Subsidence and thermal history of an inverted Late Jurassic-Early Cretaceous extensional basin (Cameros, North-central Spain) affected by very low- to low-grade metamorphism.

The Cameros Basin (North Spain) is a Late Jurassic-Early Cretaceous extensional basin, which was inverted during the Cenozoic. It underwent a remarkable thermal evolution, as indicated by the record of anomalous high temperatures in its deposits. In this work the subsidence and thermal history of the basin is reconstructed, using subsidence analysis and 2D thermal modeling. Tectonic subsidence curves provide evidence of the occurrence of two rapid subsidence phases during the syn-extensional stage. In the first phase (Tithonian-Early Berriasian), the largest accommodation space was formed in the central sector of the basin, whereas in the second (Early Barremian-Early Albian), it was formed in the northern sector. These rapid subsidence phases could correspond to relevant tectonic events affecting the Iberian Plate at that time. By distinguishing between the initial and thermal subsidence and defining their relative magnitudes, Royden's (1986) method was used to estimate the heat flow at the end of the extensional stage. A maximum heat flow of 60-65 mW/m2 is estimated, implying only a minor thermal disturbance associated with extension. In contrast with these data, very high vitrinite reflectance, anomalously distributed in some case with respect to the typical depth-vitrinite reflectance relation, was measured in the central-northern sector of the basin. Burial and thermal data are used to construct a 2D thermal basin model, to elucidate the role of the processes involved in sediment heating. Calibration of the thermal model with the vitrinite reflectance (%Ro) and fluid inclusion (FI) data indicates that in the central and northern sectors of the basin, an extra heat source, other than a typical rift, is required to explain the observed thermal anomalies. The distribution of the %Ro and FI values in these sectors suggests that the high temperatures and their distribution are related to the circulation of hot fluids. Hot fluids were attributed to the hydrothermal metamorphic events affecting the area during the early post-extensional and inversion stages of the basin

Transfers of Care between Healthcare Professionals in Obstetric Units of Different Sizes across Spain and in a hospital in Ireland : the MidconBirth Study

Abstract: Background: In Europe, the majority of healthy women give birth at conventional obstetric units with the assistance of registered midwives. This study examines the relationships between the intrapartum transfer of care (TOC) from midwife to obstetrician-led maternity care, obstetric unit size (OUS) with different degrees of midwifery autonomy, intrapartum interventions and birth outcomes. Methods: A prospective, multicentre, cross-sectional study promoted by the COST Action IS1405 was carried out at eight public hospitals in Spain and Ireland between 2016-2019. The primary outcome was TOC. The secondary outcomes included type of onset of labour, oxytocin stimulation, epidural analgesia, type of birth, episiotomy/perineal injury, postpartum haemorrhage, early initiation of breastfeeding and early skin-to-skin contact. A logistic regression was performed to ascertain the effects of studied co-variables on the likelihood that participants had a TOC; Results: Out of a total of 2,126 low-risk women, those whose intrapartum care was initiated by a midwife (1772) were selected. There were statistically significant differences between TOC and OUS (S1 = 29.0%, S2 = 44.0%, S3 = 52.9%, S4 = 30.2%, p < 0.001). Statistically differences between OUS and onset of labour, oxytocin stimulation, type of birth and episiotomy or perineal injury were observed (p = 0.009, p < 0.001, p < 0.001, p < 0.001 respectively); Conclusions: Findings suggest that the model of care and OUS have a significant effect on the prevalence of intrapartum TOC and the birth outcomes. Future research should examine how models of care differ as a function of the OUS in a hospital, as well as the cost-effectiveness for the health care system

Background. Abacavir-lamivudine and tenofovir DF-emtricitabine fixed-dose combinations are commonly used as first-line antiretroviral therapies. However, few studies have comprehensively compared their relative safety profiles. Methods. In this European, multicenter, open-label, 96-week study, antiretroviral-naive adult subjects with human immunodeficiency virus (HIV) infection were randomized to receive either abacavir-lamivudine or tenofovir- emtricitabine with efavirenz. Primary analyses were conducted after 48 weeks of treatment. Bone mineral density (BMD), a powered secondary end point, was assessed by dual energy x-ray absorptiometry. Bone turnover markers (osteocalcin, procollagen 1 N-terminal propeptide, bone specific alkaline phosphatase, and type 1 collagen cross-linked C telopeptide [CTx]) were assessed in an exploratory analysis. Results. A total of 385 subjects were enrolled in the study. BMD loss was observed in both treatment groups, with a significant difference in the change from baseline in both total hip (abacavir-lamivudine group, -1.9%; tenofovir-emtricitabine group, -3.6%; P= 6% was more common in the tenofovir-emtricitabine group (13% of the tenofovir-emtricitabine group vs 3% of the abacavir-lamivudine group had a loss of >= 6% in the hip; 15% vs 5% had a loss of >= 6% in the spine). Bone turnover markers increased in both treatment groups over the first 24 weeks, stabilizing or decreasing thereafter. Increases in all markers were significantly greater in the tenofovir-emtricitabine treatment group than in the abacavir-lamivudine group at week 24. All but CTx remained significantly different at week 48 (eg, osteocalcin: abacavir-lamivudine group, +8.07 mg/L; tenofovir-emtricitabine group, +11.92 mg/L; P Conclusions. This study demonstrated the impact of first-line treatment regimens on bone. Greater increases in bone turnover and decreases in BMD were observed in subjects treated with tenofovir-emtricitabine than were observed in subjects treated with abacavir-lamivudine

International audienceIn 31 participants who started first-line antiretroviral therapy in the NEAT 001/ANRS 143 clinical trial, we found after 96 weeks a statistically significant increase in blood telomere length (TL) of 0.04 (T/S Ratio) (p = 0.03). This increase was positively correlated with both the change in the percentage of CD4+ T-cells and with the decrease of CD38+ molecules on Central Memory CD8+ and negatively correlated with the change in the percentage of CD4+ Effector Memory cells. Increase in TL could be an expression of immune reconstitution and the associated decrease in immune activation. We acknowledge for the low statistical power due to the small sample size and the potential for false positive results due to multiple testing. Hence, further studies are needed to confirm these observations
