    Characteristics of Belgian 'life-ending acts without explicit patient request' : a large-scale death certificate survey revisited

    BACKGROUND: “Life-ending acts without explicit patient request,” as identified in robust international studies, are central in current debates on physician-assisted dying. Despite their contentiousness, little attention has been paid to their actual characteristics and to what extent they truly represent nonvoluntary termination of life. METHODS: We analyzed the 66 cases of life-ending acts without explicit patient request identified in a large-scale survey of physicians certifying a representative sample of deaths (n = 6927) in Flanders, Belgium, in 2007. The characteristics we studied included physicians’ labelling of the act, treatment course and doses used, and patient involvement in the decision. RESULTS: In most cases (87.9%), physicians labelled their acts in terms of symptom treatment rather than in terms of ending life. By comparing drug combinations and doses of opioids used, we found that the life-ending acts were similar to intensified pain and symptom treatment and were distinct from euthanasia. In 45 cases, there was at least 1 characteristic inconsistent with the common understanding of the practice: either patients had previously expressed a wish for ending life (16/66, 24.4%), physicians reported that the administered doses had not been higher than necessary to relieve suffering (22/66, 33.3%), or both (7/66, 10.6%). INTERPRETATION: Most of the cases we studied did not fit the label of “nonvoluntary life-ending” for at least 1 of the following reasons: the drugs were administered with a focus on symptom control; a hastened death was highly unlikely; or the act was taken in accordance with the patient’s previously expressed wishes. Thus, we recommend a more nuanced view of life-ending acts without explicit patient request in the debate on physician-assisted dying

    Adaptive neural reward processing during anticipation and receipt of monetary rewards in mindfulness meditators

    Reward seeking is ubiquitous and adaptive in humans. But excessive reward seeking behavior, such as chasing monetary rewards, may lead to diminished subjective well-being. This study examined whether individuals trained in mindfulness meditation show neural evidence of lower susceptibility to monetary rewards. Seventy-eight participants (34 meditators, 44 matched controls) completed the monetary incentive delay task while undergoing functional magnetic resonance imaging. The groups performed equally on the task, but meditators showed lower neural activations in the caudate nucleus during reward anticipation, and elevated bilateral posterior insula activation during reward anticipation. Meditators also evidenced reduced activations in the ventromedial prefrontal cortex during reward receipt compared with controls. Connectivity parameters between the right caudate and bilateral anterior insula were attenuated in meditators during incentive anticipation. In summary, brain regions involved in reward processingboth during reward anticipation and receipt of rewardresponded differently in mindfulness meditators than in nonmeditators, indicating that the former are less susceptible to monetary incentives

    The role of the right temporoparietal junction in perceptual conflict: detection or resolution?

    The right temporoparietal junction (rTPJ) is a polysensory cortical area that plays a key role in perception and awareness. Neuroimaging evidence shows activation of rTPJ in intersensory and sensorimotor conflict situations, but it remains unclear whether this activity reflects detection or resolution of such conflicts. To address this question, we manipulated the relationship between touch and vision using the so-called mirror-box illusion. Participants' hands lay on either side of a mirror, which occluded their left hand and reflected their right hand, but created the illusion that they were looking directly at their left hand. The experimenter simultaneously touched either the middle (D3) or the ring finger (D4) of each hand. Participants judged, which finger was touched on their occluded left hand. The visual stimulus corresponding to the touch on the right hand was therefore either congruent (same finger as touch) or incongruent (different finger from touch) with the task-relevant touch on the left hand. Single-pulse transcranial magnetic stimulation (TMS) was delivered to the rTPJ immediately after touch. Accuracy in localizing the left touch was worse for D4 than for D3, particularly when visual stimulation was incongruent. However, following TMS, accuracy improved selectively for D4 in incongruent trials, suggesting that the effects of the conflicting visual information were reduced. These findings suggest a role of rTPJ in detecting, rather than resolving, intersensory conflict

    Bipolar ablation for deep intra-myocardial circuits: human ex vivo development and in vivo experience.

