    Kinematic analysis of the Pakuashan fault tip fold, west central Taiwan: Shortening rate and age of folding inception

    The Pakuashan anticline is an active fault tip fold that constitutes the frontal most zone of deformation along the western piedmont of the Taiwan Range. Assessing seismic hazards associated with this fold and its contribution to crustal shortening across central Taiwan requires some understanding of the fold structure and growth rate. To address this, we surveyed the geometry of several deformed strata and geomorphic surfaces, which recorded different cumulative amounts of shortening. These units were dated to ages ranging from ~19 ka to ~340 ka using optically stimulated luminescence (OSL). We collected shallow seismic profiles and used previously published seismic profiles to constrain the deep structure of the fold. These data show that the anticline has formed as a result of pure shear with subsequent limb rotation. The cumulative shortening along the direction of tectonic transport is estimated to be 1010 ± 160 m. An analytical fold model derived from a sandbox experiment is used to model growth strata. This yields a shortening rate of 16.3 ± 4.1 mm/yr and constrains the time of initiation of deformation to 62.2 ± 9.6 ka. In addition, the kinematic model of Pakuashan is used to assess how uplift, sedimentation, and erosion have sculpted the present-day fold topography and morphology. The fold model, applied here for the first time on a natural example, appears promising in determining the kinematics of fault tip folds in similar contexts and therefore in assessing seismic hazards associated with blind thrust faults

    A critical appraisal of guidelines for the management of knee osteoarthritis using Appraisal of Guidelines Research and Evaluation criteria

    Clinical practice guidelines have been elaborated to summarize evidence related to the management of knee osteoarthritis and to facilitate uptake of evidence-based knowledge by clinicians. The objectives of the present review were summarizing the recommendations of existing guidelines on knee osteoarthritis, and assessing the quality of the guidelines using a standardized and validated instrument – the Appraisal of Guidelines Research and Evaluation (AGREE) tool. Internet medical literature databases from 2001 to 2006 were searched for guidelines, with six guidelines being identified. Thirteen clinician researchers participated in the review. Each reviewer was trained in the AGREE instrument. The guidelines were distributed to four groups of three or four reviewers, each group reviewing one guideline with the exception of one group that reviewed two guidelines. One independent evaluator reviewed all guidelines. All guidelines effectively addressed only a minority of AGREE domains. Clarity/presentation was effectively addressed in three out of six guidelines, scope/purpose and rigour of development in two guidelines, editorial independence in one guideline, and stakeholder involvement and applicability in none. The clinical management recommendation tended to be similar among guidelines, although interventions addressed varied. Acetaminophen was recommended for initial pain treatment, combined with exercise and education. Nonsteroidal anti-inflammatory drugs were recommended if acetaminophen failed to control pain, but cautiously because of gastrointestinal risks. Surgery was recommended in the presence of persistent pain and disability. Education and activity management interventions were superficially addressed in most guidelines. Guideline creators should use the AGREE criteria when developing guidelines. Innovative and effective methods of knowledge translation to health professionals are needed

    The use of Real World Evidence in the European context: An analysis of key expert opinion

    Randomised controlled trials (RCT), traditionally seen as the gold standard in drug approval requirement terms, are becoming more difficult due to, among other reasons, budget constraints, increasing complexities and the shrinking of patient populations. Real world evidence (RWE), data used for decision making that is not derived from traditional RCT, may in future play an increasing role in market access and reimbursement decisions. This paper analyses key pricing and reimbursement stakeholders’ opinions of RWE across five European countries via a focus group-style discussion. Areas probed included regulatory implications and the role of RWE in the study countries, RWE processes and implementation on decision making, meaningful outcomes from RWE and priorities for future focus and industry support. Results showed that RWE was used to some extent in all countries, generally in accelerated access and re-review situations, with accepted endpoints including overall survival, morbidity, avoidable mortality and quality of life among others, but that there were a number of areas where improvement was necessary if RWE use was to become more common place

    Benchmarking clinical management of spinal and non-spinal disorders using quality of life: results from the EPI3-LASER survey in primary care

