    Epidemiología de las lesiones del baloncesto de los últimos 22 años

    Introducción: El baloncesto es un deporte de contacto con movimientos complejos que incluyen saltos, giros y cambios de dirección donde el esguince de tobillo es de las lesiones más comunes. De todas formas, en nuestra práctica clínica se observa una elevada frecuencia de lesiones musculares. Actualmente, se dispone de poca información sobre la magnitud de este problema en equipos profesionales. Tampoco se conoce en detalle cómo puede utilizarse dicha información para diseñar estrategias útiles para así manejar y prevenir cada una de este tipo de lesiones. Objetivos: Reportar, informar y describir las lesiones de un equipo profesional de baloncesto durante 22 temporadas. Además, en esta tesis se pretende describir el patrón de las tasas de lesiones musculares y de tobillo en un equipo profesional de baloncesto durante 9 temporadas. Metodología: La presente tesis se centra principalmente en dos estudios, los están en fase de publicación en revistas indexadas del Journal Citation Reports (JCR). Cada uno de los estudios presenta una metodología propia con una línea común: la epidemiología lesional en el baloncesto profesional. Resultados: Los resultados de la tesis son los resultados de cada uno de los dos estudios (artículos) que la componen: 1) En las 22 temporadas el fisioterapeuta reportó un total de 3452 lesiones, de las cuales 310 causaron baja (“time-loss”). La localización más frecuente fue en la extremidad inferior con un total de 1.799 lesiones (52,1%). El tipo de lesión más frecuente fue las musculares y tendinosas con un total de 2.168 lesiones (63,1%), seguidas de las ligamentosas y articulares (n=853;24,8%). Por posición, los pívots presentaron más lesiones (n=1.046; 29,6%). 2) En 9 temporadas se analizaron 463 lesiones donde la tasa global era de 10.8 lesiones por 1000 jugadores-hora (95%CI: 9.9-11.9). La razón de tasas de incidencia de lesiones musculares en todo el período de estudio era 1.8 veces más grande (95%CI: 1.28 - 2.49) que la de las tasas lesiones de tobillo. Conclusiones: El tipo de lesión más frecuente en el baloncesto profesional son las lesiones musculo-tendinosas y los esguinces ligamentosos de tobillo y rodilla. En esta tesis, la incidencia lesional de las lesiones musculares es más alta que los esguinces de tobillo, tanto en lesiones que causan baja (“Time Loss”) como las que no (“Medical Attention”).     Development of support material for health professionals who are implementing Shared Decision-making in breast cancer screening: validation using the Delphi technique

    Background The Literature is no report support material on Shared Decision-making applied to breast cancer screening that is intended for Spanish health professionals. The researcher created both a handbook and a guide for this topic using an adaption of the Three-talk model. Objective A Delphi method will be used to reach an agreement among experts on the contents and design of a manual and guide, designed by the research team, and to be used by health professionals in the application of SDM in breast cancer screening. Design A qualitative study. The content and design of the handbook and the guide was discussed by 20 experts. The Delphi techniques was in an online mode between July and October 2020 and researchers used Google forms in three rounds with open and closed questions. The criterion established for consensus was a coefficient of concordance (Cc) above 75, for questions using a Likert scale of 1-6-in which 1 meant 'completely disagree' and 6 'completely agree'-with a cut-off point equal to or higher than 4. Results Participants considered the Three-talk model suitable for the screening context. The handbook sections and level of detail were considered satisfactory (Cc=90). The summary provided by the clinical practice guide was considered necessary (Cc=75), as it was the self-assessment tool for professionals (Cc=85). Content was added: addressing the limitations of the SDM model; extending the number of sample dialogues for health professionals; providing supplementary resources on using Patient Decisions aids and adding references on communication skills. Conclusions and applications The first handbook and clinical practice guide providing unique SDM support material for health professionals have been developed. The handbook and guide are useful and innovative as supporting material for health professionals, but training strategies for SDM and a piloting plan for the use of materials are requested, in order to facilitate its implementation

    Women’s preference to apply shared decision-making in breast cancer screening: a discrete choice experiment

