    Risk factors for infection, predictors of severe disease, and antibody response to COVID-19 in patients with inflammatory rheumatic diseases in Portugal: a multicenter, nationwide study

No use, distribution or reproduction is permitted which does not comply with these terms.Objective: To identify risk factors for SARS-CoV-2 infection and for severe/critical COVID-19, and to assess the humoral response after COVID-19 in these patients. Methods: Nationwide study of adult patients with inflammatory RMDs prospectively followed in the Rheumatic Diseases Portuguese Register-Reuma.pt-during the first 6 months of the pandemic. We compared patients with COVID-19 with those who did not develop the disease and patients with mild/moderate disease with those exhibiting severe/critical COVID-19. IgG antibodies against SARS-CoV-2 were measured ≥3 months after infection and results were compared with matched controls. Results: 162 cases of COVID-19 were registered in a total of 6,363 appointments. Patients treated with TNF inhibitors (TNFi; OR = 0.160, 95% CI 0.099-0.260, P < 0.001) and tocilizumab (OR 0.147, 95% CI 0.053-0.408, P < 0.001) had reduced odds of infection. Further, TNFi tended to be protective of severe and critical disease. Older age, major comorbidities, and rituximab were associated with an increased risk of infection and worse prognosis. Most patients with inflammatory RMDs (86.2%) developed a robust antibody response. Seroconversion was associated with symptomatic disease (OR 13.46, 95% CI 2.21-81.85, P = 0.005) and tended to be blunted by TNFi (OR 0.17, 95% CI 0.03-1.05; P = 0.057). Conclusions: TNFi and tocilizumab reduced the risk of infection by SARS-CoV-2. Treatment with TNFi also tended to reduce rates of severe disease and seroconversion. Older age, general comorbidities and rituximab were associated with increased risk for infection and worse prognosis, in line with previous reports.     Gestão de cuidado na segurança do paciente grave: revisão integrativa

    Introduction: Care management in the ICU is carried out by the nurse, who must organize her team to provide assistance to critical patients in all their therapeutic complexity, using technology, science and humanization. Objective: To evaluate care management in critically ill patient safety. Methodology: Integrative literature review research, guided by the following steps: construction of the research question; delimitation of inclusion and exclusion criteria; choice of databases; search and selection of studies; analysis and presentation of results. Results and Discussion: 1,500 studies were identified, after applying the language inclusion criteria, remaining 50 studies, and then reading the titles, in which 1,450 articles were excluded, as they were not related to the objective of this article. The summaries of the 50 articles selected based on the analysis of the titles were read. 10 articles were included in the work. Conclusion: Therefore, we can conclude the objective of the work was concluded by identifying that occurrences of errors and adverse events within the ICU, for the most part, are the responsibility of the nursing team due to the direct and constant provision of patient care and are linked to medications, failure in communication between the nursing team, lack of training and professional overload, in addition to carrying out procedures.Introdução: A gestão do cuidado na UTI é feita pela enfermeira, na qual deve organizar sua equipe para prestar assistência ao paciente crítico em toda sua complexidade terapêutica, utilizando tecnologia, ciência e humanização. Objetivo: Avaliar a gestão do cuidado na segurança do paciente grave. Metodologia: Pesquisa de revisão integrativa da literatura, guiada pelas seguintes etapas: construção da questão de pesquisa; delimitação dos critérios de inclusão e exclusão; escolha das bases de dados; busca e seleção dos estudos; análise e exposição dos resultados. &nbsp;Resultados e Discussão: Foram identificados 1.500 estudos, depois da aplicação do critério de inclusão do idioma, permanecendo os 50 estudos, e em seguida a leitura dos títulos, nos quais foram excluídos 1.450 artigos, por não estarem relacionados ao objetivo deste artigo. Houve a leitura dos resumos dos 50 artigos selecionados a partir da análise dos títulos. Foram incluídos 10 artigos no trabalho. Conclusão: Sendo assim, podemos concluir o objetivo do trabalho foi concluído identificando que ocorrências de erros e eventos adversos dentro da UTI, em sua maioria, são de competência são de competência da equipe de enfermagem devido à realização direta e constante dos cuidados ao paciente e estão ligadas a medicamentos, falha na comunicação da equipe de enfermagem, falta de capacitação e sobrecarga do profissional, além de realização de procedimentos

