    Intrinsic Noise of the Single Electron Box

    We quantify the intrinsic noise of the Single Electron Box arising from stochastic cyclic electron tunnelling between a quantum dot and a resevoir driven by a periodic gate voltage. We use both a master equation formalism and Markov Monte Carlo simulations to calculate the gate noise current, and find it to be generated by a cyclostationary process which displays significant spectral correlations at large excitation amplitudes and high tunnel rates. We model noise filtering through an electrical resonator and detection via synchronous demodulation to evaluate the effective noise spectral density in rf-reflectometry qubit readout applications, and determine the conditions under which the intrinsic noise limit could be measured experimentally. Our results have implications in the ultimate sensitivity of SEBs for fast, high-fidelity readout of spin qubits.Comment: 6 pages, 4 figure

    Multi-module microwave assembly for fast read-out and charge noise characterization of silicon quantum dots

    Fast measurements of quantum devices is important in areas such as quantum sensing, quantum computing and nanodevice quality analysis. Here, we develop a superconductor-semiconductor multi-module microwave assembly to demonstrate charge state readout at the state-of-the-art. The assembly consist of a superconducting readout resonator interfaced to a silicon-on-insulator (SOI) chiplet containing quantum dots (QDs) in a high-κ\kappa nanowire transistor. The superconducting chiplet contains resonant and coupling elements as well as LCLC filters that, when interfaced with the silicon chip, result in a resonant frequency f=2.12f=2.12~GHz, a loaded quality factor Q=680Q=680, and a resonator impedance Z=470Z=470~Ω\Omega. Combined with the large gate lever arms of SOI technology, we achieve a minimum integration time for single and double QD transitions of 2.77~ns and 13.5~ns, respectively. We utilize the assembly to measure charge noise over 9 decades of frequency up to 500~kHz and find a 1/ff dependence across the whole frequency spectrum as well as a charge noise level of 4~μ\mueV/Hz\sqrt{\text{Hz}} at 1~Hz. The modular microwave circuitry presented here can be directly utilized in conjunction with other quantum device to improve the readout performance as well as enable large bandwidth noise spectroscopy, all without the complexity of superconductor-semiconductor monolithic fabrication.Comment: Main: 7 pages, 4 figures. Emotional bodily feelings represent a 'state of self', analysed as a sense of one's body affording certain patterns of interaction with the environment. Recognising that there are two sources of intentional content in everyday emotional experience allows us to reconcile the diverging intuitions that people have about emotional states, and to understand better the long-standing debate between bodily feeling-based and appraisal-based theories of emotion

    Neonatal head and torso vibration exposure during inter-hospital transfer

    Inter-hospital transport of premature infants is increasingly common, given the centralisation of neonatal intensive care. However, it is known to be associated with anomalously increased morbidity, most notably brain injury, and with increased mortality from multifactorial causes. Surprisingly, there have been relatively few previous studies investigating the levels of mechanical shock and vibration hazard present during this vehicular transport pathway. Using a custom inertial datalogger, and analysis software, we quantify vibration and linear head acceleration. Mounting multiple inertial sensing units on the forehead and torso of neonatal patients and a preterm manikin, and on the chassis of transport incubators over the duration of inter-site transfers, we find that the resonant frequency of the mattress and harness system currently used to secure neonates inside incubators is ~9Hz. This couples to vehicle chassis vibration, increasing vibration exposure to the neonate. The vibration exposure per journey (A(8) using the ISO 2631 standard) was at least 20% of the action point value of current European Union regulations over all 12 neonatal transports studied, reaching 70% in two cases. Direct injury risk from linear head acceleration (HIC15) was negligible. Although the overall hazard was similar, vibration isolation differed substantially between sponge and air mattresses, with a manikin. Using a Global Positioning System datalogger alongside inertial sensors, vibration increased with vehicle speed only above 60 km/h. These preliminary findings suggest there is scope to engineer better systems for transferring sick infants, thus potentially improving their outcomes

    What carcinoembryonic antigen level should trigger further investigation during colorectal cancer follow-up? A systematic review and secondary analysis of a randomised controlled trial

    Background Following primary surgical and adjuvant treatment for colorectal cancer, many patients are routinely followed up with blood carcinoembryonic antigen (CEA) testing. Objective To determine how the CEA test result should be interpreted to inform the decision to undertake further investigation to detect treatable recurrences. Design Two studies were conducted: (1) a Cochrane review of existing studies describing the diagnostic accuracy of blood CEA testing for detecting colorectal recurrence; and (2) a secondary analysis of data from the two arms of the FACS (Follow-up After Colorectal Surgery) trial in which CEA testing was carried out. Setting and participants The secondary analysis was based on data from 582 patients recruited into the FACS trial between 2003 and 2009 from 39 NHS hospitals in England with access to high-volume services offering surgical treatment of metastatic recurrence and followed up for 5 years. CEA testing was undertaken in general practice. Results In the systematic review we identified 52 studies for meta-analysis, including in aggregate 9717 participants (median study sample size 139, interquartile range 72–247). Pooled sensitivity at the most commonly recommended threshold in national guidelines of 5 µg/l was 71% [95% confidence interval (CI) 64% to 76%] and specificity was 88% (95% CI 84% to 92%). In the secondary analysis of FACS data, the diagnostic accuracy of a single CEA test was less than was suggested by the review [area under the receiver operating characteristic curve (AUC) 0.74, 95% CI 0.68 to 0.80]. At the commonly recommended threshold of 5 µg/l, sensitivity was estimated as 50.0% (95% CI 40.1% to 59.9%) and lead time as about 3 months. About four in 10 patients without a recurrence will have at least one false alarm and six out of 10 tests will be false alarms (some patients will have multiple false alarms, particularly smokers). Making decisions to further investigate based on the trend in serial CEA measurements is better (AUC for positive trend 0.85, 95% CI 0.78 to 0.91), but to maintain approximately 70% sensitivity with 90% specificity it is necessary to increase the frequency of testing in year 1 and to apply a reducing threshold for investigation as measurements accrue. Limitations The reference standards were imperfect and the main analysis was subject to work-up bias and had limited statistical precision and no external validation. Conclusions The results suggest that (1) CEA testing should not be used alone as a triage test; (2) in year 1, testing frequency should be increased (to monthly for 3 months and then every 2 months); (3) the threshold for investigating a single test result should be raised to 10 µg/l; (4) after the second CEA test, decisions to investigate further should be made on the basis of the trend in CEA levels; (5) the optimal threshold for investigating the CEA trend falls over time; and (6) continuing smokers should not be monitored with CEA testing. Further research is needed to explore the operational feasibility of monitoring the trend in CEA levels and to externally validate the proposed thresholds for further investigation.     Protocol to evaluate the effectiveness and cost-effectiveness of an environmental nutrition and physical activity intervention in nurseries (Nutrition and Physical Activity Self Assessment for Child Care - NAP SACC UK): a multicentre cluster randomised controlled trial

