    The Contribution of Participatory Research: On-Farm Research

    Participatory research in agriculture may range from research and technology development (R&D), carried out on a research station with some involvement of farmers, through to genuine participatory research involving researchers and farmers working together. The latter involves the end-user in actually carrying out aspects of the research and/or in the development and evaluation of technology that is appropriate to commercial enterprises. Researchers often question the validity of the ‘findings of on-farm participatory research’ as they are more comfortable with the ‘controlled’ environment of the research station. However if research is to be applied appropriately on farms, it must go through a period of evaluation on-farm. This paper summarises perspectives relating to participatory on-farm research, highlighting some opportunities that new technology is providing by considering five key areas as follows: • Participatory research within the agricultural research enterprise (current situation); • On-farm research compared with in-station research (options); • A New Zealand example of on-farm research; • New opportunities in on-farm research; • Future models for participatory on-farm research

    The Importance of Organic Content to Fractal Floc Properties in Estuarine Surface Waters: Insights From Video, LISST, and Pump Sampling

    To better understand the nature of flocs of varying organic content in estuarine surface waters, Laser in situ Scattering and Transmissometry, video settling, and pump sampling were deployed in the York River estuary. A new in situ method was developed to simultaneously solve the floc fractal dimension (F), primary particle size (d p ), and primary particle density (ρ p ) by fitting a simple fractal model to observations of effective floc density (∆ρ) as a function of floc diameter (d f ), while ensuring that the integrated particle size distribution was consistent with measurements of bulk apparent density (ρ a ). When fractal fits were statistically justified, application of the above methods showed the bulk fraction of organic matter (f org ) to be well correlated to multiple floc properties. As f org increased, d p and ρ a also increased, while ρ p , total suspended solids (TSS), and median floc size decreased. Notably for microflocs, neither F nor ∆ρ was significantly related to either f org or TSS. This indicates that organic matter may partially displace water content within microflocs without fundamentally changing the flocs’ inorganic structure. When pooling multiple samples, a marked decrease in F was seen at the transition to macroflocs, and most strongly for high f org cases. This suggested that settling velocities \u3e_ ~1 mm/s may produce turbulent stresses that tend to tear macroflocs apart. This study also found that when the fractal theory held, ρ p had a near 1:1 correlation with the bulk dry density of filtered TSS, implying that primary particles are tightly bound aggregates of combined mineral and organic component

    Ambivalence related to potential lifestyle changes following preventive cardiovascular consultations in general practice: A qualitative study

    <p>Abstract</p> <p>Background</p> <p>Motivational interviewing approaches are currently recommended in primary prevention and treatment of cardiovascular disease (CVD) in general practice in Denmark, based on an empirical and multidisciplinary body of scientific knowledge about the importance of motivation for successful lifestyle change among patients at risk of lifestyle related diseases. This study aimed to explore and describe motivational aspects related to potential lifestyle changes among patients at increased risk of CVD following preventive consultations in general practice.</p> <p>Methods</p> <p>Individual interviews with 12 patients at increased risk of CVD within 2 weeks after the consultation. Grounded theory was used in the analysis.</p> <p>Results</p> <p>Ambivalence related to potential lifestyle changes was the core motivational aspect in the interviews, even though the patients rarely verbalised this experience during the consultations. The patients experienced ambivalence in the form of conflicting feelings about lifestyle change. Analysis showed that these feelings interacted with their reflections in a concurrent process. Analysis generated a typology of five different ambivalence sub-types: perception, demand, information, priority and treatment ambivalence.</p> <p>Conclusion</p> <p>Ambivalence was a common experience in relation to motivation among patients at increased risk of CVD. Five different ambivalence sub-types were found, which clinicians may use to explore and resolve ambivalence in trying to aid patients to adopt lifestyle changes. Future research is needed to explore whether motivational interviewing and other cognitive approaches can be enhanced by exploring ambivalence in more depth, to ensure that lifestyle changes are made and sustained. Further studies with a wider range of patient characteristics are required to investigate the generalisability of the results.</p

