Comparing estimates of cost effectiveness submitted to the National Institute for Clinical Excellence (NICE) by different organisations: retrospective study

DESIGN: Retrospective pairwise comparison of evidence submitted to the technology appraisal programme of the National Institute for Clinical Excellence (NICE) by manufacturers of the relevant healthcare technologies and by contracted university based assessment groups. DATA SOURCES: Data from the first 62 appraisals. MAIN OUTCOME MEASURE: Incremental cost effectiveness ratios. RESULTS: Data from 27 of the 62 appraisals could be compared. The analysis of 54 pairwise comparisons showed that manufacturers' estimates of incremental cost effectiveness ratios were lower (suggesting a more cost effective use of resources) than those produced by the assessment groups (25 were lower, 29 were the same, none were higher, P < 0.01). Restriction of this dataset to include only one pairwise comparison per appraisal (27 pairs) produced a similar result (21 were lower, two were the same, four were higher, P < 0.001). CONCLUSIONS: The estimated incremental cost effectiveness ratios submitted by manufacturers were on average significantly lower than those submitted by the assessment groups. These results show that an important role of NICE's appraisal committee, and of decision makers in general, is to determine which economic evaluations, or parts of evaluations, should be given more credence
Heterogeneity of Glia in the Retina and Optic Nerve of Birds and Mammals
We have recently described a novel type of glial cell that is scattered across the inner layers of the avian retina [1]. These cells are stimulated by insulin-like growth factor 1 (IGF1) to proliferate, migrate distally into the retina, and up-regulate the nestin-related intermediate filament transitin. These changes in glial activity correspond with increased susceptibility of neurons to excitotoxic damage. This novel cell-type has been termed the Non-astrocytic Inner Retinal Glia-like (NIRG) cells. The purpose of the study was to investigate whether the retinas of non-avian species contain cells that resemble NIRG cells. We assayed for NIRG cells by probing for the expression of Sox2, Sox9, Nkx2.2, vimentin and nestin. NIRG cells were distinguished from astrocytes by a lack of expression for Glial Fibrilliary Acidic Protein (GFAP). We examined the retinas of adult mice, guinea pigs, dogs and monkeys (Macaca fasicularis). In the mouse retina and optic nerve head, we identified numerous astrocytes that expressed GFAP, S100Ξ², Sox2 and Sox9; however, we found no evidence for NIRG-like cells that were positive for Nkx2.2, nestin, and negative for GFAP. In the guinea pig retina, we did not find astrocytes or NIRG cells in the retina, whereas we identified astrocytes in the optic nerve. In the eyes of dogs and monkeys, we found astrocytes and NIRG-like cells scattered across inner layers of the retina and within the optic nerve. We conclude that NIRG-like cells are present in the retinas of canines and non-human primates, whereas the retinas of mice and guinea pigs do not contain NIRG cells
Refined saddle-point preconditioners for discretized Stokes problems
This paper is concerned with the implementation of efficient solution algorithms for elliptic problems with constraints. We establish theory which shows that including a simple scaling within well-established block diagonal preconditioners for Stokes problems can result in significantly faster convergence when applying the preconditioned MINRES method. The codes used in the numerical studies are available online
On the gauge boson's properties in a candidate technicolor theory
The technicolor scenario replaces the Higgs sector of the standard model with
a strongly interacting sector. One candidate for a realization of such a sector
is two-technicolor Yang-Mills theory coupled to two degenerate flavors of
adjoint, massless techniquarks. Using lattice gauge theory the properties of
the technigluons in this scenario are investigated as a function of the
techniquark mass towards the massless limit. For that purpose the minimal
Landau gauge two-point and three-point correlation functions are determined,
including a detailed systematic error analysis. The results are, within the
relatively large systematic uncertainties, compatible with a behavior very
similar to QCD at finite techniquark mass. However, the limit of massless
techniquarks exhibits features which could be compatible with a

Revisiting global fossil fuel and biofuel emissions of ethane
Recent measurements over the Northern Hemisphere indicate that the long-term decline in the atmospheric burden of ethane (C2H6) has ended and the abundance increased dramatically between 2010 and 2014. The rise in C2H6 atmospheric abundances has been attributed to oil and natural gas extraction in North America. Existing global C2H6 emission inventories are based on outdated activity maps that do not account for current oil and natural gas exploitation regions. We present an updated global C2H6 emission inventory based on 2010 satellite-derived CH4 fluxes with adjusted C2H6 emissions over the U.S. from the National Emission Inventory (NEI 2011). We contrast our global 2010 C2H6 emission inventory with one developed for 2001. The C2H6 difference between global anthropogenic emissions is subtle (7.9 versus 7.2βTgβyrβ1), but the spatial distribution of the emissions is distinct. In the 2010 C2H6 inventory, fossil fuel sources in the Northern Hemisphere represent half of global C2H6 emissions and 95% of global fossil fuel emissions. Over the U.S., unadjusted NEI 2011 C2H6 emissions produce mixing ratios that are 14β50% of those observed by aircraft observations (2008β2014). When the NEI 2011 C2H6 emission totals are scaled by a factor of 1.4, the Goddard Earth Observing System Chem model largely reproduces a regional suite of observations, with the exception of the central U.S., where it continues to underpredict observed mixing ratios in the lower troposphere. We estimate monthly mean contributions of fossil fuel C2H6 emissions to ozone and peroxyacetyl nitrate surface mixing ratios over North America of ~1% and ~8%, respectively
Terahertz Security Image Quality Assessment by No-reference Model Observers
To provide the possibility of developing objective image quality assessment
(IQA) algorithms for THz security images, we constructed the THz security image
database (THSID) including a total of 181 THz security images with the
resolution of 127*380. The main distortion types in THz security images were
first analyzed for the design of subjective evaluation criteria to acquire the
mean opinion scores. Subsequently, the existing no-reference IQA algorithms,
which were 5 opinion-aware approaches viz., NFERM, GMLF, DIIVINE, BRISQUE and
BLIINDS2, and 8 opinion-unaware approaches viz., QAC, SISBLIM, NIQE, FISBLIM,
CPBD, S3 and Fish_bb, were executed for the evaluation of the THz security
image quality. The statistical results demonstrated the superiority of Fish_bb
over the other testing IQA approaches for assessing the THz image quality with
PLCC (SROCC) values of 0.8925 (-0.8706), and with RMSE value of 0.3993. The
linear regression analysis and Bland-Altman plot further verified that the
Fish__bb could substitute for the subjective IQA. Nonetheless, for the
classification of THz security images, we tended to use S3 as a criterion for
ranking THz security image grades because of the relatively low false positive
rate in classifying bad THz image quality into acceptable category (24.69%).
Interestingly, due to the specific property of THz image, the average pixel
intensity gave the best performance than the above complicated IQA algorithms,
with the PLCC, SROCC and RMSE of 0.9001, -0.8800 and 0.3857, respectively. This
study will help the users such as researchers or security staffs to obtain the
THz security images of good quality. Currently, our research group is

