How the vision of a clinician and an educator brought the MA Dental Law and Ethics course to life.
This paper reflects on an educational development that is Dental Law and Ethics course as the course approaches its 5th anniversary. The authors outline their personal journey into developing and delivering this course as well share best practice in relation to teaching and learning dental postgraduate students who may approach the subject in different ways. It also highlights the vision behind this provision and how it is received by dental practitioners. The paper shares the learners’ perception of topics such as ethics in comparison to law, and it highlights the perspective of both authors in teaching and following the students’ journey in this course
Modeling He-rich subdwarfs through the hot-flasher Scenario
We present 1D numerical simulations aimed at studying the hot-flasher
scenario for the formation of He-rich subdwarf stars. Sequences were calculated
for a wide range of metallicities and physical assumptions, such as the stellar
mass at the moment of the helium core flash. This allows us to study the two
previously proposed flavors of the hot-flasher scenario ("deep" and "shallow"
mixing cases) and to identify a third transition type. Our sequences are
calculated by solving simultaneously the mixing and burning equations within a
diffusive convection picture, and in the context of standard mixing length
theory. We are able to follow chemical evolution during deep-mixing events in
which hydrogen is burned violently, and therefore able to present a homogeneous
set of abundances for different metallicities and varieties of hot-flashers. We
extend the scope of our work by analyzing the effects of non-standard
assumptions, such as the effect of chemical gradients, extra-mixing at
convective boundaries, possible reduction in convective velocities, or the
interplay between difussion and mass loss. Particular emphasis is placed on the
predicted surface properties of the models.
We find that the hot-flasher scenario is a viable explanation for the
formation and surface properties of He-sdO stars. Our results also show that,
during the early He-core burning stage, element diffusion may produce the
transformation of (post hot-flasher) He-rich atmospheres into He-deficient
ones. If this is so, then we find that He-sdO stars should be the progenitors

The ubiquitous nature of the Horizontal Branch second U-jump: A link with the Blue Hook scenario?
In a previous paper we reported on a discontinuity in the extreme horizontal
branch (EHB) of the Galactic globular cluster NGC6752, which we called the
second U-jump. This feature was attributed to a combination of post zero-age
horizontal branch evolution and diffusion effects. In this follow-up study we
analyze other EHB clusters and show that the second U-jump is a common feature
among EHB clusters reaching T_{eff}\ge 23,000K, and that its onset in different
clusters converges around T_{eff}\sim 21,000\pm3,000K. We also present
near-ultraviolet diagrams of \omega Cen and NGC2808, the only two objects with
spectroscopically confirmed ``blue hook'' stars (T_{eff}\ge 35,000K). We
confirm predictions of a photometric discontinuity separating late from
early-helium flashers. Moreover, we present empirical evidence that the second
U-jump population might be mainly composed by early-helium flashers. Lastly, we
revisit the discussion on the ubiquitous nature of the gaps and jumps so far
identified in the blue HB tails, suggesting a possible discrete nature of the

Food meanings in HIV and AIDS caregiving trajectories: Ritual, optimism and anguish among caregivers in Lesotho
The article describes the caregiving responsibility to provide food for chronically ill family members and the meanings attached to food and eating when ill created stress for family caregivers. The results come from a qualitative phenomenological study using in-depth interviews with 21 family caregivers of chronically ill HIV and AIDS patients in one district in Lesotho. Analysis of the interview data showed that the caregivers attached profound meanings to food and feeding care recipients. Their perceptions about food as part of family life and caring, the role of food and eating in curbing disease progression, the link between food and medical efficacy and the link between food and life led to ritualised behaviour around food, and moments of optimism and anguish in caregiving. Patients’ behaviour in relation to food was in most instances inconsistent with the caregivers’ goals, thus leading this aspect of caregiving to induce stress. Services intended to support home-based caregivers and patients could contribute to the reduction of stress associated with food through suitably tailored food assistance and professional support to caregivers to enhance their competences and understanding of the dynamics of food intake as AIDS progressed
Observations of the pulsating subdwarf B star Feige 48: Constraints on evolution and companions
Since pulsating subdwarf B (sdBV or EC14026) stars were first discovered
(Kilkenny et al, 1997), observational efforts have tried to realize their
potential for constraining the interior physics of extreme horizontal branch
(EHB) stars. Difficulties encountered along the way include uncertain mode
identifications and a lack of stable pulsation mode properties. Here we report
on Feige 48, an sdBV star for which follow-up observations have been obtained
spanning more than four years, which shows some stable pulsation modes.
