Earliest signs and management of leakage after bariatric surgeries: Single institute experience
Background: The aim of this study was to describe the clinical presentation and outcomes of treatment in patients who develop gastrointestinal leaks after different bariatric surgeries.Methods: Retrospective review of 632 consecutive bariatric surgical procedures performed from 1999–2009 in Alexandria University Hospital, Egypt.Results: Leakage occurred in 10 patients. Symptoms and signs included tachycardia, fever, tachypnea, left shoulder pain, abdominal pain, chest pain, and/or change in the nature of the drain effluent. The earliest signs of presentation were tachycardia and unilateral decrease in air entry in all patients. The average time to diagnosis was 3.9± 2.6 days. In four patients contrast study was negative (40%). Six leaks occurred after laparoscopic sleeve gastrectomy (6.3%), 2 after laparoscopic gastric bypass (3.6%), one after open gastric bypass (2.3%), and 1 after laparoscopic vertical banded gastroplasty (2.4%). The most common leak location was at the esophagogastric junction (70%). Four patients (40%) required reoperations. A percutaneous abdominal drainage was placed in five patients (50%). In 2 patients (20%), the prophylactic drain was maintained in situ till cessation of leakage. Two patients (20%) died. Mean hospital length of stay was 13.9 ± 7.8 days.Conclusions: Tachycardia and unequal breath sound in the early postoperative course are worrisome signs that warrant laparoscopic exploration even if contrast studies were negative. Patients with signs of sepsis or hemodynamic instability require emergent exploration. Leaks that are more insidious may be treated successfully with percutaneous drainage or maintenance of prophylactic drains.Keywords: Morbid obesity; Bariatric surgery; Complication; Lea

The Structure and Psychometric Properties of Wellness for A Sample of Teachers in Egypt
The present study attempts to examine the structure of wellness for a sample of teachers in Egypt and examine psychometric properties of wellness Inventory. The study questions addressed are: is the structure of wellness multidimensional or uni-dimensional?, Is wellness inventory has appropriate psychometric properties?. A total of 417 teachers from Egypt completed the inventory of wellness, including males (N= 260) and females (N= 157).The teachers ranged in age from 29 to 48 with an average age 36.7 and S.D. 1.7 . The inventory that used in this study is referred to as the multidimensional organizational wellness inventory (MOWI) (Fasone, 2017), Confirmatory factor analysis (CFA) is used. The results indicates that The structure of wellness is multidimensional which includes physical, emotional, social, intellectual, spiritual, occupational , and environmental. Unrealistic Optimism Among a Sample of University Students in Egypt and Saudi Arabia (Cross-cultural Study)
The present study considered as cross-cultural study attempts to examine the psychometric properties of unrealistic optimism in light of the scale used in the study for Egyptian and Saudi Arabia university students and Is the measurement equivalent of the scale terms for Egyptian and Saudi Arabia university students? . A total of (336) male students from university in Egypt and in Saudi Arabia from different specialties completed the scale of unrealistic optimism (prepared by the researchers), Confirmatory factor analysis (CFA) is used. The results indicate that the structure of model of unrealistic optimism is fit and the scale has appropriate psychometric properties. And the result indicate that the unrealistic optimism scale have the same structure in Egypt and Saudi Arabia , that means the factorial invariance of unrealistic optimism in Egypt and Saudi Arabia . Identification and genetic characterization of a gibberellin 2-oxidase gene that controls tree stature and reproductive growth in plum
