Implantação de prótese valvular aórtica por via percutânea
Trabalho final de mestrado integrado em Medicina área cientifica de Cardiologia, apresentado à Faculdade de Medicina da Universidade de CoimbraActualmente, a estenose aórtica é a mais frequente valvulopatia na Europa, tem uma longa e
latente fase assintomática, mas quando os sintomas finalmente ocorrem, a deterioração clínica
pode ser rápida.
Pacientes com estenose aórtica severa sintomática têm uma sobrevida média de dois anos e a
substituição cirúrgica da válvula aórtica continua a ser o único tratamento efectivo que
melhora o prognóstico e a qualidade de vida. Embora os resultados sejam excelentes com a
cirurgia convencional, alguns pacientes com sintomas severos, são considerados de elevado
risco para este procedimento. Para estes pacientes até aproximadamente oito anos atrás não
havia outra opção se não serem muitas vezes deixados com estratégias subóptimas como o
tratamento médico ou a valvuloplastia por balão, como medida paliativa ou como ponte para a
cirurgia de coração aberto. Os últimos oito anos de experiência com técnicas percutâneas de
implantação de prótese valvular aórtica mostram que esta técnica menos invasiva pode tornarse
uma promissora opção terapêutica para pacientes com estenose aórtica severa sintomática.
Nesta revisão, são apresentados os tipos de válvulas aórticas implantáveis por via percutânea
que estão comercializadas, a técnica, vantagens e limitações. Temos por objectivo analisar a
literatura desde o desenvolvimento experimental até à aplicação clínica desta nova
modalidade de tratamento para pacientes com estenose aórtica severa e alto risco cirúrgico,
sendo feita uma revisão dos principais aspectos do procedimento, resultados clínicos e
hemodinâmicos a curto e médio prazo.
O primeiro caso, num humano, de implantação de prótese valvular aórtica por via percutânea
para o tratamento da estenose aórtica sintomática realizou-se em 2002. Seguiram-se uma série
de registos que demonstraram que esta nova tecnologia era viável para o tratamento de
pacientes inoperáveis ou de alto risco para a cirurgia clássica de substituição de válvula
aórtica. Resultados de recentes ensaios multicêntricos usando quer a válvula Edwards
SAPIEN quer a CoreValve Revalving System têm mostrado que o procedimento é seguro e
eficaz. Na maioria dos ensaios, as duas tecnologias estavam associadas a uma taxa de sucesso
>90% e a uma mortalidade aos 30 dias do procedimento <10% apesar dos ensaios envolverem
pacientes de risco muito elevado. Também se deve notar que numerosos estudos têm
demonstrado que estas próteses valvulares têm boas características hemodinâmicas a curto e
médio prazo.Currently the aortic valve stenosis is the most frequent valvular heart disease in Europe. It has
a long and asymptomatic phase, but when the symptoms occur, the clinical deterioration can
be rapid. Patients with severe symptomatic aortic stenosis have a survival rate of two years
and the surgical replacement of the aortic valve is still the only effective treatment which
improves the prognosis and the quality of life. Despite the excellent results of the
conventional surgery, some patients with severe symptoms are considered of high-risk for this
procedure. Until eight years ago for these patients there was no other option either than going
through medical treatment or the balloon aortic valvuloplasty as a palliative strategy or as a
bridge to open heart surgery. The last eight years of experiments with percutaneous
techniques of implantation of prosthetic cardiac valves show that this less invasive technique
can become a promising therapeutical option for patients with severe symptomatic aortic
stenosis.
This study shows the different types of aortic valves commercialized for percutaneous
implantation, the technique, advantages and limitations. Our aim is to analyse the literature
from the experimental development to the clinical application of the new treatment modality
for patients with severe symptomatic aortic stenosis and high-surgical risk. We also focused
the principle aspects of the procedure, the clinical results and hemodynamics in a short and
medium term.