We resolve the temporal spectrum into five stable pulsation periods in the
range 340 to 380 seconds with amplitudes less than 1%, and two additional
periods that appear in one dataset each. The three largest amplitude
periodicities are nearly equally spaced, and we explore the consequences of
identifying them as a rotationally split l=1 triplet by consulting with a
representative stellar model.
The general stability of the pulsation amplitudes and phases allows us to use
the pulsation phases to constrain the timescale of evolution for this sdBV
star. Additionally, we are able to place interesting limits on any stellar or

Why Hot Horizontal Branch Stars Can Appear Redder Than Red Giants
In this paper we report on a curious feature in the V, (U-B) color-magnitude
diagrams of globular clusters. In our database, we find that a considerable
fraction of blue horizontal branch stars, hotter than the instability strip and
cooler than the Grundahl et al. (1999) jump (i.e., 6000 <~ T_eff K <~ 10000),
have (U-B) colors redder than their red giant progenitors. This red incursion
is not expected on theoretical grounds, as horizontal branch stars (whose
convective regions are less extended than in red giant structures) should not
``appear'' cooler than a red giant. Analyzing data from different telescopes we
show that: 1) the horizontal branch red incursion is strongly dependent on the
shape of the adopted U filter and to a lesser extent, on the B filter; 2) the
photometry done with U filters which do not encompass the Balmer jump, do not
show the blue horizontal branch red incursion; 3) the occurrence of this
feature is also due to the peculiar dependence of the U and B magnitudes on
stars effective temperature, gravity, and metallicity; 4) theoretical tracks
can reproduce the observed horizontal branch morphology, provided that the
appropriate (i.e. exactly responding to the filters effectively used in the
observations) transmission curve efficiencies are used for deriving color-T_eff

Altered thymic differentiation and modulation of arthritis by invariant NKT cells expressing mutant ZAP70
Various subsets of invariant natural killer T (iNKT) cells with different cytokine productions develop in the mouse thymus, but the factors driving their differentiation remain unclear. Here we show that hypomorphic alleles of Zap70 or chemical inhibition of Zap70 catalysis leads to an increase of IFN-gamma-producing iNKT cells (NKT1 cells), suggesting that NKT1 cells may require a lower TCR signal threshold. Zap70 mutant mice develop IL-17-dependent arthritis. In a mouse experimental arthritis model, NKT17 cells are increased as the disease progresses, while NKT1 numbers negatively correlates with disease severity, with this protective effect of NKT1 linked to their IFN-gamma expression. NKT1 cells are also present in the synovial fluid of arthritis patients. Our data therefore suggest that TCR signal strength during thymic differentiation may influence not only IFN-gamma production, but also the protective function of iNKT cells in arthritis
Long Sun-Exposures Influencing High Sub-Cutaneous Synthesis of Vitamin-D3 may be Associated with Exacerbation of Symptoms in Allergic-Asthma.