    Reviewing evidence of marine ecosystem change off South Africa

    Recent changes have been observed in South African marine ecosystems. The main pressures on these ecosystems are fishing, climate change, pollution, ocean acidification and mining. The best long-term datasets are for trends in fishing pressures but there are many gaps, especially for non-commercial species. Fishing pressures have varied over time, depending on the species being caught. Little information exists for trends in other anthropogenic pressures. Field observations of environmental variables are limited in time and space. Remotely sensed satellite data have improved spatial and temporal coverage but the time-series are still too short to distinguish long-term trends from interannual and decadal variability. There are indications of recent cooling on the West and South coasts and warming on the East Coast over a period of 20 - 30 years. Oxygen concentrations on the West Coast have decreased over this period. Observed changes in offshore marine communities include southward and eastward changes in species distributions, changes in abundance of species, and probable alterations in foodweb dynamics. Causes of observed changes are difficult to attribute. Full understanding of marine ecosystem change requires ongoing and effective data collection, management and archiving, and coordination in carrying out ecosystem research.DHE

    A synthesis of evidence for the effects of interventions to conserve peatland vegetation: Overview and critical discussion

    Peatlandsare valuable but threatenedecosystems. Intervention to tackle direct threats is often necessary, but should be informed by scientific evidence to ensure it is effective and efficient. Herewe discuss a recent synthesis of evidence for the effects of interventions to conserve peatland vegetation -a fundamental component of healthy, functioning peatland ecosystems. The synthesis is unique in its broad scope (global evidence for a comprehensive list of 125 interventions) and practitioner-focused outputs (short narrative summaries in plain English, integrated into a searchable online database). Systematicliteraturesearches, supplemented by recommendationsfrom an international advisory board, identified162 publications containing 296 distinct tests of 66 of the interventions. Most of the articles studiedopen bogs or fens in Europe or North America. Only 36 interventions weresupported by sufficient evidence to assess their overall effectiveness. Mostof these interventions(85%) hadpositiveeffects, overall,on peatland vegetation-although this figure is likelyto have beeninflated by publication bias. We discuss how to use the synthesis, critically,to informconservation decisions.Reflecting on the content of the synthesiswe make suggestions for the future of peatland conservation,from monitoring overappropriate timeframes to routinely publishing resultsto build up the evidence base.MAVA, Arcadi

    Ultrasound-Guided Radiofrequency Thermal Ablation of Uterine Fibroids: Medium-Term Follow-Up

    Previous studies have shown that radiofrequency thermal ablation (RFA) of uterine fibroids through a percutaneous ultrasound (US)-guided procedure is an effective and safe minimally invasive treatment, with encouraging short-term results. The aim of this study was to assess the results in terms of volume reduction and clinical symptoms improvement in the midterm follow-up of fibroids with a diameter of up to 8 cm. Eleven premenopausal females affected by symptomatic fibroids underwent percutaneous US-guided RFA. Symptom severity and reduction in volume were evaluated at 1, 3, 6, 9, and 12 months. The mean symptom score (SSS) before the procedure was 50.30 (range 31.8–67.30), and the average quality of life (QOL) score value was 62 (range 37.20–86.00). The mean basal diameter was 5.5 cm (range 4.4–8) and the mean volume was 101.5 cm3 (range 44.58–278 cm3). The mean follow-up was 9 months (range 3–12 months). The mean SSS value at the end of the follow-up was 13.38 (range 0–67.1) and the QOL 90.4 (range 43.8–100). At follow-up the mean diameter was 3.0 cm (range 1.20–4.5 cm), and the mean volume was 18 cm3 (range 0.90–47.6 cm3). In 10 of 11 patients we obtained total or partial regression of symptoms. In one case the clinical manifestations persisted and it was thus considered unsuccessful. In conclusion, US-guided percutaneous RFA is a safe and effective treatment even for fibroids up to 8 cm

    Hormone Treatment without Surgery for Patients Aged 75 Years or Older with Operable Breast Cancer

