Catheters are used for effective drainage of the bladder, either temporally or permanently, in the presence of physiological and anatomical defects or obstruction of the lower urinary tract. Catheters are used for a variety of reasons, as follows, to maintain bladder drainage during and following surgery or epidurals anesthesia for minimizing and prevention of the risk of distension injuries; investigations, for accurate urine output measurement, and measurement of post-micturition residuals; treatments, to relieve urinary retention or for chemotherapy instillation; intractable incontinence, as the final option for containment

Characterization of the symbolic abilities of children with Down syndrome

OBJETIVO: Caracterizar as habilidades simbólicas de um grupo de crianças com síndrome de Down. MÉTODOS: Participaram do estudo 26 crianças com idades entre 12 e 36 meses, divididas em dois grupos: grupo síndrome de Down (GSD) e grupo controle (GC) - crianças com desenvolvimento normal. Os grupos foram subdivididos de acordo com a idade: GSD I e GC I, compostos por crianças de 12 a 24 meses; GSD II e GC II, com crianças de 25 a 36 meses. Os dados foram coletados por meio da interação com a examinadora em situação lúdica, durante 30 minutos o GSD e 20 minutos o GC, de acordo com a proposta do protocolo de observação comportamental. RESULTADOS: Comparando ambos os grupos controle encontramos diferença (p<0,05) para as formas de manipulação dos objetos, para o nível de desenvolvimento do simbolismo e para o desempenho geral no protocolo. Em ambos os grupos síndrome de Down houve diferença para o nível de desenvolvimento do simbolismo. Na comparação inter-grupos de acordo com as faixas etárias encontramos diferenças quanto à forma de manipulação dos objetos, nível de desenvolvimento do simbolismo e desempenho geral no protocolo. A imitação sonora e gestual não se diferenciou significativamente nessa pesquisa. CONCLUSÃO: Os resultados confirmaram a hipótese de atraso do desenvolvimento das habilidades simbólicas para crianças com síndrome de Down. O exame da linguagem e simbolismo em contexto funcional possibilitou a confrontação das manifestações observadas neste grupo e as descritas para crianças com desenvolvimento linguístico e simbólico considerados normais, sendo o nível de desenvolvimento simbólico o melhor parâmetro de análise e acompanhamento para o grupo.PURPOSE: To characterize the symbolic abilities of a group of children with Down syndrome. METHODS: The study included 26 children with ages ranging from 12 to 36 months, divided into two groups: Down syndrome group (DSG) and control group (CG) - children within normal development. The groups were subdivided according to age range: DSG I and CG I comprised children from 12 to 24 months old; DSG II and CG II, children from 25 to 36 months. Data were gathered during a 30- (DSG) or 20-minute (CG) interaction session with the examiner, in a playful situation, according to the proposal of the behavioral observation protocol. RESULTS: The comparison between the control groups showed differences (p<0.05) in the forms of objects' manipulation, the level of symbolic development, and in overall performance on the protocol. In both Down syndrome groups there was a difference in the level of symbolic development. The between-group comparison according to age groups indicated differences in how children manipulated objects, in the level of symbolic development, and in overall performance on the protocol. The imitation of sounds and gestures did not differ significantly in this study. CONCLUSION: The results confirmed the hypothesis of delay in the development of symbolic abilities of children with Down syndrome. The evaluation of language and symbolism in a functional context allowed the confrontation of manifestations observed in this group with those described for children within normal language and symbolic development, showing that the level of symbolic development was the best measure for analyzing and monitoring the group.Fundação de Amparo à Pesquisa do Estado de São Paulo (FAPESP

Current Trends in Urinary Diversion in Men

Prior to the introduction of the ileal conduit more than four decades ago, the options for urinary diversion after cystectomy were extremely limited. Direct cutaneous anastomoses of the collecting system (cutaneous pyelostomies, ureterostomies) offered patients a short-term diversion, but the benefits were outweighed by significant complications: recession or stenosis of the stoma. The first choice of diversion was the ureterosigmoidostomy with or without antireflux technique. Then it fell in popularity and was replaced with continent/non-continent uretero-ileo-cutaneous diversions. Only in the last years the continent orthotopic neobladder has been widely employed as first procedure choice. OS PAGAMENTOS POR SERVIÇOS AMBIENTAIS (PSA) COMO ALTERNATIVA NA CONSTRUÇÃO DA SUSTENTABILIDADE AMBIENTAL EM PAÍSES EUROPEUS E AMERICANOS

