    Cross-cultural validation and psychometric evaluation of the Self-Medication Assessment Tool (SMAT) for assessing and optimizing medication therapy management of older people

    Background, rationale and objectives: The assessment of medication management ability in the elderly can be performed using specific tools, such as the Self-Medication Assessment Tool, which considers real and simulated regimens. The objective of this study was to perform the linguistic and cultural adaptation of the Self-Medication Assessment Tool to European Portuguese and determine its psychometric properties. Methods: The adaptation commenced with the translation/back translation cycle completed by 4 independent bilingual experts. The cultural component was accomplished through an external expert meeting and a longitudinal screening of concepts and construct. The pilot study was carried out in a sample of 150 Portuguese community-dwelling elders. Descriptive data, correlations, internal reliability, response consistency and exploratory factor analysis was conducted using SPSS Statistics (v22). Results: The pilot study was carried out in a sample of 150 community-dwelling elders: 112 (74.7%) participants were women; mean age was 74.73 ± 6.43 years. The Self-Medication Assessment Tool (Portuguese version) standard regimen (simulated medication regimen) mean scores were 20.92 ± 6.83 in functional ability and 38.75 ± 5.92 in cognitive ability; the real regimen (medication taken by the elderly) mean scores were 83.74 ± 15.86 in medication recall, 96.96 ± 11.39 in adherence self-report and 4.82 ± 10.1 in intentional non-adherence. Cronbach's α were 0.87 (functional ability), 0.84 (cognitive ability), 0.57 (medication recall), 0.94 (adherence self-report) and 0.79 (intentional non-adherence). The response consistency between test and re-test was verified. Conclusions: We have developed the European Portuguese version of the Self-Medication Assessment Tool with acceptable psychometric properties which can now be employed in the study of the elderly in clinical and research contexts

    This paper presents the results of a systematic review of the literature (SRL) in the field of Participatory Design (PD), undertaken under the scope of LOCUS – Playful Connected Rural Territories. The project employs an ethnographically, participatory and agile approach to the process of co-designing, developing and evaluating an IoT system to support playful intergenerational engagement in exploring cultural heritage of Portuguese rural territories, by engaging inhabitants, stakeholders and visitors. The SRL aimed to understand how older adults have been integrated and engaged into technology PD teams and processes. This paper focus on the most used methodological approaches and participation methods, along with the challenges in integrating older adults and the strategies to overcome them, which are discussed in the light of project aims. / Cet article présente les résultats d'une revue systématique de la littérature dans le domaine du design participatif, effectuée dans le cadre du Projet LOCUS - Territoires ruraux connectés et ludiques. Le projet utilise une approche ethnographique, participative et agile du processus de co-design, développement et évaluation d'un système d’Internet des objets pour soutenir un engagement intergénérationnel ludique dans l'exploration du patrimoine culturel des territoires ruraux portugais, en impliquant les habitants, les parties prenantes et les visiteurs. La revue systématique de la littérature vise à comprendre comment les personnes âgées ont été intégrées et engagées dans des équipes et des processus de design participatif des technologiques. Cet article se concentre sur les approches méthodologiques et les méthodes de participation les plus utilisées, ainsi que sur les défis de l'intégration des personnes âgées et les stratégies pour les surmonter, qui sont discutés à la lumière des objectifs du projet

    Community-based pulmonary rehabilitation during acute exacerbations of COPD

    The role of pulmonary rehabilitation (PR) during acute exacerbations of chronic obstructive pulmonary disease (AECOPD) is controversial. Most studies have been conducted in hospitalised and severe patients, despite more than 80% of AECOPD being managed on an outpatient basis. Thus, this study assessed the effects of a community-based PR program during AECOPD. 23 patients with AECOPD were voluntarilly allocated to intervention (IG: n=12, 69±7yrs, FEV1 52±27pp) or control group (CG: n=11, 66±9yrs, FEV1 55±22pp). The IG received standard medication plus 6 sessions (2*/week) of PR (i.e., breathing control and airway clearance techniques, thoracic mobility and expansion exercises, exercise training and psychoeducational support). The CG received medication only. Respiratory rate (RR), COPD Assessment Test (CAT) and quadriceps muscle strength (QMS) were assessed within 48h of the AECOPD onset (Pre) and after PR (Post). Wilcoxon tests were used for Pre/Post comparisons within groups. The Pre/Post differences per group were pooled and groups were compared with Mann-Whitney tests. Only the EG improved significantly in all outcomes (Fig. 1). The addition of PR in the management of AECOPD seems to be more effective than only usual medication in improving patients’ RR, impact of the disease and QMS, parameters associated with an increased risk of AECOPD and poor prognosis.     Intergenerational Perspectives on Media and Fake News During Covid-19: Results From Online Intergenerational Focus Groups

