    Revisión sistemática de los métodos diagnósticos no basados en cultivo para candidiasis invasora en pacientes adultos no neutropénicos críticamente enfermos

    Las infecciones invasoras por especies del genero Candida constituyen entidades en aumento en diferentes poblaciones específicas de pacientes. Una de estas poblaciones de pacientes en las que la candidiasis invasora ha mostrado un incremento progresivo en sus tasas de incidencia es la de los pacientes críticamente enfermos. El bajo rendimiento de los métodos microbiológicos dirigidos al aislamiento de este hongo en muestras biológicas ha llevado a la implementación sucesiva de técnicas sucedáneas, que detectan antígenos, metabolitos o anticuerpos relacionados con el patógeno y la respuesta del hospedero. La detección y cuantificación de estos antígenos, anticuerpos y metabolitos en el escenario clínico muestra rendimientos operativos distintos, con valores más consistentes en las poblaciones de pacientes inmunocomprometidos. El objetivo del presente estudio fue el de realizar una revisión sistemática de la literatura de métodos diagnósticos para candidiasis invasora, diferentes al cultivo, y dirigidos a la población de pacientes en cuidado critico no neutropénicos. Se realizó una revisión cualitativa de aquellos estudios que cumplieron con los criterios de selección y calidad, así como también meta-análisis de 4 estudios para la utilización del 1-3 β-D glucano en pacientes críticos quirúrgicos.Abstract. . Invasive infections by species of the genera Candida have become entities of growing incidence in specific patient populations. One of these groups of patients in whom invasive candidiasis has shown increasing incidence is critically ill patients. The low diagnostic yield of microbiological techniques that procure the isolation of this fungus in biological samples, has opened the use of other tests, detecting antigens, metabolites or antibodies related to the pathogen itself or the host immune response. The detection and measurement of these antigens, antibodies and metabolites in clinical practice show different diagnostic yields, more consistent among immune compromised patients. The objective of this study was to make a systematic review of the literature on non-culture based diagnostic tests for invasive candidiasis, specifically in critically ill non neutropenic patients. A narrative review of those studies that fulfilled selection and quality criteria was made, and a meta-analysis was done including 4 articles with 1-3 β D glucan for diagnosis in critically ill surgical patients.Otr

    Avaliação de clusters baseados em sistemas em um chip para a computação de alto desempenho: uma revisão

    High-performance computing systems are the maximum expression in the field of processing for large amounts of data. However, their energy consumption is an aspect of great importance, which was not considered decades ago. Hence, software developers and hardware providers are obligated to approach new challenges to address energy consumption, and costs. Constructing a computational cluster with a large amount of systems on a chip can result in a powerful, ecologic platform, with the capacity to offer sufficient performance for different applications, as long as low costs and minimum energy consumption can be maintained. As a result, energy efficient hardware has an opportunity to impact upon the area of high-performance computing. This article presents a systematic review of the evaluations conducted on clusters of  ystems on a Chip for High-Performance computing in the research setting.Los sistemas de computación de alto desempeño son la máxima expresión en el campo de procesamiento para grandes cantidades de datos. Sin embargo, su consumo de energía es un aspecto de gran importancia que no era tenido en cuenta en décadas pasadas. Por lo tanto, desarrolladores de software y proveedores de hardware están obligados a enfocarse en nuevos retos para abordar el consumo de energía y costos. Construir un clúster informático con una gran cantidad de sistemas en un chip puede dar como resultado una plataforma poderosa, ecológica y capaz de ofrecer el rendimiento suficiente para diferentes aplicaciones, siempre y cuando se puedan mantener bajos costos y el menor consumo de energía posible. Como resultado, el hardware eficiente en el consumo de energía tiene la oportunidad de tener un impacto en el área de la computación de alto desempeño. En este artículo se presenta una revisión sistemática para conocer las evaluaciones realizadas a clústeres de sistemas en un chip para computación de alto desempeño en el ámbito investigativo. Os sistemas de computação de alto desempenho são a máxima expressão no campo de processamento para grandes quantidades de dados. No entanto, seu consumo de energia é um aspecto de grande importância que não era levado em consideração em décadas passadas. Portanto, desenvolvedores de software e provedores de hardware estão obrigados a focar-se em novos desafios para abordar o consumo de energia e  ustos. Construir um cluster informático com uma grande quantidade de sistemas em um chip pode dar como resultado uma plataforma poderosa, ecológica e capaz de oferecer o rendimento suficiente para diferentes aplicações, desde que possam ser mantidos baixos custos e o menor consumo de energia possível. Como resultado, o hardware eficiente no consumo de energia tem a oportunidade de ter um impacto na área da computação de alto desempenho. Neste artigo, apresenta-se uma revisão sistemática para conhecer as avaliações realizadas a clusters de sistemas em um chip para computação de alto desempenho no âmbito investigativo.&nbsp

    Prevalence of hearing loss in a population of schoolchildren with Down syndrome from Bogotá, Colombia.

