    This report presents the results of a Hazardous Material Commodity Flow Analysis for the Louisville Metro area and Jefferson County. Study components were conducted by Western Kentucky University in partnership with Louisville / Jefferson County Emergency Management Agency. The study area was focused in Louisville and Jefferson County, Louisville Metro area, and included monitoring in Bullet County and Oldham County, as shown in Figure 1.1. As part of the study, hazardous material transport was monitored at the following sites: Interstate-64 (I-64), East and West bound lanes at The Belvedere; I-64, East and West bound lanes at Blankenbaker exit location; Interstate-65 (I-65, North and South bound lanes at the Jewish Hospital; I-65, North and South bound lanes at Brooks, KY exit location; Interstate-71 (I-71), North and South bound lanes at Oldham County rest areas; Hwy 841, North and South bound Lanes at Westport Road. The purpose of this report is to present information on patterns of hazardous material commodity flow along I-64, I-65, I-71 and Highway 841, as observed from June 11, 2012 to August 2, 2012. This report also summarizes incidents involving hazardous materials over the previous 3 years, August 2008 to June 2011, in the Louisville Metro area. Finally, this report assesses survey information collected from fixed facilities that ship and receive hazardous materials in the Louisville Metro Area

    Madison County, Kentucky Hazardous Materials Commodity Flow Analysis

    This report presents the results of a Commodity Flow Analysis of Hazardous Materials for Interstate-75 (I-75) (North and South Bound lanes) conducted by Western Kentucky University in partnership with the Madison County (Kentucky) Local Emergency Planning Committee (LEPC). The only Kentucky County within the study area is Madison County as shown in Figure 1.1. The purpose of report is to give information on patterns of hazardous materials being transported along I-75 as observed from July 25th 2011 to August 5th 2011. A secondary purpose is to summarize incidents involving hazardous materials over the previous 6 years (January 2006 – June 2011). Finally, this report assesses survey information collected from fixed facilities that ship and receive hazardous materials in the I-75 highway. Commodity flow analysis is necessary in order for the LEPC to prepare for future hazardous material releases that may occur along this section of I-75. Data collected from this study will aid the emergency planning process for specific hazardous materials that were observed to frequent the study area during the study period

    Azimuthal temperature variations in ISO-Oph2 from multi-frequency ALMA observations

    Environmental effects, such as stellar fly-bys and external irradiation, are thought to affect the evolution of protoplanetary disks in clustered star formation. Previous ALMA images at 225 GHz of the ISO-Oph 2 binary revealed a peculiar morphology in the disk of the primary, perhaps due to a possible fly-by with the secondary. Here we report on new ALMA continuum observations of this system at 97.5 GHz, 145 GHz and 405 GHz, which reveal strong morphological variations. Multi-frequency positional alignment allows to interpret these spectral variations in terms of underlying physical conditions. ISO-Oph 2A is remarkably offset from the centroid of its ring, at all frequencies, and the disk is lopsided, pointing at gravitational interactions. However, the dust temperature also varies in azimuth, with two peaks whose direction connects with HD 147889, the earliest-type star in the Ophiuchus complex, suggesting that it is the dominant heat source.     Black-White Risk Differentials in COVID-19 (SARS-COV2) Transmission, Mortality and Case Fatality in the United States: Translational Epidemiologic Perspective and Challenges

    Background: Social and health inequities predispose vulnerable populations to adverse morbidity and mortality outcomes of epidemics and pandemics. While racial disparities in cumulative incidence (CmI) and mortality from the influenza pandemics of 1918 and 2009 implicated Blacks with survival disadvantage relative to Whites in the United States, COVID-19 currently indicates comparable disparities. We aimed to: (a) assess COVID-19 CmI by race, (b) determine the Black-White case fatality (CF) and risk differentials, and (c) apply explanatory model for mortality risk differentials. Methods: COVID-19 data on confirmed cases and deaths by selective states health departments were assessed using a cross-sectional ecologic design. Chi-square was used for CF independence, while binomial regression model for the Black-White risk differentials. Results: The COVID-19 mortality CmI indicated Blacks/AA with 34% of the total mortality in the United States, albeit their 13% population size. The COVID-19 CF was higher among Blacks/AA relative to Whites; Maryland, (2.7% vs. 2.5%), Wisconsin (7.4% vs. 4.8%), Illinois (4.8% vs. 4.2%), Chicago (5.9% vs. 3.2%), Detroit (Michigan), 7.2% and St. John the Baptist Parish (Louisiana), 7.9%. Blacks/AA compared to Whites in Michigan were 15% more likely to die, CmI risk ratio (CmIRR) = 1.15, 95% CI, 1.01-1.32. Blacks/AA relative to Whites in Illinois were 13% more likely to die, CmIRR = 1.13, 95% CI, 0.93-1.39, while Blacks/AA compared to Whites in Wisconsin were 51% more likely to die, CmIRR = 1.51, 95% CI, 1.10-2.10. In Chicago, Blacks/AA were more than twice as likely to die, CmIRR = 2.24, 95% CI, 1.36-3.88. Conclusion: Substantial racial/ethnic disparities are observed in COVID-19 CF and mortality with Blacks/AA disproportionately affected across the United States

