    Early results of intra-articular micro-fragmented lipoaspirate treatment in patients with late stages knee osteoarthritis: a prospective study

    Aim To analyze clinical and functional effects of intra-articular injection of autologous micro-fragmented lipoaspirate (MLA) in patients with late stage knee osteoarthritis (KOA). Secondary aims included classifying cell types contributing to the treatment effect, performing detailed MRIbased classification of KOA, and elucidating the predictors for functional outcomes. Methods This prospective, non-randomized study was conducted from June 2016 to February 2018 and enrolled 20 patients with late stage symptomatic KOA (Kellgren Lawrence grade III, n = 4; and IV, n = 16) who received an intra-articular injection of autologous MLA in the index knee joint. At baseline radiological KOA grade and MRI were assessed in order to classify the morphology of KOA changes. Stromal vascular fraction cells obtained from MLA samples were stained with antibodies specific for cell surface markers. Patients were evaluated at baseline and 12-months after treatment with visual analog scale (VAS), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), and Knee Injury and Osteoarthritis Outcome Score (KOOS).Results Three patients (15%) received a total knee replacement and were not followed up completely. Seventeen patients (85%) showed a substantial pattern of KOOS and WOMAC improvement, significant in all accounts. KOOS score improved from 46 to 176% when compared with baseline, WOMAC decreased from 40 to 45%, while VAS rating decreased from 54% to 82% (all P values were <0.001). MLA contained endothelial progenitor cells, pericytes, and supra-adventitial adipose stromal cells as most abundant cell phenotypes. Conclusion This study is among the first to show a positive effect of MLA on patients with late stages KOA

    Immunophenotyping of a Stromal Vascular Fraction from Microfragmented Lipoaspirate Used in Osteoarthritis Cartilage Treatment and Its Lipoaspirate Counterpart

    Osteoarthritis (OA) is a degenerative joint disease accompanied by pain and loss of function. Adipose tissue harbors mesenchymal stem/stromal cells (MSC), or medicinal signaling cells as suggested by Caplan (Caplan, 2017), used in autologous transplantation in many clinical settings. The aim of the study was to characterize a stromal vascular fraction from microfragmented lipoaspirate (SVF-MLA) applied for cartilage treatment in OA and compare it to that of autologous lipoaspirate (SVF-LA). Samples were first stained using a DuraClone SC prototype tube for the surface detection of CD31, CD34, CD45, CD73, CD90, CD105, CD146 and LIVE/DEAD Yellow Fixable Stain for dead cell detection, followed by DRAQ7 cell nuclear dye staining, and analyzed by flow cytometry. In SVF-LA and SVF-MLA samples, the following population phenotypes were identified within the CD45- fraction: CD31+CD34+CD73Ā±CD90Ā±CD105Ā±CD146Ā± endothelial progenitors (EP), CD31+CD34-CD73Ā±CD90Ā±CD105-CD146Ā± mature endothelial cells, CD31-CD34-CD73Ā±CD90+CD105-CD146+ pericytes, CD31-CD34+CD73Ā±CD90+CD105-CD146+ transitional pericytes, and CD31-CD34+CD73highCD90+CD105-CD146- supra-adventitial-adipose stromal cells (SA-ASC). The immunophenotyping profile of SVF-MLA was dominated by a reduction of leukocytes and SA-ASC, and an increase in EP, evidencing a marked enrichment of this cell population in the course of adipose tissue microfragmentation. The role of EP in pericyte-primed MSC-mediated tissue healing, as well as the observed hormonal implication, is yet to be investigated

    ā€œNew Alien Mediterranean Biodiversity Recordsā€ (March 2021)

    This article includes twenty (20) new records of alien and cryptogenic species in the Mediterranean Sea, belonging to six (6) Phyla (Rhodophyta, Tracheophyta, Mollusca, Arthropoda, Bryozoa, and Chordata) distributed from the Eastern Mediterranean to the Sea of Alboran. The records are reported from nine (9) countries and can be classified into two categories: new records for the Mediterranean Sea and new records of non-indigenous species expanding within the Mediterranean Sea. The first category includes the gastropod Turbo radiatus from Lebanon coasts, the portunid crab Charybdis (Charybdis) natator from Tunis southern lagoon, the mollusc Thuridilla mazda from South Spain, and the nudibranch Okenia picoensis from the Alboran coasts of Spain and from Malta. The second category includes the bivalve Nudiscintilla cf. glabra from the Aegean coast of Turkey, the rhodophyte Colaconema codicola from the North Aegean coasts of Greece, the naked band gaper Champsodon nudivittis from the Sea of Marmara, Turkey. Also, the brachyuran Gonioinfradens giardi from the Greek Ionian waters, the codlet Bregmaceros nectabanus from the Croatian coasts of the Adriatic Sea, and the bryozoan Arbopercula tenella and copepod Parvocalanus crassirostris both from the Gulf of Trieste, Slovenian and Italian coasts, respectively. New records were also reported for the ascidian Distaplia bermudensis from brackish the Gulf of Naples, Italy, the damselfish Abudefduf cf. saxatilis and the seagrass Halophila stipulacea from Sardinia, Tyrrhenian Sea, and for the fish Paranthias furcifer from the harbour of Almeria, Alboran Sea, Spain. Through these records, an understanding of the expanding mechanisms and processes and, if possible, the development of mitigation measures within the region will be further facilitated