    Antiulcerogenic activity of the hydroalcoholic extract of leaves of Annona muricata Linnaeus in mice

    AbstractAnnona muricata Linnaeus, popularly known as “graviola” and also called soursop, is a species typical of countries with a tropical climate, and it is used in folk medicine as an anticancer, analgesic and antispasmodic agent. The aim of the present study was to validate the gastroprotective activity of the hydroalcoholic extract of the leaves of A. muricata (HEAM) and to investigate the underlying mechanisms of action for this effect. Gastric lesions were induced in mice by absolute ethanol, acidified ethanol or indomethacin. Before, the animals were pretreated with saline, omeprazole or HEAM orally at doses of 50–400mg/kg. To determine the mechanism of action of the extract, we investigated, using specific inhibitors, the involvement of nitric oxide (NO), prostaglandins (PGEs), ATP-dependent K+ channels and α2-noradrenergic receptors. HEAM showed significant antiulcer activity against lesions induced by absolute ethanol, acidified ethanol or indomethacin, which was mediated by endogenous gastric prostaglandins

    COVID-19 symptoms at hospital admission vary with age and sex: results from the ISARIC prospective multinational observational study

    Background: The ISARIC prospective multinational observational study is the largest cohort of hospitalized patients with COVID-19. We present relationships of age, sex, and nationality to presenting symptoms. Methods: International, prospective observational study of 60 109 hospitalized symptomatic patients with laboratory-confirmed COVID-19 recruited from 43 countries between 30 January and 3 August 2020. Logistic regression was performed to evaluate relationships of age and sex to published COVID-19 case definitions and the most commonly reported symptoms. Results: ‘Typical’ symptoms of fever (69%), cough (68%) and shortness of breath (66%) were the most commonly reported. 92% of patients experienced at least one of these. Prevalence of typical symptoms was greatest in 30- to 60-year-olds (respectively 80, 79, 69%; at least one 95%). They were reported less frequently in children (≤ 18 years: 69, 48, 23; 85%), older adults (≥ 70 years: 61, 62, 65; 90%), and women (66, 66, 64; 90%; vs. men 71, 70, 67; 93%, each P &lt; 0.001). The most common atypical presentations under 60 years of age were nausea and vomiting and abdominal pain, and over 60 years was confusion. Regression models showed significant differences in symptoms with sex, age and country. Interpretation: This international collaboration has allowed us to report reliable symptom data from the largest cohort of patients admitted to hospital with COVID-19. Adults over 60 and children admitted to hospital with COVID-19 are less likely to present with typical symptoms. Nausea and vomiting are common atypical presentations under 30 years. Confusion is a frequent atypical presentation of COVID-19 in adults over 60 years. Women are less likely to experience typical symptoms than men

