    Nivel de conocimiento de los padres sobre salud bucal de preescolares de una institución educativa pública de Lima, 2022

    El objetivo principal del presente estudio es determinar el nivel de conocimiento de los padres sobre salud bucal de preescolares de una institución educativa pública de Lima, 2022. El tipo de investigación es básica y diseño no experimental, transversal y descriptivo; se aplicó una encuesta a 132 padres, para evaluar su nivel de conocimiento en salud bucal de preescolares. Los resultados hallaron que el 47.7% de los padres obtuvieron un nivel de conocimiento bueno en salud bucal; según sexo, fue bueno y predominó en masculino con 55.6% y en femenino con 47.2% respectivamente; según grupo etario, obtuvieron un nivel bueno y predominó en adulto maduro con 31.8% y en adulto joven con 15.9% respectivamente; según grado de instrucción, en secundaria fue bueno con 40.2%, mientras que en superior predominó un nivel muy bueno con 9.1%; según el conocimiento de los padres, la mayoría conoce sobre la dieta saludable preventiva de caries con 98.5%, mientras que desconoce sobre definición de placa bacteriana en 81.1%.     Inflammatory Myofibroblastic Tumor in the Adrenal Gland: A Case Report

    Inflammatory myofibroblastic tumors (IMTs) were first described by Harold Brunn in 1939. IMTs are mainly found in the lungs and other sites of the body; hence, its occurrence in the adrenal gland is exceptional. In the literature, less than 10 cases of IMTs in the adrenal gland have been reported. The etiology of IMT remains unknown, with post-inflammatory changes and a neoplastic origin being proposed. We present a case of a 19-year-old woman and adrenal gland IMT. The patient presented with abdominal pain and low cardiac output without hypovolemic shock. Computed tomography revealed a tumor in the adrenal gland measuring 11.4 cm with extravasation of contrast medium within the tumor. Treatment included conservative management with selective embolization due to minimal invasion of the inferior artery of the adrenal gland. The patient was then discharged with possibility of future elective surgery. Four months later, the size of the tumor decreased to 6.3 cm, and her Eastern Cooperative Oncology Group physical status was 0. The Multidisciplinary Tumor Board suggested surgical management. The final histopathology report was compatible with an IMT of the adrenal gland, with the immunohistochemical report showing positivity for anti-actin muscle-specific and anti-actin smooth muscle and negativity for anaplastic lymphoma kinase. IMTs of the adrenal gland may be treated electively through multidisciplinary management together with interventional radiology and surgery, achieving a favorable outcome for the patient

    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    Biochar research activities and their relation to development and environmental quality. A meta-analysis

    Biochar is the solid product that results from pyrolysis of organic materials. Its addition to highly weathered soils changes physico-chemical soil properties, improves soil functions and enhances crop yields. Highly weathered soils are typical of humid tropics where agricultural productivity is low and needs to be raised to reduce human hunger and poverty. However, impact of biochar research on scientists, politicians and end-users in poor tropical countries remains unknown; assessing needs and interests on biochar is essential to develop reliable knowledge transfer/translation mechanisms. The aim of this publication is to present results of a meta-analysis conducted to (1) survey global biochar research published between 2010 and 2014 to assess its relation to human development and environmental quality, and (2) deduce, based on the results of this analysis, priorities required to assess and promote the role of biochar in the development of adapted and sustainable agronomic methods. Our main findings reveal for the very first time that: (1) biochar research associated with less developed countries focused on biochar production technologies (26.5 ± 0.7%), then on biochars’ impact on chemical soil properties (18.7 ± 1.2%), and on plant productivity (17.1 ± 2.6%); (2) China dominated biochar research activities among the medium developed countries focusing on biochar production technologies (26.8 ± 0.5%) and on use of biochar as sorbent for organic and inorganic compounds (29.1 ± 0.4%); and (3) the majority of biochar research (69.0±2.9%) was associated with highly developed countries that are able to address a higher diversity of questions. Evidently, less developed countries are eager to improve soil fertility and agricultural productivity, which requires transfer and/or translation of biochar knowledge acquired in highly developed countries. Yet, improving local research capacities and encouraging synergies across scientific disciplines and countries are crucial to foster development of sustainable agronomy in less developed countries

