    Role of erythropoietin in the angiogenic activity of bone marrow endothelial cells of MGUS and multiple myeloma patients

    Increasing evidences suggest several biological roles for erythropoietin and its receptor (Epo and EpoR), unrelated to erythropoiesis, including angiogenesis. Here, we detected the expression of EpoR in bone marrow-derived endothelial cells from monoclonal gammopathy of undetermined significance (MGUS) and multiple myeloma (MM) patients (MGECs and MMECs, respectively) and assessed whether Epo plays a role in MGECs- and MMECs-mediated angiogenesis. We show that EpoR is expressed by both MGECs and MMECs even though at a higher level in the first ones. Both EC types respond to rHuEpo in terms of cell proliferation, whereas other responses, including activation of JAK2/STAT5 and PI3K/Akt pathways, cell migration and capillarogenesis are enhanced by Epo in MGECs, but not in MMECs. In addition, the conditioned media of both Epo-treated cells induce a strong angiogenic response in vivo in the chorioallantoic membrane assay, comparable to that of vascular endothelial growth factor (VEGF). Overall, these data highlight the effect of Epo on MGECs- and MMECs-mediated angiogenesis: MGECs are more responsive to Epo treatment than MMECs, probably because over-angiogenic phenotype of MMECs is already activated by their autocrine/paracrine loops occurring in the "angiogenic switch" from MGUS

    A HGF/cMET Autocrine Loop Is Operative in Multiple Myeloma Bone Marrow Endothelial Cells and May Represent a Novel Therapeutic Target

    Purpose: The aim of this study was to investigate the angiogenic role of the hepatocyte growth factor (HGF)/cMET pathway and its inhibition in bone marrow endothelial cells (EC) from patients with multiple myeloma versus from patients with monoclonal gammopathy of undetermined significance (MGUS) or benign anemia (control group). Experimental Design: The HGF/cMET pathway was evaluated in ECs from patients with multiple myeloma (multiple myeloma ECs) at diagnosis, at relapse after bortezomib- or lenalidomide-based therapies, or on refractory phase to these drugs; in ECs from patients with MGUS (MGECs); and in those patients from the control group. The effects of a selective cMET tyrosine kinase inhibitor (SU11274) on multiple myeloma ECs' angiogenic activities were studied in vitro and in vivo. Results: Multiple myeloma ECs express more HGF, cMET, and activated cMET (phospho (p)-cMET) at both RNAand protein levels versus MGECs and control ECs. Multiple myeloma ECs are able to maintain the HGF/cMET pathway activation in absence of external stimulation, whereas treatment with anti-HGF and anti-cMET neutralizing antibodies (Ab) is able to inhibit cMET activation. The cMET pathway regulates several multiple myeloma EC activities, including chemotaxis, motility, adhesion, spreading, and whole angiogenesis. Its inhibition by SU11274 impairs these activities in a statistically significant fashion when combined with bortezomib or lenalidomide, both in vitro and in vivo. Conclusions: An autocrine HGF/cMET loop sustains multiple myeloma angiogenesis and represents an appealing new target to potentiate the antiangiogenic management of patients with multiple myeloma

    AMPA receptors (AMPARs) are tetrameric ligand-gated channels made up of combinations of GluA1-4 subunits encoded by GRIA1-4 genes. GluA2 has an especially important role because, following post-transcriptional editing at the Q607 site, it renders heteromultimeric AMPARs Ca2+-impermeable, with a linear relationship between current and trans-membrane voltage. Here, we report heterozygous de novo GRIA2 mutations in 28 unrelated patients with intellectual disability (ID) and neurodevelopmental abnormalities including autism spectrum disorder (ASD), Rett syndrome-like features, and seizures or developmental epileptic encephalopathy (DEE). In functional expression studies, mutations lead to a decrease in agonist-evoked current mediated by mutant subunits compared to wild-type channels. When GluA2 subunits are co-expressed with GluA1, most GRIA2 mutations cause a decreased current amplitude and some also affect voltage rectification. Our results show that de-novo variants in GRIA2 can cause neurodevelopmental disorders, complementing evidence that other genetic causes of ID, ASD and DEE also disrupt glutamatergic synaptic transmission