Radio-frequency dressed state potentials for neutral atoms
Potentials for atoms can be created by external fields acting on properties
like magnetic moment, charge, polarizability, or by oscillating fields which
couple internal states. The most prominent realization of the latter is the
optical dipole potential formed by coupling ground and electronically excited
states of an atom with light. Here we present an experimental investigation of
the remarkable properties of potentials derived from radio-frequency (RF)
coupling between electronic ground states. The coupling is magnetic and the
vector character allows to design state dependent potential landscapes. On atom
chips this enables robust coherent atom manipulation on much smaller spatial
scales than possible with static fields alone. We find no additional heating or
collisional loss up to densities approaching atoms / cm compared
to static magnetic traps. We demonstrate the creation of Bose-Einstein
condensates in RF potentials and investigate the difference in the interference
between two independently created and two coherently split condensates in
identical traps. All together this makes RF dressing a powerful new tool for
micro manipulation of atomic and molecular systems
GABA Maintains the Proliferation of Progenitors in the Developing Chick Ciliary Marginal Zone and Non-Pigmented Ciliary Epithelium
GABA is more than the main inhibitory neurotransmitter found in the adult CNS. Several studies have shown that GABA regulates the proliferation of progenitor and stem cells. This work examined the effects of the GABAA receptor system on the proliferation of retinal progenitors and non-pigmented ciliary epithelial (NPE) cells. qRT-PCR and whole-cell patch-clamp electrophysiology were used to characterize the GABAA receptor system. To quantify the effects on proliferation by GABAA receptor agonists and antagonists, incorporation of thymidine analogues was used. The results showed that the NPE cells express functional extrasynaptic GABAA receptors with tonic properties and that low concentration of GABA is required for a baseline level of proliferation. Antagonists of the GABAA receptors decreased the proliferation of dissociated E12 NPE cells. Bicuculline also had effects on progenitor cell proliferation in intact E8 and E12 developing retina. The NPE cells had low levels of the Clβtransporter KCC2 compared to the mature retina, suggesting a depolarising role for the GABAA receptors. Treatment with KCl, which is known to depolarise membranes, prevented some of the decreased proliferation caused by inhibition of the GABAA receptors. This supported the depolarising role for the GABAA receptors. Inhibition of L-type voltage-gated Ca2+ channels (VGCCs) reduced the proliferation in the same way as inhibition of the GABAA receptors. Inhibition of the channels increased the expression of the cyclin-dependent kinase inhibitor p27KIP1, along with the reduced proliferation. These results are consistent with that when the membrane potential indirectly regulates cell proliferation with hyperpolarisation of the membrane potential resulting in decreased cell division. The increased expression of p27KIP1 after inhibition of either the GABAA receptors or the L-type VGCCs suggests a link between the GABAA receptors, membrane potential, and intracellular Ca2+ in regulating the cell cycle
Expression and Localization of CLC Chloride Transport Proteins in the Avian Retina
Members of the ubiquitously expressed CLC protein family of chloride channels and transporters play important roles in regulating cellular chloride and pH. The CLCs that function as Clβ/H+ antiporters, ClCs 3β7, are essential in particular for the acidification of endosomal compartments and protein degradation. These proteins are broadly expressed in the nervous system, and mutations that disrupt their expression are responsible for several human genetic diseases. Furthermore, knock-out of ClC3 and ClC7 in the mouse result in the degeneration of the hippocampus and the retina. Despite this evidence of their importance in retinal function, the expression patterns of different CLC transporters in different retinal cell types are as yet undescribed. Previous work in our lab has shown that in chicken amacrine cells, internal Clβ can be dynamic. To determine whether CLCs have the potential to participate, we used PCR and immunohistochemical techniques to examine CLC transporter expression in the chicken retina. We observed a high level of variation in the retinal expression levels and patterns among the different CLC proteins examined. These findings, which represent the first systematic investigation of CLC transporter expression in the retina, support diverse functions for the different CLCs in this tissue
