Effects of transcutaneous electrical nerve stimulation on myocardial protection in patients undergoing aortic valve replacement: a randomized clinical trial
Abstract
Background
Cardiopulmonary bypass-related myocardial ischemia-reperfusion injury is a major contributor to postoperative morbidity. Although transcutaneous electrical nerve stimulation (TENS) has been found to have cardioprotective effects in animal studies and healthy volunteers, its effects on cardiac surgery under cardiopulmonary bypass patients have not been evaluated. We investigated the effects of TENS on myocardial protection in patients undergoing aortic valve replacement surgery using cardiopulmonary bypass.
Methods
Thirty patients were randomized to receive TENS or sham in three different anesthetic states â pre-anesthesia, sevoflurane, or propofol (each nâ=â5). TENS was applied with a pulse width of 385âÎŒs and a frequency of 10âHz using two surface electrodes at the upper arm for 30âmin. Sham treatment was provided without stimulation. The primary outcome was the difference in myocardial infarct size following ischemia-reperfusion injury in rat hearts perfused with pre- and post-TENS plasma dialysate obtained from the patients using Langendorff perfusion system. The cardioprotective effects of TENS were determined by assessing reduction in infarct size following treatment.
Results
There were no differences in myocardial infarct size between pre- and post-treatment in any group (41.4â±â4.3% vs. 36.7â±â5.3%, 39.8â±â7.3% vs. 27.8â±â12.0%, and 41.6â±â2.2% vs. 37.8â±â7.6%; pâ=â0.080, 0.152, and 0.353 in the pre-anesthesia, sevoflurane, and propofol groups, respectively).
Conclusions
In our study, TENS did not show a cardioprotective effect in patients undergoing aortic valve replacement surgery.

Dielectric functions and electronic band structure of lead zirconate titanate thin films
We measure pseudodielectric functions in the visible-deep ultraviolet spectral range of Pb(ZrxTi1âx)O3 (x=0.2,0.56,0.82) (PZT), Pb0.98Nb0.04 (Zr0.2Ti0.8)0.96O3, Pb0.91La0.09 (Zr0.65Ti0.35)0.98O3, and Pb0.85La0.15Ti0.96O3 films grown on platinized silicon substrates using a sol-gel method and on (0001) sapphire using a radio-frequency sputtering method. Using a parametric optical constant model, we estimate the dielectric functions(Ï”) of the perovskite oxide thin films. Taking the second derivative of the fitted layer dielectric functions and using the standard critical-point model, we determine the parameters of the critical points. In the second derivative spectra, the lowest band-gapenergy peak near 4 eVis fitted as a double peak for annealed PZTs due to the perovskite phase. As-grown PZTs have mainly pyrochlore phase and the lowest band-gap peak is fitted as a single peak. We also examine the effect of dopants La and Nb, which substitute at Pb and Zr (Ti) sites, respectively. We found three band gaps Ea(âŒ3.9eV), Eb (âŒ4.5eV), and Ec (âŒ6.5eV) in the order of increasing energy. The Ea and Eb band-gap energies were not sensitive to Zr composition. We discuss the change of critical-point parameters for PZTs in comparison to the band-structure calculations based on local-density approximation. The near constancy of the lowest band-gap energy independent of Zr composition is consistent with the band-structure calculations
Accelerated Bone Regeneration by Two-Photon Photoactivated Carbon Nitride Nanosheets
Human bone marrow-derived mesenchymal stem cells (hBMSCs) present promising opportunities for therapeutic medicine. Carbon derivatives showed only marginal enhancement in stem cell differentiation toward bone formation. Here we report that red-light absorbing carbon nitride (C3N4) sheets lead to remarkable proliferation and osteogenic differentiation by runt-related transcription factor 2 (Runx2) activation, a key transcription factor associated with osteoblast differentiation. Accordingly, highly effective hBMSCs-driven mice bone regeneration under red light is achieved (91% recovery after 4 weeks compared to 36% recovery in the standard control group in phosphate-buffered saline without red light). This fast bone regeneration is attributed to the deep penetration strength of red light into cellular membranes via tissue and the resulting efficient cell stimulation by enhanced photocurrent upon two-photon excitation of C3N4 sheets near cells. Given that the photoinduced charge transfer can increase cytosolic Ca2+ accumulation, this increase would promote nucleotide synthesis and cellular proliferation/differentiation. The cell stimulation enhances hBMSC differentiation toward bone formation, demonstrating the therapeutic potential of near-infrared two-photon absorption of C3N4 sheets in bone regeneration and fracture healing.ope
Brain structural changes after multiâstrategic metamemory training in older adults with subjective memory complaints: A randomized controlled trial