The first human case of percutaneous implantation of a prosthetic cardiac valve took place in
2002. A series of other registered cases showed that this technology was possible for the
treatment of inoperable patients as well as for those who faced a high-surgical risk for the
classical aortic valve replacement surgery. The results of some recent multicentric
experiments using both the Edwards SAPIEN valve and the CoreValve Revalving System
have shown that the procedure is safe and effective. In most experiments both techniques
were associated to a success rate of >90% and a mortality by the end of the 30 days of the
procedure of <10%, although the experiments involved high risk patients. It must be noted
that many studies have shown that these valve prosthesis have good hemodynamics in a short
and medium term
Development of a novel platform for high-throughput gene design and artificial gene synthesis to produce large libraries of recombinant venom peptides for drug discovery
Tese de Doutoramento em Ciências Veterinárias na Especialidade de Ciências Biológicas e BiomédicasAnimal venoms are complex mixtures of biologically active molecules that, while presenting low immunogenicity, target with high selectivity and efficacy a variety of membrane receptors. It is believed that animal venoms comprise a natural library of more than 40 million different natural compounds that have been continuously fine-tuned during the evolutionary process to disturb cellular function. Within animal venoms, reticulated peptides are the most attractive class of molecules for drug discovery. However, the use of animal venoms to develop novel pharmacological compounds is still hampered by difficulties in obtaining these low molecular mass cysteine-rich polypeptides in sufficient amounts. Here, a high-throughput gene synthesis platform was developed to produce synthetic genes encoding venom peptides. The final goal of this project is the production of large libraries of recombinant venom peptides that can be screened for drug discovery. A robust and efficient Polymerase Chain Reaction (PCR) methodology was refined to assemble overlapping oligonucleotides into small artificial genes (< 500 bp) with high-fidelity. In addition, two bioinformatics tools were constructed to design multiple optimized genes (ATGenium) and overlapping oligonucleotides (NZYOligo designer), in order to allow automation of the high-throughput gene synthesis platform. The platform can assemble 96 synthetic genes encoding venom peptides simultaneously, with an error rate of 1.1 mutations per kb. To decrease the error rate associated with artificial gene synthesis, an error removal step using phage T7 endonuclease I was designed and integrated into the gene synthesis methodology. T7 endonuclease I was shown to be highly effective to specifically recognize and cleave DNA mismatches allowing a dramatically reduction of error frequency in large synthetic genes, from 3.45 to 0.43 errors per kb. Combining the knowledge acquired in the initial stages of the work, a comprehensive study was performed to investigate the influence of gene design, presence of fusion tags, cellular localization of expression, and usage of Tobacco Etch Virus (TEV) protease for tag removal, on the recombinant expression of disulfide-rich venom peptides in Escherichia coli. Codon usage dramatically affected the levels of recombinant expression in E. coli. In addition, a significant pressure in the usage of the two cysteine codons suggests that both need to be present at equivalent levels in genes designed de novo to ensure high levels of expression. This study also revealed that DsbC was the best fusion tag for recombinant expression of disulfide-rich peptides, in particular when expression of the fusion peptide was directed to the bacterial periplasm. TEV protease was highly effective for efficient tag removal and its recognition sites can tolerate all residues at its C-terminal, with exception of proline, confirming that no extra residues need to be incorporated at the N-terminus of recombinant venom peptides. This study revealed that E. coli is a convenient heterologous host for the expression of soluble and potentially functional venom peptides. Thus, this novel high-throughput gene synthesis platform was used to produce ~5,000 synthetic genes with a low error rate. This genetic library supported the production of the largest library of recombinant venom peptides constructed until now. The library contains 2736 animal venom peptides and it is presently being screened for the discovery of novel drug leads related to different diseases.RESUMO - Desenvolvimento de uma nova plataforma de alta capacidade para desenhar e sintetizar genes artificiais, para a produção de péptidos venómicos recombinantes - Os venenos animais são misturas complexas de moléculas biologicamente activas que se ligam com elevada selectividade e eficácia a uma grande variedade de receptores de membrana. Embora apresentem baixa imunogenicidade, os venenos podem afectar a função celular actuando ao nível dos seus receptores. Actualmente, pensa-se que os venenos de animais constituam uma biblioteca natural de mais de 40 milhões de moléculas diferentes que têm sido continuamente aperfeiçoadas ao longo do processo evolutivo. Tendo em conta a composição dos venenos, os péptidos reticulados são a classe mais atractiva de moléculas com interesse farmacológico. No entanto, a utilização de venenos para o desenvolvimento de novos fármacos está limitada por dificuldades em obter estas moléculas em quantidades adequadas ao seu estudo. Neste trabalho desenvolveu-se uma plataforma de alta capacidade para a síntese de genes sintéticos codificadores de péptidos venómicos, com o objectivo de produzir bibliotecas de péptidos venómicos recombinantes que possam ser rastreadas para a descoberta de novos medicamentos. Com o objectivo de sintetizar genes pequenos (< 500 pares de bases) com elevada fidelidade e em simultâneo, desenvolveu-se uma metodologia de PCR (polymerase chain reaction) robusta e eficiente, que se baseia na extensão de oligonucleótidos sobrepostos. Para possibilitar a automatização da plataforma de síntese de genes, foram construídas duas ferramentas bioinformáticas para desenhar simultaneamente dezenas a milhares de genes optimizados para a expressão em Escherichia coli (ATGenium) e os respectivos oligonucleótios sobrepostos (NZYOligo designer). Esta plataforma foi optimizada para sintetizar em simultâneo 96 genes sintéticos, tendo-se obtido uma taxa de erro de 1.1 mutações por kb de DNA sintetizado. A fim de diminuir a taxa de erro associada à produção de genes sintéticos, desenvolveu-se um método para remoção de erros utilizando a enzima T7 endonuclease I. A enzima T7 endonuclease I mostrou-se muito eficaz no reconhecimento e clivagem de moléculas DNA que apresentam emparelhamentos incorrectos, reduzindo drasticamente a frequência de erros identificados em genes grandes, de 3.45 para 0.43 erros por kb de DNA sintetizado. Investigou-se também a influência do desenho dos genes, da presença de tags de fusão, da localização celular da expressão e da actividade da protease Tobacco Etch Virus (TEV) para a remoção eficiente de tags, na expressão de péptidos venómicos ricos em cisteínas em E. coli. A utilização de codões meticulosamente escolhidos afectou drasticamente os níveis de expressão em E. coli. Para além disso, os resultados mostram que existe uma pressão significativa no uso dos dois codões que codificam para o resíduo cisteína, o que sugere que ambos os codões têm de estar presentes, em níveis equivalentes, nos genes que foram desenhados e optimizados para garantir elevados níveis de expressão. Este trabalho indicou também que o tag de fusão DsbC foi o mais apropriado para a expressão eficiente de péptidos venómicos ricos em cisteínas, particularmente quando os péptidos recombinantes foram expressos no periplasma bacteriano. Confirmou-se que a protease TEV é eficaz na remoção de tags de fusão, podendo o seu local de reconhecimento conter quaisquer aminoácidos na extremidade C-terminal, com excepção da prolina. Desta forma, verificou-se não ser necessário incorporar qualquer aminoácido extra na extremidade N-terminal dos péptidos venómicos recombinantes. Reunindo todos os resultados, verificou-se que a E. coli é um hospedeiro adequado para a expressão, na forma solúvel, de péptidos venómicos potencialmente funcionais. Por último, foram produzidos, com uma taxa de erro reduzida, ~5000 genes sintéticos codificadores de péptidos venómicos utilizando a nova plataforma de elevada capacidade para a síntese de genes aqui desenvolvida. A nova biblioteca de genes sintéticos foi usada para produzir a maior biblioteca de péptidos venómicos recombinantes construída até agora, que inclui 2736 péptidos venómicos. Esta biblioteca recombinante está presentemente a ser rastreada com o objectivo de descobrir novas drogas com interesse para a saúde humana
Dehumanization revisited: the humanization of ingroup and outgroup members depends on the perception of social relatedness
Others with whom we do not relate are sometimes likened to objects or machines. Perceiving
others as relational partners could contribute to their acknowledgment as human beings.
