, and nanotechnology, are emerging as companion tests to innovative medicines. In this Opinion, we present the rationale for promulgating an ‘‘Essential Diagnostics List.’’ Additionally, we explain the ways in which adopting a vision for ‘‘Health in All Policies’’ could link essential diagnostics with robust and timely societal outcomes such as sustainable development, human rights, gender parity, and alleviation of poverty. We do so in three ways. First, we propose the need for a new, ‘‘see through’’ taxonomy for knowledge-based innovation as we transition from the material industries (e.g., textiles, plastic, cement, glass) dominant in the 20th century to the anticipated knowledge industry of the 21st century. If knowledge is the currency of the present century, then it is sensible to adopt an approach that thoroughly examines scientific knowledge, starting with the production aims, methods, quality, distribution, access, and the ends it purports to serve. Second, we explain that this knowledge trajectory focus on innovation is crucial and applicable across all sectors, including public, private, or public–private partnerships, as it underscores the fact that scientific knowledge is a co-product of technology, human values, and social systems. By making the value systems embedded in scientific design and knowledge co-production transparent, we all stand to benefit from sustainable and transparent science. Third, we appeal to the global health community to consider the necessary qualities of good governance for 21st century organizations that will embark on developing essential diagnostics. These have importance not only for science and knowledge based innovation, but also for the ways in which we can build open, healthy, and peaceful civil societies today and for future generations

Bu çalışma; Seyyid Yahyâ-i Şirvânî'nin hayatı, Beyânu'l-ilm, Kıssa-i Mansûr, Gazeliyyât isimli eserlerinin tenkitli metinleri ve tercümelerinden ibarettir. Osmanlı Devleti için büyük önem arz eden Halvetiyye tarikatının asıl mimarı olan Seyyid Yaóyâ'yı ve eserlerini tanıtmak amacımız olmuştur. Anahtar Kelimeler: Seyyid Yahyâ-i Şirvânî, Halvetîlik, Beyânu'l-ilm, Kıssa-i Mansûr, Gazeliyyât.Levent Menteşe, Güngör, "Yahyâ-i Şirvânî's Persian Works (Beyânu'l-ilm, Kıssa-i Mansûr, Ghazeliyyât)", Master's Thesis, Kırıkkale, 2018. Seyyid Yahyâ-i Şirvânî was born in 1397 and died in Baku in 1466. He was a sufi poet who tended to sophism after completing his education. He discussed sophistic issues in detail in his verse and prose works written in Turkish, Arabic and Persian. Seyyid Yahyâ-i Şirvânî, who had tens of thousands of followers, sent his caliphs into many places in the world and ensured the sect of Khalvetî to improve and spread via these caliphs. It is clear that he had great influence on both systematization of the sect of Khalvetî and its spread into many regions. It is certain that Ottoman Empire is the place where the sect of Khalvetî was widespread. It is clear evidence that Khalvetîs were supported by Ottoman rulers and they had influence on this land. This study is composed of Seyyid Yahyâ-i Şirvânî's life, the descriptions, translations and spelling features of his works called Beyanu'l-ilm, Kıssa-i Mansûr, Ghazeliyyât. Our aim is to introduce Seyyid Yahyâ who is the real founder of the sect of Khalvetî, which has great importance for Ottoman Empire, and his works.