However, the critical components of reentrant VT circuits may be located at deep intramural sites. We hypothesized that bipolar ablations would create deeper lesions than unipolar ablation in human hearts.Ablation was performed on nine explanted human hearts at the time of transplantation. Following explant, the hearts were perfused by using a Langendorff perfusion setup. For bipolar ablation, the endocardial catheter was connected to the generator as the active electrode and the epicardial catheter as the return electrode. Unipolar ablation was performed at 50 W with irrigation of 25 mL/min, with temperature limit of 50°C. Bipolar ablation was performed with the same settings. Subsequently, in a patient with an incessant septal VT, catheters were positioned on the septum from both the ventricles and radiofrequency was delivered with 40 W. In the explanted hearts, there were a total of nine unipolar ablations and four bipolar ablations. The lesion depth was greater with bipolar ablation, 14.8 vs. 6.1 mm (P < 0.01), but the width was not different (9.8 vs. 7.8 mm). All bipolar lesions achieved transmurality in contrast to the unipolar ablations. In the patient with a septal focus, bipolar ablation resulted in termination of VT with no inducible VTs.By using a bipolar ablation technique, we have demonstrated the creation of significantly deeper lesions without increasing the lesion width, compared with standard ablation. Further clinical trials are warranted to detail the risks of this technique

    ERPs and their brain sources in perceptual and conceptual prospective memory tasks: commonalities and differences between the two tasks

    The present study examined whether Event-Related Potential (ERP) components and their neural generators are common to perceptual and conceptual prospective memory (PM) tasks or specific to the form of PM cue involved. We used Independent Component Analysis (ICA) to study the contributions of brain source activities to scalp ERPs across the different phases of two event-based PM-tasks: (1) holding intentions during a delay (monitoring) (2) detecting the correct context to perform the delayed intention (cue detection) and (3) carrying out the action (realisation of delayed intentions). Results showed that monitoring for both perceptual and conceptual PM-tasks was characterised by an enhanced early occipital negativity (N200). In addition the conceptual PM-task showed a long-lasting effect of monitoring significant around 700 ms. Perceptual PM-task cues elicited an N300 enhancement associated with cue detection, whereas a midline N400-like response was evoked by conceptual PM-task cues. The Prospective Positivity associated with realisation of delayed intentions was observed in both conceptual and perceptual tasks. A common frontal-midline brain source contributed to the Prospective Positivity in both tasks and a strong contribution from parieto-frontal brain sources was observed only for the perceptually cued PM-task. These findings support the idea that: (1) The enhanced N200 can be understood as a neural correlate of a ‘retrieval mode’ for perceptual and conceptual PM-tasks, and additional strategic monitoring is implemented according the nature of the PM task; (2) ERPs associated with cue detection are specific to the nature of the PM cues; (3) Prospective Positivity reflects a general PM process, but the specific brain sources contributing to it depend upon the nature of the PM task

    International audienceDetermining liquid-vapor phase equilibrium is often required in multiphase flow computations. Existing equilibrium solvers are either accurate but computationally expensive, or cheap but inaccurate. The present paper aims at building a fast and accurate specific phase equilibrium solver, specifically devoted to unsteady multiphase flow computations. Moreover, the solver is efficient at phase diagram bounds, where non-equilibrium pure liquid and pure gas are present. It is systematically validated against solutions based on an accurate (but expensive) solver. Its capability to deal with cavitating, evaporating and condensing two-phase flows is highlighted on severe test problems both 1D and 2D

    Exceptionally monotone models : the rank correlation model class for exceptional model mining

    Exceptional Model Mining strives to find coherent subgroups of the dataset where multiple target attributes interact in an unusual way. One instance of such an investigated form of interaction is Pearson's correlation coefficient between two targets. EMM then finds subgroups with an exceptionally linear relation between the targets. In this paper, we enrich the EMM toolbox by developing the more general rank correlation model class. We find subgroups with an exceptionally monotone relation between the targets. Apart from catering for this richer set of relations, the rank correlation model class does not necessarily require the assumption of target normality, which is implicitly invoked in the Pearson's correlation model class. Furthermore, it is less sensitive to outliers

    Complex patterns of spontaneous initiations and terminations of reentrant circulation in a loop of cardiac tissue

    A two-component model is developed that consists of a discrete loop of cardiac cells that circulates action potentials together with a cardiac pacing mechanism. Physiological properties of cells such as restitutions of refractoriness and of conduction velocity are given via experimentally measured functions. The dynamics of circulating pulses and their interactions with the pacer are regulated by two threshold relations. Patterns of spontaneous initiations and terminations of reentry (SITR) generated by this system are studied through numerical simulations and analytical observations.