    Concerns have been raised regarding sub-optimal utilization of analgesics and psychotropic drugs in the treatment of patients with chronic musculoskeletal disorders (MSDs) and their associated co-morbidities. The objective of this study was to describe drug prescriptions for the management of spinal and non-spinal MSDs contrasted against a standardized measure of quality of life. A representative population sample of 1,756 MSDs patients [38.5% with spinal disorder (SD) and 61.5% with non-spinal MSDs (NS-MSD)] was drawn from the EPI3-LASER survey of 825 general practitioners (GPs) in France. Physicians recorded their diagnoses and prescriptions on that day. Patients provided information on socio-demographics, lifestyle and quality of life using the Short Form 12 (SF-12) questionnaire. Chronicity of MSDs was defined as more than 12 weeks duration of the current episode. Chronic SD and NS-MSD patients were prescribed less analgesics and non-steroidal anti-inflammatory drugs than their non-chronic counterpart [odds ratios (OR) and 95% confidence intervals (CI), respectively: 0.4, 0.2–0.7 and 0.5, 0.3–0.6]. They also had more anxio-depressive co-morbidities reported by their physicians (SD: 16.1 vs.7.4%; NS-MSD: 21.6 vs. 9.5%) who prescribed more antidepressants and anxiolytics with a difference that was statistically significant only for spinal disorder patients (OR, 95% CI: 2.0, 1.1–3.6). Psychotropic drugs were more often prescribed in patients in the lower quartile of SF-12 mental score and prescriptions of analgesics in the lower quartile of SF-12 physical score (P < 0.001). In conclusion, anxiety and depressive disorders were commonly reported by GPs among chronic MSD patients. Their prescriptions of psychotropic and analgesic drugs were consistent with patients’ self-rated mental and physical health

    Kinematics of fault-related folding derived from a sandbox experiment

    We analyze the kinematics of fault tip folding at the front of a fold-and-thrust wedge using a sandbox experiment. The analog model consists of sand layers intercalated with low-friction glass bead layers, deposited in a glass-sided experimental device and with a total thickness h = 4.8 cm. A computerized mobile backstop induces progressive horizontal shortening of the sand layers and therefore thrust fault propagation. Active deformation at the tip of the forward propagating basal décollement is monitored along the cross section with a high-resolution CCD camera, and the displacement field between pairs of images is measured from the optical flow technique. In the early stage, when cumulative shortening is less than about h/10, slip along the décollement tapers gradually to zero and the displacement gradient is absorbed by distributed deformation of the overlying medium. In this stage of detachment tip folding, horizontal displacements decrease linearly with distance toward the foreland. Vertical displacements reflect a nearly symmetrical mode of folding, with displacements varying linearly between relatively well defined axial surfaces. When the cumulative slip on the décollement exceeds about h/10, deformation tends to localize on a few discrete shear bands at the front of the system, until shortening exceeds h/8 and deformation gets fully localized on a single emergent frontal ramp. The fault geometry subsequently evolves to a sigmoid shape and the hanging wall deforms by simple shear as it overthrusts the flat ramp system. As long as strain localization is not fully established, the sand layers experience a combination of thickening and horizontal shortening, which induces gradual limb rotation. The observed kinematics can be reduced to simple analytical expressions that can be used to restore fault tip folds, relate finite deformation to incremental folding, and derive shortening rates from deformed geomorphic markers or growth strata

    Streaming potential measurements 2. Relationship between electrical and hydraulic flow patterns from rock samples during deformation

    Streaming potential and resistivity measurements have been performed on Fontainebleau sandstone and Villejust quartzite samples in a triaxial device during compaction, uniaxial compression, and rupture. Measurements on individual samples do not show any clear intrinsic dependence of the streaming potential coefficient with permeability. An apparent dependence of the streaming potential coefficient with permeability is, however, observed during deformation. The effect of surface conductivity is taken into account and is small compared with the observed changes in the streaming potential coefficient. The observed dependence is therefore interpreted in terms of a difference in the evolution of the electrical and hydraulic connectivity patterns during deformation. This effect causes the streaming potential coefficient, and consequently the inferred ξ potential, to be reduced by a geometrical factor R_G representing the electrical efficiency of the hydraulic network. Estimates of the R_G factor varying between 0.2 and 0.8 for electrolyte resistivity larger than 100 Ωm are obtained by comparing the values of the ξ potential inferred from intact rock samples with the values obtained from crushed rock samples, where the geometrical effects are assumed to be negligible. The reduction of the streaming potential coefficient observed during compaction or uniaxial compression suggests that the tortuosity of the hydraulic network increases faster than the tortuosity of the electrical network. Before rupture, an increase in the streaming potential coefficient associated with the onset of dilatancy was observed for three samples of Fontainebleau sandstone and one sample of Villejust quartzite. The changes in streaming potential coefficient prior to failure range from 30% to 50%. During one experiment, an increase in the concentration of sulfate ions was also observed before failure. These experiments suggest that observable streaming potential and geochemical variations could occur before earthquakes

