    As crianças no processo migratório: Uma realidade que continua vigente

    O processo migratório tem sido uma atividade constante para o ser humano desde o início, em nível global, e as razões pelas quais se toma a decisão de migrar respondem às necessidades de sobrevivência ao longo da história. No caso latino-americano e, particularmente, no do território equatoriano, a crise econômica e a instabilidade política levaram ao crescimento do desemprego e à instabilidade social. A referida migração foi realizada em condições de ilegalidade e por meio de grandes dívidas com pessoas ou entidades ilegais, que impunham juros elevados e, portanto, tais dívidas deveriam ser pagas o mais rápido possível. Para isso, aqueles que migravam tinham de enfrentar condições de vida quase sub-humanas, com o objetivo de economizar e poder enviar as remessas necessárias para o pagamento dos seus bens. Tal situação colocou quem migrava em posição heróica dentro do círculo familiar e da sociedade como um todo, porque, devido às remessas que chegavam ao país, e à dolarização, a economia conseguiu se estabilizar em nível nacional, aumentando a circulação do dinheiro e, com isso, o acesso a bens e serviços que anos antes não estavam ao alcance da população em geral, devido aos altos custos de aquisição. Neste contexto, a ausência de pais e/ou mães estava justiicada dentro do entorno familiar, recebendo carta branca em benefício de maiores acessos econômicos e práticas sociais diferentes das práticas dos progenitores.palavras chave: migração, jovens, família, crianças

    Prioritization of Alternative Agricultural Products Based on Market Signals as a Tool for the Development of Innovative Projects

    The Decentralized Autonomous Government of the province of Chimborazo promotes the agricultural productive development of the province by promoting entrepreneurship, and improving the capacities of producers, access to production factors, and integration of actors and subjects of agriculture and management on the basis to strategic planning. The construction of the Chimborazo Competitiveness Agenda was a participatory process that identified opportunities to improve the agricultural sector, where agricultural production at the primary level was recognized as a vocation and strength of the province; however, a strategic necessity was to establish possibilities of adding value and diversifying agricultural production. The definition of a set of agricultural products with high potential at the primary level and/or in agroindustrial processes that are not deeply explored required determining market signals to identify new medium- and long-term commercial possibilities for Chimborazo farmers. Additionally, it was recognized that the management of the province of Chimborazo was based on its management of the Millennium Development Goals, the Constitution of the Republic, the National Plan for Development in a Supra-Provincial Framework, as well as the Plan Development of Chimborazo, the Territorial Planning Plan and others that promoted planning with a focus on territorial and solidarity sustainable development. Resumen El Gobierno Autónomo Descentralizado de la Provincia de Chimborazo impulsa el fomento productivo agrícola de la provincia mediante la promoción del emprendimiento, el mejoramiento de las capacidades de los productores, acceso a factores de producción, integración de los actores y sujetos del agro y gestión en base a una planificación estratégica. Como parte de dicho accionar, la construcción de la Agenda de Competitividad Chimborazo fue un proceso participativo que identificó oportunidades para mejorar el sector agrícola, donde se reconoció la producción agrícola a nivel primario como una vocación y fortaleza de la provincia, no obstante, una necesidad estratégica fue el establecer posibilidades de agregación de valor y de diversificar la producción agrícola. La definición de un conjunto de productos agrícolas con alto potencial a nivel primario y/o en procesos agroindustriales que no están profundamente explorados requirió determinar señales de mercado para identificar posibilidades comerciales nuevas a mediano y largo plazo para los agricultores de Chimborazo. Adicionalmente, se reconoció que la gestión de la provincia de Chimborazo tuvo como base los Objetivos del Milenio, la Constitución de la República, el Plan Nacional de Desarrollo en un marco supra-provincial, así como el Plan de Desarrollo de Chimborazo, el Plan de Ordenamiento Territorial y otros en su conjunto que promovieron una planificación con un enfoque de desarrollo sostenible territorial y solidario.     Supervivencia global en pacientes con cáncer gástrico avanzado o metastásico en los últimos 10 años en el Centro Médico Nacional «20 de noviembre del ISSSTE»