    ObjectiveTo analyse women's stated preferences for establishing the relative importance of each attribute of shared decision-making (SDM) and their willingness to pay (WTP) for more participatory care in breast cancer screening programmes (BCSP). DesignA discrete choice experiment was designed with 12 questions (choice tasks). It included three attributes: 'How the information is obtained', regarding benefits and harms; whether there is a 'Dialogue for scheduled mammography' between the healthcare professional and the woman; and, 'Who makes the decision', regarding participation in BCSP. Data were obtained using a survey that included 12 choice tasks, 1 question on WTP and 7 socioeconomic-related questions. The analysis was performed using conditional mixed-effect logit regression and stratification according to WTP. SettingData collection related to BCSP was conducted between June and November 2021 in Catalonia, Spain. ParticipantsSixty-five women aged between 50 and 60. Main outcome measuresWomen's perceived utility of each attribute, trade-off on these attributes and WTP for SDM in BCSP. ResultThe only significant attribute was 'Who makes the decision'. The decision made alone (coefficient=2.879; 95% CI=2.297 to 3.461) and the decision made together with a healthcare professional (2.375; 95% CI=1.573 to 3.177) were the options preferred by women. The former contributes 21% more utility than the latter. Moreover, 52.3% of the women stated a WTP of Euro10 or more for SDM. Women's preferences regarding attributes did not influence their WTP.ConclusionsThe participant women refused a current paternalistic model and preferred either SDM or informed decision-making in BCSP

    Life table analysis of Diaphorina citri (Hemiptera: Psyllidae) infesting sweet orange (Citrus sinensis) in São Paulo

    An ecological life table for eggs and nymphs of Diaphorina citri Kuwayama (Hemiptera: Psyllidae) was constructed with data obtained from orange orchards (Citrus sinensis Osbeck) in 2 regions of the State of São Paulo, over 4 generations in the period from XI-2006 to V-2007, comprising spring, summer, and fall seasons. Young growing shoots with D. citri eggs present were identifed, and live individuals were counted until adult emergence. No predatory arthropods were observed in association with D. citri eggs and nymphs during the study. The mean parasitism of fourth- and ffth-instar nymphs by Tamarixia radiata Waterston (Hymenoptera: Eulophidae) was 2.3%. The durations of the egg–adult period were similar among the 4 generations, ranging from 18.0 to 24.7 d (at mean temperatures ranging from 21.6 to 26.0 °C) and followed the temperature requirement models obtained in the laboratory for D. citri. However, survival from the egg to the adult stage for the same period varied considerably from 1.7 to 21.4%; the highest mortalities were observed in the egg and small nymphal (frst- to thirdinstar) stages, which were considered to be key phases for population growth of the pest.Uma tabela de vida ecológica foi construída para ovos e ninfas de Diaphorina citri Kuwayama (Hemiptera: Psyllidae) com dados obtidos em pomares de laranja (Citrus sinensis Osbeck) em 2 regiões do estado de São Paulo, com 4 gerações, no período de novembro de 2006 a maio de 2007, compreendendo as estações de primavera, verão e outono. Ramos jovens em crescimento com a presença de ovos de D. citri foram identificados e os indivíduos vivos foram contados até a emergência dos adultos. Nenhum predador foi observado associado a ovos e ninfas de D. citri durante o estudo. A taxa média de parasitismo de ninfas de quarto e quinto ínstares por Tamarixia radiata Waterson (Hymenoptera: Eulophidae) foi de 2.3%. A duração do período de ovo a adulto foi semelhante entre as quatro gerações, variando de 18.0 a 24.7 dias (com temperaturas médias de 21.6 a 26.0 °C) e seguiram os modelos de exigencias térmicas obtidas em laboratório para D. citri. Todavia, a sobrevivencia de ovo até o estágio adulto variou consideravelmente para o mesmo período, de 1.7 a 21.4%, sendo que as maiores mortalidades foram observadas nos estágios de ovos e ninfas pequenas (de primeiro a terceiro ínstares), as quais foram consideradas fases chaves para o crescimento populacional desta praga.info:eu-repo/semantics/publishedVersio

    How hepatitis C virus modifies the immunological profile of Sjögren syndrome: analysis of 783 patients.

