    Costos logísticos y exportaciones de la Asociación Valle del Chira, Piura 2022

    El objetivo principal del presente estudio fue determinar la relación existente entre los costos logísticos y las exportaciones de banano orgánico de la Asociación Valle del Chira en Piura en el 2022. Fue de tipo sustantiva, tiene un alcance descriptivo y sigue un enfoque cuantitativo. Asimismo, tiene un diseño no experimental y transversal y utiliza una encuesta de 22 preguntas aplicada sobre una muestra de 40 colaboradores. Los resultados descriptivos determinaron que el nivel de costos logísticos es alto con un 47,5% mientras que 37,5% lo considera moderada, y sólo el 15% baja; para las exportaciones el nivel es moderado con un 45%; mientras un 30% lo ubica en el nivel alto y un 25% lo considera baja. Además, se puede evidenciar que existe una asociación directa entre las variables de estudio demostrado por el estadístico Chi cuadrado = 25,156 y p valor =0,000; por tanto, mientras va subiendo el nivel de los Costos logísticos, la variable Exportación presenta menores niveles en su intercepción respectiva, existiendo una asociación inversa. Respecto a los resultados inferenciales se probó la Ha que existe una relación significativa (p=0,000<0,05) entre costos logísticos y exportaciones cuyo coeficiente de correlación de Spearman de RS= -0,755 que determina una relación significativa inversa alta

    Pesquisa descritiva, exploratória com abordagem qualitativa realizada em Hospital Universitário da Rede PúblicaFederal de Fortaleza, Ceará, Brasil. Teve como objetivos compreender a percepção dos enfermeiros assistenciaissobre o cuidado de enfermagem; conhecer as ações desenvolvidas pelos enfermeiros que são reconhecidas comoatividades de cuidado. Participaram oito enfermeiras da unidade de terapia intensiva pós-operatória. Utilizou-secomo instrumento de coleta de dados um questionário com perguntas abertas e análise ocorreu através da análisede conteúdo com formação de categorias temáticas e um quadro que permitiram a melhor visualização dos resultados.Evidenciou-se que embora conheçam e conceituem o cuidado de enfermagem, as enfermeiras têm dificuldadesem realizar o cuidado amplo, talvez por estarem sobrecarregadas por procedimentos técnicos realizados no ambientehospitalar

    IL-17RA-signaling modulates CD8+ T Cell survival and exhaustion during trypanosoma cruzi infection

    The IL-17 family contributes to host defense against many intracellular pathogens by mechanisms that are not fully understood. CD8+ T lymphocytes are key elements against intracellular microbes, and their survival and ability to mount cytotoxic responses are orchestrated by several cytokines. Here, we demonstrated that IL-17RA-signaling cytokines sustain pathogen-specific CD8+ T cell immunity. The absence of IL-17RA and IL-17A/F during Trypanosoma cruzi infection resulted in increased tissue parasitism and reduced frequency of parasite-specific CD8+ T cells. Impaired IL-17RA-signaling in vivo increased apoptosis of parasite-specific CD8+ T cells, while in vitro recombinant IL-17 down-regulated the pro-Apoptotic protein BAD and promoted the survival of activated CD8+ T cells. Phenotypic, functional, and transcriptomic profiling showed that T. cruzi-specific CD8+ T cells derived from IL-17RA-deficient mice presented features of cell dysfunction. PD-L1 blockade partially restored the magnitude of CD8+ T cell responses and parasite control in these mice. Adoptive transfer experiments established that IL-17RA-signaling is intrinsically required for the proper maintenance of functional effector CD8+ T cells. Altogether, our results identify IL-17RA and IL-17A as critical factors for sustaining CD8+ T cell immunity to T. cruzi.Fil: Tosello Boari, Jimena.     The aim was to describe the contributions of the Fundamentals of Nursing II monitoring in the academic education of Nursing students. Descriptive study, type of experience report, carried out in a Higher Education Institution with nursing students in the city of Rio de Janeiro. It was possible to observe that the monitoring facilitated the learning of nursing students in Fundamentals of Nursing II, provided the academic monitor with the teaching experience, helped in the mediation between nursing students and a professor of Fundamentals of Nursing II and made it possible for academics to train students. better quality in this discipline as future nursing professionals. It should be emphasized the monitoring contributions of this subject, both for academics and for the academic monitor, as well as for the teacher who teaches this course.Objetivou-se descrever as contribuições da monitoria de Fundamentos de Enfermagem II na formação acadêmica de estudantes de Enfermagem. Estudo descritivo, tipo relato de experiência, realizado em uma Instituição de Ensino Superior com estudantes de enfermagem na cidade do Rio de Janeiro. Foi possível observar que a monitoria facilitou o aprendizado dos acadêmicos de enfermagem em Fundamentos de Enfermagem II, proporcionou ao acadêmico-monitor a experiência de docência, auxiliou na mediação entre acadêmicos de enfermagem e docente de Fundamentos de Enfermagem II e possibilitou aos acadêmicos a formação de melhor qualidade nesta disciplina como futuros profissionais da enfermagem. Deve-se ressaltar as contribuições da monitoria desta matéria, tanto para os acadêmicos quanto para o acadêmico-monitor, bem como para o docente que ministra esta unidade curricular. &nbsp

