    Neighborhood and weight-related health behaviors in the Look AHEAD (Action for Health in Diabetes) Study

    <p>Abstract</p> <p>Background</p> <p>Previous studies have shown that neighborhood factors are associated with obesity, but few studies have evaluated the association with weight control behaviors. This study aims to conduct a multi-level analysis to examine the relationship between neighborhood SES and weight-related health behaviors.</p> <p>Methods</p> <p>In this ancillary study to Look AHEAD (Action for Health in Diabetes) a trial of long-term weight loss among individuals with type 2 diabetes, individual-level data on 1219 participants from 4 clinic sites at baseline were linked to neighborhood-level data at the tract level from the 2000 US Census and other databases. Neighborhood variables included SES (% living below the federal poverty level) and the availability of food stores, convenience stores, and restaurants. Dependent variables included BMI, eating patterns, weight control behaviors and resource use related to food and physical activity. Multi-level models were used to account for individual-level SES and potential confounders.</p> <p>Results</p> <p>The availability of restaurants was related to several eating and weight control behaviors. Compared to their counterparts in neighborhoods with fewer restaurants, participants in neighborhoods with more restaurants were more likely to eat breakfast (prevalence Ratio [PR] 1.29 95% CI: 1.01-1.62) and lunch (PR = 1.19, 1.04-1.36) at non-fast food restaurants. They were less likely to be attempting weight loss (OR = 0.93, 0.89-0.97) but more likely to engage in weight control behaviors for food and physical activity, respectively, than those who lived in neighborhoods with fewer restaurants. In contrast, neighborhood SES had little association with weight control behaviors.</p> <p>Conclusion</p> <p>In this selected group of weight loss trial participants, restaurant availability was associated with some weight control practices, but neighborhood SES was not. Future studies should give attention to other populations and to evaluating various aspects of the physical and social environment with weight control practices.</p

    Promotion of couples' voluntary counselling and testing for HIV through influential networks in two African capital cities

    <p>Abstract</p> <p>Background</p> <p>Most new HIV infections in Africa are acquired from cohabiting heterosexual partners. Couples' Voluntary Counselling and Testing (CVCT) is an effective prevention strategy for this group. We present our experience with a community-based program for the promotion of CVCT in Kigali, Rwanda and Lusaka, Zambia.</p> <p>Methods</p> <p>Influence Network Agents (INAs) from the health, religious, non-governmental, and private sectors were trained to invite couples for CVCT. Predictors of successful promotion were identified using a multi-level hierarchical analysis.</p> <p>Results</p> <p>In 4 months, 9,900 invitations were distributed by 61 INAs, with 1,411 (14.3%) couples requesting CVCT. INAs in Rwanda distributed fewer invitations (2,680 vs. 7,220) and had higher response rates (26.9% vs. 9.6%), than INAs in Zambia. Context of the invitation event, including a discreet location such as the INA's home (OR 3.3–3.4), delivery of the invitation to both partners in the couple (OR 1.6–1.7) or to someone known to the INA (OR 1.7–1.8), and use of public endorsement (OR 1.7–1.8) were stronger predictors of success than INA or couple-level characteristics.</p> <p>Conclusion</p> <p>Predictors of successful CVCT promotion included strategies that can be easily implemented in Africa. As new resources become available for Africans with HIV, CVCT should be broadly implemented as a point of entry for prevention, care and support.</p

    NGTS-28Ab:a short period transiting brown dwarf

    We report the discovery of a brown dwarf orbiting a M1 host star. We first identified the brown dwarf within the Next Generation Transit Survey data, with supporting observations found in TESS sectors 11 and 38. We confirmed the discovery with follow-up photometry from the South African Astronomical Observatory, SPECULOOS-S, and TRAPPIST-S, and radial velocity measurements from HARPS, which allowed us to characterize the system. We find an orbital period of ∼1.25 d, a mass of 69.0+5.3-4.8 MJ, close to the hydrogen burning limit, and a radius of 0.95 ± 0.05 RJ. We determine the age to be &gt;0.5 Gyr, using model isochrones, which is found to be in agreement with spectral energy distribution fitting within errors. NGTS-28Ab is one of the shortest period systems found within the brown dwarf desert, as well as one of the highest mass brown dwarfs that transits an M dwarf. This makes NGTS-28Ab another important discovery within this scarcely populated region.</div