To test the idea that social relatedness constitutes a fundamental dimension of humanness
perception is the main goal of this thesis. It is hypothesized that perceiving the outgroup as
taking part in relations with the ingroup reduces outgroup dehumanization to a greater extent
than when its members are not taking part in such relations. It also aims to explore the extent
to which the ingroup is (de)humanized according to its participation in social relations.
It was found that social relations between ingroup and outgroup decreased outgroup
dehumanization when compared to the absence of relation. Evidence on the moderating role
of status in the perceived humanness of high and low status, fictional and natural groups, was
also presented. Additionally, social relations between outgroup members (in which ingroup
members were not involved) also had an effect on the perceived humanness of both groups.
Finally, initial findings of studies using a minimal groups’ paradigm were presented. In these
studies, outgroup dehumanization also occurred but was not moderated by (anticipated) social
relations between the groups.
This thesis represents a new take on this phenomenon by providing a relational approach to
the (de)humanization of ingroup and outgroup. Therefore, social relatedness is advanced as a
dimension that underlies how human we perceive our own group and other groups to be.A ausência de relação com os outros por vezes leva a que estes sejam comparados a objectos
ou máquinas. Percebê-los enquanto parceiros relacionais poderia contribuir para o seu
reconhecimento enquanto seres humanos.
O principal objectivo desta tese é o de testar a ideia de que as relações sociais constituem uma
dimensão fundamental da percepção de humanidade. Hipotetiza-se que perceber o exogrupo
como participando em relações com o endogrupo reduz a dehumanização do exogrupo em
maior grau do que a ausência de relação. Pretende-se também explorar o grau em que o
endogrupo é (des)humanizado de acordo com a sua participação em relações sociais.
Verificou-se que as relações sociais entre o endogrupo e o exogrupo diminuíram a
desumanização exogrupal quando comparadas com a ausência de relação. Mostrou-se também
evidências sobre o papel moderador do estatuto na humanidade percebida de grupos de alto e
baixo estatuto, tanto fictícios como naturais. Adicionalmente, constatou-se que as relações
entre membros do exogrupo (nas quais o endogrupo não estava envolvido) também afectaram
a humanidade percebida de ambos os grupos. Finalmente, resultados iniciais de estudos
usando um paradigma de grupos mínimos foram apresentados. Nestes estudos, ocorreu
desumanização do exogrupo mas esta não foi moderada pelas relações sociais (antecipadas)
entre os grupos.
Esta tese representa uma nova visão sobre este fenómeno através de uma abordagem
relacional sobre a (des)humanização do endogrupo e do exogrupo. As relações sociais são
então avançadas como uma dimensão subjacente à forma como percebemos o nosso próprio
grupo e outros enquanto humanos
Atitudes face à sexualidade na deficiência intelectual: a importância da formação em educação sexual
Dissertação de mest., Psicologia da Educação, Faculdade de Ciências Humanas e Sociais, Univ. do Algarve, 2013A educação sexual assume-se como uma importante influência no desenvolvimento de atitudes e comportamentos relativos à manifestação da sexualidade e à forma como se está no mundo, como nos percecionamos nós e como percecionamos o outro, exercendo elevado impacto no modo de interagir com o exterior.
A presente investigação tem como objetivo principal analisar as atitudes face à sexualidade na deficiência intelectual, considerando a importância da formação em educação sexual.