    ResumenAntecedentesLa introducción de nuevos fármacos para el tratamiento de los pacientes con cáncer gástrico irresecable, recurrente o metastásico ha reportado un modesto incremento en la supervivencia libre de progresión (SLP) y la supervivencia global (SG).ObjetivoDeterminar el impacto de la aplicación de los nuevos esquemas de quimioterapia paliativa en la SG de los pacientes con cáncer gástrico del Centro Médico Nacional 20 de noviembre, ISSSTE.Material y métodosEstudio retrospectivo, descriptivo. Analizamos expedientes de pacientes con cáncer gástrico irresecable o metastásico, tratados con quimioterapia paliativa de enero de 2002 a diciembre de 2012 en el Centro Médico Nacional 20 de noviembre, ISSSTE. Se evaluó la SG, SLP y los esquemas de quimioterapia más frecuentemente utilizados. Se asignaron los casos a 2 cohortes de acuerdo a la fecha de inicio de la quimioterapia, conformando la cohorte A los pacientes tratados de enero de 2002 hasta diciembre de 2006 y la cohorte B de enero de 2007 a diciembre de 2012. Se estableció estos periodos dado que representaba el cambio en los esquemas de tratamiento utilizados; es decir; la introducción de esquemas que incluyen antraciclinas, oxaliplatino, capecitabina y docetaxel.ResultadosRevisamos los expedientes de 291 pacientes con cáncer gástrico; excluimos a 221 pacientes por estar en etapas tempranas (i, ii o iii) resecables, recibir tratamiento con quimioterapia fuera de la unidad o no ser candidatos a quimioterapia. Finalmente incluimos 70 casos con tratamiento de primera línea. La mediana de SG para pacientes de la cohorte A fue de 11.2 vs. 10.5 meses para los pacientes de la cohorte B. La mediana de SLP en primera línea de tratamiento fue 8.5 vs. 5.2 meses respectivamente. Ambos resultados sin diferencia estadísticamente significativa.ConclusiónEn nuestro centro, no hay impacto de los nuevos esquemas de tratamiento en SLP o SG. Un tamaño de muestra pequeño y el hecho de que pacientes con pobre estado funcional recibieran quimioterapia podrían ser factores que influyeron en los resultados del estudio, en el cual se observó una tendencia a favor de los nuevos esquemas de quimioterapia, pero sin demostrar significación estadística.AbstractBackgroundThe introduction of new drugs for the treatment of patients with advanced, recurrent or metastatic gastric cancer has resulted in a small benefit in overall survival (OS) and progression free survival (PFS).ObjectiveTo determine the impact of new chemotherapy schedules on the OS of patients with advanced or metastic gastric cancer treated at the Centro Medico Nacional 20 de noviembre, ISSSTE.Material and methodologyRetrolective, descriptive study, the clinical files of patients with advanced, recurrent or metastatic gastric cancer treated with chemotherapy at the Centro Medico Nacional 20 de noviembre, ISSSTE, from january 2002 to december 2012, were analyzed. Chemotherapy schedules, OS and PFS were evaluated. Patients were assigned to two cohorts: those treated from january 2002 to december 2006 were included in cohort A and those treated from january 2007 to december 2012 in cohort B. These time periods were determined based on the years when newer chemotherapy agents (anthracyclines, oxaliplatin, capecitabine and docetaxel) were introduced in our institution.Results291 clinical files were analyzed; 221 patients were excluded for they had clinical stage I, II or resectable III disease, started first line chemotherapy as outpatients of our institution or were not candidates for chemotherapy. 70 cases treated with first line chemotherapy were included. OS for patients in cohort A was 11.2 months vs 10.5 months for patients in cohort B. PFS was 8.5 months vs 5.2, respectively. There was no statistical difference in either comparison.ConclusionThere was no impact of the introduction of newer chemotherapy agents in OS or PFS in patients treated in our institution. A small sample size and the fact that patients with poor performance status received chemotherapy could have had influenced the results of our study, in which tendency towards a better outcome for patients treated with newer chemotherapy schemes was observed, although a statistically significant benefit was not proven

    PDR-1/hParkin negatively regulates the phagocyosis of apoptotic cell corpses in Caenorhabditis elegans

    Apoptotic cell death is an integral part of cell turnover in many tissues, and proper corpse clearance is vital to maintaining tissue homeostasis in all multicellular organisms. Even in tissues with high cellular turnover, apoptotic cells are rarely seen because of efficient clearance mechanisms in healthy individuals. In Caenorhabditis elegans, two parallel and partly redundant conserved pathways act in cell corpse engulfment. The pathway for cytoskeletal rearrangement requires the small GTPase CED-10 Rac1 acting for an efficient surround of the dead cell. The CED-10 Rac pathway is also required for the proper migration of the distal tip cells (DTCs) during the development of the C. elegans gonad. Parkin, the mammalian homolog of the C. elegans PDR-1, interacts with Rac1 in aged human brain and it is also implicated with actin dynamics and cytoskeletal rearrangements in Parkinsons's disease, suggesting that it might act on engulfment. Our genetic and biochemical studies indicate that PDR-1 inhibits apoptotic cell engulfment and DTC migration by ubiquitylating CED-10 for degradation

    BET Inhibition-Induced GSK3β Feedback Enhances Lymphoma Vulnerability to PI3K Inhibitors

