    Effectiveness of the Bridge/Adapt Program on Functional Skill Generalization After Acquired Brain Injury

    This study explored the effectiveness of the Bridge/Adapt program for generalizing increased cognition to functional skills. Three participants, identified as having significant cognitive impairments as measured by the Cognistat assessment, participated in the Bridge/Adapt program, an eight-week program that includes both remedial and compensatory components. The remedial component used was a computer-based cognitive rehabilitation program called Parrot Software. Past studies have proven computer-based cognitive rehabilitation to be effective in increasing overall cognition. The Bridge/Adapt module is the compensatory component that utilized a variety of strategies and everyday tasks to facilitate the generalization of improved cognition to functional performance. A homework component was also implemented for participants to incorporate the strategies learned in the Bridge/Adapt program to their own meaningful occupations. This study utilized a pretest posttest design using the medication box assessment to measure functional performance. Results of the medication box assessment indicated that one of the three participants demonstrated generalization of skills from improved cognition to functional performance. Future research should include re-evaluating the Bridge/Adapt modules and the medication box assessment. Recommendations to improve future implementation are provided to increase likelihood of generalization

    ‘The Secret is the Power, not the Knowledge’: Reconfiguring the Discourse of HIV/AIDS-related Stigma in Durham, NC

    What are the conditions in which stigma is held to be a common sense factor of HIV/AIDS, yet is mutable in the ways people experience stigma? My research analyzes the terrain in which this question is precipitated, and I will evaluate stigma as a lens for understanding broader questions of politics and care that are embedded with American notions of self-hood. The national history of HIV/AIDS, Goffman’s work on stigma, and neoliberal policies, laws, and education measures have converged to create certain imaginations of attribution and individuality for those who live with the disease. People who are HIV-positive are feared, seen as immoral and tainted, and they are blamed for contracting the virus and failing to receive the proper care. The dominant discourse creates feelings of isolation and has led to beliefs that stigma is permanent and universally affects HIV/AIDS patients, so one must remain confidential about a diagnosis for protection from stigma. From policies that move towards privatized healthcare to the rationale that stigma can be localized to a body, these aspects of life rely on ideals of self-help and have precipitated perceptions of HIV/AIDS as solely individual experiences. Instead of seeking ways to confront these perceptions, society ultimately advocates for concealing diagnoses to mitigate stigma. The localization of stigma ignores the social construction of stigma that is informed by history, culture, and power, and in this very context, my participants have demonstrated agency by disclosing their stories. By illuminating social ties through dialogue, they have contested the dominant discourse that they are secluded from the rest due to their mark of stigma. In this ethnography, then, I will explore how stigma functions and is maintained in a space where discursive practices, neoliberalism, and medical intervention intersect.Honors Thesi

    <p>Abstract</p> <p>Health Issue</p> <p>Cardiovascular disease (CVD) is the leading cause of death in Canadian women and men. In general, women present with a wider range of symptoms, are more likely to delay seeking medial care and are less likely to be investigated and treated with evidence-based medications, angioplasty or coronary artery bypass graft than men.</p> <p>Key Findings</p> <p>In 1998, 78,964 Canadians died from CVD, almost half (39,197) were women. Acute myocardial infarction, which increases significantly after menopause, was the leading cause of death among women.</p> <p>Cardiovascular disease accounted for 21% of all hospital admissions for Canadian women over age 50 in 1999. Admissions to hospital for ischemic heart disease were more frequent for men, but the mean length of hospital stay was longer for women.</p> <p>Mean blood pressure increases with age in both men and women. After age 65, however, high blood pressure is more common among Canadian women. More than one-third of postmenopausal Canadian women have hypertension.</p> <p>Diabetes increases the mortality and morbidity associated with CVD in women more than it does in men. Depression also contributes to the incidence and recovery from CVD, particularly for women who experience twice the rate of depression as men.</p> <p>Data Gaps and Recommendations</p> <p>CVD needs to be recognized as a woman's health issue given Canadian mortality projections (particularly heart failure). Health professionals should be trained to screen, track, and address CVD risk factors among women, including hypertension, elevated lipid levels, smoking, physical inactivity, depression, diabetes and low socio-economic status.</p