O trabalho de investigação decorreu em três instituições distintas. A amostra que integra o estudo é constituída por 77 indivíduos (N=77), 32 participantes integram o subgrupo de jovens com deficiência intelectual, 11 dos indivíduos integram o subgrupo dos pais, e 34 dos indivíduos são profissionais que trabalham com a população deficiente intelectual. Os instrumentos que serviram de suporte no estudo dizem respeito a três questionários desenvolvidos para a presente investigação, que se destinam a cada subgrupo que constituem à amostra. O questionário elaborado para os jovens teve como base um instrumento adaptado por Macário (2010), “Questionário sobre Educação Sexual em Contexto Escolar- Alunos”- QUESCE-A. Relativamente aos questionários destinados aos pais e profissionais estes foram elaborados a partir de duas escalas utilizadas em estudos anteriores, nomeadamente “Atitudes dos pais face à afetividade e sexualidade do jovem com deficiência mental” (Ramos, 2005) e a escala “Sexuality and Mentally Retarded Attitudes Inventory (SMRAI)” (Félix, 2001).
De acordo com os resultados a generalidade os inquiridos consideraram a educação sexual como um domínio com relevância nas suas vidas. Permitindo, concluir que a educação sexual é uma área que assume importância na formação de qualquer pessoa, tornando-se mais significativa na população com deficiência intelectual. Conclui-se, assim, que a implementação de um programa de educação sexual exerce impacto ao nível das atitudes e conhecimentos de jovens, pais e profissionais, melhorando desta forma a atuação nos diversos contextos de vida que integram
Estudo exploratório para a definição de uma tipologia do comportamento criminal no homicídio
As causas que conduzem um indivíduo a matar outro são muitas e existem a vários níveis. Assim, o fenómeno homicida, devido à sua multifactoriedade, contribui para revelar características da conduta humana, onde factores biológicos, psicopatológicos e psicossociais influenciam todo o processo.
O presente trabalho possui como objectivo geral a definição de uma tipologia do comportamento criminal no homicídio. Para a obtenção de tal tipologia, este estudo visa a elaboração de perfis identificando padrões de condutas criminais. Assim, avaliam-se, não só os comportamentos criminais, mas também as características sócio – demográficas do agressor e vítima, bem como as características psicológicas e jurídico – penais do agressor. Ademais, como objectivo específico deste estudo, pretende-se também evidenciar a multiplicidade de factores que vão condicionar o sujeito no momento em que comete o delito, já que estudar com exactidão o fenómeno ajudar-nos-á a ter uma noção mais objectiva desta realidade.
As análises estatísticas efectuadas revelaram três perfis criminais com características distintas. No primeiro, expressivo – impulsivo, os homicídios são motivados por uma discussão acalorada, ocorrem predominantemente entre conhecidos, frequentemente originados por questões triviais e em estado de desinibição alcoólica. O segundo, expressivo – íntimo, é geralmente perpetrado por indivíduos do sexo masculino, com psicopatologia prévia frequente e que cometem homicídio – suicídio num contexto em que se sentem rejeitados ou traídos. Por fim, o instrumental – cognitivo caracteriza-se pela presença de crimes sexuais e roubo, múltiplas feridas, ocultação da participação pelo agressor e ocultação do cadáver após o delito, bem como por motivos de natureza económica.There are many causes that lead an individual to kill another and which reside at various levels. Thus, the homicidal phenomenon, because of the numerous factors that describe it, reveals characteristics of human conduct, where biological, psychopathological and psychosocial factors influence the whole process.
Therefore, this study aims the definition of a typology of criminal behaviours on homicide. To obtain such a typology, this study consists of the elaboration of profiles through the identification of patterns of criminal conducts. For that reason, criminal behaviours, socio-demographic and psychological characteristics of the offender and victim, as well as the legal features of the offender are assessed. Moreover, a specific goal of this study is also to demonstrate the multitude of factors that influence the subject at the time he commits the crime, since accurately studying this phenomenon will help gaining a more objective notion of such reality.
