    Biliary peritonitis due to a ruptured amebic liver abscess mimicking a periampullary tumor and liver metastases with the elevation of CA 19-9 and CA 125: a case report

    Introduction: An amebic liver abscess is the most common presentation of extraintestinal amebiasis. This condition is the result of a parasite infection caused by Entamoeba histolytica. Materials and Methods: We report a case of a 53-year-old male who presented with abdominal pain in the right upper quadrant, jaundice, and a 10-kg weight loss within a 1-month span. Results and Conclusion: A wide range of symptoms and findings in the imaging tests suggestive of neoplasia, elevated levels of CA 19-9 and CA 125, and the presentation of biliary peritonitis as a complication makes this case a challenge for its approach and management.Revisión por pare

    Place of Preoperative Treatment of Acromegaly with Somatostatin Analog on Surgical Outcome: A Systematic Review and Meta-Analysis

    Context: Transsphenoidal neurosurgery is the accepted first-line treatment of acromegaly in the majority of patients. Previous studies addressing preoperative somatostatin analog (SSA) treatment and subsequent surgical cure rates are conflicting, reporting either benefits or no significant differences. Objective: The aim of this study, based on a meta-analysis of all published reports, was to investigate whether treatment with SSA before surgery improves the surgical outcome of acromegaly. Data sources: All studies of preoperative treatment of acromegaly with SSA were systematically reviewed up to December 2011. We searched the Medline, Embase, Cochrane and Google Scholar electronic databases. Study selection: The primary endpoint was the biochemical postoperative cure rate. We identified 286 studies, out of which 10 studies (3.49%) fulfilling the eligibility criteria were selected for analysis; five retrospective studies with a control group, two prospective non-randomized trials, and three prospective controlled trials. The meta-analysis was conducted using the random-effects model. Data extraction: Data were extracted from published reports by two independent observers. Data synthesis: A borderline effect was detected in the analysis of all of the trials with control groups, with a pooled odds ratio (OR) for biochemical cure with SSA treatment of 1.62 (95% CI, 0.93-2.82). In the analysis of the three prospective controlled trials, a statistically significant effect was identified OR: 3.62 (95% CI, 1.88-6.96). Conclusions: Preoperative treatment with SSA og GH-secreting pituitary adenomas shows a significant improvement on surgical results. This meta-analysis suggests that in centers without optimal results all patients with a GH-secreting pituitary macroadenoma should be treated with a long-acting SSA prior to surgical treatment

    Synovial incorporation of polyacrylamide hydrogel after injection into normal and osteoarthritic animal joints

    Polyacrylamide hydrogel (PAAG) is a non-toxic, non-degradable synthetic product, used for years in the augmentation of soft tissues. Preliminary results in animals and humans have suggested long-lasting beneficial effects on symptoms of osteoarthritis (OA). The aim of this histopathological study was to investigate whether intra-articular injection of PAAG is integrated into synovial tissue in normal and OA animal joints, and if this integration is sustained.(A) A prospective, controlled, longitudinal study of normal knee joints injected with PAAG was performed in 10 rabbits, following the animals up to 1 year, and (B) a post mortem examination was carried out up to 2 years post-injection on 18 horse joints which had previously been treated with 1-2 injections of 2 ml PAAG for clinically and radiologically diagnosed OA.Integration of the injected gel was evident at day 10 in the rabbit and by day 14 in the horse, with proliferation and invasion of synovial cells into the gel. By day 90 in rabbit joints and day 30 in horse joints, the gel had formed a sub-synovial layer, which was traversed by thin strands of connective tissue with vessels and covered by a synovial lining facing the joint cavity. This histological appearance persisted up to 2 years post-injection in horse joints.Intra-articular injection of PAAG results in a stable, long-lasting sub-synovial layer of gel traversed with thin strands of connective tissue. Further studies to explore potential effects on synovial inflammation and pain are warranted

    Time-series analysis of fissure-fed multi-vent activity: a snapshot from the July 2014 eruption of Etna volcano (Italy)

    The April to May 2010 eruption of Eyjafjallajökull (Iceland) volcano was characterized by a large compositional variability of erupted products. To contribute to the understanding of the plumbing system dynamics of this volcano, we present new EMPA and LA-ICP-MS data on groundmass glasses of ash particles and minerals erupted between April 15 and 22. The occurrence of disequilibrium textures in minerals, such as resorption and inverse zoning, indicate that open system processes were involved in determining the observed compositional variability. The variation of major and trace element data of glasses corroborates this hypothesis indicating that mixing between magma batches with different compositions interacted throughout the whole duration of the eruption. In particular, the arrival of new basaltic magma into the plumbing system of the volcano destabilized and remobilized magma batches of trachyandesite and rhyolite compositions that, according to geophysical data, might have intruded as sills over the past 20 years beneath the Eyjafjallajökull edifice. Two mixing processes are envisaged to explain the time variation of the compositions recorded by the erupted tephra. The first occurred between basaltic and trachyandesitic end-members. The second occurred between trachyandesite and rhyolites. Least-squares modeling of major elements supports this hypothesis. Furthermore, investi- gation of compositional histograms of trace elements allows us to estimate the initial proportions of melts that interacted to generate the compositional variability triggered by mixing of trachyandesites and rhyolites.Published515V.     Clonación, expresión y evaluación inmunológica de la proteína Omp31 de Brucella melitensis y evaluación de su posible uso para el diagnóstico en brucelosis bovina