Several dwarf plum genotypes (Prunus salicina L.), due to deficiency of unknown gibberellin (GA) signalling, were identified. A cDNA encoding GA 2-oxidase (PslGA2ox), the major gibberellin catabolic enzyme in plants, was cloned and used to screen the GA-deficient hybrids. This resulted in the identification of a dwarf plum hybrid, designated as DGO24, that exhibits a markedly elevated PslGA2ox signal. Grafting ‘Early Golden’ (EG), a commercial plum cultivar, on DGO24 (EG/D) enhanced PslGA2ox accumulation in the scion part and generated trees of compact stature. Assessment of active GAs in such trees revealed that DGO24 and EG/D accumulated relatively much lower quantities of main bioactive GAs (GA1 and GA4) than control trees (EG/M). Moreover, the physiological function of PslGA2ox was studied by determining the molecular and developmental consequences due to ectopic expression in Arabidopsis. Among several lines, two groups of homozygous transgenics that exhibited contrasting phenotypes were identified. Group-1 displayed a dwarf growth pattern typical of mutants with a GA deficiency including smaller leaves, shorter stems, and delay in the development of reproductive events. In contrast, Group-2 exhibited a ‘GA overdose’ phenotype as all the plants showed elongated growth, a typical response to GA application, even under limited GA conditions, potentially due to co-suppression of closely related Arabidopsis homologous. GNP-GAPDH1-22 nanovaccines prevent neonatal listeriosis by blocking microglial apoptosis and bacterial dissemination
Clinical cases of neonatal listeriosis are associated with brain disease and fetal loss due to complications in early or late pregnancy, which suggests that microglial function is altered. This is believed to be the first study to link microglial apoptosis with neonatal listeriosis and listeriosis-associated brain disease, and to propose a new nanovaccine formulation that reverses all effects of listeriosis and confers Listeria monocytogenes (LM)-specific immunity. We examined clinical cases of neonatal listeriosis in 2013-2015 and defined two useful prognostic immune biomarkers to design listeriosis vaccines: high anti-GAPDH1-22 titres and tumor necrosis factor (TNF)/interleukin (IL)-6 ratios. Therefore, we developed a nanovaccine with gold glyco-nanoparticles conjugated to LM peptide 1-22 of GAPDH (Lmo2459), GNP-GAPDH1-22 nanovaccinesformulated with a pro-inflammatory Toll-like receptor 2/4-targeted adjuvant. Neonates born to non-vaccinated pregnant mice with listeriosis, showed brain and vascular diseases and significant microglial dysfunction by induction of TNF-?-mediated apoptosis. This programmed TNF-mediated suicide explains LM dissemination in brains and livers and blocks production of early pro-inflammatory cytokines such as IL-1? and interferon-?/?. In contrast, neonates born to GNP-GAPDH1-22-vaccinated mothers before LM infection, did not develop listeriosis or brain diseases and had functional microglia. In nanovaccinated mothers, immune responses shifted towards Th1/IL-12 pro-inflammatory cytokine profiles and high production of anti-GAPDH1-22 antibodies, suggesting good induction of LM-specific memory
Complementary regulation of four Eucalyptus CBF genes under various cold conditions
CBF transcription factors play central roles in the control of freezing tolerance in plants. The isolation of two additional CBF genes, EguCBF1c and EguCBF1d, from E. gunnii, one of the cold-hardiest Eucalyptus species, is described. While the EguCBF1D protein sequence is very similar to the previously characterized EguCBF1A and EguCBF1B sequences, EguCBF1C is more distinctive, in particular in the AP2-DBD (AP2-DNA binding domain). The expression analysis of the four genes by RT-qPCR reveals that none of them is specific to one stress but they are all preferentially induced by cold, except for the EguCBF1c gene which is more responsive to salt. The calculation of the transcript copy number enables the quantification of constitutive CBF gene expression. This basal level, significant for the four genes, greatly influences the final EguCBF1 transcript level in the cold. A cold shock at 4 °C, as well as a progressive freezing which mimics a natural frost episode, trigger a fast and strong response of the EguCBF1 genes, while growth at acclimating temperatures results in a lower but more durable induction. The differential expression of the four EguCBF1 genes under these cold regimes suggests that there is a complementary regulation. The high accumulation of the CBF transcript, observed in response to the different types of cold conditions, might be a key for the winter survival of this evergreen broad-leaved tree