    This paper aims at evaluating in the first place the different terms used in order to define the individuals involved in non-professional theater practice, from the historical groups to the present national associations. Secondly making use of an empirical inquiry realized by the author in 2015 focusing on a sample of non-professional acting companies belonging to the area of Rome, a short summary of the purposes and of their main dramaturgical tendencies rovided with the submission by considering few case studies.L'elaborato intende, in prima battuta, dare una panoramica dei differenti termini utilizzati per definire gli operatori non professionisti di teatro, analizzandone varie accezioni significative. Viene poi ripercorso brevemente lo sviluppo del teatro amatoriale nel Novecento, dai gruppi filodrammatici alle associazioni nazionali di oggi. Attraverso i risultati di un'indagine empirica condotta dalla stessa autrice nel 2015 su di un campione di compagnie amatoriali appartenenti alla provincia di Roma, viene inoltre fornito un rapido quadro relativo alle intenzioni e alle scelte drammaturgiche che caratterizzano l'attività teatrale degli amatori, proponendone alcuni esempi pratici

    The new COVID-19 disease has swept the world in recent months, causing enormous disruption to social, economic and health systems. Given the diversity of international health systems and conditions differ from one location to another. In all cases, however, it was decided to limit the elective surgical practices considered non-urgent. Plastic surgery departments have also seen a change in their normal business. The aim of this study was to investigate how the pandemic changed the activity of the Plastic Surgery Department of the "San Giovanni di Dio and Ruggi d'Aragona" University Hospital in Salerno (Italy). In particular, starting from the hospital discharge forms for the two-year period 2019-2020, Gender, Age, Date of admission, Date of discharge, Diagnostic Related Group (DRG) weight and Hospital admission procedures for patients were extracted. Statistical analysis and logistic regression were used to compare the activity of 2019, used in this study as a reference, with that of 2020 in the midst of the pandemic. The analysis showed a statically significant reduction in the Length of Stay (LOS), thus improving appropriateness and achieving a reduction in spending

    Bone marrow angiogenesis and progression in multiple myeloma

    Multiple myeloma plasma cells home and expand in the bone marrow where cause an unbalanced bone remodelling with increased bone resorption and low bone formation that represent the typical feature in the majority of patients. A clinically relevant aspect of the interactions of multiple myeloma plasma cells in the bone marrow microenvironment is neovascularization, a constant hallmark of disease progression. This process is only partially supported by factors such as vascular endothelial growth factor, fibroblast growth factor-2 and metalloproteinases, which are directly secreted by the tumor cells. In fact, the presence in the bone marrow microenvironment of cytokines, in particular interleukin-6, as a consequence of plasma cell-stromal cell interactions, induces the production and secretion of angiogenic factors by other cells present in the bone microenvironment, thus contributing to the angiogenic switch during the progression of the disease. Near angiogenesis vasculogenesis occur in the bone marrow of myeloma patients and contribute to the vascular three formation. In the bone marrow of myeloma patients haematopoietic stem cells are recruited and induced to differentiate into endothelial cells by the angiogenic cytokines present in the microenvironment. Myeloma plasma cells also induce angiogenesis indirectly via recruitment and activation of stromal inflammatory cells (i.e.: macrophages and mast cells) to secrete their own angiogenic factors. They are recruited and activated by tumor plasma cells through the secretion of fibroblast growth factor-2, interleukin-8, and other chemokines, such as ITAC, Mig, IP-10. When macrophages and mast cells are activated they secrete their angiogenic factors: fibroblast growth factor-2, vascular endothelial growth factor, granulocyte-colony stimulating factor, granulocyte macrophage-colony stimulating factor, which contribute to enhance the tumor neovascularization. Finally, myeloma macrophages when exposed to vascular endothelial growth factor and fibroblast growth factor-2 secreted by plasma cells shows vasculogenic ability and acquire endothelial cell markers and transform into cells functionally and phenotypically similar to paired bone marrow endothelial cells. So they participate to the formation of the bone marrow capillary network (vasculogenic mimicry)

    Since the first case recorded in China in 2019, CoViD-19 has overwhelmed the health systems of all countries. The highly complex request for assistance combined with the fear of contagion have changed the normal activity of hospitals. Conversely, as the pandemic spreads, fewer people are going to the Emergency Department (ED) for non-CoViD diseases. In this study, logistic regression and statistical analysis were used to investigate how the pandemic changed the activity of the Emergency Medicine Department of "San Giovanni di Dio and Ruggi d'Aragona" University Hospital of Salerno (Italy). Patients admitted in 2020 have a higher Length of Stay (LOS) and the mode of discharge is mostly "at home". While the discharge modality 'transferred to another regime in the same hospital' had significantly decreased in order to counter the internal contagion