    This article reflects on intergenerational perspectives on media habits and fake news during Covid-19. Active participation is closely linked to the citizens’ media literacy competencies. Due to the Covid-19 pandemic, inequalities in access, use, and understanding of the information conveyed by the media became more evident. Digital skills are essential to encourage co-learning and active ageing among different generations. This article relies on data collected during two online intergenerational focus groups with family pairs of different ages (grandparents and grandchildren) conducted in Portugal in the context of the European project SMaRT-EU. The focus groups addressed subjects such as news, fake news, critical perspective towards social networks and digital communication, and younger and older people’s perspectives regarding these matters. The thematic analysis of the Portuguese data suggests that, by placing grandparents and grandchildren side by side, the online intergenerational focus groups promoted sharing and exchange of knowledge, valuing the intergenerational encounter and the voices of one of society’s most fragile groups. Data also shows that participants have different perspectives on communication and digitally mediated interaction, mainly related to age factors and media literacy skills. As for fake news, although grandparents and grandchildren show awareness of the phenomenon, for the youngest participant it was complex to identify characteristics or the spaces where they are disseminated. The young adult participant was the most proficient and autonomous digital media user. Results further indicate that, although the online environment contributed to continuing research in times of pandemic, bringing together family members with different media literacy skills and ages poses difficulties related to the recruitment of participants

    Qualidade de Vida Relacionada com a Saúde Oral:   Perceção Parental das Crianças em Idade Pré‐Escolar

    Resumo Enquadramento: O conhecimento dos determinantes de saúde que afetam a saúde oral e a qualidade de vida das crianças são da maior importância para alavancar a definição de políticas de redução do risco de doença e de promoção da saúde (DGS, 2008b). Objetivo: Avaliar a perceção dos pais sobre a Qualidade de Vida Relacionada com a Saúde Oral das crianças em idade pré-escolar. Metodologia: Estudo descritivo-correlacional e transversal. Foi utilizado o Questionário Qualidade de Vida Relacionado com a Saúde Oral das Crianças/Adolescentes (Q2VRSOC/A), construído por Pahel, Rozier e Slade, (2007) e traduzido e validado por Bica-Costa (2013), numa amostra de 412 pais/educadores de crianças (50,5% meninos e 49,5% meninas), com idades compreendidas entre os 3 e 6 anos ( ݔ̅=4,33; DP=0,934), a frequentar jardins de infância do distrito de Viseu. Resultados: Apurou-se que 47,5% das crianças/adolescentes apresenta bons hábitos de higiene oral; 31,9% fracos e 20,6% razoáveis. A Qualidade de Vida Relacionada com a Saúde Oral é influenciada pela idade da criança (p=.000); pela sua preferência alimentar (p=.014); pelo estado de saúde geral (p=.034) e oral (p=.000); pela abordagem da saúde e higiene oral realizada pelo enfermeiro (p=.026); pela observação da boca e dentes das crianças pelos pais (p=.006); pela consulta no dentista (p=.000) e pela presença de cáries dentárias (p=.000). Conclusão: Pelos resultados obtidos infere-se que quanto maior o impacto dos problemas orais, pior é a qualidade de vida relacionada com a saúde oral das crianças e, sendo assim, é de primordial importância desenvolver ações de educação para a saúde oral.Abstract Background: The knowledge of the determinants of health that affect oral health and quality of life of children are of paramount importance to leverage the definition of policies to reduce risk and promote health (DGS, 2008b). A questionnaire was used Quality of Life Related to Oral Health for Children / Teens (Q2VRSOC/A) in a sample of 412 parents/carers of children (50.5% boys and 49.5% girls), aged between 3 and 6 years (ݔ̅= 4.33; DP=0,934), who attended the kindergartens from the district of Viseu. Results: The majority of oral hygiene habits are good (47.5%), 31.9% are poor and 20.6% reasonable. The overall score Q2VRSOC/A is influenced by age (p = .000), food preference (p = .014), general and oral health (p = .034 p = .000, respectively); approach to health and oral hygiene performed by nurses (p = .026); observation of the mouth and teeth of children (p = .006); consultation at the dentist (p = .000); and for the presence of dental caries (p = .000).
