    Direct measurement of diurnal polar motion by ring laser gyroscopes

    We report the first direct measurements of the very small effect of forced diurnal polar motion, successfully observed on three of our large ring lasers, which now measure the instantaneous direction of Earth's rotation axis to a precision of 1 part in 10^8 when averaged over a time interval of several hours. Ring laser gyroscopes provide a new viable technique for directly and continuously measuring the position of the instantaneous rotation axis of the Earth and the amplitudes of the Oppolzer modes. In contrast, the space geodetic techniques (VLBI, SLR, GPS, etc.) contain no information about the position of the instantaneous axis of rotation of the Earth, but are sensitive to the complete transformation matrix between the Earth-fixed and inertial reference frame.     Improving irrigation efficiency will be insufficient to meet future water demand in the Nile Basin

    The Nile River Basin covers an area of approximately 3.2 million km2 and is shared by 11 countries. Rapid population growth is expected in the region. The irrigation requirements of Nile riparian countries of existing (6.4 million ha) and additional planned (3.8 million ha, 2050) irrigation schemes were calculated, and the likely water savings through improved irrigation efficiency were evaluated. We applied SPARE:WATER to calculate irrigation demands on the basis of the well-known FAO56 Crop Irrigation Guidelines. Egypt (67 km3 yr-1) and Sudan (19 km3 yr-1) consume the highest share of the 84 km3 yr-1 total (2011). Assuming today’s poor irrigation infrastructure, the total consumption was predicted to increase to 123 km3 yr-1 (2050), an amount far exceeding the total annual yield of the Nile Basin. Therefore, a key challenge for water resources management in the Nile Basin is balancing the increasing irrigation water demand basin-wide with the available water supply. We found that water savings from improved irrigation technology will not be able to meet the additional needs of planned areas. Under a theoretical scenario of maximum possible efficiency, the deficit would still be 5 km3 yr-1. For more likely efficiency improvement scenarios, the deficit ranged between 23 and 29 km3 yr-1. Our results suggest that that improving irrigation efficiency may substantially contribute to decreasing water stress on the Nile system but would not completely meet the demand.     Sampling frequency trade-offs in the assessment of mean transit times of tropical montane catchment waters under semi-steady-state conditions

    Precipitation event samples and weekly based water samples from streams and soils were collected in a tropical montane cloud forest catchment for 2 years and analyzed for stable water isotopes in order to understand the effect of sampling frequency in the performance of three lumped-parameter distribution functions (exponential-piston flow, linear-piston flow and gamma) which were used to estimate mean transit times of waters. Precipitation data, used as input function for the models, were aggregated to daily, weekly, bi-weekly, monthly and bi-monthly sampling resolutions, while analyzed frequencies for outflows went from weekly to bi-monthly. By using different scenarios involving diverse sampling frequencies, this study reveals that the effect of lowering the sampling frequency depends on the water type. For soil waters, with transit times on the order of few weeks, there was a clear trend of over predictions. In contrast, the trend for stream waters, which have a more damped isotopic signal and mean transit times on the order of 2 to 4 years, was less clear and showed a dependence on the type of model used. The trade-off to coarse data resolutions could potentially lead to misleading conclusions on how water actually moves through the catchment, notwithstanding that these predictions could reach better fitting efficiencies, fewer uncertainties, errors and biases. For both water types an optimal sampling frequency seems to be 1 or at most 2 weeks. The results of our analyses provide information for the planning of future fieldwork in similar Andean or other catchments

    Therapies in Refractory Metastatic Renal Cell Carcinoma

    As the therapeutic landscape for metastatic clear cell renal cell carcinoma (mccRCC) expands to include vascular endothelial growth factor receptor tyrosine kinase inhibitors (VEGFR TKIs) and immunotherapies, new challenges are in place for evaluating and treating refractory disease. Assessing and managing refractory disease has several elements: (1) the mechanism(s) of front-line treatment, (2) timing of progressive disease, (3) rapidity and sites of progressing disease, (4) use of adjuvant therapy, and (5) incorporation of surgical and radiation techniques. These variables all have distinct impact on the biology of refractory or resistant mccRCC. A better understanding of the essential mechanisms of both primary and secondary immunotherapy resistance will inform biomarker development and therapeutic strategies in the refractory setting. This paper addresses the current understanding of treatment sequencing in refractory mccRCC, focusing on treatment options with prospective clinical trial data, considers refractory mccRCC after adjuvant immunotherapy, and incorporates radiation or surgical resection for oligoprogressive disease

