Patologia Psiquiátrica nos Cuidados Primários de Saúde
É consensual a alta prevalência de patologia psiquiátrica ao nível dos Cuidados Primários Saúde. Consensual será também o papel fulcral que estes mesmos Cuidados desempenham enquanto primeiro filtro na abordagem dos problemas psiquiátricos, reconhecendo-se que, frequentemente, o Clínico Geral é o agente melhor posicionado para uma correcta abordagem diagnóstica e terapêutica deste tipo de problemas. Assume-se assim como necessidade objectiva que o Clínico Geral tome a seu cargo a maioria destes doentes e proceda à referenciação criteriosa duma minoria verdadeiramente necessitada de cuidados mais especializados. Em termos práticos, configura-se como prioritária uma progressiva melhoria da capacidade de actuação dos Clínicos Gerais, na área da Saúde Mental. É portanto neste contexto que surge a importância da articulação dos Serviços de Psiquiatria com os Centros de Saúde e a relevância do trabalho Consiliar e de Ligação do Psiquiatra junto dos Clínicos Gerais. No presente artigo os autores tecem algumas considerações e lançam diversas questões sobre o tema em apreço, procurando abordar determinados aspectos centrais, nomeadamente, as particularidades de apresentação da patologia psiquiátrica nos Cuidados de Saúde Primários, em particular da “depressão”, e decorrendo deste facto, as dificuldades na avaliação e abordagem das perturbações mentais neste setting particular
Episiotomía, ¿Práctica electiva o rutinaria?
La episiotomía es una incisión de la pared posterior de la vagina que agranda el introito vaginal para facilitar el parto y prevenir laceraciones, las cuales son más difíciles de reparar. Hay médicos que creen que la episiotomía de rutina prevé laceraciones severas durante el parto vaginal. Por otra parte, una episiotomía garantiza trauma perineal y la utilización de suturas. METODOLOGÍA: Desde la exposición de un escenario clínico se planteó una pregunta estructurada para luego realizar una búsqueda bibliográfica con el fin de dar respuesta a la pregunta sobre si la episiotomía de rutina en mujeres con trabajo de parto disminuye el riesgo de desgarro perineal o vaginal. Se realizó una búsqueda en PubMed utilizando los Mesh (Medical Subject Headings) "episiotomy AND selective", se aplicaron distintos filtros, tras lo cual se llegó al número final de 34 artículos. Tras una lectura de los títulos y resúmenes de los artículos, se escogió el artículo "Selective versus routine use of episiotomy for vaginal birth (Review)". ANÁLISIS DEL ARTÍCULO: Nos hemos centrado en la variable sobre el trauma vaginal/perineal con el fin de responder a la pregunta clínica planteada. Para ello se realizó un análisis secundario de los datos de las distintas variables con el fin de definir los valores de la prevalencia de cada grupo, el riesgo relativo (RR), la reducción de riesgo relativo (RRA) y el número necesario a perjudicar (NNP), con sus respectivos intervalos de confianza del 95% (IC95%). RECOMENDACIÓN FINAL: Por el momento, con la evidencia disponible, se recomienda la realización de la episiotomía de forma selectiva. En mujeres donde se realizó una episiotomía selectiva hubo una menor cantidad de pacientes con trauma vaginal o perineal severo, por lo que no estaría justificado la implementación de la episiotomía de forma rutinaria, ya que no hay evidencia que justifique su uso de esta manera
Risk assessment for the spread of Serratia marcescens within dental-unit waterline systems using Vermamoeba vermiformis
Vermamoeba vermiformis is associated with the biofilm ecology of dental-unit waterlines (DUWLs). This study investigated whether V. vermiformis is able to act as a vector for potentially pathogenic bacteria and so aid their dispersal within DUWL systems. Clinical dental water was initially examined for Legionella species by inoculating it onto Legionella selective-medium plates. The molecular identity/profile of the glassy colonies obtained indicated none of these isolates were Legionella species. During this work bacterial colonies were identified as a non-pigmented Serratia marcescens. As the water was from a clinical DUWL which had been treated with Alpron™ this prompted the question as to whether S. marcescens had developed resistance to the biocide. Exposure to Alpron™ indicated that this dental biocide was effective, under laboratory conditions, against S. marcescens at up to 1x108 colony forming units/millilitre (cfu/ml). V. vermiformis was cultured for eight weeks on cells of S. marcescens and Escherichia coli. Subsequent electron microscopy showed that V. vermiformis grew equally well on S. marcescens and E. coli (p = 0.0001). Failure to detect the presence of S. marcescens within the encysted amoebae suggests that V. vermiformis is unlikely to act as a vector supporting the growth of this newly isolated, nosocomial bacterium
Novel strategies lead to pre-elimination of malaria in previously high-risk areas in Suriname, South America
<p>Abstract</p> <p>Background</p> <p>Suriname was a high malaria risk country before the introduction of a new five-year malaria control program in 2005, the Medical Mission Malaria Programme (MM-MP). Malaria was endemic in the forested interior, where especially the stabile village communities were affected.</p> <p>Case description</p> <p>The interventions of the MM-MP included new strategies for prevention, vector control, case management, behavioral change communication (BCC)/information, education and communication (IEC), and strengthening of the health system (surveillance, monitoring and evaluation and epidemic detection system). After a slow first year with non-satisfying scores for the performance indicators, the MM-MP truly engaged in its intervention activities in 2006 and kept its performance up until the end of 2009. A total of 69,994 long-lasting insecticide-treated nets were distributed and more than 15,000 nets re-impregnated. In high-risk areas, this was complemented with residual spraying of insecticides. Over 10,000 people were screened with active case detection in outbreak and high-risk areas. Additional notification points were established and the national health system was strengthened.</p> <p>Discussion and evaluation</p> <p>In the current paper, the MM-MP is evaluated both on account of the targets established within the programme and on account of its impact on the malaria situation in Suriname. Malaria vector populations, monitored in sentinel sites, collapsed after 2006 and concurrently the number of national malaria cases decreased from 8,618 in 2005 to 1,509 in 2009. Malaria transmission risk shifted from the stabile village communities to the mobile gold mining communities, especially those along the French Guiana border.</p> <p>Conclusions</p> <p>The novel strategies for malaria control introduced in Suriname within the MM-MP have led to a significant decrease in the national malaria burden. The challenge is to further reduce malaria using the available strategies as appropriate in the affected areas and populations. Elimination of malaria in the country will require a thorough understanding of transmission dynamics and a dedicated investment in key effective interventions.</p