    Background: One in seven UK children have obesity when starting school, with higher prevalence associated with deprivation. Most pre-school children do not meet UK recommendations for physical activity and nutrition. Formal childcare settings provide opportunities to deliver interventions to improve nutritional quality and physical activity to the majority of 3–4-year-olds. The nutrition and physical activity self-assessment for childcare (NAP SACC) intervention has demonstrated effectiveness in the USA with high acceptability in the UK. The study aims to evaluate the effectiveness and cost-effectiveness of the NAP SACC UK intervention to increase physical activity, reduce sedentary time and improve nutritional intake. Methods: Multi-centre cluster RCT with process and economic evaluation. Participants are children aged 2 years or over, attending UK early years settings (nurseries) for ≥ 12 h/week or ≥ 15 h/week during term time and their parents, and staff at participating nurseries. The 12-month intervention involves nursery managers working with a Partner (public health practitioner) to self-assess policies and practices relating to physical activity and nutrition; nursery staff attending one physical activity and one nutrition training workshop and setting goals to be achieved within 6 months. The Partner provides support and reviews progress. Nursery staff receive a further workshop and new goals are set, with Partner support for a further 6 months. The comparator is usual practice. Up to 56 nurseries will be stratified by area and randomly allocated to intervention or comparator arm with minimisation of differences in level of deprivation. Primary outcomes: accelerometer-assessed mean total activity time on nursery days and average total energy (kcal) intake per eating occasion of lunch and morning/afternoon snacks consumed within nurseries. Secondary outcomes: accelerometer-assessed mean daily minutes of moderate-to-vigorous physical activity and sedentary time per nursery day, total physical activity on nursery days compared to non-nursery days, average serving size of lunch and morning/afternoon snacks in nursery per day, average percentage of core and non-core food in lunch and morning/afternoon snacks, zBMI, proportion of children who are overweight/obese and child quality-of-life. A process evaluation will examine fidelity, acceptability, sustainability and context. An economic evaluation will compare costs and consequences from the perspective of the local government, nursery and parents. Trial registration: ISRCTN33134697, 31/10/2019

    Composition of woody species in a dynamic forest-woodland-savannah mosaic in Uganda: implications for conservation and management

    ForestÂżwoodlandÂżsavannah mosaics are a common feature in the East African landscape. For the conservation of the woody species that occur in such landscapes, the species patterns and the factors that maintain it need to be understood. We studied the woody species distribution in a forestÂżwoodlandÂżsavannah mosaic in Budongo Forest Reserve, Uganda. The existing vegetation gradients were analyzed using data from a total of 591 plots of 400 or 500 m2 each. Remotely sensed data was used to explore current vegetation cover and the gradients there in for the whole area. A clear species gradient exists in the study area ranging from forest, where there is least disturbance, to wooded grassland, where frequent fire disturbance occurs. Most species are not limited to a specific part of the gradient although many show a maximum abundance at some point along the gradient. Fire and accessibility to the protected area were closely related to variation in species composition along the ordination axis with species like Cynometra alexandri and Uvariopsis congensis occurring at one end of the gradient and Combretum guenzi and Lonchocarpus laxiflorus at the other. The vegetation cover classes identified in the area differed in diversity, density and, especially, basal area. All vegetation cover classes, except open woodland, had indicator species. Diospyros abyssinica, Uvariopsis congensis, Holoptelea grandis and all Celtis species were the indicator species for the forest class, Terminalia velutina and Albizia grandbracteata for closed woodland, Grewia mollis and Combretum mole for very open woodland and Lonchocarpus laxiflorus, Grewia bicolor and Combretum guenzi for the wooded grassland class. Eleven of the species occurred in all cover classes and most of the species that occurred in more than one vegetation cover class showed peak abundance in a specific cover class. Species composition in the study area changes gradually from forest to savannah. Along the gradient, the cover classes are distinguishable in terms of species composition and vegetation structure. These classes are, however, interrelated in species composition. For conservation of the full range of the species within this East African landscape, the mosaic has to be managed as an integrated whole. Burning should be varied over the area with the forest not being burnt at all and the wooded grassland burnt regularly.