Existe certo consenso entre os diversos segmentos da sociedade mundial em aceitar a constatação de que nas últimas décadas houve um incremento à utilização dos recursos naturais não renováveis como matriz energética para diversos setores produtivos. A crescente devastação ambiental tem constantemente exigido do Estado e da iniciativa privada a adoção de novas formas de gestão ambiental para a redução dos impactos causados pela exploração dos recursos naturais. A análise de casos práticos tem demonstrado que apenas a aplicação de sanções – seja no âmbito criminal ou civil - àqueles que degradam o meio ambiente, não resolve o problema. O conceito do modelo de desenvolvimento a ser adotado, ao seu turno, varia de acordo com os interesses em jogo, neste sistema mundial de produção capitalista, o que implica diretamente na forma de atuar em face deste avanço da utilização desordenada dos recursos naturais. Adotando-se como parâmetro a ideia de sociedade de risco e do Estado de Direito Ambiental, que, resptivamente, identifica um modelo de produção mundial que promove relevantes alterações negativas no meio ambiente, e que coloca o Estado como ente protagonista no papel de promover medidas voltadas à transformar este cenário, o presente estudo procura analisar uma das formas de promover um meio ambiente equilibrado: o pagamento por serviços ambientais – PSA, e o atual estágio de evolução nos ambientes americano e europeu. Busca analisar a relevância dos pagamentos por serviços ambientais enquanto mecanismo de fomento à preservação dos ecossistemas e da biodiversidade. Nesse cenário, é cada vez mais frequente a criação de mecanismos jurídicos atrelados a conceitos econômicos que surgem como forma incentivar a preservação e o uso sustentável dos recursos naturais. Traz ao debate os pagamentos por serviços ambientais, demonstrando-se a viabilidade da implementação destes modelos como forma de garantir a efetivação de políticas públicas de cunho ambiental, a exemplo do que já ocorre em países americanos e europeu.

Development of a questionnaire specifically for patients with Ileal Orthotopic Neobladder (IONB)

The ileal orthotopic neobladder (IONB) is often used in patients undergoing radical cystectomy. The IONB allows to void avoiding the disadvantages of the external urinary diversion.In IONB patients the quality of life (QoL) appears compromised by the need to urinate voluntarily. The patients need to wake up at night interrupting the sleep-wake rhythm with consequences on social and emotional life.At present the QoL in IONB patients is evaluated by generic questionnaires. These are useful when IONB patients are compared with patients with different urinary diversions but they are less effective when only IONB patients are evaluated. To address this problem a specific questionnaire-the IONB-PRO-was developed. METHODS: A) Based on a conceptual framework, narrative-based interviews were conducted on 35 IONB patients. A basic pool of 43 items was produced and organized throughout two clinical and four QoL dimensions. An additional 15 IONB patients were interviewed for face validity testing.B) Psychometric testing was conducted on 145 IONB patients. Both classic test strategy and Rasch analysis were applied. Psychometric properties of the resulting scales were comparatively tested against other QoL-validated scales. RESULTS: The IONB-PRO questionnaire includes two sections: one on the QoL and a second section on the capability of the patient to manage the IONB. For evaluation of the QoL, three versions were delivered: 1) a basic 23-item QoL version (3 domains 23-items; alpha 0.86÷ 9.69), 2) a short-form 12-item QoL scale (alpha = 0.947), and 3) a short-form 15-item Rasch QoL scale (alpha = 0.967). Correlations of the long version scales with the corresponding dimensions of the EORTC-QLQ C30 and the EORTC-BLM30 were significant. The short forms exhibited significant correlations with the global health dimension of the EORTC-QLQ and with the urinary subscales of the EORTC-BLM30. The effect size was approximately 1.00 between patients at the 1-year follow-up period and those with 3, 5, and > 5-year follow-up periods for all scales. No relevant differences were observed between the 12-item short-form and the Rasch scale. CONCLUSIONS: The IONB-PRO long and short-forms demonstrated a high level of internal consistency and reliability with an excellent discriminanting validity

Cinq notes sur la relation entre espaces composites et automatisations gestuelles

Ce texte aborde, de manière transversale, plusieurs de mes productions vidéographiques, réalisées entre 2005 et aujourd’hui. À travers cinq notes analytiques, j’interrogerai certains rapports entre les notions d’espace composite et d’automatisation gestuelle. Il s’agira de penser la manière avec laquelle l’activité de post-production vidéo, plus particulièrement celle que nous nommons communément le compositing[^1], permettent de réinvestir certaines articulations classiques entre fond (ou arrière-plan contextuel) et forme, à travers l’articulation entre lieu de l’action scénique et personnages.This text questions, in a transversal way, several of my video art productions, made between 2005 and today. Sintetizador de frequencias empregando a tecnica de sintese digital direta coerente