    Women physicians and stress

    Most women physicians enjoy better than average physical health and lead satisfying and productive lives. However, higher than average rates of depression, anxiety, marital problems, and substance abuse have been reported by some, but not all, authors. This quantitative survey of 196 women physicians and qualitative focus groups with 48 other women physicians was conducted to determine perceptions of their health, stress, satisfaction, knowledge, and abuse rates in medical practice. Eight specialties plus family practice physicians participated. The average age was 44.1 years (SD 8.8, range 23–77). Seventy-four percent of women physicians were married, with children. Specialists and family physicians were similar in all demographic characteristics except that family physicians were more significantly likely to be divorced, separated, or widowed (p ≤ 0.01). Specialists perceived their personal physical health to be better than that of family doctors (p ≤ 0.05), and family physicians rated their medical knowledge better than that of specialists (p ≤ 0.0001). Women physicians over age 50 or with children over age 19 reported the best mental health (p ≤ 0.0001 and 0.003, respectively). Overall, 49% of women physicians reported usually having high levels of stress, 44% felt mentally tired, and 17% took antidepressant drugs. Seventy-three percent reported verbal abuse at work (71% in the last year), and 33% reported physical assault at work (11% in the last year). Focus groups identified three major sources of stress: high expectations, multiple roles, and work environment. These results are discussed and compared with the literature. Both personal and systemic strategies are required to solve the problems identified. Women physicians can facilitate the adoption of some of these strategies by sharing information about successes, challenges, and solutions

    Expression and Roles of Teneurins in Zebrafish

    The teneurins, also known as Ten-m/Odz, are highly conserved type II transmembrane glycoproteins widely expressed throughout the nervous system. Functioning as dimers, these large cell-surface adhesion proteins play a key role in regulating neurodevelopmental processes such as axon targeting, synaptogenesis and neuronal wiring. Synaptic specificity is driven by molecular interactions, which can occur either in a trans-homophilic manner between teneurins or through a trans-heterophilic interaction across the synaptic cleft between teneurins and other cell-adhesion molecules, such as latrophilins. The significance of teneurins interactions during development is reflected in the widespread expression pattern of the four existing paralogs across interconnected regions of the nervous system, which we demonstrate here via in situ hybridization and the generation of transgenic BAC reporter lines in zebrafish. Focusing on the visual system, we will also highlight the recent developments that have been made in furthering our understanding of teneurin interactions and their functionality, including the instructive role of teneurin-3 in specifying the functional wiring of distinct amacrine and retinal ganglion cells in the vertebrate visual system underlying a particular functionality. Based on the distinct expression pattern of all teneurins in different retinal cells, it is conceivable that the combination of different teneurins is crucial for the generation of discrete visual circuits. Finally, mutations in all four human teneurin genes have been linked to several types of neurodevelopmental disorders. The opportunity therefore arises that findings about the roles of zebrafish teneurins or their orthologs in other species shed light on the molecular mechanisms in the etiology of such human disorders

    Interventions to reduce suicides at suicide hotspots: a systematic review

    BACKGROUND: 'Suicide hotspots' include tall structures (for example, bridges and cliffs), railway tracks, and isolated locations (for example, rural car parks) which offer direct means for suicide or seclusion that prevents intervention. METHODS: We searched Medline for studies that could inform the following question: 'What interventions are available to reduce suicides at hotspots, and are they effective?' RESULTS: There are four main approaches: (a) restricting access to means (through installation of physical barriers); (b) encouraging help-seeking (by placement of signs and telephones); (c) increasing the likelihood of intervention by a third party (through surveillance and staff training); and (d) encouraging responsible media reporting of suicide (through guidelines for journalists). There is relatively strong evidence that reducing access to means can avert suicides at hotspots without substitution effects. The evidence is weaker for the other approaches, although they show promise.     Uncertainties in limits on TeV-gravity from neutrino-induced showers

    In models with TeV-scale gravity, ultrahigh energy cosmic rays can generate microscopic black holes in the collision with atmospheric and terrestrial nuclei. It has been proposed that stringent bounds on TeV-scale gravity can be obtained from the absence of neutrino cosmic ray showers mediated by black holes. However, uncertainties in the cross section of black hole formation and, most importantly, large uncertainties in the neutrino flux affects these bounds. As long as the cosmic neutrino flux remains unknown, the non-observation of neutrino induced showers implies less stringent limits than present collider limits.Comment: Changes to match published versio