    Introduction: We conducted a study to analyze how infection by hepatitis C virus (HCV) may influence the immunological serum pattern of patients with Sjögren syndrome (SS). Methods: Since 1994, we have tested serum HCV-IgG antibodies in 783 patients with SS diagnosed according to the 1993 European classification criteria. The immunological profile at diagnosis was compared according to the presence or absence of HCV. Results: Of the 783 patients with SS, 105 (13.4 %) tested positive for HCV-IgG antibodies (88 females, 17 males,mean age at SS diagnosis: 62.9 years). Multivariate analysis showed that patients with SS-HCV had a higher mean age and a higher frequency of low C3/C4 levels, cryoglobulins, and hematological neoplasia compared with patients without HCV. The frequency of anti-La antibodies compared with anti-Ro antibodies was higher in patients with SS-HCV (17 % vs. 15 %) and lower in patients without HCV infection (30 % vs. 43 %). The frequency of concomitant detection of the three main cryoglobulin-related markers (cryoglobulins, rheumatoid factor activity, and C4 consumption) was threefold higher in patients with SS-HCV compared with patients without HCV. SS-HCV patients with genotype 1b showed the highest frequencies of immunological abnormalities related to cryoglobulins and the lowest frequencies of anti-Ro/La antibodies. Conclusions: We found HCV infection in 13 % of a large series of Spanish patients with SS. The HCV-driven autoimmune response was characterized by a lower frequency of anti-Ro/La antibodies, an abnormal predominance of anti-La among anti-Ro antibodies, and a higher frequency of cryoglobulinemic-related immunological markers in comparison with patients without HCV infection. This immunological pattern may contribute to the poor outcomes found in patients with SS-HCV

    Expression atlas of the Deubiquitinating enzymes in the adult mouse retina, their evolutionary diversification and phenotypic roles

    Ubiquitination is a relevant cell regulatory mechanism to determine protein fate and function. Most data has focused on the role of ubiquitin as a tag molecule to target substrates to proteasome degradation, and on its impact in the control of cell cycle, protein homeostasis and cancer. Only recently, systematic assays have pointed to the relevance of the ubiquitin pathway in the development and differentiation of tissues and organs, and its implication in hereditary diseases. Moreover, although the activity and composition of ubiquitin ligases has been largely addressed, the role of the deubiquitinating enzymes (DUBs) in specific tissues, such as the retina, remains mainly unknown. In this work, we undertook a systematic analysis of the transcriptional levels of DUB genes in the adult mouse retina by RT-qPCR and analyzed the expression pattern by in situ hybridization and fluorescent immunohistochemistry, thus providing a unique spatial reference map of retinal DUB expression. We also performed a systematic phylogenetic analysis to understand the origin and the presence/absence of DUB genes in the genomes of diverse animal taxa that represent most of the known animal diversity. The expression landscape obtained supports the potential subfunctionalization of paralogs in those families that expanded in vertebrates. Overall, our results constitute a reference framework for further characterization of the DUB roles in the retina and suggest new candidates for inherited retinal disorders

    Actividades de divulgación del patrimonio geológico en geoparques

    p. 61-68En los últimos años se ha producido un incremento en la consideración del patrimonio geológico como recurso que enfatiza el valor científico, educativo y recreativo de la geología. Una de las consecuencias de este crecimiento es la delimitación de áreas con estrategias de desarrollo socioeconómico y cultural centradas en la gea. Este es el caso de los territorios con categoría de parques geológicos y geoparques, en los cuales es habitual el desarrollo de diferentes estrategias de divulgación geológica que pueden ser útiles en la enseñanza formal de la geología. En este artículo se describen cuatro experiencias de divulgación geológica desarrolladas en territorios con alguna de estas categorías: el Geoparque de la Costa Vasca, la Fundación Dinópolis de Teruel, el Geoparque de la Comarca de Molina de Aragón y del Alto Tajo, y el Parque geológico de Chera.S

    Modulation of food intake by differential TAS2R stimulation in rat

    Metabolic surgery modulates the enterohormone profile, which leads, among other effects, to changes in food intake. Bitter taste receptors (TAS2Rs) have been identified in the gastrointestinal tract and specific stimulation of these has been linked to the control of ghrelin secretion. We hypothesize that optimal stimulation of TAS2Rs could help to modulate enteroendocrine secretions and thus regulate food intake. To determine this, we have assayed the response to specific agonists for hTAS2R5, hTAS2R14 and hTAS2R39 on enteroendocrine secretions from intestinal segments and food intake in rats. We found that hTAS2R5 agonists stimulate glucagon-like peptide 1 (GLP-1) and cholecystokinin (CCK), and reduce food intake. hTAS2R14 agonists induce GLP1, while hTASR39 agonists tend to increase peptide YY (PYY) but fail to reduce food intake. The effect of simultaneously activating several receptors is heterogeneous depending on the relative affinity of the agonists for each receptor. Although detailed mechanisms are not clear, bitter compounds can stimulate differentially enteroendocrine secretions that modulate food intake in rats