    The phosphatidylinositol 3 kinase (PI3K)-glycogen synthase kinase \u3b2 (GSK3\u3b2) axis plays a central role in MYC-driven lymphomagenesis, and MYC targeting with bromodomain and extraterminal protein family inhibitors (BETi) is a promising treatment strategy in lymphoma. In a high-throughput combinatorial drug screening experiment, BETi enhance the antiproliferative effects of PI3K inhibitors in a panel of diffuse large B cell lymphoma (DLBCL) and Burkitt lymphoma cell lines. BETi or MYC silencing upregulates several PI3K pathway genes and induces GSK3\u3b2 S9 inhibitory phosphorylation, resulting in increased \u3b2-catenin protein abundance. Furthermore, BETi or MYC silencing increases GSK3\u3b2 S9 phosphorylation levels and \u3b2-catenin protein abundance through downregulating the E2 ubiquitin conjugating enzymes UBE2C and UBE2T. In a mouse xenograft DLBCL model, BETi decrease MYC, UBE2C, and UBE2T and increase phospho-GSK3\u3b2 S9 levels, enhancing the anti-proliferative effect of PI3K inhibitors. Our study reveals prosurvival feedbacks induced by BETi involving GSK3\u3b2 regulation, providing a mechanistic rationale for combination strategies. In this study, Derenzini et al. demonstrate that BET inhibitors enhance lymphoma vulnerability to PI3K inhibitors by inducing GSK3\u3b2 feedback in a MYC-dependent manner and by downregulating E2-ubiquitin conjugating enzymes, which further enhance the feedback. These data provide the rationale for combining BET and PI3K inhibitors in lymphoma therapy

    Repeat protein scaffolds: Ordering photo- and electroactive molecules in solution and solid state

    The precise control over the organization of photoactive components at the nanoscale is one of the main challenges for the generation of new and sophisticated macroscopically ordered materials with enhanced properties. In this work we present a novel bioinspired approach using protein-based building blocks for the arrangement of photo- and electroactive porphyrin derivatives. We used a designed repeat protein scaffold with demonstrated unique features that allow for the control of their structure, functionality, and assembly. Our designed domains act as exact biomolecular templates to organize porphyrin molecules at the required distance. The hybrid conjugates retain the structure and assembly properties of the protein scaffold and display the spectroscopic features of orderly aggregated porphyrins along the protein structure.     Sex- and age-specific trends in mortality from suicide and undetermined death in Germany 1991–2002

    BACKGROUND: Over the last decade, significant downward linear time trends in suicide mortality were observed in most Western countries. To date, it is not established whether those favourable time trends developed homogeneously for sex and age groups and how they were affected by the number of undetermined deaths. METHODS: Data on suicide mortality and undetermined death from 1991 to 2002 in Germany were obtained from the German Federal Statistical Office. For each year, the age-standardised suicide rate (SR), undetermined death rate (UDR) and total rate (SR+UDR) was calculated by direct standardisation separately for men and women. Time trends were analyzed by Poisson regression estimating the average annual percentage change (AAPC) of the rates for sex and four age groups (15–24, 25–44, 45–74, ≥ 75 years). RESULTS: A significant decline of the SR was observed in all age groups but was less pronounced among the younger ages, particularly among men aged 15–24 years (AAPC -0.7%, p = 0.041). The SR in the oldest male age group (≥ 75 years) declined much stronger (AAPC -3.5%, p < 0.001). In women, the AAPC of the SR ranged from -1.7% to -4.6%. The average annual percentage changes in the age groups 25 – 74 years did not differ substantially for SR and SR+UDR. In contrast, due to an increase of undetermined deaths for subjects ≥ 75 years, time trends in this age group were affected by the number of undetermined deaths, especially in women. CONCLUSION: Observing downward trends in suicide mortality with lower declines for younger subjects, prevention strategies should focus in particular on younger subjects

    Patterns of adherence to and compliance with the Portuguese smoke-free law in the leisure-hospitality sector

    CIEC – Research Centre on Child Studies, UM (FCT R&D 317)Background: In 2008, the Portuguese smoke-free law came into effect including partial bans in the leisure-hospitality (LH) sector. The objective of the study is to assess the prevalence of smoking control policies (total ban, smoking permission and designated smoking areas) adopted by the LH sector in Portugal. The levels of noncompliance with each policy are investigated as well as the main factors associated with smoking permission and noncompliance with the law. Methods: Cross-sectional study conducted between January 2010 and May 2011. A random sample of venues was selected from the Portuguese LH sector database, proportionally stratified according to type, size and geographical area. All venues were assessed in loco by an observer. The independent effects of venues’ characteristics on smoking permission and the level of noncompliance with the law were explored using logistic regression. Results: Overall, 1.412 venues were included. Total ban policy was adopted by 75.9% of venues, while 8.4% had designated smoking areas. Smoking ban was more prevalent in restaurants (85.9%). Only 29.7% of discos/bars/pubs opted for complete ban. Full or partial smoking permission was higher in discos/bar/pubs (OR = 7.37; 95%CI 4.87 to 11.17). Noncompliance with the law was higher in venues allowing smoking and lower in places with complete ban (33.6% and 7.6% respectively, p, 0.001). Discos/bars/pubs with full smoking permission had the highest level of noncompliance (OR = 3.31; 95%CI 1.40 to 7.83). Conclusions: Our findings show a high adherence to smoking ban policy by the Portuguese LH sector. Nonetheless, one quarter of the venues is fully or partially permissive towards smoking, with the discos/bars/pubs considerably contributing to this situation. Venues with smoking permission policies were less compliant with the legislation.