    Mestrado em Arte MultimédiaBiografia e Autobiografia Universal: para um modelo de análise dos rituais de produção e consumo na arte multimédia trata do estudo e análise de formas alternativas que questionam o modo como criamos a nossa memória cultural colectiva por meio de objectos e rituais. O campo de investigação aqui exposto permanece distinto da História ou Crítica da arte. A abordagem aos rituais é realizada pelo do ponto de vista sócio-cultural, ambicionando reconhecer um modelo de profusão cultural. Inicialmente, é analisado o papel da memória na construção de conteúdos culturais. Com esse intuito, é discutida a validade de dispositivos externos, como a hermenêutica e os rituais nessa construção. Abordando temas gerais como a Memória, Cultura e Linguagem estabelecem-se dicotomias entre Hermenêutica e Rituais, Texto e Imagem, de modo a perceber uma evolução dos rituais tradicionais para rituais contemporâneos em actividades artísticas. É questionado, nos rituais contemporâneos, o seu potencial de, como manifestações performativas, que se centram no indivíduo, constituírem o formato privilegiado para uma linguagem transversal, independente de religiões, nacionalidade, ou grupo. Sendo a investigação desenvolvida no âmbito do Mestrado em Arte Multimédia, avançou-se por uma análise de propostas artísticas que discutem a amplitude dos conceitos de multimédia e performance. A presença do subtítulo Para um modelo de análise dos rituais de produção e consumo na arte multimédia representa um enquadramento da dinâmica presente nos rituais contemporâneos em manifestações de trabalho artístico de tendência biográfica e autobiográfica. The field of investigation here shown stands apart from areas of History or Art criticism. The approach to rituals is made from the Socio-cultural view point, aspiring to recognize a model of cultural profusion. Foremost, the role of memory in the construction of cultural content is here analyzed. With that intent, the validity between external devices, such as hermeneutics and rituals, is discussed on that construction. Approaching broad themes such as Memory, Culture, Language, dichotomies between Hermeneutics and Rituals, Text and Image are established to understand an evolution from traditional rituals to contemporary ones in artistic activity. It is questioned and suggested that contemporary rituals, with their potential of being performative manifestations that centre on the individual, constitute a privileged format for a transverse language, independent of religion, nationality or group. Being the investigation developed in the scope of the Master in Multimedia Art, an analysis of artistic proposals was followed, that discusses the breadth of the multimedia and performance concepts. The presence of the sub-heading For a model of analysis of the rituals of production and consumption in multimedia art , represents a framing of the dynamic which is present on contemporary rituals in biographical and autobiographical artistic work.     Tese de Doutoramento em Design, apresentada na Faculdade de Arquitetura da Universidade de Lisboa para obtenção do grau de Doutor.A construção de um arquivo, elemento estruturante e declarado de organização de informação, implica a consciência dos processos atuais de gestão de dados. A gestão dessa informação liga-se à potencialidade de interpretar e ler informações que de outro modo são indecifráveis ou que permanecem invisíveis. O design de informação e nomeadamente a visualização de informação são terreno fértil para a conceptualização, criação e origem de novas formas de organização visual. Nesse contexto, estudámos, observámos e inferimos sobre como pode ser um modelo de implementação de novos arquivos – arquivos visuais digitais. Com este estudo, procuramos analisar o que está por detrás dos modelos de arquivos digitais. Tem por objectivo analisar questões de percepção, cultura e léxico visuais, fornecendo linhas orientadoras para a criação e construção de arquivos visuais digitais, no contexto dos jornais digitais online. A metodologia adoptada, com recurso a revisão literária e técnicas como trabalho de experimentação laboratorial de um modelo, entrevistas e inquéritos, permitiu concluir que existem modelos visuais de organização de dados e que a visualização de informação pode caminhar para a criação de estruturas de arquivo visual digital mais amplas. Ao aplicar e adaptar conhecimentos desenvolvidos por diversos autores, constatámos que existe um território sistémico de potencialidade de organização visual. Conseguimos, desse modo, identificar elementos para um léxico visual, retirando considerações globais com características estruturantes para a área do design, e classificá-los à luz das necessidades do Jornal P3 e da sua organização.ABSTRACT: The building of an Archive, a structured and declared element of information, implies the awareness of existing data organization processes. The management of this information will connect with the capability to interpret and read information that are otherwise impervious. Design, archiving and image are fertile ground for the conceptualization, creation and origin of new visual media in information visualization. In this context, we study, observe and infer on how they can become an implementation model for new archives – visual digital archives. With this study, we seek to analyze what is behind the digital archive model. It aims to infer on perception, culture and visual lexicon issues, giving guidelines to the creation and construct of digital visual archives, in the context of online digital newspapers. The methodology, using literary review and technics such as experimental laboratorial work, interviews and surveys, allowed the conclusion that the- re are visual models of organizing data and that information visualization can set the creation of broader digital visual archives structures. By applying and adapting the knowledge developed by several authors, we found that there is a systemic territory of visual organization capability. We managed, thereby, to identify the elements for a visual lexicon, removing global considerations with structural features for the field of design, and classify them in the light of the needs of the P3 newspaper and its organization.N/