    ABSTRACT. Objective. To assess the prevalence of autoimmune diseases in first-degree relatives of patients with systemic sclerosis (SSc), and to compare those results with control families in order to identify patterns of autoimmune diseases in relatives. Methods. A retrospective case-control postal questionnaire survey was performed in France to recruit patients with SSc belonging to an association of patients with SSc and unrelated age-matched and sex-matched controls. Each participant was asked to self-report on the existence of autoimmune diseases in their first-degree relatives. The prevalence of autoimmune diseases in the families of patients with SSc was compared with the corresponding prevalence in the families of controls. Results. A total of 121 families out of 373 (32.4%) with a member having SSc reported at least 1 autoimmune disease in 1 or more first-degree relatives. The most frequent autoimmune diseases in SSc families when adjusted for family size were autoimmune thyroid disease (AITD; 4.9%), rheumatoid arthritis (4.1%), psoriasis (3.9%), and type 1 diabetes mellitus (2.9%). Compared with control families, AITD and connective tissue diseases (SSc, systemic lupus erythematosus, or Sjögren&apos;s syndrome) were more likely to occur in families with SSc (p = 0.01 and p = 0.01, respectively), with OR of 3.20 (95% CI 1.25-8.18) and 5.20 (95% CI 1.22-21.8). In contrast, inflammatory bowel disease was less likely to occur within families with SSc (p = 0.02, OR 0.29, 95% CI 0.11-0.80). In addition, the coexistence of more than 1 autoimmune disease in the index SSc case was associated with familial aggregation of autoimmune diseases. Conclusion. Our results show that autoimmune diseases cluster within families of patients with SSc. This supports the notion that these diseases might arise on a shared genetic basis underlying several autoimmune phenotypes. (J Rheumatol First Release Jan 15 2012

    Streaming potential measurements 1. Properties of the electrical double layer from crushed rock samples

    The ξ potential has been inferred from streaming potential measurements with crushed rock samples as a function of pH and electrolyte concentration for various salts. The value obtained for crushed Fontainebleau sandstone at pH = 5.7 and a KCl solution with a resistivity of 400 Ω m is −40 ± 5 mV, where the error is dominated by sample to sample variations. The sensitivity of the ξ potential to the electrolyte resistivity for KCl is given experimentally by ρ_f^(0.23±0.014) where ρ_f is the electrolyte resistivity. The point of zero charge (pzc) is observed for pH = 2.5 ± 0.1, and the ξ potential is positive for pH pzc. For pH > 5 the variations of the ξ potential with pH can be approximated by ξ(pH)/ξ(5.7) = 1 + (0.068 ± 0.004)(pH - 5.7) for ρ_f = 100 Ω m. The ξ potential has been observed to be sensitive to the valence of the ions and is approximately reduced by the charge of the cation, unless specific adsorption takes place like in the case of Al^3+. The experimental results are well accounted for by a three-layer numerical model of the electrical double layer, and the parameters of this model can be evaluated from the experimental data. The sensitivity of the ξ potential to the rock minerals has also been studied. The ξ potential obtained for granitic rocks is comparable to that obtained for Fontainebleau sandstone but can be reduced by a factor of 2–4 for sandstones containing significant fractions of carbonates or clay. To take into account the effect of the chemical composition of the electrolyte, a chemical efficiency is defined as the ratio of the ξ potential to the ξ potential measured for KCl. This chemical efficiency is measured to be ∼80% for typical groundwater but can be as low as 40% for a water with a high dissolved carbonate content. The set of empirical laws derived from our measurements can be used to assess the magnitude of the streaming potentials expected in natural geophysical systems