    Evaluation of Lu-177-Dotatate treatment in patients with metastatic neuroendocrine tumors and prognostic factors

    BACKGROUND: (177)Lu peptide receptor radionuclide therapy (PRRT) is a recently approved therapy in Spain that has been demonstrated to be a well-tolerated therapy for positive somatostatin receptor advanced gastroenteropancreatic neuroendocrine tumors. AIM: To determine the impact of PRRT on quality of life, radiologic and metabolic response, overall survival, prognostic factors and toxicity. METHODS: Thirty-six patients treated with (177)Lu-PRRT from 2016 to 2019 were included. The most frequent location of the primary tumor was the gastrointestinal tract (52.8%), pancreas (27.8%), and nongastropancreatic neuroendocrine tumor (11.1%). The liver was the most common site of metastasis (91.7%), followed by distant nodes (50.0%), bone (27.8%), peritoneum (25.0%) and lung (11.1%). Toxicity was evaluated after the administration of each dose. Treatment efficacy was evaluated by two parameters: stable disease and disease progression in response evaluation criteria in solid tumors 1.1 criterion and prognostic factors were tested. RESULTS: From 36 patients, 55.6% were men, with a median age of 61.1 +/- 11.8 years. Regarding previous treatments, 55.6% of patients underwent surgery of the primary tumor, 100% of patients were treated with long-acting somatostatin analogues, 66.7% of patients were treated with everolimus, 27.8% of patients were treated with tyrosine kinase inhibitor, and 27.8% of patients were treated with interferon. One patient received radioembolization, three patients received chemoembolization, six patients received chemotherapy. Hematological toxicity was registered in 14 patients (G1-G2: 55.5% and G3: 3.1%). Other events presented were intestinal suboclusion in 4 cases, cholestasis in 2 cases and carcinoid crisis in 1 case. The median follow-up time was 3 years. Currently, 24 patients completed treatment. Nineteen are alive with stable disease, two have disease progression, eight have died, and nine are still receiving treatment. The median overall survival was 12.5 mo (95% confidence interval range: 9.8-15.2), being inversely proportional to toxicity in previous treatments (P < 0.02), tumor grade (P < 0.01) and the presence of bone lesions (P = 0.009) and directly proportional with matching lesion findings between Octreoscan and computed tomography pre-PRRT (P < 0.01), , primary tumor surgery (P = 0.03) and metastasis surgery (P = 0.045). In a multivariate Cox regression analysis, a high Ki67 index (P = 0.003), a mismatch in the lesion findings between Octreoscan and computed tomography pre-PRRT (P < 0.01) and a preceding toxicity in previous treatments (P < 0.05) were risk factors to overall survival. CONCLUSION: Overall survival was inversely proportional to previous toxicity, tumor grade and the presence of bone metastasis and directly proportional to matching lesion findings between Octreoscan and computed tomography pre-PRRT and primary tumor and metastasis surgery

    A visão dos agentes comunitários de saúde sobre a importância do nutricionista na atenção básica/ Community health agents 'view on the importance of the nutritionist in basic care

