    Changes in population characteristics and their implication on public health research

    Population estimates are generally drawn from one point in time to study disease trends over time; changes in population characteristics over time are usually not assessed and included in the study design. We evaluated whether population characteristics remained static and assessed the degree of population shifts over time. The analysis was based on the New York State 1990 and 2000 census data with adjustments for changes in geographic boundaries. Differences in census tract information were quantified by calculating the mean, median, standard deviation, and the percent of change for each population characteristic. Between 1990 and 2000, positive and negative fluctuations in population size created a U-shaped bimodal pattern of population change which increased the disparities in demographics and socioeconomic status for many census tracts. While 268 (10%) census tracts contracted by 10%, twice as many census tracts (21%, N = 557) grew at least 10%. Notably, the non-Hispanic African-American population grew 10% or more in 152 tracts. Although there were overall reductions in working class and undereducated populations and gains in incomes, most census tracts experienced growing income inequalities and an increased poverty rate. These changes were most pronounced in urban census tracts. Differences in population characteristics in a decade showed growing disparities in demographics and socioeconomic status. This study elucidates that important population shifts should be taken into account when conducting longitudinal research

    Charting a Path to Location Intelligence for STD Control

    This article describes the New York State Department of Health's GeoDatabase project, which developed new methods and techniques for designing and building a geocoding and mapping data repository for sexually transmitted disease (STD) control. The GeoDatabase development was supported through the Centers for Disease Control and Prevention's Outcome Assessment through Systems of Integrated Surveillance workgroup. The design and operation of the GeoDatabase relied upon commercial-off-the-shelf tools that other public health programs may also use for disease-control systems. This article provides a blueprint of the structure and software used to build the GeoDatabase and integrate location data from multiple data sources into the everyday activities of STD control programs

    Braving difficult choices alone: children's and adolescents' medical decision making.

    OBJECTIVE: What role should minors play in making medical decisions? The authors examined children's and adolescents' desire to be involved in serious medical decisions and the emotional consequences associated with them. METHODS: Sixty-three children and 76 adolescents were presented with a cover story about a difficult medical choice. Participants were tested in one of four conditions: (1) own informed choice; (2) informed parents' choice to amputate; (3) informed parents' choice to continue a treatment; and (4) uninformed parents' choice to amputate. In a questionnaire, participants were asked about their choices, preference for autonomy, confidence, and emotional reactions when faced with a difficult hypothetical medical choice. RESULTS: Children and adolescents made different choices and participants, especially adolescents, preferred to make the difficult choice themselves, rather than having a parent make it. Children expressed fewer negative emotions than adolescents. Providing information about the alternatives did not affect participants' responses. CONCLUSIONS: Minors, especially adolescents, want to be responsible for their own medical decisions, even when the choice is a difficult one. For the adolescents, results suggest that the decision to be made, instead of the agent making the decision, is the main element influencing their emotional responses and decision confidence. For children, results suggest that they might be less able than adolescents to project how they would feel. The results, overall, draw attention to the need to further investigate how we can better involve minors in the medical decision-making process

    Triggered aseismic fault slip from nearby earthquakes, static or dynamic effect?

    Observations show that an earthquake can affect aseismic slip behavior of nearby faults and produce “triggered aseismic fault slip.” Two types of stress changes are often examined by researchers as possible triggering sources. One is the static stress change associated with the faulting process and the other is the dynamic stress change or transient deformation generated by the passage of seismic waves. No consensus has been reached, however, regarding the mechanism(s) of triggered aseismic fault slip. We evaluate the possible triggering role of static stress changes by examining observations made after 10 large earthquakes in California. Most of the nearby fault segments that slipped aseismically were encouraged to move by the imposed positive changes in static Coulomb Failure Stress (CFS). Nonetheless, three discrepancies or failures with this model exist, which implies that static stress triggering either is or is not the sole mechanism causing the observed triggered slip. We then use a spring-slider system as a simplified fault model to study its slip behavior and the impact of transient (dynamic) loading on it. We show that a two-state-variable rate-dependent and state-dependent frictional law can generate creep events. Transient loads are then put into the system. Certain types of them can cause a large time advance of (or trigger) the next creep event. While our work examines triggered creep events near the surface, it may well have implications for the occurrence of similar events near the bottom of the seismogenic zone where a transition in frictional stability occurs

    Peer review of research articles is a core part of our scholarly communication system. In spite of its importance, the status and purpose of peer review is often contested. What is its role in our modern digital research and communications infrastructure? Does it perform to the high standards with which it is generally regarded? Studies of peer review have shown that it is prone to bias and abuse in numerous dimensions, frequently unreliable, and can fail to detect even fraudulent research. With the advent of web technologies, we are now witnessing a phase of innovation and experimentation in our approaches to peer review. These developments prompted us to examine emerging models of peer review from a range of disciplines and venues, and to ask how they might address some of the issues with our current systems of peer review. We examine the functionality of a range of social Web platforms, and compare these with the traits underlying a viable peer review system: quality control, quantified performance metrics as engagement incentives, and certification and reputation. Ideally, any new systems will demonstrate that they out-perform and reduce the biases of existing models as much as possible. We conclude that there is considerable scope for new peer review initiatives to be developed, each with their own potential issues and advantages. We also propose a novel hybrid platform model that could, at least partially, resolve many of the socio-technical issues associated with peer review, and potentially disrupt the entire scholarly communication system. Success for any such development relies on reaching a critical threshold of research community engagement with both the process and the platform, and therefore cannot be achieved without a significant change of incentives in research environments
