Herbal extracts modulate the amplitude and frequency of slow waves in circular smooth muscle of mouse small intestine
Background: Herbal preparations like STW 5 (Iberogast(R)) are widely used drugs in the treatment of dyspepsia and motility-related disorders of the gastrointestinal tract. STW 5 is a phytotherapeutic agent consisting of a fixed mixture of 9 individual plant extracts. The electrophysiological mechanisms of action of STW 5 remain obscure. Aim: The aim of the present study was to investigate whether herbal extracts influence electrophysiological parameters of the small intestine. For this purpose, the resting membrane potential (RMP) and the slow wave rhythmicity of smooth muscle cells of mouse small intestine were observed. Methods: Intracellular recordings of smooth muscle cells of the circular muscle layer of mouse small intestine were performed using standard microelectrode techniques. After dissection of the mucosa, the small intestine was placed in an organ bath and a microelectrode was applied on a circular smooth muscle cell. The RMP and the amplitude of slow waves were measured in millivolts. Results: The RMP of smooth muscle cells was - 59 +/- 1.3 mV. This RMP was significantly depolarized by STW 5 ( 9.6 +/- 1.6 mV); the depolarizing effects can be mainly attributed to the constituents of matricariae flos, angelicae radix and chelidonii herba. The basal frequency of small intestinal slow waves was 39.5 +/- 1.4 min(-1) and the amplitude was 23.1 +/- 0.9 mV. STW 5 significantly reduced the amplitude and frequency of the slow waves ( 11.7 +/- 0.8 mV; 33.5 +/- 3.4 min(-1)). This effect on slow waves represents the sum of the effects of the 9 phytoextracts. Whereas angelicae radix and matricariae flos completely blocked slow wave activity, Iberis amara increased the frequency and amplitude, chelidonii herba reduced the frequency and amplitude of the slow waves, mentae piperitae folium reduced the frequency and left amplitude unchanged and liquiritae radix, carvi fructus and melissae folium had no effects. Conclusion: Herbal extracts cause changes in smooth muscle RMP and slow wave rhythmicity, up to reversible abolition, by blockade of large conductance Ca2+ channels and other not yet identified mechanisms. In herbal preparations like STW 5 these effects add up to a total effect and this study indicates that herbal preparations which are widely used in dyspepsia and motility-related disorders have characteristic, reproducible, reversible effects on small intestinal electrophysiology. Implementation research for the prevention of antimicrobial resistance and healthcare-associated infections; 2017 Geneva infection prevention and control (IPC)-think tank (part 1)
Background
Around 5–15% of all hospital patients worldwide suffer from healthcare-associated infections (HAIs), and years of excessive antimicrobial use in human and animal medicine have created emerging antimicrobial resistance (AMR). A considerable amount of evidence-based measures have been published to address these challenges, but the largest challenge seems to be their implementation.
Methods
In June 2017, a total of 42 experts convened at the Geneva IPC-Think Tank to discuss four domains in implementation science: 1) teaching implementation skills; 2) fostering implementation of IPC and antimicrobial stewardship (AMS) by policy making; 3) national/international actions to foster implementation skills; and 4) translational research bridging social sciences and clinical research in infection prevention and control (IPC) and AMR.
Results
Although neglected in the past, implementation skills have become a priority in IPC and AMS. They should now be part of any curriculum in health care, and IPC career paths should be created. Guidelines and policies should be aligned with each other and evidence-based, each document providing a section on implementing elements of IPC and AMS in patient care. International organisations should be advocates for IPC and AMS, framing them as patient safety issues and emphasizing the importance of implementation skills. Healthcare authorities at the national level should adopt a similar approach and provide legal frameworks, guidelines, and resources to allow better implementation of patient safety measures in IPC and AMS. Rather than repeating effectiveness studies in every setting, we should invest in methods to improve the implementation of evidence-based measures in different healthcare contexts. For this, we need to encourage and financially support collaborations between social sciences and clinical IPC research.