    General practitioners' experience and benefits from patient evaluations

    <p>Abstract</p> <p>Background</p> <p>It has now for many years been recognised that patient evaluations should be undertaken as an integral part of the complex task of improving the quality of general practice care. Yet little is known about the general practitioners' (GPs') benefit from patient evaluations. Aim 1 was to study the impact on the GPs of a patient evaluation and subsequent feedback of results presented at a plenary session comprising a study guide for the results and group discussions. Aim 2 was to study possible facilitators and barriers to the implementations of the results raised by the patient evaluation process.</p> <p>Methods</p> <p>A patient evaluation survey of 597 voluntarily participating GPs was performed by means of the EUROPEP questionnaire. Evaluation results were fed back to the GPs as written reports at a single feedback meeting with group discussions of the results. Between 3 and 17 months after the feedback, the 597 GPs received a questionnaire with items addressing their experience with and perceived benefit from the evaluations.</p> <p>Results</p> <p>79.4% of the GPs responded. 33% of the responding GPs reported that the patient evaluation had raised their attention to the patient perspective on the quality of general practice care. Job satisfaction had improved among 26%, and 21% had developed a more positive attitude to patient evaluations. 77% of the GPs reported having learnt from the evaluation. 54% had made changes to improve practice, 82% would recommend a patient evaluation to a colleague and 75% would do another patient evaluation if invited. 14% of the GPs had become less positive towards patient evaluations, and job satisfaction had decreased among 3%.</p> <p>Conclusions</p> <p>We found a significant impact on the GPs regarding satisfaction with the process and attitude towards patient evaluations, GPs' attention to the patients' perspective on care quality and their job satisfaction. Being benchmarked against the average seemed to raise barriers to the concept of patient evaluations and difficulties interpreting the results may have formed a barrier to their implementation which was partly overcome by adding qualitative data to the quantitative results. The GPs' significant willingness to share and discuss the results with others may have served as a facilitator.</p

    Breast cancer incidence and mortality in Tyrol/Austria after fifteen years of opportunistic mammography screening

    <p>Abstract</p> <p>Background</p> <p>The aim of this study was to analyse breast cancer incidence and mortality in Tyrol from 1970 to 2006, namely after performing more than a decade of opportunistic mammography screening and just before piloting an organised screening programme. Our investigation was conducted on a population level.</p> <p>Methods</p> <p>To study time trends in breast cancer incidence and mortality, we applied the age-period-cohort model by Poisson regression to the official mortality data covering more than three decades from 1970 to 2006 and to the incidence data ranging from 1988 to 2006. In addition, for incidence data we analysed data on breast cancer staging and compared these with EU guidelines.</p> <p>Results</p> <p>For the analysis of time trend in breast cancer mortality in age groups 40-79, an age-period-cohort model fits well and shows for years 2002-2006 a statistically significant reduction of 26% (95% CI 13%-36%) in breast cancer mortality as compared to 1992-1996.</p> <p>We see only slight non-significant increases in breast cancer incidence. For the past five years, incidence data show a 10% proportion of in situ cases, and of 50% for cases in stages II+.</p> <p>Conclusions</p> <p>The opportunistic breast cancer screening programme in Tyrol has only in part exploited the mortality reduction known for organised screening programmes. There seems to be potential for further improvement, and we recommend that an organised screening programme and a detailed screening database be introduced to collect all information needed to analyse the quality indicators suggested by the EU guidelines.</p

    Gravitational Waves From Known Pulsars: Results From The Initial Detector Era

    We present the results of searches for gravitational waves from a large selection of pulsars using data from the most recent science runs (S6, VSR2 and VSR4) of the initial generation of interferometric gravitational wave detectors LIGO (Laser Interferometric Gravitational-wave Observatory) and Virgo. We do not see evidence for gravitational wave emission from any of the targeted sources but produce upper limits on the emission amplitude. We highlight the results from seven young pulsars with large spin-down luminosities. We reach within a factor of five of the canonical spin-down limit for all seven of these, whilst for the Crab and Vela pulsars we further surpass their spin-down limits. We present new or updated limits for 172 other pulsars (including both young and millisecond pulsars). Now that the detectors are undergoing major upgrades, and, for completeness, we bring together all of the most up-to-date results from all pulsars searched for during the operations of the first-generation LIGO, Virgo and GEO600 detectors.     We summarize the sensitivity achieved by the LIGO and Virgo gravitational wave detectors for compact binary coalescence (CBC) searches during LIGO's fifth science run and Virgo's first science run. We present noise spectral density curves for each of the four detectors that operated during these science runs which are representative of the typical performance achieved by the detectors for CBC searches.     First LIGO search for gravitational wave bursts from cosmic (super)strings

    We report on a matched-filter search for gravitational wave bursts from cosmic string cusps using LIGO data from the fourth science run (S4) which took place in February and March 2005. No gravitational waves were detected in 14.9 days of data from times when all three LIGO detectors were operating. We interpret the result in terms of a frequentist upper limit on the rate of gravitational wave bursts and use the limits on the rate to constrain the parameter space (string tension, reconnection probability, and loop sizes) of cosmic string models.Comment: 11 pages, 3 figures.