    Purpose. To evaluate the trend in the use of primary endocrine treatment (PET) for elderly patients with operable breast cancer and to study mean time to response (TTR), local control, time to progression (TTP), and overall survival.Methods. Data of 184 patients aged >= 75 years, diagnosed with breast cancer in the south of the Netherlands between 2001 and 2008 and receiving PET, were analyzed.Results. The percentage of women >= 75 years with breast cancer receiving PET in the south of the Netherlands decreased from 23% in the period 1988-1992 to 12% in 1997-2000, and increased to 29% in 2005-2008. Mean age at diagnosis of 184 patients treated with PET in the period 2001-2008 was 84 years (range 75-89 years). Mean length of follow-up was 2.6 years. In 107 patients (58%), an initial response was achieved (mean TTR 7 months), 21 patients (12%) showed stable disease. A total of 64 patients (35%), with or without prior response, eventually displayed progression (mean TTP 20 months). No differences in TTR and TTP were observed between the patients starting with tamoxifen or an aromatase inhibitor. One hundred nineteen (65%) of 184 patients had died by January 1, 2010. In 17 patients (14%), breast cancer was the cause of death.Conclusions. Tumor progression was observed in a substantial proportion of the cohort, but only a small number of patients died of breast cancer.     Promoting the use of the PI-QUAL score for prostate MRI quality: results from the ESOR Nicholas Gourtsoyiannis teaching fellowship

    OBJECTIVES: The Prostate Imaging Quality (PI-QUAL) score is a new metric to evaluate the diagnostic quality of multiparametric magnetic resonance imaging (MRI) of the prostate. This study assesses the impact of an intervention, namely a prostate MRI quality training lecture, on the participant's ability to apply PI-QUAL. METHODS: Sixteen participants (radiologists, urologists, physicists, and computer scientists) of varying experience in reviewing diagnostic prostate MRI all assessed the image quality of ten examinations from different vendors and machines. Then, they attended a dedicated lecture followed by a hands-on workshop on MRI quality assessment using the PI-QUAL score. Five scans assessed by the participants were evaluated in the workshop using the PI-QUAL score for teaching purposes. After the course, the same participants evaluated the image quality of a new set of ten scans applying the PI-QUAL score. Results were assessed using receiver operating characteristic analysis. The reference standard was the PI-QUAL score assessed by one of the developers of PI-QUAL. RESULTS: There was a significant improvement in average area under the curve for the evaluation of image quality from baseline (0.59 [95 % confidence intervals: 0.50-0.66]) to post-teaching (0.96 [0.92-0.98]), an improvement of 0.37 [0.21-0.41] (p < 0.001). CONCLUSIONS: A teaching course (dedicated lecture + hands-on workshop) on PI-QUAL significantly improved the application of this scoring system to assess the quality of prostate MRI examinations. KEY POINTS: • A significant improvement in the application of PI-QUAL for the assessment of prostate MR image quality was observed after an educational intervention. • Appropriate training on image quality can be delivered to those involved in the acquisition and interpretation of prostate MRI. • Further investigation will be needed to understand the impact on improving the acquisition of high-quality diagnostic prostate MR examinations

    Do knowledge infrastructure facilities support Evidence-Based Practice in occupational health? An exploratory study across countries among occupational physicians enrolled on Evidence-Based Medicine courses

    <p>Abstract</p> <p>Background</p> <p>Evidence-Based Medicine (EBM) is an important method used by occupational physicians (OPs) to deliver high quality health care. The presence and quality of a knowledge infrastructure is thought to influence the practice of EBM in occupational health care. This study explores the facilities in the knowledge infrastructure being used by OPs in different countries, and their perceived importance for EBM practice.</p> <p>Methods</p> <p>Thirty-six OPs from ten countries, planning to attend an EBM course and to a large extent recruited via the European Association of Schools of Occupational Medicine (EASOM), participated in a cross-sectional study.</p> <p>Results</p> <p>Research and development institutes, and knowledge products and tools are used by respectively more than 72% and more than 80% of the OPs and they are rated as being important for EBM practice (more than 65 points (range 0–100)). Conventional knowledge access facilities, like traditional libraries, are used often (69%) but are rated as less important (46.8 points (range 0–100)) compared to the use of more novel facilities, like question-and-answer facilities (25%) that are rated as more important (48.9 points (range 0–100)). To solve cases, OPs mostly use non evidence-based sources. However, they regard the evidence-based sources that are not often used, e.g. the Cochrane library, as important enablers for practising EBM. The main barriers are lack of time, payment for full-text articles, language barrier (most texts are in English), and lack of skills and support.</p> <p>Conclusion</p> <p>This first exploratory study shows that OPs use many knowledge infrastructure facilities and rate them as being important for their EBM practice. However, they are not used to use evidence-based sources in their practice and face many barriers that are comparable to the barriers physicians face in primary health care.</p