    The global burden of adolescent and young adult cancer in 2019 : a systematic analysis for the Global Burden of Disease Study 2019

    Background In estimating the global burden of cancer, adolescents and young adults with cancer are often overlooked, despite being a distinct subgroup with unique epidemiology, clinical care needs, and societal impact. Comprehensive estimates of the global cancer burden in adolescents and young adults (aged 15-39 years) are lacking. To address this gap, we analysed results from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019, with a focus on the outcome of disability-adjusted life-years (DALYs), to inform global cancer control measures in adolescents and young adults. Methods Using the GBD 2019 methodology, international mortality data were collected from vital registration systems, verbal autopsies, and population-based cancer registry inputs modelled with mortality-to-incidence ratios (MIRs). Incidence was computed with mortality estimates and corresponding MIRs. Prevalence estimates were calculated using modelled survival and multiplied by disability weights to obtain years lived with disability (YLDs). Years of life lost (YLLs) were calculated as age-specific cancer deaths multiplied by the standard life expectancy at the age of death. The main outcome was DALYs (the sum of YLLs and YLDs). Estimates were presented globally and by Socio-demographic Index (SDI) quintiles (countries ranked and divided into five equal SDI groups), and all estimates were presented with corresponding 95% uncertainty intervals (UIs). For this analysis, we used the age range of 15-39 years to define adolescents and young adults. Findings There were 1.19 million (95% UI 1.11-1.28) incident cancer cases and 396 000 (370 000-425 000) deaths due to cancer among people aged 15-39 years worldwide in 2019. The highest age-standardised incidence rates occurred in high SDI (59.6 [54.5-65.7] per 100 000 person-years) and high-middle SDI countries (53.2 [48.8-57.9] per 100 000 person-years), while the highest age-standardised mortality rates were in low-middle SDI (14.2 [12.9-15.6] per 100 000 person-years) and middle SDI (13.6 [12.6-14.8] per 100 000 person-years) countries. In 2019, adolescent and young adult cancers contributed 23.5 million (21.9-25.2) DALYs to the global burden of disease, of which 2.7% (1.9-3.6) came from YLDs and 97.3% (96.4-98.1) from YLLs. Cancer was the fourth leading cause of death and tenth leading cause of DALYs in adolescents and young adults globally. Interpretation Adolescent and young adult cancers contributed substantially to the overall adolescent and young adult disease burden globally in 2019. These results provide new insights into the distribution and magnitude of the adolescent and young adult cancer burden around the world. With notable differences observed across SDI settings, these estimates can inform global and country-level cancer control efforts. Copyright (C) 2021 The Author(s).     Sirenomelia- A rare congenital anomaly: Case report

    Sirenomelia is an extremely uncommon congenital deformity in which neonate has fused lower limbs associated with various visceral malformations including urogenital and gastrointestinal tract, pulmonary hypoplasia, and potter's facies. The prevalence is 0.8 to 1 per 100,000 newborns. About 300 cases were recorded till date. We describe a case of 31-year G4P1L1A2 with previous lower segment caesarean section with gestational age of 22 weeks 5 days with anomaly scan suggestive of single live intrauterine pregnancy of 21 weeks 5 days with bilateral renal agenesis with placenta previa and no demonstrable amniotic fluid pocket seen. Her pregnancy was uneventful without any medical disorder, drug history, and no congenital anomaly in the family. Termination of pregnancy done by emergency hysterotomy and delivered anomalous foetus with fused lower limb with absent toes, low set ears, absent left ear ostium, imperforate anus, and absent external genitalia. Sirenomelia is fatal congenital anomaly with unclear etiology. Early antenatal diagnosis and termination of pregnancy is the treatment
