    Flexible nurse staffing based on hourly bed census predictions

    Workload on nursing wards depends highly on patient arrivals and patient lengths of stay, which are both inherently variable. Predicting this workload and staffing nurses accordingly is essential for guaranteeing quality of care in a cost effective manner. This paper introduces a stochastic method that uses hourly census predictions to derive efficient nurse staffing policies. The generic analytic approach minimizes staffing levels while satisfying so-called nurse-to-patient ratios. In particular, we explore the potential of flexible staffing policies which allow hospitals to dynamically respond to their fluctuating patient population by employing float nurses. The method is applied to a case study of the surgical inpatient clinic of the Academic Medical Center (AMC) Amsterdam. This case study demonstrates the method's potential to study the complex interaction between staffing requirements and several interrelated planning issues such as case mix, care unit partitioning and size, and surgical block planning. Inspired by the numerical results, the AMC decided that this flexible nurse staffing methodology will be incorporated in the redesign of the inpatient care operations during the upcoming years

    Integral resource capacity planning for inpatient care services based on hourly bed census predictions

    The design and operations of inpatient care facilities are typically largely historically shaped. A better match with the changing environment is often possible, and even inevitable due to the pressure on hospital budgets. Effectively organizing inpatient care requires simultaneous consideration of several interrelated planning issues. Also, coordination with upstream departments like the operating theater and the emergency department is much-needed. We present a generic analytical approach to predict bed census on nursing wards by hour, as a function of the Master Surgical Schedule (MSS) and arrival patterns of emergency patients. Along these predictions, insight is gained on the impact of strategic (i.e., case mix, care unit size, care unit partitioning), tactical (i.e., allocation of operating room time, misplacement rules), and operational decisions (i.e., time of admission/discharge). The method is used in the Academic Medical Center Amsterdam as a decision support tool in a complete redesign of the inpatient care operations

    Global surface-ocean pCO2 and sea–air CO2 flux variability from an observation-driven ocean mixed-layer scheme

    A temporally and spatially resolved estimate of the global surface-ocean CO<sub>2</sub> partial pressure field and the sea–air CO<sub>2</sub> flux is presented, obtained by fitting a simple data-driven diagnostic model of ocean mixed-layer biogeochemistry to surface-ocean CO<sub>2</sub> partial pressure data from the SOCAT v1.5 database. Results include seasonal, interannual, and short-term (daily) variations. In most regions, estimated seasonality is well constrained from the data, and compares well to the widely used monthly climatology by Takahashi et al. (2009). Comparison to independent data tentatively supports the slightly higher seasonal variations in our estimates in some areas. We also fitted the diagnostic model to atmospheric CO<sub>2</sub> data. The results of this are less robust, but in those areas where atmospheric signals are not strongly influenced by land flux variability, their seasonality is nevertheless consistent with the results based on surface-ocean data. From a comparison with an independent seasonal climatology of surface-ocean nutrient concentration, the diagnostic model is shown to capture relevant surface-ocean biogeochemical processes reasonably well.     Interplay of Peltier and Seebeck effects in nanoscale nonlocal spin valves

    We have experimentally studied the role of thermoelectric effects in nanoscale nonlocal spin valve devices. A finite element thermoelectric model is developed to calculate the generated Seebeck voltages due to Peltier and Joule heating in the devices. By measuring the first, second and third harmonic voltage response non locally, the model is experimentally examined. The results indicate that the combination of Peltier and Seebeck effects contributes significantly to the nonlocal baseline resistance. Moreover, we found that the second and third harmonic response signals can be attributed to Joule heating and temperature dependencies of both Seebeck coefficient and resistivity.Comment: 4 pages, 4 figure