Structuring of Bacterioplankton Diversity in a Large Tropical Bay
Structuring of bacterioplanktonic populations and factors that determine the structuring of specific niche partitions have been demonstrated only for a limited number of colder water environments. In order to better understand the physical chemical and biological parameters that may influence bacterioplankton diversity and abundance, we examined their productivity, abundance and diversity in the second largest Brazilian tropical bay (Guanabara Bay, GB), as well as seawater physical chemical and biological parameters of GB. The inner bay location with higher nutrient input favored higher microbial (including vibrio) growth. Metagenomic analysis revealed a predominance of Gammaproteobacteria in this location, while GB locations with lower nutrient concentration favored Alphaproteobacteria and Flavobacteria. According to the subsystems (SEED) functional analysis, GB has a distinctive metabolic signature, comprising a higher number of sequences in the metabolism of phosphorus and aromatic compounds and a lower number of sequences in the photosynthesis subsystem. The apparent phosphorus limitation appears to influence the GB metagenomic signature of the three locations. Phosphorus is also one of the main factors determining changes in the abundance of planktonic vibrios, suggesting that nutrient limitation can be observed at community (metagenomic) and population levels (total prokaryote and vibrio counts)
Phytochemicals as antibiotic alternatives to promote growth and enhance host health
There are heightened concerns globally on emerging drug-resistant superbugs and the lack of new antibiotics for treating human and animal diseases. For the agricultural industry, there is an urgent need to develop strategies to replace antibiotics for food-producing animals, especially poultry and livestock. The 2nd International Symposium on Alternatives to Antibiotics was held at the World Organization for Animal Health in Paris, France, December 12-15, 2016 to discuss recent scientific developments on strategic antibiotic-free management plans, to evaluate regional differences in policies regarding the reduction of antibiotics in animal agriculture and to develop antibiotic alternatives to combat the global increase in antibiotic resistance. More than 270 participants from academia, government research institutions, regulatory agencies, and private animal industries from >25 different countries came together to discuss recent research and promising novel technologies that could provide alternatives to antibiotics for use in animal health and production; assess challenges associated with their commercialization; and devise actionable strategies to facilitate the development of alternatives to antibiotic growth promoters (AGPs) without hampering animal production. The 3-day meeting consisted of four scientific sessions including vaccines, microbial products, phytochemicals, immune-related products, and innovative drugs, chemicals and enzymes, followed by the last session on regulation and funding. Each session was followed by an expert panel discussion that included industry representatives and session speakers. The session on phytochemicals included talks describing recent research achievements, with examples of successful agricultural use of various phytochemicals as antibiotic alternatives and their mode of action in major agricultural animals (poultry, swine and ruminants). Scientists from industry and academia and government research institutes shared their experience in developing and applying potential antibiotic-alternative phytochemicals commercially to reduce AGPs and to develop a sustainable animal production system in the absence of antibiotics.Fil: Lillehoj, Hyun. Procollagen Triple Helix Assembly: An Unconventional Chaperone-Assisted Folding Paradigm
Fibers composed of type I collagen triple helices form the organic scaffold of bone and many other tissues, yet the energetically preferred conformation of type I collagen at body temperature is a random coil. In fibers, the triple helix is stabilized by neighbors, but how does it fold? The observations reported here reveal surprising features that may represent a new paradigm for folding of marginally stable proteins. We find that human procollagen triple helix spontaneously folds into its native conformation at 30–34°C but not at higher temperatures, even in an environment emulating Endoplasmic Reticulum (ER). ER-like molecular crowding by nonspecific proteins does not affect triple helix folding or aggregation of unfolded chains. Common ER chaperones may prevent aggregation and misfolding of procollagen C-propeptide in their traditional role of binding unfolded polypeptide chains. However, such binding only further destabilizes the triple helix. We argue that folding of the triple helix requires stabilization by preferential binding of chaperones to its folded, native conformation. Based on the triple helix folding temperature measured here and published binding constants, we deduce that HSP47 is likely to do just that. It takes over 20 HSP47 molecules to stabilize a single triple helix at body temperature. The required 50–200 µM concentration of free HSP47 is not unusual for heat-shock chaperones in ER, but it is 100 times higher than used in reported in vitro experiments, which did not reveal such stabilization