Through statistical analysis three criminal profiles with distinct characteristics were obtained. In the first one, expressive - impulsive, the homicides are motivated by a heated discussion, they occur predominantly among acquaintances, often caused by trivial matters and in a state of alcoholic disinhibition. The second, expressive - intimate, is generally perpetrated by male subjects, often with prior psychopathology and who commit murder-suicide since they feel rejected or betrayed. Finally, the instrumental – cognitive, is characterized by the presence of sexual crimes, theft, multiple wounds, concealment of participation by the offender, concealment of the body after the crime, as well as economic motivations
Modelo lógico de um programa de intervenção comunitária – GerAcções
Os modelos lógicos são uma maneira sucinta de
mostrar e ilustrar como um projecto foi concebido e
está a ser desenvolvido e resume os principais
elementos do projecto. O modelo lógico do programa
GerAccões é apresentado. Foi desenvolvido como
uma forma de planeamento estratégico, que vem sendo
utilizado para uma melhoria contínua do programa e,
agora, no final deste, para o planeamento da avaliação.
Os componentes do modelo lógico do GerAcções são:
(1) Missão do programa – envolver as pessoas que
moram ou trabalham na Freguesia de Santa Maria de
Belém (Lisboa), como os membros-chave na
promoção dos seus interesses e na resolução dos seus
problemas, afim de construir uma comunidade
saudável; (2) Recursos – parcerias, coligações
comunitárias e patrocinadores; (3) Objectivos –
Gerais: envolvimento de crianças e jovens, famílias e
seniores no seu próprio processo de desenvolvimento,
bem como o envolvimento da comunidade e parceiros
da rede social para uma intervenção integrada no
processo de construção de uma comunidade saudável,
e Específicos: o empowerment comunitário, a
promoção da saúde e prevenção da violência, o bemestar
e qualidade de vida; (4) Actividades – Conselho
Consultivo, a intervenção nas escolas, Roteiro de
espaços lúdicos, Centro Jovem e Centro Sénior,
Formação Parental, Comissão Social de Freguesia, Dia
Europeu dos Vizinhos, Publicações; (5) Cronograma –
desde Março de 2006 até Fevereiro de 2010; (6)
Resultados – maior conhecimento em relação aos
comportamentos positivos para a saúde, o aumento das
competências pessoais e sociais para as crianças e os
jovens, os pais aumentarem seus conhecimentos e
competências de comunicação positiva, aumentar a
participação comunitária, estabelecimento de redes
de suporte formais e informais; (7) Indicadores –
número de participantes, conhecimentos e competências
alcançados, mudança de atitudes e de condições;
(8) Medidas – quantitativa como inquéritos e aplicação
de escalas, e qualitativas como grupos de discussão e
entrevistas; (9) Sustentabilidade; e (10) Avaliação.
A avaliação do programa GerAcções é pertinente,
neste momento, providenciando informação sobre o
valor e a importância do programa através de investigação
sistemática. A avaliação tem um objectivo e as
informações fornecidas serão usadas para facilitar a
decisão do rumo das acções.