BACKGROUND: Metamemory is the process of monitoring and controlling one's memory. Improving metamemory may reduce the memory problem in old age. We hypothesized that metamemory training (MMT) would improve cognition in older adults with subjective memory complaints and change the brain region related to metacognition. METHOD: We recruited and randomized older adults to the multi-strategic memory training of 10 weekly 90-min sessions, based on the metamemory concept or usual care. Cognitive tests including the Elderly Verbal Learning Test, Simple Rey Figure Test, Digit Span, Spatial Span, Categorical Fluency, and the Boston Naming Test were done in 201 participants, together with magnetic resonance imaging (MRI) in 49 participants before and after training. RESULTS: A total of 112 in the training group and 89 in the control group participated. The training group had a significant increase in long-term delayed free recall, categorical fluency, and the Boston Naming test. In MRI, the mean diffusivity of the bundles of axon tracts passing from the frontal lobe to the posterior end of the lateral sulcus decreased in the training group. CONCLUSION: These results indicate that the MMT program has a positive impact on enhancing older people' cognitive performance. Improved white matter integrity in the anterior and posterior cerebrum and increased cortical thickness of prefrontal regions, which related to metacognition, possibly suggest that the effects of the MMT would be induced via the enhancement of cognitive control
Chronic allergen challenge induces bronchial mast cell accumulation in BALB/c but not C57BL/6 mice and is independent of IL-9
As genetically engineered mutant mice deficient in single genes are usually generated on a C57BL/6 background, to study mast cell trafficking in mutant mice, we initially investigated whether mast cells accumulated in bronchi in C57BL/6 mice challenged with OVA allergen acutely or chronically for 1 to 3 months. The total number of bronchial mast cells were quantitated using toluidine blue staining in airways of different sizes, i.e. , small (<90 ”m), medium (90â155 ”m), or large (>150 ”m) airways. Non-OVA challenged and acute OVA challenged mice (C57BL/6 and BALB/c) had no detectable bronchial mast cells. Chronic OVA challenge in BALB/c mice for 1 or 3 months induced a significant increase in the number of bronchial mast cells in small-, medium-, and large-sized airways but minimal change in the number of bronchial mast cells in C57BL/6 mice. Both BALB/c and C57BL/6 mice developed significant lung eosinophilia following acute or chronic OVA challenge. Studies of IL-9-deficient mice on a BALB/c background demonstrated a significant increase in the number of bronchial mast cells in IL-9-deficient mice suggesting that IL-9 was not required for the bronchial accumulation of mast cells. Overall, these studies demonstrate that the chronic OVA challenge protocol we have utilized in BALB/c mice provides a model to study the mechanism of bronchial mast cell accumulation and that bronchial mast cell accumulation in chronic OVA challenged mice is independent of IL-9 in this model
Effects of red grape, wild grape and black raspberry wines on ground pork during refrigerated storage
The effects of red grape, wild grape and black raspberry wines on the quality of ground pork during a 15 days refrigerated storage period were investigated. The levels of phenolic compounds were the highest in black raspberry wine (P0.05). The addition of 5% and 10% wine influenced the quality of ground pork by decreasing pH, inhibiting the progression of lipid oxidation and the formation of total volatile basic nitrogen (TVB-N), and stabilizing the red colour of the ground pork compared to control samples to which no wine was added. In ground pork, addition of red grape wine led to lower concentrations of thiobarbituric acid reactive substances (TBARS, 0.19â0.39 mg kgâ1) and TVB-N values (69.1â119.9 mg kgâ1) than wild grape (0.16â0.43 mg kgâ1 and 72.0â194.1 mg kgâ1, respectively) or black raspberry wine (0.33â0.58 mg kgâ1 and 81.7â225.4 mg kgâ1, respectively) up to 10 days of storage. Results from the present study suggested that the quality of ground pork was affected by wine type and storage period. These effects could be due to phenolic compounds as well as other chemical components of the wines
Measurement of Leptonic Asymmetries and Top Quark Polarization in ttbar Production
We present measurements of lepton (l) angular distributions in ttbar -> W+ b
W- b -> l+ nu b l- nubar bbar decays produced in ppbar collisions at a
center-of-mass energy of sqrt(s)=1.96TeV, where l is an electron or muon. Using
data corresponding to an integrated luminosity of 5.4fb^-1, collected with the
D0 detector at the Fermilab Collider, we find that the angular distributions of
l- relative to anti-protons and l+ relative to protons are in agreement with
each other. Combining the two distributions and correcting for detector
acceptance we obtain the forward-backward asymmetry A^l_FB = (5.8 +- 5.1(stat)
+- 1.3(syst))%, compared to the standard model prediction of A^l_FB (predicted)
= (4.7 +- 0.1)%. This result is further combined with the measurement based on
the analysis of the l+jets final state to obtain A^l_FB = (11.8 +- 3.2)%.
Furthermore, we present a first study of the top-quark polarization.Comment: submitted versio
Precise measurement of the top quark mass in the dilepton channel at D0
We measure the top quark mass (mt) in ppbar collisions at a center of mass
energy of 1.96 TeV using dilepton ttbar->W+bW-bbar->l+nubl-nubarbbar events,
where l denotes an electron, a muon, or a tau that decays leptonically. The
data correspond to an integrated luminosity of 5.4 fb-1 collected with the D0
detector at the Fermilab Tevatron Collider. We obtain mt = 174.0 +- 1.8(stat)
+- 2.4(syst) GeV, which is in agreement with the current world average mt =
173.3 +- 1.1 GeV. This is currently the most precise measurement of mt in the
dilepton channel.Comment: 7 pages, 4 figure
Search for Decay
We have searched for the charmless hadronic decay of B0 mesons into two
neutral pions. Using 9.13fb^-1 taken at the Upsilon(4S) with the CLEO detector,
we obtain an improved upper limit for the branching fraction BR(B0-->pi0pi0) <
5.7*10^-6 at the 90% confidence level.Comment: pages postscript, also available through
