    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    Las relaciones de América latina con Estados Unidos parecieran congelarse alrededor de la muralla que separa al país del norte con México. Trump definió su política hacia la región a través de sus ataques obsesivos contra todo lo que representa la cultura mexicana y, por extensión, de América Latina. La muralla hay que extenderla y hacerla más impenetrable no tanto para frenar la migración laboral del sur o para controlar el tráfico de drogas ilícitas. El inquilino de la Casa Blanca quiere levantar la muralla como símbolo para evitar el ingreso a Estados Unidos de los antivalores que representa la herencia mestiza (hispana o latina). El libro Estados Unidos y la nueva correlación de fuerzas internacionales contiene unos 20 artículos de especialistas de 9 países de la región latinoamericana. Son trabajos preparados antes de las elecciones de Estados Unidos de 2016. Sin embargo, todos presentan el escenario en que los norteamericanos fueron a las urnas y sus análisis permiten entender mejor el triunfo electoral de Trump. Incluso, se desprenden de los diferentes capítulos las razones por las cuales el nuevo presidente obtuvo los votos electorales necesarios para triunfar, a pesar de perder el voto popular por un margen superior a los tres millones de sufragios

    A global metagenomic map of urban microbiomes and antimicrobial resistance

    We present a global atlas of 4,728 metagenomic samples from mass-transit systems in 60 cities over 3 years, representing the first systematic, worldwide catalog of the urban microbial ecosystem. This atlas provides an annotated, geospatial profile of microbial strains, functional characteristics, antimicrobial resistance (AMR) markers, and genetic elements, including 10,928 viruses, 1,302 bacteria, 2 archaea, and 838,532 CRISPR arrays not found in reference databases. We identified 4,246 known species of urban microorganisms and a consistent set of 31 species found in 97% of samples that were distinct from human commensal organisms. Profiles of AMR genes varied widely in type and density across cities. Cities showed distinct microbial taxonomic signatures that were driven by climate and geographic differences.