    In vitro antioxidant and in vivo hypoglycemic activity of biophenols and polyunsaturated fatty acids from Vitis vinifera L. muscat and quebranta seeds from the Valley of Ica-Peru

    Currently, there is a greater interest in using natural products in various chronic diseases such as type 2 diabetes. However, these investigations have not considered the components of grape seeds. In this context, the current study explored the in vitro antioxidant and in vivo hypoglycemic activity of biophenols and total polyunsaturated fatty acids (PUFA) from Vitis vinifera L. muscat and quebranta seeds from the Ica Valley, Peru. The total polyphenol content (TPC) of muscat (1.57±0.015 mg GAE/g) and quebranta (1.43±0.015 mg GAE/g) seeds was estimated by the Folin-Ciocalteu method. In vitro antioxidant activity was determined by DPPH• free radical assay for muscat and quebranta (IC50: 38.60±0.624 µg/mL and 42.83±0.306 µg/mL, respectively) and by FRAP 0.79±0.030 μg TEAC/g for muscat and 0.61±0.038 μg TEAC/g for quebranta. After inducing experimental hyperglycemia with alloxane in Rattus norvegicus strain Holtzman, treatment was carried out for 7 days and glucose levels were measured at 1, 2 and 4 hours. At a dose of 500 mg/kg, orally, of biophenols/PUFA from muscat and quebranta seeds, a hypoglycemic effect was observed; whose results were verified with the Shapiro-Wilk test (p-value > α = 0.05), Tukey’s multiple comparisons test (p-value 0.0001 α = 0.05 for 2 and 4 hours on day 7, indicates a small probability of difference; in ANOVA results the mean difference is significant (p-value 0.0001 < α 0.05). The Pearson analysis found a strong correlation [0.50 ≤ (0.9530–0.9827) < 1.0] between glibenclamide/biophenols-PUFA glucose levels. Current data show an in vitro antioxidant effect and hypoglycemic activity of the seeds of grapes of the muscat and quebranta varieties. Graphical abstrac

    Pharmacogenetic variants of CYP2C9 and CYP2C19 associated with adverse reactions induced by antiepileptic drugs used in Peru

    Epilepsy is the most common neurological disorder with a worldwide incidence of 20% and a treatment failure rate of 25–30%. The fluctuation in serum levels, efficacy and safety of antiepileptic drugs can be attributed to single nucleotide polymorphisms of genes encoding their respective proteins involved in drug metabolism. The present study attempted to evaluate the pharmacogenetic variants of CYP2C9 and CYP2C19 associated with adverse reactions induced by antiepileptic drugs used in Peru. Few studies were found to significantly associate the CYP2C9*2, CYP2C9*3, CYP2C19*2, and CYP2C19*3 single nucleotide polymorphisms with elevated serum levels of valproic acid and carbamazepine, and valproic acid induction of hyperammonemia, and adverse reactions cutaneous for carbamazepine. There is further evidence of a significant association of CYP2C9*2/CYP2C9*3 with severe cutaneous adverse reactions (SCARs) such as Stevens-Johnson syndrome (SJS) and epidermal necrolysis (TEN) phenytoin-induced. CYP2C9*3 may be a pharmacogenetic biomarker for such a drug. It is proposed to reduce the dose of drugs for intermediate and poor metabolizers. No pharmacogenetic studies were found in patients with epilepsy in Peruvian populations. It is concluded that this review could help physicians in the prediction and prevention of adverse reactions induced by antiepileptic drugs, as well as to improve their pharmacotherapeutic results. It could also be used as scientific evidence to carry out pharmacogenetic and precision medicine studies in Peruvian patients with epilepsy, due to their tricontinental and Latin American ancestry
