    A Dashboard to Support Decision-Making Processes in Learning Ecosystems

    There are software solutions to solve most of the problems related to information management in any company or institutions, but still, there is a problem for transforming information into knowledge. Technological ecosystems emerge as a solution to combine existing tools and human resources to solve different problems of knowledge management. In particular, when the ecosystem is focused on learning processes associated with knowledge are named learning ecosystems. The learning ecosystem metamodel defined in previous works solves several problems related to the definition and implementation of these solutions. However, there are still challenges associated with improving the analysis and visualization of information as a way to discover knowledge and support decision making processes. On the other hand, there is a metamodel proposal to define customized dashboards for supporting decision-making processes. This proposal aims to integrate both metamodels as a way to improve the definition of learning ecosystems

    An Approach for the Automated Generation of Engaging Dashboards

    Organizations use Key Performance Indicators (KPIs) to monitor whether they attain their goals. To support organizations at tracking the performance of their business, software vendors offer dash boards to these organizations. For the development of the dashboards that will engage organizations and enable them to make informed deci sions, software vendors leverage dashboard design principles. However, the dashboard design principles available in the literature are expressed as natural language texts. Therefore, software vendors and organizations either do not use them or spend significant efforts to internalize and apply them literally in every engaging dashboard development process. We show that engaging dashboards for organizations can be automati cally generated by means of automatically visualized KPIs. In this con text, we present our novel approach for the automated generation of engaging dashboards for organizations. The approach employs the deci sion model for visualizing KPIs that is developed based on the dashboard design principles in the literature.     Influencia de un programa de empleo con apoyo en la calidad de vida percibida de sus usuarios

    Esta investigación evaluó el impacto del empleo con apoyo en la calidad de vida de las personas con discapacidad y los factores que influyen en la misma. Para ello se aplicó la adaptación española de la Escala de Calidad de Vida de Schalock y Keith (1993) y el Formulario de Datos del Trabajador (lNICO, 2004) a una muestra de 1065 trabajadores con discapacidad que se encontraban trabajando gracias al Programa Empleo con Apoyo de Caja Madrid entre 2005 y 2007. Los resultados indicaron que, en general, los trabajadores muestran una elevada calidad de vida percibida. Se plantean los modelos explicativos de las dimensiones de la calidad de vida en función del tipo de discapacidad

    Pediatric Intensive Care Unit admission criteria for haemato-oncological patients: a basis for clinical guidelines implementation

    Recent advances in supportive care and progress in the development and use of chemotherapy have considerably improved the prognosis of many children with malignancy, thus the need for intensive care admission and management is increasing, reaching about 40% of patients throughout the disease course. Cancer remains a major death cause in children, though outcomes have considerably improved over the past decades. Prediction of outcome for children with cancer in Pediatric Intensive Care Unit (PICU) obviously requires clinical guidelines, and these are not well defined, as well as admission criteria. Major determinants of negative outcomes remain severe sepsis/septic shock association and respiratory failure, deserving specific approach in children with cancer, particularly those receiving a bone marrow transplantation. A nationwide consensus should be achieved among pediatric intensivists and oncologists regarding the threshold clinical conditions requiring Intensive Care Unit (ICU) admission as well as specific critical care protocols. As demonstrated for the critically ill non-oncologic child, it appears unreasonable that pediatric patients with malignancy can be admitted to an adult Intensive Care Unit ICU. On a national basis a pool of refecence institutions should be identified and early referral to an oncologic PICU is warranted

    Producción CientíficaEl objetivo del tratamiento quirúrgico para la escoliosis, cifosis y otro tipo de deformidades del raquis es la fusión de las vértebras para que la columna vertebral no pueda curvarse. Se colocan implantes metálicos (barras, tornillos, alambres, placas, etc.), para mantener el raquis mientras las vértebras se fusionan. La artrodesis vertebral se potencia con injerto (autogénico y/o alogénico) de hueso. El abordaje quirúrgico puede ser posterior, anterior o combinación de ambos. La cirugía toracoscópica se puede utilizar en el abordaje anterior del raquis dorsal.     Varespladib and cardiovascular events in patients with an acute coronary syndrome: the VISTA-16 randomized clinical trial