    Introduction: Children with Down syndrome have a higher prevalence of hearing disorders, secondary to anatomical and physiological variations. No universal hearing screening is implemented in Colombia, so there are no data available on the prevalence of hearing loss in this population. The objective of this study was to determine the frequency and type of such disorders in this population based on hearing tests.Patients and methods: This was a cross-sectional study conducted at two institutions specialized in accompanying and supporting the families of children with Down syndrome in Bogota, D.C., Colombia. Between October 2017 and September 2018, a behavioral and / or pure-tone audiometry and an impedance audiometry were done in 40 and 37 children respectively. Subjects attended the institutions twice a week and were integrated at school, up to fifth grade. Prevalence was estimated and characteristics, clinical history, and language developmental milestones were described.Results: The sample was made up of 40 children (22 girls) aged 6-18 years (mean: 11.23). A history of otitis was found in 19 children and of mild to moderate hearing loss, in 17, predominately conductive. A delay in language developmental milestones was observed, apparently not associated with hearing loss.Conclusions: A sample of children with Down syndrome from Bogota showed a high prevalence of hearing loss, consistent with the results of previous studies. These data stress the need for a strict control of hearing status based on standardized parameters and guidelines

    Introducción: Los niños con síndrome de Down presentan mayor prevalencia de alteraciones de la capacidad auditiva, secundaria a variaciones anatómicas y fisiológicas. En Colombia, no se realiza tamizaje auditivo universal, y no hay datos sobre la prevalencia de hipoacusia en esta población. El propósito de este estudio fue determinar la frecuencia y tipo de estas alteraciones en nuestra población por medio de evaluación audiológica. Pacientes y métodos: Se realizó un estudio de corte trasversal en dos instituciones especializadas en el acompañamiento y terapia de familias de niños con síndrome de Down en Bogotá D. C., Colombia. Entre octubre de 2017 y septiembre de 2018, se realizó audiometría comportamental y/o tonal e impedanciometría en 40 y en 37 niños respectivamente. Los sujetos asistían a las instituciones dos veces por semana y estaban integrados en diferentes grados escolares hasta quinto de primaria. Se estimó la prevalencia y se describieron las características, antecedentes clínicos e hitos del desarrollo del lenguaje. Resultados: Se obtuvo una muestra de 40 niños (22 mujeres) con edades entre 6 y 18 años (media: 11,23). Se encontró antecedente de otitis en 19 sujetos e hipoacusia de leve a moderada en 17 niños, predominantemente, conductiva. Hubo retraso en hitos del desarrollo del lenguaje, en apariencia, no asociado a hipoacusia. Conclusiones: En una muestra de niños con síndrome de Down en Bogotá, se encontró una alta prevalencia de hipoacusia, consistente con resultados de estudios previos. Estos datos resaltan la necesidad de un control estricto de la capacidad auditiva basado en parámetros y guías estandarizadas

    Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)

    Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters. Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs). Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001). Conclusions: Tocilizumab might be useful in COVID-19 patients with a hyperinflammatory state and should be prioritized for randomized trials in this situatio

    El sistema web para la gestión del registro de ingreso y salida en los parqueaderos de la Facultad tecnológica (automóviles, motocicletasy bicicletas) de la universidad distrital Francisco José de caldas, hará posible que por medio del registro con huella dactilar de los usuarios se logre optimizar el proceso que actualmente se está llevando en el parqueadero de ciclas, de motos y de vehículos, de una manera eficiente brindando beneficios para el personal a cargo de dichos espacios, la generación de reportes y rápido acceso a la información registrada serán características del sistema de información. Para el desarrollo de este sistema se implementó Las herramientas a utilizar para la elaboración en el sistema de información son Asp.net como estructura base de la plataforma cuya sintaxis permite un amplio trabajo lógico y de interfaz, la explotación del lenguaje Asp.net es llevada a cabo por el entorno de trabajo que ofrece Visual Studio.