    The serological diagnosis of brucellosis is carried out by the detection of antibodies generated against the liposaccharide «LPS» or bacterial extracts of whole cells by ELISA or agglutination tests. The objective of this study was to evaluate a recombinant protein that can be used in the serological diagnosis of bovine brucellosis. The Omp31 gene of Brucella melitensis 16M was chosen. The gene was cloned from DNA of B. melitensis; the expression of the Omp31r protein was performed in a prokaryotic expression system using the pET28a vector and purification by metal affinity chromatography (IMAC). An indirect ELISA was standardized using control sera from positive and negative bovines. The Omp31r protein reacted with the positive control sera and succeeded in discriminating against the negative sera. The sensitivity and specificity of the ELISA assay was 77.2% and 90.6%, respectively. This indirect ELISA system can be useful for the rapid diagnosis of large numbers of samples in the diagnosis of bovine brucellosis.El diagnóstico serológico de la brucelosis se lleva a cabo por la detección de anticuerpos generados contra el liposacárido “LPS” o extractos bacterianos de células enteras por ELISA o pruebas de aglutinación. El objetivo del trabajo fue evaluar una proteína recombinante que pueda ser usada en el diagnóstico serológico de la brucelosis bovina. Se escogió el gen de la Omp31 de Brucella melitensis 16M. El gen fue clonado a partir de ADN de B. melitensis. La expresión de la proteína Omp31r se realizó en un sistema de expresión procariota utilizando el vector pET28a y la purificación por cromatografía de afinidad a metales (IMAC). Un ELISA indirecto fue estandarizado utilizando sueros controles de bovinos positivos y negativos. La proteína Omp31r reaccionó con los sueros controles positivos y logró discriminarlos frente a los sueros negativos. La sensibilidad y especificidad del ensayo de ELISA fue de 77.2% y de 90.6%, respectivamente. Este sistema de ELISA indirecto puede ser útil para los diagnósticos rápidos de grandes números de muestras en el diagnóstico de la brucelosis bovina

    Interspecific Genetic Differences and Historical Demography in South American Arowanas (Osteoglossiformes, Osteoglossidae, Osteoglossum)

    The South American arowanas (Osteoglossiformes, Osteoglossidae, Osteoglossum) are emblematic species widely distributed in the Amazon and surrounding basins. Arowana species are under strong anthropogenic pressure as they are extensively exploited for ornamental and food purposes. Until now, limited genetic and cytogenetic information has been available, with only a few studies reporting to their genetic diversity and population structure. In the present study, cytogenetic and DArTseq-derived single nucleotide polymorphism (SNP) data were used to investigate the genetic diversity of the two Osteoglossum species, the silver arowana O. bicirrhosum, and the black arowana O. ferreirai. Both species differ in their 2n (with 2n = 54 and 56 for O. ferreirai and O. bicirrhosum, respectively) and in the composition and distribution of their repetitive DNA content, consistent with their taxonomic status as different species. Our genetic dataset was coupled with contemporary and paleogeographic niche modeling, to develop concurrent demographic models that were tested against each other with a deep learning approach in O. bicirrhosum. Our genetic results reveal that O. bicirrhosum colonized the Tocantins-Araguaia basin from the Amazon basin about one million years ago. In addition, we highlighted a higher genetic diversity of O. bicirrhosum in the Amazon populations in comparison to those from the Tocantins-Araguaia basin. © 2019 by the authors.     Phylogeographic Analysis of HIV-1 Subtype C Dissemination in Southern Brazil

    The HIV-1 subtype C has spread efficiently in the southern states of Brazil (Rio Grande do Sul, Santa Catarina and Paraná). Phylogeographic studies indicate that the subtype C epidemic in southern Brazil was initiated by the introduction of a single founder virus population at some time point between 1960 and 1980, but little is known about the spatial dynamics of viral spread. A total of 135 Brazilian HIV-1 subtype C pol sequences collected from 1992 to 2009 at the three southern state capitals (Porto Alegre, Florianópolis and Curitiba) were analyzed. Maximum-likelihood and Bayesian methods were used to explore the degree of phylogenetic mixing of subtype C sequences from different cities and to reconstruct the geographical pattern of viral spread in this country region. Phylogeographic analyses supported the monophyletic origin of the HIV-1 subtype C clade circulating in southern Brazil and placed the root of that clade in Curitiba (Paraná state). This analysis further suggested that Florianópolis (Santa Catarina state) is an important staging post in the subtype C dissemination displaying high viral migration rates from and to the other cities, while viral flux between Curitiba and Porto Alegre (Rio Grande do Sul state) is very low. We found a positive correlation (r2 = 0.64) between routine travel and viral migration rates among localities. Despite the intense viral movement, phylogenetic intermixing of subtype C sequences from different Brazilian cities is lower than expected by chance. Notably, a high proportion (67%) of subtype C sequences from Porto Alegre branched within a single local monophyletic sub-cluster. These results suggest that the HIV-1 subtype C epidemic in southern Brazil has been shaped by both frequent viral migration among states and in situ dissemination of local clades
