    Risk of exposure to chemical compounds In wood processing workers

    Objetivos: identificar los riesgos por exposición a disolventes orgánicos y las manifestaciones en sus  condiciones de salud de una muestra de trabajadores dedicados a la actividad de transformación de la  madera; a partir de valoración de matriz de riesgo y encuesta de identificación sociodemográfica en un  sector de economía informal (trabajadores por cuenta propia) en Sincelejo, año 2010. Materiales y Métodos: estudio descriptivo observacional transversal en una población laboral total de 132 trabajadores, de los que  finalmente 55 fueron seleccionados por cumplir los criterios de inclusión exigidos: antigüedad en el oficio (5  años o más), consentimiento para ser parte del estudio y uso con efectos estadísticos de sus datos. La  información se recolectó mediante la aplicación de la matriz de identificación de peligro (Guía Técnica  Colombiana 045) y del formato de identificación de condiciones sociodemográficas y salud; el análisis fue  efectuado a través del programa estadístico Epi-Info 3.5.3. Resultados: se encontró riesgo por exposición a  los siguientes disolventes orgánicos: laca (52,7%), resina (57%), tintilla (20%), formol (12,7%), pinturas  (70,9%),  barnices (40,6%), thiner (70,9%), gasolina (87,3%), petróleo (21,8%), goma (61,8%), sellador (58,2%),  esmalte (43,6%) y mate (41,8%). Las manifestaciones clínicas detectadas y la afectación a sus condiciones de  salud referidas han sido: cefalea (38,2%), insomnio (14,5%), alergias (18,2%), dificultad respiratoria (16,4%),  alteraciones dermatológicas (14,5%), parestesia (32,7%), convulsiones (1,8%) y desmayos (1,8%).  Conclusiones: se comprobó la existencia de riesgo por exposición a disolventes orgánicos, inadecuada  protección específica colectiva e individual y clínica derivada de la exposición; se recomienda ampliar la  investigación con muestras poblacionales mayores, control de la clínica y la adopción de medidas  preventivas, así como análisis de la relación causa-efecto, y seguimiento mediante vigilancia epidemiológica  del colectivo afectado.Objectives: to identify risks from exposure to organic solvents and demonstrations in their health condition  in a sample of workers engaged in the activity of wood processing, from the assessment of risk matrix and  socialdemographic identify survey in an area of informal (self-employed) economy in Sincelejo in 2010.  Materials and Methods: observational cross-sectional descriptive study in a total workforce of 132 workers,  of which 55 were finally selected to meet the required inclusion criteria: seniority in office (5 years or more),  consent to be part of the study and to use their data for statistical purposes. Results: risk following exposure to organic solvents was found: lacquer  (52.7%), resin (57%), dark stain (20%), formaldehyde (12.7%), paint (70.9%), varnish (40.6%), thinner (70.9%),  gasoline (87.3%), petroleum (21.8%), rubber (61.8%), sealant (58.2%), enamel (43, 6%) and matt 41.8%. The  clinical manifestations detected and affectations to their health conditions were: cephalalgia (38.2%),  insomnia (14.5%), allergies (18.2%), respiratory distress (16.4%), dermatological alterations (14.5%),  paresthesia (32.7%), seizures (1.8%) and fainting (1.8%). Conclusions: The existence of risk from exposure to  organic solvents, inadequate specific, collective. individual and clinic protection derived from exposure were  confirmed. Further research with larger sample populations, clinical control and adoption of preventive  measurements is recommended, as well as cause and effect relationship analysis, and follow up through  epidemiologic surveillance of the affected group

    Identification of Human Papillomavirus genotypes in juvenile laryngeal papillomatosis. Three-year experience in a concentration hospital in Puebla, Mexico

    uvenile laryngeal papillomatosis (JLP) is a chronic benign disease of viral etiology, with an aggressive clinical course. In Mexico, the genotypes of the Human Papilloma Virus (HPV) that cause the disease have been poorly studied. The aim of this study was to identify HPV genotypes in PLJ patients. A descriptive and retrospective study was carried out, the records of patients with PLJ treated in a hospital in Mexico were reviewed, in the period 2018-2021. HPV was identied in all patients for genomes 6, 11, 16, and 18. nine patients were included, 56% women, mean age 9.5 ± 5.7 years; seven patients registered positivity for HPV-11 and 2 for HPV-6. The average age at diagnosis was 2.35 ± 1.77 years, with an average of 12 ± 11.56 surgical procedures.     Compromising between European and US allergen immunotherapy schools: Discussions from GUIMIT, the Mexican immunotherapy guidelines

