    Femtocell operator entry decision with spectrum bargaining and service competition

    This paper analyzes the effect of the entry of a femtocell operator into a mobile communications market where a macrocell operator exists. The analysis is conducted using a game-theory-based model, specifically a threelevel multi leaderfollower game, where different solution concepts are applied: Shapley value, Nash equilibrium and Wardrop equilibrium. It aims to answer the question of which benefit mobile communication users get from the entry of a femtocell operator into the market. The equilibrium is assessed from the point of view of each agent (e.g. profits and utilities), and of the whole (e.g. social welfare).     Long-term Follow-up of Sexual Quality of Life after Laparoscopic Surgery in Patients with Deep Infiltrating Endometriosis

    Study Objective: We performed a long-term follow-up to quantify the impairment of sexual quality of life (SQL) and health-related QL (HRQL) in sexually active women after laparoscopic excision of deep infiltrating endometriosis (DIE). Design: Prospective case-control study. Setting: Hospital Clinic of Barcelona. Patients: A total of 193 patients (after dropout and exclusions) were divided into 2 groups: one hundred twenty-nine premenopausal women with DIE (DIE group) and 64 healthy women who underwent tubal ligation (C group). Interventions: All patients underwent laparoscopic surgery: laparoscopic endometriosis surgery in the DIE group and laparoscopic tubal ligation in the C group. All women were followed for at least 36 months, and they completed the Medical Outcomes Study 36-item short form questionnaire to assess their HRQL and 3 self-administered questionnaires that evaluate different aspects of SQL: the generic Sexual Quality of Life−Female questionnaire, the Female Sexual Distress Scale to evaluate 'sexually related distress,' and the Brief Profile of Female Sexual Function to screen hypoactive sexual desire disorder. The patients with DIE as well as the controls completed the 4 questionnaires before surgery, and the patients with DIE also completed the questionnaires at 6 and 36 months after surgery. Measurements and Main Results: A comparison of the patients and controls before surgery showed a statistically significant impairment in SQL and HRQL among the patients with DIE. A statistically significant improvement in SQL and HRQL was observed in the DIE group 6 months after surgery, with scores being similar to those of the C group. An evaluation 36 months after surgery showed that SQL and HRQL were better than presurgical SQL and HRQL in the DIE group, with a slight reduction compared with the 6-month evaluation. Conclusion: SQL and HRQL improved in patients with DIE undergoing complete laparoscopic endometriosis resection and were comparable to those of healthy women at 6 months after surgery, showing a slight reduction at 36 months of follow-up

    Lift devices in the flight of Archaeopteryx

    Archaeopteryx has played a central role in the debates on the origins of avian (and dinosaurian) flight, even though as a flier it probably represents a relatively late stage in the beginnings of fl ight. We report on aerodynamic tests using a life-sized model of Archaeopteryx performing in a low turbulence wind tunnel. Our results indicate that tail deflection significantly decreased take-off velocity and power consumption, and that the first manual digit could have functioned as the structural precursor of the alula. Such results demonstrate that Archaeopteryx had already evolved high-lift devices, which are functional analogues of those present in today's birds