Conclusions
Experts of the 2017 Geneva Think Tank on IPC and AMS, CDC, and WHO agreed that sustained efforts on implementation of IPC and AMS strategies are required at international, country, and hospital management levels, to provide an adequate multimodal framework that addresses (not exclusively) leadership, resources, education and training for implementing IPC and AMS. Future strategies can build on this agreement to make strategies on IPC and AMS more effective
The Wow Factor? A Comparative Study of the Development of Student Music Teachers' Talents in Scotland and Australia
For some time there has been debate about differing perspectives on musical gift and musical intelligence. One view is that musical gift is innate: that it is present in certain individuals from birth and that the task of the teacher is to develop the potential which is there. A second view is that musical gift is a complex concept which includes responses from individuals to different environments and communities (Howe and Sloboda, 1997). This then raises the possibility that musical excellence can be taught. We have already explored this idea with practising musicians (Stollery and McPhee, 2002). Our research has now expanded to include music teachers in formation, and, in this paper, we look at the influences in their musical development which have either 'crystallised' or 'paralysed' the musical talent which they possess. Our research has a comparative dimension, being carried out in Scotland and in Australia. We conclude that there are several key influences in the musical development of the individual, including home and community support, school opportunities and teaching styles and that there may be education and culture-specific elements to these influences
Conceptualising organisational cultural lag: Marriage equality and Australian sport
This article develops a theoretical framework to understand how sexuality can be institutionalised through debates about marriage equality. We first examine 13 Australian sporting organisations concerning their support for marriage equality and sexual minority inclusion before showing they drew cultural capital from supporting episodes of equality exogenous to their organisation, while failing to promote internal inclusion. We use online content analysis alongside the identification of institutional speech acts within policy to analyse results through three conceptual lenses: Ahmed’s (2006) institutional diversity work, Ogburn’s (1922) cultural lag, and Evan’s (1966) organisational lag, from which we propose a hybrid, Organisational Cultural Lag, as a theoretical tool within social movement theory
An infrared survey of brightest cluster galaxies: Paper I
We report on an imaging survey with the Spitzer Space Telescope of 62
brightest cluster galaxies with optical line emission. These galaxies are
located in the cores of X-ray luminous clusters selected from the ROSAT All-Sky
Survey. We find that about half of these sources have a sign of excess infrared
emission; 22 objects out of 62 are detected at 70 microns, 18 have 8 to 5.8
micron flux ratios above 1.0 and 28 have 24 to 8 micron flux ratios above 1.0.
Altogether 35 of 62 objects in our survey exhibit at least one of these signs
of infrared excess. Four galaxies with infrared excesses have a 4.5/3.6 micron
flux ratio indicating the presence of hot dust, and/or an unresolved nucleus at
8 microns. Three of these have high measured [OIII](5007A)/Hbeta flux ratios
suggesting that these four, Abell 1068, Abell 2146, and Zwicky 2089, and
R0821+07, host dusty active galactic nuclei (AGNs). 9 objects (including the
four hosting dusty AGNs) have infrared luminosities greater than 10^11 L_sol
and so can be classified as luminous infrared galaxies (LIRGs). Excluding the
four systems hosting dusty AGNs, the excess mid-infrared emission in the
remaining brightest cluster galaxies is likely related to star formation.Comment: accepted for publication in ApJ
Trypanocidal and leishmanicidal activity of six limonoids
Six limonoids [kotschyienone A and B (1, 2), 7-deacetylgedunin (3), 7-deacetyl-7-oxogedunin (4), andirobin (5) and methyl angolensate (6)] were investigated for their trypanocidal and leishmanicidal activities using bloodstream forms of Trypanosoma brucei and promastigotes of Leishmania major. Whereas all compounds showed anti-trypanosomal activity, only compounds 1–4 displayed anti-leishmanial activity. The 50% growth inhibition (GI 50) values for the trypanocidal and leishmanicidal activity of the compounds ranged between 2.5 and 14.9 μM. Kotschyienone A (1) was found to be the most active compound with a minimal inhibition concentration (MIC) value of 10 μM and GI 50 values between 2.5 and 2.9 μM. Only compounds 1 and 3 showed moderate cytotoxicity against HL-60 cells with MIC and GI 50 values of 100 μM and 31.5–46.2 μM, respectively. Compound 1 was also found to show activity against intracellular amastigotes of L. major with a GI 50 value of 1.5 μM. The results suggest that limonoids have potential as drug candidates for the development of new treatments against trypanosomiasis and leishmaniasis
The core fundamental plane of B2 radio galaxies
The photometric, structural and kinematical properties of the centers of
elliptical galaxies, harbor important information of the formation history of
the galaxies. In the case of non active elliptical galaxies these properties
are linked in a way that surface brightness, break radius and velocity
dispersion of the core lie on a fundamental plane similar to that found for
their global properties. We construct the Core Fundamental Plane (CFP) for a
sizeable sample of low redshift radio galaxies and compare it with that of non
radio ellipticals. To pursue this aim we combine data obtained from high
resolution HST images with medium resolution optical spectroscopy to derive the
photometric and kinematic properties of ~40 low redshift radio galaxies. We
find that the CFPs of radio galaxies is indistinguishable from that defined by
non radio elliptical galaxies of similar luminosity. The characteristics of the
CFP of radio galaxies are also consistent (same slope) with those of the
Fundamental Plane (FP) derived from the global properties of radio (and non
radio) elliptical galaxies. The similarity of CFP and FP for radio and non
radio ellipticals suggests that the active phase of these galaxies has minimal

Real world challenges in delivering person centred care: A community based case study
Community nurses face many challenges when trying to practice evidence-based, person-centred care. Ongoing concerns regarding the impact of the 2013 Francis Report (Ford and Lintern, 2017) suggest that individualised and holistic care is an impossible dream, one made harder when the client appears uncooperative. This paper presents a case study that sets out how some of these challenges were met in a potentially difficult situation experienced by a student nurse and her mentor in practice, in which the student was supported to further examine and explore issues that may have influenced the situation. In this instance, the solution came with the recognition that the client had expertise and knowledge that needed to be taken into account, alongside that of the nurses looking after him. His care became a partnership, not an imposition of expertise; a principle which is transferable to many other situations. Underpinning it was the recognition of our shared humanity, wherein lies the essence of truly holistic care, and student nurses learning this, through the guidance and support of their mentor.