    Background: Allergen immunotherapy (AIT) has a longstanding history and still remains the only disease-changing treatment for allergic rhinitis and asthma. Over the years 2 different schools have developed their strategies: the United States (US) and the European. Allergen extracts available in these regions are adapted to local practice. In other parts of the world, extracts from both regions and local ones are commercialized, as in Mexico. Here, local experts developed a national AIT guideline (GUIMIT 2019) searching for compromises between both schools. Methods: Using ADAPTE methodology for transculturizing guidelines and AGREE-II for evaluating guideline quality, GUIMIT selected 3 high-quality Main Reference Guidelines (MRGs): the European Academy of Allergy, Asthma and Immunology (EAACI) guideines, the S2k guideline of various German-speaking medical societies (2014), and the US Practice Parameters on Allergen Immunotherapy 2011. We formulated clinical questions and based responses on the fused evidence available in the MRGs, combined with local possibilities, patient's preference, and costs. We came across several issues on which the MRGs disagreed. These are presented here along with arguments of GUIMIT members to resolve them. GUIMIT (for a complete English version, see Supplementary data) concluded the following: Results: Related to the diagnosis of IgE-mediated respiratory allergy, apart from skin prick testing complementary tests (challenges, in vitro testing and molecular such as species-specific allergens) might be useful in selected cases to inform AIT composition. AIT is indicated in allergic rhinitis and suggested in allergic asthma (once controlled) and IgE-mediated atopic dermatitis. Concerning the correct subcutaneous AIT dose for compounding vials according to the US school: dosing tables and formula are given; up to 4 non-related allergens can be mixed, refraining from mixing high with low protease extracts. When using European extracts: the manufacturer's indications should be followed; in multi-allergic patients 2 simultaneous injections can be given (100% consensus); mixing is discouraged. In Mexico only allergoid tablets are available; based on doses used in all sublingual immunotherapy (SLIT) publications referenced in MRGs, GUIMIT suggests a probable effective dose related to subcutaneous immunotherapy (SCIT) might be: 50–200% of the monthly SCIT dose given daily, maximum mixing 4 allergens. Also, a table with practical suggestions on non-evidence-existing issues, developed with a simplified Delphi method, is added. Finally, dissemination and implementation of guidelines is briefly discussed, explaining how we used online tools for this in Mexico. Conclusions: Countries where European and American AIT extracts are available should adjust AIT according to which school is followed

    Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)

    Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters. Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs). Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001). Conclusions: Tocilizumab might be useful in COVID-19 patients with a hyperinflammatory state and should be prioritized for randomized trials in this situatio

    Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people.     Este volumen acoge la memoria académica de la Cuarta edición del Congreso Internacional de Ciencia, Tecnología e Innovación para la Sociedad, CITIS 2017, desarrollado entre el 29 de noviembre y el 1 de diciembre de 2017 y organizado por la Universidad Politécnica Salesiana (UPS) en su sede de Guayaquil. El Congreso ofreció un espacio para la presentación, difusión e intercambio de importantes investigaciones nacionales e internacionales ante la comunidad universitaria que se dio cita en el encuentro. El uso de herramientas tecnológicas para la gestión de los trabajos de investigación como la plataforma Open Conference Systems y la web de presentación del Congreso http://citis.blog.ups.edu.ec/, hicieron de CITIS 2017 un verdadero referente entre los congresos que se desarrollaron en el país. La preocupación de nuestra Universidad, de presentar espacios que ayuden a generar nuevos y mejores cambios en la dimensión humana y social de nuestro entorno, hace que se persiga en cada edición del evento la presentación de trabajos con calidad creciente en cuanto a su producción científica.