    O Agente Comunitário de Saúde (ACS) é um articulador do processo de trabalho da equipe, por residir na sua área de atuação torna o trabalho mais simples por conhecer muito bem a comunidade. Feito isso eles comunicam à equipe do PSF quais são os agravos e voltam à comunidade com orientações, encaminhamentos e outras formas de solucionar os problemas. O objetivo foi compreender a visão dos agentes comunitários de saúde sobre a importância do nutricionista na atenção básica. A pesquisa foi realizada em um posto de saúde em Fortaleza- CE pertencente a Regional II. Quanto à coleta de dados, ocorreu no período de fevereiro a março de 2019. Foi realizada uma entrevista semiestruturada com 10 ACS, sendo esta gravada para avaliar a percepção dos participantes durante a participação na pesquisa, sobre a temática abordada. Constatamos que os ACS não tinham conhecimento sobre a abrangência de todo o campo de atuação do nutricionista, relataram maior prevalência de excesso de peso, diabéticos e hipertensos na comunidade. Observamos que os ACS’s não estavam cientes de todas as contribuições que o nutricionista pode realizar em uma Unidade Básica de Saúde. Também foi possível constatar a deficiência dos mesmos em relação aos conhecimentos básicos sobre alimentação saudável, na prevenção e tratamento de determinadas patologias

    The Pattern of Methylation and Polymorphism in Interleukin-6 Promoter Gene Are Related to the Development of Rheumatoid Arthritis?

    Rheumatoid Arthritis (RA) is a complex disease with higher level of IL-6. In order to elucidate the alterations related to IL-6 gene in RA, we evaluated the -174 G/C IL-6 gene polymorphism and methylation pattern of its promoter. A total of 120 RA patients and 120 healthy controls were recruited for polymorphism analysis, and 30 individuals of both groups were randomly selected for methylation analysis. The IL-6 gene polymorphism was analyzed by PCR-RFLP and methylation pattern was analyzed by MSP-PCR. Regarding IL-6 gene polymorphism, no significant difference was found between RA patients and controls (P>0.05). For DNA, two CpG regions of IL-6 promoter gene was analyzed. The first region (−1069, −1061, −1057 and −1001 CpG) did not show significant differences, whereas the second region (−628, −610, −574 and −491 CpG) showed a significant hypermethylated status (P=0.0004) in RA patients as compared with controls.These results, suggest a possible relationship between methylation pattern and the susceptibility to RA in our studied population

    Lower insulin-dose adjusted A1c (IDAA1c) is associated with less complications in Individuals with Type 1 Diabetes treated with hematopoetic stem-cell transplantation and conventional therapy

    Objective: To evaluate the association between insulin-dose adjusted A1C (IDAA1c) and microvascular complications (MC) and hypoglycemia in a representative Brazilian population of Type 1 diabetes mellitus (T1DM) patients. Research Design and Methods: This was a cross-sectional study based on a previous study, “Microvascular Complications in Type 1 Diabetes: a comparative analysis of patients treated with autologous nonmyeloablative hematopoietic stem-cell transplantation (AHST) and conventional medical therapy (CT)”. The 168 patients in that study (144 from CT plus 24 from AHST) were re-subdivided into two groups, according to their IDAA1c values (30 patients had IDAA1c ≤ 9; 138 had IDAA1c > 9). Then, the prevalence of MC (diabetic renal disease, neuropathy, and retinopathy), hypoglycemia (blood glucose <60 mg/dL), and severe hypoglycemic (episode of hypoglycemia that required the assistance of another person to treat) events were compared between the groups. The groups were well-matched on these factors: duration of disease, sex, and age at the time of diagnosis of T1DM. Results: After an average of 8 years after diagnosis, only 6.6% (2/30) of the patients from IDAA1c ≤ 9 group developed any MC, whereas 21.0% (29/138) from the IDAA1c > 9 group had at least one complication (p = 0.044). Regarding hypoglycemic events, the proportion of individuals who reported at least 1 episode of hypoglycemia in the last month was 43.3 and 64.7% from the IDAA1c ≤ 9 and IDAA1c > 9 groups, respectively (p = 0.030). Regarding severe hypoglycemia, the proportion of patients presenting at least one episode in the last month and the rate of episode/patient/month were similar between groups (6.7 vs. 13.2%; p = 0.535; and 0.1/patient/month vs. 0.25/patient/month; p = 0.321). Conclusion: In a representative Brazilian population of T1DM patients, those with IDAA1c ≤ 9 presented a lower frequency of MC, as well as fewer episodes of hypoglycemia, in the month prior to the analysis.publishedVersio