    Guided Self-Change Treatment for alcohol abuse in Spain: a single-case study

    Aunque existe evidencia de la efectividad de intervenciones breves para disminuir el consumo problemático de alcohol, son escasos los datos en población española. El objetivo de este estudio es analizar, por primera vez en España, la efectividad del Programa de Auto-Cambio Dirigido (Guided Self-Change Treatment; GSCT). Método: Se aplicaron 4 sesiones individuales con frecuencia semanal del programa a una consumidora de alto riesgo de alcohol. Resultados: Los resultados indican una disminución del consumo de alcohol de 42 a 0 unidades de bebida estándar por semana y un aumento de la autoeficacia percibida, manteniéndose en el seguimiento a 6 meses. Conclusiones: Como ya señalaron estudios anteriores en población latinoamericana, el GSCT parece ser efectivo en la reducción del consumo de alcohol y las consecuencias negativas asociadas al mismoAlthough there is evidence of the effectiveness of brief interventions at reducing problem drinking, there is a lack of studies in Spanish population. This study aims to analyze, for the first time in Spain, the effectiveness of the Guided Self-Change Treatment (GSCT).Methods: One-day a week, the 4-sesion program was individually applied to a woman with a pattern of high-risk alcohol consumption. Results: After treatment, thefrequency of drinking decreased from 42 to 0 standard drinks per week and self-efficacywas clinically significantimproved. Changes were maintained at 6-months follow up. Conclusions: To conclude, the GSCT seems to be effective at reducing problem drinking and the negative consequences associated with it, which is consistent with the results of previous studies in Latin American population

    Entry, competition and regulation in cognitive radio scenarios: a simple game theory model

    [EN] Spectrum management based on private commons is argued to be a realistic scenario for cognitive radio deployment within the current mobile market structure. A scenario is proposed where a secondary entrant operator leases spectrum from a primary incumbent operator. The secondary operator innovates incorporating cognitive radio technology, and it competes in quality of service and price against the primary operator in order to provide service to users. We aim to assess which benefit users get from the entry of secondary operators in the market. A game theory-based model for analyzing both the competition between operators and the subscription decision by users is proposed. We conclude that an entrant operator adopting an innovative technology is better off entering the market, and that a regulatory authority should intervene first allowing the entrant operator to enter the market and then setting a maximum amount of spectrum leased.     High exposure to tacrolimus is associated to spontaneous remission of recurrent membranous nephropathy after kidney transplantation

    Introduction We aimed to characterize the incidence and clinical presentation of membranous nephropathy (MN) after kidney transplantation (KT), and to assess allograft outcomes according to proteinuria rates and immunosuppression management. Methods Multicenter retrospective cohort study including patients from six Spanish centers who received a KT between 1991-2019. Demographic, clinical, and histological data were collected from recipients with biopsy-proven MN as primary kidney disease (n = 71) or MN diagnosed de novo after KT (n = 4). Results Up to 25.4% of patients with biopsy-proven MN as primary kidney disease recurred after a median time of 18.1 months posttransplant, without a clear impact on graft survival. Proteinuria at 3-months post-KT was a predictor for MN recurrence (rMN, HR 4.28; P = 0.008). Patients who lost their grafts had higher proteinuria during follow-up [1.0 (0.5-2.5) vs 0.3 (0.1-0.5) g/24 h], but only eGFR after recurrence treatment predicted poorer graft survival (eGFR < 30 ml/min: RR = 6.8). We did not observe an association between maintenance immunosuppression and recurrence diagnosis. Spontaneous remission after rMN was associated with a higher exposure to tacrolimus before recurrence (trough concentration/dose ratio: 2.86 vs 1.18; P = 0.028). Up to 94.4% of KT recipients received one or several treatments after recurrence onset: 22.2% rituximab, 38.9% increased corticosteroid dose, and 66.7% ACEi/ARBs. Only 21 patients had proper antiPLA2R immunological monitoring. Conclusions One-fourth of patients with biopsy-proven MN as primary kidney disease recurred after KT, without a clear impact on graft survival. Spontaneous remission after rMN was associated with a higher exposure to tacrolimus before recurrence.12 página

    Article Germacrone Derivatives as new Insecticidal and Acaricidal Compounds: A Structure-Activity Relationship