    Optical spectrum of the post-AGB star HD56126 in the region 4010-8790 AA

    We studied in detail the optical spectrum of the post-AGB star HD56126 (IRAS07134+1005). We use high resolution spectra (R=25000 and 60000) obtained with the echelle spectrographs of the 6-m telescope. About one and a half thousand absorptions of neutral atoms and ions, absorption bands of C_2, CN, and CH molecules, and interstellar bands (DIBs) are identified in the 4010 to 8790 AA wavelength region, and the depths and radial velocities of these spectral features are measured. Differences are revealed between the variations of the radial velocities measured from spectral features of different excitation. In addition to the well-known variability of the Halpha profile, we found variations in the profiles of a number of FeII, YII, and BaII lines. We also produce an atlas of the spectrum of HD56126 and its comparison staralpha Per.     Towards a Relativistic Description of Exotic Meson Decays

    This work analyses hadronic decays of exotic mesons, with a focus on the lightest one, the JPC=1+J^{PC}=1^{-+} π1\pi_{1}, in a fully relativistic formalism, and makes comparisons with non-relativistic results. We also discuss Coulomb gauge decays of normal mesons that proceed through their hybrid components. The relativistic spin wave functions of mesons and hybrids are constructed based on unitary representations of the Lorentz group. The radial wave functions are obtained from phenomenological considerations of the mass operator. Fully relativistic results (with Wigner rotations) differ significantly from non-relativistic ones. We also find that the decay channels π1πb1,πf1,KK1\pi_{1}\to\pi b_{1}, \pi f_{1}, KK_{1} are favored, in agreement with results obtained using other models.Comment: 14 pages, 7 figure

    Infrared High-Resolution Spectroscopy of Post-AGB Circumstellar Disks. I. HR 4049 - The Winnowing Flow Observed?

    High-resolution infrared spectroscopy in the 2.3-4.6 micron region is reported for the peculiar A supergiant, single-lined spectroscopic binary HR 4049. Lines from the CO fundamental and first overtone, OH fundamental, and several H2O vibration-rotation transitions have been observed in the near-infrared spectrum. The spectrum of HR 4049 appears principally in emission through the 3 and 4.6 micron region and in absorption in the 2 micron region. The 4.6 micron spectrum shows a rich 'forest' of emission lines. All the spectral lines observed in the 2.3-4.6 micron spectrum are shown to be circumbinary in origin. The presence of OH and H2O lines confirm the oxygen-rich nature of the circumbinary gas which is in contrast to the previously detected carbon-rich material. The emission and absorption line profiles show that the circumbinary gas is located in a thin, rotating layer near the dust disk. The properties of the dust and gas circumbinary disk and the spectroscopic orbit yield masses for the individual stars, M_AI~0.58 Msolar and M_MV~0.34 Msolar. Gas in the disk also has an outward flow with a velocity of \gtrsim 1 km/s. The severe depletion of refractory elements but near-solar abundances of volatile elements observed in HR 4049 results from abundance winnowing. The separation of the volatiles from the grains in the disk and the subsequent accretion by the star are discussed. Contrary to prior reports, the HR 4049 carbon and oxygen isotopic abundances are typical AGB values: 12C/13C=6^{+9}_{-4} and 16O/17O>200.Comment: 42 pages, 14 figures, Accepted by Ap

    Interest in bariatric surgery among obese patients with obstructive sleep apnea

    BACKGROUND: Standard obstructive sleep apnea (OSA) therapies are poorly tolerated. Bariatric surgery is a potential alternative but the level of interest in this intervention among OSA patients is unknown. OBJECTIVES: Determine the proportion of OSA patients who would be interested in bariatric surgery. SETTING: Sleep clinics, United States. METHODS: Consecutive adult patients with untreated severe OSA and a body mass index of 35-45 kg/m2 were approached. Patients at low perioperative risk and no urgent indication for OSA treatment were invited to a separate informational visit about bariatric surgery as primary treatment for OSA. RESULTS: Of 767 eligible patients, 230 (30.0%) were not at low perioperative risk, 49 (6.4%) had drowsy driving, and 16 (2.1%) had no insurance coverage for bariatric surgery. Of the remaining 482 patients, over one third (35.5%) were interested in bariatric surgery. Surgical interest was 47.2% in women versus 27.6% in men (
