A study of low level nitrogen laser therapy in the treatment of non-responding tubercular lymphadenopathy and sinuses
Forty-four cases of tubercular lymphadenopathy and sinuses, who were taking anti-tubercular treatment for more than 6 months and not responding to it, were randomly selected in this study. Overall cure rate in cases of lymphadenopathy was 93.99% , sinus 77.77% and cold abscess 100%. Mean age of the patients was 30.13 (Male: female1:4.5). Most common site of lymphadenopathy was cervical , smear was positive in 19(43.18%) cases and culture in 25(56.81%) cases. Low Level Nitrogen Laser Therapy may be used as an adjuvant to anti-tubercular drugs in cases of chronic non-responding tubercular lymphadenopathy and sinuses
Design and baseline characteristics of the finerenone in reducing cardiovascular mortality and morbidity in diabetic kidney disease trial
Background: Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials.
Patients and Methods: The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate >= 25 mL/min/1.73 m(2) and albuminuria (urinary albumin-to-creatinine ratio >= 30 to <= 5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level alpha = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure.
Conclusions: FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen.

A cross-sectional observational Study to assess the factors responsible for resolution of tubercular pleural effusion
Tuberculous pleuritis is the most common form of EPTB and common cause of pleural effusion in India. TPE is more common in young healthy adults, results from Mycobacterium tuberculosis infection of the pleura, characterized by an intense chronic accumulation of fluid and inflammatory cells in pleural space. A total of 200 patients were selected whom are having Lymphocytic Exudative non malignant pleural effusion with ADA >70 or histopathologically diagnosed TB pleural effusionand not on ATT. There is preponderance of male gender among the patients 69.5% were male 30.5% were female. The mean age of participant is 36.96 ±3.01 yearsmost of participant were belong to middle and lower class. Smoking, preexisting diabetes, hypertensionand low BMI affect the resolution. Our study showed that the good economic status, young age, normal BMI help in resolving the pleural effusion
Retroacetabular Stress-shielding in THA
We conducted a randomized clinical trial to compare periacetabular bone density changes after total hip arthroplasty using press-fit components with soft and hard liner materials. Bone density changes were assessed using quantitative computed tomography-assisted osteodensitometry. Twenty press-fit cups with alumina ceramic liners and 20 press-fit cups with highly cross-linked polyethylene liners were included; the nonoperated contralateral side was used as the control. Computed tomography scans were performed postoperatively and 1Â year after the index operation. At the 1-year followup, we found no differences of periacetabular bone density changes between the alumina and polyethylene liner cohorts. However, we observed marked periacetabular cancellous bone density loss (up to â34%) in both cohorts. In contrast, we observed only moderate cortical bone density changes. The decrease of periacetabular cancellous bone density with retention of cortical bone density after THA suggests stress transfer to the cortical bone
Frequency of Missed Findings on Chest Radiographs (CXRs) in an International, Multicenter Study: Application of AI to Reduce Missed Findings
Background: Missed findings in chest X-ray interpretation are common and can have serious consequences. Methods: Our study included 2407 chest radiographs (CXRs) acquired at three Indian and five US sites. To identify CXRs reported as normal, we used a proprietary radiology report search engine based on natural language processing (mPower, Nuance). Two thoracic radiologists reviewed all CXRs and recorded the presence and clinical significance of abnormal findings on a 5-point scale (1—not important; 5—critical importance). All CXRs were processed with the AI model (Qure.ai) and outputs were recorded for the presence of findings. Data were analyzed to obtain area under the ROC curve (AUC). Results: Of 410 CXRs (410/2407, 18.9%) with unreported/missed findings, 312 (312/410, 76.1%) findings were clinically important: pulmonary nodules (n = 157), consolidation (60), linear opacities (37), mediastinal widening (21), hilar enlargement (17), pleural effusions (11), rib fractures (6) and pneumothoraces (3). AI detected 69 missed findings (69/131, 53%) with an AUC of up to 0.935. The AI model was generalizable across different sites, geographic locations, patient genders and age groups. Conclusion: A substantial number of important CXR findings are missed; the AI model can help to identify and reduce the frequency of important missed findings in a generalizable manner
Quantitative CT-assisted osteodensitometry of femoral adaptive bone remodelling after uncemented total hip arthroplasty
The aim of this prospective study was to measure bone density changes and to assess adaptive bone remodelling after uncemented total hip arthroplasty with a taper-design femoral component using quantitative computer-tomography-assisted osteodensitometry. This method is able to differentiate cortical and cancellous bone structures. Twenty-seven consecutive patients (29 hips) with degenerative joint disease were enrolled in the study. Serial clinical, radiological and CT-osteodensitometry assessments were performed after the index operation. At the 2-year follow-up, the clinical outcome was rated satisfactory in all hips. The radiological assessment showed signs of osteointegration and stable fixation of all cups and stems. We observed a â17% decrease of cortical bone density and â22% decrease of cancellous bone density in the greater trochanter and femoral neck region. Cortical and cancellous bone density decrease at the level of the lesser trochanter was â9% and respectively â4%. We observed small changes of cortical bone density in the diaphyseal regions; in contrast, cancellous bone density increased (range 6% to 27%) in the diaphyseal regions. Overall, a trend of bone density recovery was observed throughout the follow-up period. Periprosthetic bone density changes at the 2-year follow-up are suggestive of stable osteointegration with proximal femoral diaphysis load transfer and moderate metaphyseal stress-shielding