However, its excessive use leads to the development of pesticide resistance, a contamination of the environment, toxicity to non-target organisms, and risks for human health. With the ultimate aim of contributing to the develop of a more sustainable pest management, we used the natural product germacrone (compound 1), reported to possess significant insecticidal activity, as starting material for the generation of molecular diversity (2–24). Some of the generated derivatives are natural compounds, such as 1,10-epoxygermacrone (2), 4,5-epoxygermacrone (3), gajutsulactone A (7), germacrol (11), isogermacrone (14), 9-hydroxyeudesma-3,7(11)dien-6-one (19), eudesma-4,7(11),dien-8-one (20), eudesma-3,7(11)-dien-8-one (21) and eudesma-4(15),7(11)-dien-8-one (22). Compounds, 7,11-9,10-diepoxigermacr-4,5-en-8-ol (17), 7,11-epoxieudesma-4,7(11)-dien-8-one (23) and 7,11-epoxieudesma-3,7(11)-dien-8-one (24) are described for the first time. The biocidal activity of most of these compounds was assayed against the tick Hyalomma lusitanicum. The acaricidal effects of compound 24 were four times higher than that of germacrone (1).     Use of Alkylarsonium Directing Agents for the Synthesis and Study of Zeolites

This article may be used for non-commercial purposes in accordance with Wiley Terms and Conditions for Self-Archiving.[EN] Expanding the previously known family of -onium (ammonium, phosphonium, and sulfonium) organic structure-directing agents (OSDAs) for the synthesis of zeolite MFI, a new member, the arsonium cation, is used for the first time. The new group of tetraalkylarsonium cations has allowed the synthesis of the zeolite ZSM-5 with several different chemical compositions, opening a route for the synthesis of zeolites with a new series of OSDA. Moreover, the use of As replacing N in the OSDA allows the introduction of probe atoms that facilitate the study of these molecules by powder X-ray diffraction (PXRD), solid-state nuclear magnetic resonance (MAS NMR), and X-ray absorption spectroscopy (XAS). Finally, the influence of trivalent elements such as B, Al, or Ga isomorphically replacing Si atoms in the framework structure and its interaction with the As species has been studied. The suitability of the tetraalkylarsonium cation for carrying out the crystallization of zeolites is demonstrated along with the benefit of the presence of As atoms in the occluded OSDA, which allows its advanced characterization as well as the study of its evolution during OSDA removal by thermal treatments.Program Severo Ochoa SEV-2016-0683 and Maria de Maeztu MDM-2015-0538 are gratefully acknowledged.     Current HHT genetic overview in Spain and its phenotypic correlation: data from RiHHTa registry

    Background: Hereditary hemorrhagic telangiectasia (HHT) is a rare vascular disease with autosomal dominant inheritance. Disease-causing variants in endoglin (ENG) and activin A receptor type II-like 1 (ACVRL1) genes are detected in more than 90% of cases submitted to molecular diagnosis. Methods: We used data from the RiHHTa (Computerized Registry of Hereditary Hemorrhagic Telangiectasia) registry to describe genetic variants and to assess their genotype-phenotype correlation among HHT patients in Spain. Results: By May 2019, 215 patients were included in the RiHHTa registry with a mean age of 52.5 ± 16.5 years and 136 (63.3%) were women. Definitive HHT diagnosis defined by the Curaçao criteria were met by 172 (80%) patients. Among 113 patients with genetic test, 77 (68.1%) showed a genetic variant in ACVRL1 and 36 (31.8%) in ENG gene. The identified genetic variants in ACVRL1 and ENG genes and their clinical significance are provided. ACVRL1 mutations were more frequently nonsense (50%) while ENG mutations were more frequently, frameshift (39.1%). ENG patients were significantly younger at diagnosis (36.9 vs 45.7 years) and had pulmonary arteriovenous malformations (AVMs) (71.4% vs 24.4%) and cerebral AVMs (17.6% vs 2%) more often than patients with ACVRL1 variants. Patients with ACVRL1 variants had a higher cardiac index (2.62 vs 3.46), higher levels of hepatic functional blood tests, and anemia (28.5% vs 56.7%) more often than ENG patients. Conclusions: ACVRL1 variants are more frequent than ENG in Spain. ACVRL1 patients developed symptomatic liver disease and anemia more often than ENG patients.