Does excessive sun-exposure, non-use of sunscreen and/or high doses of vitamin-D3 supplements provoke exacerbation of asthma? Clinical examinations, retrospective records-access and questionnaire surveys were distributed to a convenience sample of allergic-asthma patient (n=183). Patients (19-89 years) attending the outpatient respiratory clinics at Maidstone Hospital were enrolled. 90.3% of patients (total IgE levels ≥75 kU/L ; n=103) exposed to direct sunlight of ≥ 15 minutes per day continuously for 6-7 days presented with wheeze (χ2(1) = 7.46; p< 0.05) compared to only 9.7% patients of similar atopy-status, presenting with wheeze if exposed to sunlight of < 15 minutes per day for 6-7 days. 68.9% patients (with IgE levels ≥ 75 kU/L ; n=103), non-users of sunscreen (SPF 30 and above), exposed to direct sunlight of ≥ 15 minutes per day continuously for 6-7 days developed a wheeze, compared to fewer users of sunscreen (9.7%, n=103), exposed to the same duration of sunlight who developed asthma symptoms (p< 0.05). Vitamin-D3 supplementation in asthma-patients with clinical signs of hypovitaminosis-D (n=21), produced symptoms of morning chest-tightness (76.2%), allergic rhinitis (61.9%) and wheeze (100%), 2 weeks after initiation of treatment. Our results advocate direct sunlight exposure < 15 minutes per day and use of sunscreen as a novel approach to preventing atopic-asthma symptoms in allergic-asthma patients.. Activated vitamin-D3 is well-recognised to shift the immune-balance towards Th2 predominance, favouring allergic asthma. These results suggest that limiting subcutaneous synthesis of vitamin-D3 in asthma patients and re-addressing dosage of vitamin-D3 supplementation is necessary may contribute to prevent exacerbation of symptoms
Pregnancy-induced changes in cell-fate in the mammary gland
The protective effect of an early full-term pregnancy is a well established phenomenon; in contrast, the molecular and cell-specific mechanisms that govern parity-specific changes in the mammary gland have not been well described. Recent studies signify a dramatic advance in our understanding of this phenomenon, and indicate a 'cell fate' model for parity-related changes that lead to protection against breast cancer
Pregnancy in the mature adult mouse does not alter the proportion of mammary epithelial stem/progenitor cells
Introduction
In humans, an early full-term pregnancy reduces lifetime breast cancer risk by up to 50% whereas a later pregnancy (>35 years old) can increase lifetime risk. Several mechanisms have been suggested, including changes in levels of circulating hormones, changes in the way the breast responds to these hormones, changes in gene expression programmes which may alter susceptibility to transformation and changes to mammary stem cell numbers or behaviour. Previous studies have shown that the mammary tissue isolated from both virgin and parous mice has the ability to repopulate a cleared mammary fat pad in transplant experiments. Limited dilution transplant assays have demonstrated that early pregnancy (at 5 weeks of age) reduces stem/progenitor cell numbers in the mouse mammary epithelium by twofold. However, the effects on stem/progenitor cell numbers in the mammary epithelium of a pregnancy in older animals have not yet been tested.
Methods
Mice were put through a full-term pregnancy at 9 weeks of age, when the mammary epithelium is mature. The total mammary epithelium was purified from parous 7-week post-lactation and age-matched virgin mice and analysed by flow cytometry and limiting dilution cleared fat pad transplants.
Results
There were no significant differences in the proportions of different mammary epithelial cell populations or numbers of CD24+/Low Sca-1- CD49fHigh cells (stem cell enriched basal mammary epithelial compartment). There was no significant difference in stem/progenitor cell frequency based on limiting dilution transplants between the parous and age-matched virgin epithelium.
Conclusions
Although differences between parous and virgin mammary epithelium at later time points post lactation or following multiple pregnancies cannot be ruled out, there are no differences in stem/progenitor cell numbers between mammary epithelium isolated from parous animals which were mated at 9 weeks old and virgin animals. However, a recent report has suggested that animals that were mated at 5 weeks old have a twofold reduction in stem/progenitor cell numbers. This is of interest given the association between early, but not late, pregnancy and breast cancer risk reduction in humans. However, a mechanistic connection between stem cell numbers and breast cancer risk remains to be established