illustrate how a project was designed and is being
developed and outlines the main elements of the
project. The logic model of GerAccões’ program is
presented. It was developed as a form of strategic
planning, which has been used for a continuous
improvement program, and now at the end of program
for evaluation planning. The components of the
GerAcções logic model are: (1) Mission of the
program – involving people who live or work in the
borough of Santa Maria de Belém (Lisbon), as key
members in promoting their interests and to solve their
problems in order to build a healthy community, (2)

Novel Perspectives in Management of Angiogenesis and Cancer Therapy

FL-C PhD fellowship was funded by FCT (PD/BD/128337/2017).The activation of endothelial cells (ECs) is a crucial step on the road map of tumor angiogenesis and expanding evidence indicates that a pro-oxidant tumor microenvironment, conditioned by cancer metabolic rewiring, is a relevant controller of this process. Herein, we investigated the contribution of oxidative stress-induced ferroptosis to ECs activation. Moreover, we also addressed the anti-angiogenic effect of Propranolol. We observed that a ferroptosis-like mechanism, induced by xCT inhibition with Erastin, at a non-lethal level, promoted features of ECs activation, such as proliferation, migration and vessel-like structures formation, concomitantly with the depletion of reduced glutathione (GSH) and increased levels of oxidative stress and lipid peroxides. Additionally, this ferroptosis-like mechanism promoted vascular endothelial cadherin (VE-cadherin) junctional gaps and potentiated cancer cell adhesion to ECs and transendothelial migration. Propranolol was able to revert Erastin-dependent activation of ECs and increased levels of hydrogen sulfide (H2S) underlie the mechanism of action of Propranolol. Furthermore, we tested a dual-effect therapy by promoting ECs stability with Propranolol and boosting oxidative stress to induce cancer cell death with a nanoformulation comprising selenium-containing chrysin (SeChry) encapsulated in a fourth generation polyurea dendrimer (SeChry@PUREG4). Seasonal sensory evaluation of low commercial value or unexploited fish species from the Portuguese coast
Overfishing is increasing over time, and according to FAO (Food and Agriculture Organization), about one third of the world’s fish stocks are now overfished. Thus, diversifying the target species is essential for fisheries sustainability contributing to improve resource-effcient processes. Non-target species can be valuable resources for the development of new food products. However, those species are scarcely studied, and it is of high importance to trace their seasonal sensory profile as a first step towards their valorisation. Therefore, in this study, seasonal influence on sensory properties of five low commercial value or unexploited fish species, namely Trachurus picturatus (blue jack mackerel), Spondyliosoma cantharus (black seabream), Trigla lyra (piper gurnard), Serranus cabrilla (comber) and Capros aper (boarfish), was assessed in order to identify the most favourable season for catching each species. Fish samples were assessed by a panel of 16 semi-trained assessors for sensory attributes previously identified. The evaluation takes place every 2 months. Statistical differences were reported between attributes and seasons for all species, except for T. lyra, which did not present any di erence in its sensory attributes throughout the year.info:eu-repo/semantics/publishedVersio
Comparison with adult-onset rheumatic diseases
Objective To compare physical disability, mental health, fatigue and health-related quality of life (HRQoL) across juvenile idiopathic arthritis (JIA) categories in adulthood and between JIA and adult-onset rheumatic diseases. Methods Cross-sectional analysis nested in a cohort of adult patients with JIA registered in the Rheumatic Diseases Portuguese Register (Reuma.pt). Physical disability (Health Assessment Questionnaire - Disability Index), mental health symptoms (Hospital Anxiety and Depression Scale), fatigue (Functional Assessment of Chronic Illness Therapy - Fatigue Scale (FACIT-F)) and HRQoL (EuroQol-5D (EQ5D) and Short Form (SF-36)) were compared across JIA categories. Patients with polyarticular JIA and enthesis-related arthritis (ERA) JIA were compared respectively to patients with rheumatoid arthritis (RA) and spondyloarthritis (SpA), matched for gender and age, adjusted for disease duration and activity. Results 585 adult patients with JIA were included. Comparison across JIA categories showed that persistent oligoarthritis and patients with ERA reported a higher score in EQ5D and SF-36 physical component when compared with other JIA categories. Polyarticular JIA reported less disability and fatigue than patients with RA (median Health Assessment Questionnaire of 0.25 vs 0.63; p<0.001 and median FACIT-F score 42 vs 40; p=0.041). Polyarticular JIA had also better scores on EQ5D and all domains of SF-36, than patients with RA. Patients with ERA reported less depression and anxiety symptoms (0% vs 14.8%; p=0.003% and 9% vs 21.3%; p=0.002) and less fatigue symptoms (45 vs 41; p=0.01) than patients with SpA. Conclusion Persistent oligoarticular JIA and ERA are the JIA categories in adulthood with better HRQoL. Overall, adult polyarticular and patients with ERA JIA have lower functional impairment and better quality-of-life than patients with RA and SpA.publishersversionpublishe