    IMPORTANCE: Secretory phospholipase A2(sPLA2) generates bioactive phospholipid products implicated in atherosclerosis. The sPLA2inhibitor varespladib has favorable effects on lipid and inflammatory markers; however, its effect on cardiovascular outcomes is unknown. OBJECTIVE: To determine the effects of sPLA2inhibition with varespladib on cardiovascular outcomes. DESIGN, SETTING, AND PARTICIPANTS: A double-blind, randomized, multicenter trial at 362 academic and community hospitals in Europe, Australia, New Zealand, India, and North America of 5145 patients randomized within 96 hours of presentation of an acute coronary syndrome (ACS) to either varespladib (n = 2572) or placebo (n = 2573) with enrollment between June 1, 2010, and March 7, 2012 (study termination on March 9, 2012). INTERVENTIONS: Participants were randomized to receive varespladib (500 mg) or placebo daily for 16 weeks, in addition to atorvastatin and other established therapies. MAIN OUTCOMES AND MEASURES: The primary efficacy measurewas a composite of cardiovascular mortality, nonfatal myocardial infarction (MI), nonfatal stroke, or unstable angina with evidence of ischemia requiring hospitalization at 16 weeks. Six-month survival status was also evaluated. RESULTS: At a prespecified interim analysis, including 212 primary end point events, the independent data and safety monitoring board recommended termination of the trial for futility and possible harm. The primary end point occurred in 136 patients (6.1%) treated with varespladib compared with 109 patients (5.1%) treated with placebo (hazard ratio [HR], 1.25; 95%CI, 0.97-1.61; log-rank P = .08). Varespladib was associated with a greater risk of MI (78 [3.4%] vs 47 [2.2%]; HR, 1.66; 95%CI, 1.16-2.39; log-rank P = .005). The composite secondary end point of cardiovascular mortality, MI, and stroke was observed in 107 patients (4.6%) in the varespladib group and 79 patients (3.8%) in the placebo group (HR, 1.36; 95% CI, 1.02-1.82; P = .04). CONCLUSIONS AND RELEVANCE: In patients with recent ACS, varespladib did not reduce the risk of recurrent cardiovascular events and significantly increased the risk of MI. The sPLA2inhibition with varespladib may be harmful and is not a useful strategy to reduce adverse cardiovascular outcomes after ACS. TRIAL REGISTRATION: clinicaltrials.gov Identifier: NCT01130246.     Review: Nutritional ecology of heavy metals

    The aim of this review is to focus the attention on the nutrition ecology of the heavy metals and on the major criticisms related to the heavy metals content in animal feeds, manure, soil and animal-origin products. Heavy metals are metallic elements that have a high density that have progressively accumulated in the food chain with negative effects for human health. Some metals are essential (Fe, I, Co, Zn, Cu, Mn, Mo, Se) to maintain various physiological functions and are usually added as nutritional additives in animal feed. Other metals (As, Cd, F, Pb, Hg) have no established biological functions and are considered as contaminants/undesirable substances. The European Union adopted several measures in order to control their presence in the environment, as a result of human activities such as: farming, industry or food processing and storage contamination. The control of the animal input could be an effective strategy to reduce human health risks related to the consumption of animal-origin products and the environmental pollution by manure. Different management of raw materials and feed, animal species as well as different legal limits can influence the spread of heavy metals. To set up effective strategies against heavy metals the complex interrelationships in rural processes, the widely variability of farming practices, the soil and climatic conditions must be considered.     HLA class II DNA typing in a large series of European patients with systemic lupus erythematosus: correlations with clinical and autoantibody subsets

    We conducted this study to determine the HLA class II allele associations in a large cohort of patients of homogeneous ethnic derivation with systemic lupus erythematosus (SLE). The large sample size allowed us to stratify patients according to their clinical and serologic characteristics. We studied 577 European Caucasian patients with SLE. Antinuclear antibodies (Hep-2 cells), anti-dsDNA antibodies (Crithidia luciliae), and antibodies to extractable nuclear antigens Ro (SS-A), La (SS-B), U1-RNP, Sm, Jo1, SCL70, and PCNA, were detected in all patients. Molecular typing of HLA-DRB1, DRB3, DQA1, and DQB1 loci was performed by the polymerase chain reaction-sequence specific oligonucleotide probes (PCR-SSOP) method. We found a significantly increased frequency of DRB1*03, DRB1*15, DRB1*16, DQA1*0102, DQB1*0502, DQB1*0602, DQB1*0201, DQB1*0303, and DQB1*0304 in lupus patients as compared with healthy controls. In addition, DRB1*03 was associated with anti-Ro, anti-La, pleuritis, and involvement of lung, kidney, and central nervous system. DRB1*15 and DQB1*0602 were associated with anti-dsDNA antibodies; DQB1*0201 with anti-Ro and anti-La, leukopenia, digital skin vasculitis, and pleuritis; and DQB1*0502 was associated with anti-Ro, renal involvement, discoid lupus, and livedo reticularis. In conclusion, our study shows some new HLA clinical and serologic associations in SLE and further confirms that the role of MHC genes is mainly to predispose to particular serologic and clinical manifestations of this disease