    An old warm Jupiter orbiting the metal-poor G-dwarf TOI-5542

    We report the discovery of a 1.32−0.10+0.10 MJup planet orbiting on a 75.12 day period around the G3V 10.8−3.6+2.1 Gyr old star TOI-5542 (TIC 466206508; TYC 9086-1210-1). The planet was first detected by the Transiting Exoplanet Survey Satellite (TESS) as a single transit event in TESS Sector 13. A second transit was observed 376 days later in TESS Sector 27. The planetary nature of the object has been confirmed by ground-based spectroscopic and radial velocity observations from the CORALIE and HARPS spectrographs. A third transit event was detected by the ground-based facilities NGTS, EulerCam, and SAAO. We find the planet has a radius of 1.009−0.035+0.036 RJup and an insolation of 9.6−0.8+0.9 S⊕, along with a circular orbit that most likely formed via disk migration or in situ formation, rather than high-eccentricity migration mechanisms. Our analysis of the HARPS spectra yields a host star metallicity of [Fe/H] = −0.21 ± 0.08, which does not follow the traditional trend of high host star metallicity for giant planets and does not bolster studies suggesting a difference among low- and high-mass giant planet host star metallicities. Additionally, when analyzing a sample of 216 well-characterized giant planets, we find that both high masses (4 MJup 10 days) and hot (P 0.1). TOI-5542b is one of the oldest known warm Jupiters and it is cool enough to be unaffected by inflation due to stellar incident flux, making it a valuable contribution in the context of planetary composition and formation studies

    TOI-836 : a super-Earth and mini-Neptune transiting a nearby K-dwarf

TOI-836 is a bright (T = 8.5 mag), high proper motion (∼200 mas yr−1), low metallicity ([Fe/H]≈−0.28) K-dwarf with a mass of 0.68 ± 0.05 M⊙ and a radius of 0.67 ± 0.01 R⊙. We obtain photometric follow-up observations with a variety of facilities, and we use these data-sets to determine that the inner planet, TOI-836 b, is a 1.70 ± 0.07 R⊕ super-Earth in a 3.82 day orbit, placing it directly within the so-called ‘radius valley’. The outer planet, TOI-836 c, is a 2.59 ± 0.09 R⊕ mini-Neptune in an 8.60 day orbit. Radial velocity measurements reveal that TOI-836 b has a mass of 4.5 ± 0.9 M⊕, while TOI-836 c has a mass of 9.6 ± 2.6 M⊕. Photometric observations show Transit Timing Variations (TTVs) on the order of 20 minutes for TOI-836 c, although there are no detectable TTVs for TOI-836 b.     We present the discovery of two exoplanets transiting TOI-836 (TIC 440887364) using data from TESS Sector 11 and Sector 38. TOI-836 is a bright (T=8.5T = 8.5 mag), high proper motion (200\sim\,200 mas yr1^{-1}), low metallicity ([Fe/H]0.28\approx\,-0.28) K-dwarf with a mass of 0.68±0.050.68\pm0.05 M_{\odot} and a radius of 0.67±0.010.67\pm0.01 R_{\odot}. We obtain photometric follow-up observations with a variety of facilities, and we use these data-sets to determine that the inner planet, TOI-836 b, is a 1.70±0.071.70\pm0.07 R_{\oplus} super-Earth in a 3.82 day orbit, placing it directly within the so-called 'radius valley'. The outer planet, TOI-836 c, is a 2.59±0.092.59\pm0.09 R_{\oplus} mini-Neptune in an 8.60 day orbit.     The development and validation of a scoring tool to predict the operative duration of elective laparoscopic cholecystectomy

    Background: The ability to accurately predict operative duration has the potential to optimise theatre efficiency and utilisation, thus reducing costs and increasing staff and patient satisfaction. With laparoscopic cholecystectomy being one of the most commonly performed procedures worldwide, a tool to predict operative duration could be extremely beneficial to healthcare organisations. Methods: Data collected from the CholeS study on patients undergoing cholecystectomy in UK and Irish hospitals between 04/2014 and 05/2014 were used to study operative duration. A multivariable binary logistic regression model was produced in order to identify significant independent predictors of long (> 90 min) operations. The resulting model was converted to a risk score, which was subsequently validated on second cohort of patients using ROC curves. Results: After exclusions, data were available for 7227 patients in the derivation (CholeS) cohort. The median operative duration was 60 min (interquartile range 45–85), with 17.7% of operations lasting longer than 90 min. Ten factors were found to be significant independent predictors of operative durations > 90 min, including ASA, age, previous surgical admissions, BMI, gallbladder wall thickness and CBD diameter. A risk score was then produced from these factors, and applied to a cohort of 2405 patients from a tertiary centre for external validation. This returned an area under the ROC curve of 0.708 (SE = 0.013, p  90 min increasing more than eightfold from 5.1 to 41.8% in the extremes of the score. Conclusion: The scoring tool produced in this study was found to be significantly predictive of long operative durations on validation in an external cohort. As such, the tool may have the potential to enable organisations to better organise theatre lists and deliver greater efficiencies in care