    ClimateFish: A Collaborative Database to Track the Abundance of Selected Coastal Fish Species as Candidate Indicators of Climate Change in the Mediterranean Sea

    Under the effects of global warming, many animals and plants are undergoing rapid distribution shifts. These changes can be particularly rapid in marine fishes, and many species have responded markedly to recent increases in sea temperature. ClimateFish is an open-access database, which collates abundance data for 7 Mediterranean indigenous and 8 non-indigenous fishes, proposed as candidate indicators of climate change. These species have been selected by a network of Mediterranean scientists based on their wide distribution, responsiveness to temperature conditions and easy identification. Data are periodically collected according to a standard visual census protocol in four different depth layers. At present, the database collates data on a total number of 101'771 observed individuals belonging to the 15 target species. Counts were realized along 3142 transects carried out in 7 Mediterranean countries between 2009 and 2021. This database, associated with climate data, offers new opportunities to investigate spatiotemporal effects of climate change and to test the effectiveness of each selected indicator.     Climatic variability in Mfabeni peatlands (South Africa) since the late Pleistocene

    It has been postulated that a bipolar seesaw interhemispheric mechanism dominated the relationship between the Northern and Southern hemisphere climates since the late Pleistocene. A key test for this proposition would be to undertake palaeoenvironmental studies on terrestrial archives in climatically sensitive regions. Southern Africa's contemporary C-3 and C-4 terrestrial plant distributions display a definitive geographical pattern dictated by different growing season rainfall and temperature zones; however, the region is generally archive poor due to its overall semi-arid climate and high relief topography. The Mfabeni peatland, with a basal age of c. 47 k yrs calibrated before present (kcal yr BP), is one of the oldest continuous coastal peat deposits in Southern Africa. Molecular leaf wax isotopes (delta C-13(wax)) were generated for a 810 cm long core, and combined with previously published bulk geochemical (delta C-13(bulk), %TOC), palynological, and stratigraphic data, to reconstruct the late Pleistocene and Holocene palaeoenvironments. We interpreted environmental shifts associated with the Heinrich 4, Last Glacial Maximum, deglacial and Holocene periods, which are consistent with adjacent Indian Ocean sea surface temperature records. However, the other shorter climate perturbations during the Heinrich 5, 3, 2, 1, Antarctic cold reversal and Younger Dryas, were muted, most likely due to local hydrological overprinting on the Mfabeni record. A general anti-phase sequence was observed between the Mfabeni record and better established Northern Hemisphere events, underpinning the bipolar seesaw interhemispheric mechanism proposed for global climate forcing since the Late Pleistocene

    According to the latest reports, the Eastern Europe currently exhibits the greatest relative increase in the number of newly registered HIV infections in the world. At the same time, Central Europe remains relatively spared from the epidemic, with reported rates significantly lower than those in both Eastern and Western Europe. Croatia geographically affiliates to Central Europe, but it has two specific potential risk factors in comparison to neighboring countries: recent War events and a summer season when immigration of large number of tourists from Central and Eastern Europe is expected. Therefore, it is critical to examine AIDS attitudes in young people, increase their knowledge, monitor their behavior and warn on risks in order to prevent larger spread of epidemics from Eastern Europe to Croatia. In this study, we report on a large related survey and education program among 17-year-old high school pupils that was conducted in years immediately following the War (1996ā€“1999)

    Case report: State-of-the-art risk-modifying treatment of sudden cardiac death in an asymptomatic patient with a mutation in the SCN5A gene and a review of the literature

    Brugada syndrome is a rare hereditary disorder characterized by distinct ECG findings, complex genetics, and a high risk of sudden cardiac death. Recognition of the syndrome is crucial as it represents a paradigm of sudden death tragedy in individuals at the peak of their lives. Notably, Brugada syndrome accounts for more than 20% of sudden cardiac deaths in individuals with structurally normal hearts. Although this syndrome follows an autosomal dominant inheritance pattern, it is more prevalent and severe in males. Diagnosis is primarily based on the characteristic ECG pattern observed in the right precordial leads. Mutations in the SCN5A gene, resulting in loss of function, are the most common genetic cause. We presented a 36-year-old proband with a family history of sudden cardiac death. Although the patient was asymptomatic for Brugada syndrome, his father had experienced sudden death at the age of 36. The proband was admitted to St. Catherine's Specialty Hospital where blood was taken and subjected to next-generation sequencing (NGS) using a ā€œSudden cardiac deathā€ panel. The analysis identified a pathogenic variant in the SCN5A gene [c.4222Gā€‰&gt;ā€‰A(p.Gly1408Arg)], which is associated with autosomal dominant Brugada syndrome. Based on the positive genetic test result, the patient was referred for further examination. ECG with modified precordial lead positioning confirmed the presence of the Brugada phenotype, displaying the type-2 and type-1 ECG patterns. Therefore, we made the diagnosis and decided to implant an implantable cardioverter-defibrillator (ICD) based on the results of broad genetic NGS testing, diagnostic criteria (ECG), and considering the high burden of sudden cardiac death in the patient's family, as well as his concerns that limited his everyday activities. This case shows that genetics and personalized medicine hold immense potential in the primary prevention, diagnosis, and treatment of Brugada syndrome and sudden cardiac death
