    The application of innovative three-dimensional (3D) spheroids cell culture strategy to Parasitology offers the opportunity to closely explore host–parasite interactions. Here we present a first report on the application of 3D hepatic spheroids to unravel the immune response of canine hepatocytes exposed to Leishmania infantum. The liver, usually considered a major metabolic organ, also performs several important immunological functions and constitutes a target organ for L. infantum infection, the etiological agent of canine leishmaniasis (CanL), and a parasitic disease of major veterinary and public health concern. 3D hepatic spheroids were able to sense and immunologically react to L. infantum parasites, generating an innate immune response by increasing nitric oxide (NO) production and enhancing toll-like receptor (TLR) 2 and interleukin-10 gene expression. The immune response orchestrated by canine hepatocytes also lead to the impairment of several cytochrome P450 (CYP450) with possible implications for liver natural xenobiotic metabolization capacity. The application of meglumine antimoniate (MgA) increased the inflammatory response of 3D hepatic spheroids by inducing the expression of Nucleotide oligomerization domain (NOD)-like receptors 1 and NOD2 and TLR2, TLR4, and TLR9 and enhancing gene expression of tumour necrosis factor α. It is therefore suggested that hepatocytes are key effector cells and can activate and orchestrate the immune response to L. infantum parasites.publishersversionpublishe

    Determinantes sociais da saúde e prevenção secundária do câncer do colo do útero no Estado do Amazonas, Brasil

    Modelo do estudo: este foi um estudo ecológico transversal e descritivo. Objetivo do estudo: conhecer os determinantes sociais de saúde associados com a prevenção secundária do câncer do colo do útero no período de 2010 a 2014. Metodologia: a unidade amostral foi os 62 municípios do estado do Amazonas. Os dados secundários utilizados foram do Sistema de Informação do Câncer do Colo do Útero (SISCOLO), do Sistema de Informação da Atenção Básica (SIAB), do Instituto Brasileiro de Geografia e Estatística (IBGE) e do Atlas de Desenvolvimento Humano do Brasil. Resultados: baixa cobertura do exame Preventivo do câncer do colo do útero para mulheres de 25 a 64 anos, sendo a faixa etária com menor adesão ao rastreio, a de 60 a 64 anos. A média de analfabetismo feminino na idade de 25 anos ou mais, no Estado, foi de 25,01%, e este indicador apresentou correlação com a baixa cobertura do exame Preventivo para o câncer do colo do útero em todos os anos. A pouca cobertura da Estratégia Saúde da Família também se associou a baixa adesão ao rastreio. Além disso, a Proporção de Municípios com Amostras Insatisfatórias dos exames citopatológicos apresentou-se acima da meta preconizada pelo Ministério da Saúde. Conclusão: estratégias de promoção da saúde voltadas a adesão ao rastreio devem considerar o nível de escolaridade das mulheres e o fortalecimento da Atenção Primária à Saúde pautada no rastreio não oportunístico; estas devem ser uma das prioridades das políticas públicas nos municípios do Estado.Study design: This was a cross-sectional and ecological descriptive study. Study objective: Knowing the social determinants of health associated with the secondary prevention of cervical cancer from 2010 to 2014. Methods: The sample included the 62 counties of the state of Amazonas, and secondary data were used from the Cervical Cancer Information System, the Primary Care Information System, the Brazilian Institute of Geography and Statistics and the Atlas of Human Development of Brazil. Results: Low coverage of the preventive exam for cervical cancer for women aged 25 to 64 years was found. Also, the age group with the lowest adherence to screening was 60 to 64 years old. The average illiteracy rate among women aged 25 years and over, in the state, was 25.01%, and this indicator correlated with the low coverage of the preventive exam for cervical cancer every year. The low coverage of the Family Health Strategy was also associated with low adherence to screening. The proportion of counties with unsatisfactory samples from the cytopathological examinations was above the goal recommended by the Ministry of Health. Conclusion: Health promotion strategies regarding adherence to screening should consider women’s level of education, and the reinforcement of Primary Health Care based on non-opportunistic screening should be one of the priorities of public policies in the counties of the state

    ESTUDO EPIDEMIOLÓGICO DE PACIENTES COM LEISHMANIOSE TEGUMENTAR AMERICANA EM BURITICUPU, PRÉ-AMAZÔNIA MARANHENSE

    A LTA constitui um sério problema de saúde pública; trata-se de uma doença endêmica no Brasil, tendo distribuição em todo território nacional, com maior casuística na Pré-Amazônia Maranhense. Causada por diferentes espécies de protozoários do gênero Leishmania Ross, 1903, esta doença caracteriza-se pelo comprometimento cutâneo e, dependendo da espécie de Leishmania e de fatores inerentes ao hospedeiro, podem ocorrer lesões mutilantes nas mucosas nasobucofaríngea. Foram atendidos 35 pacientes na cidade de Buriticupu, estado do Maranhão, dos quais a maioria era do gênero masculino (85,7%), apresentava um tempo de evolução da doença inferior a 2 meses (51,4%) e média de idade de 33,9±15,9 anos. Essas características indicam que, embora a busca ao serviço diagnóstico se dê na fase aguda, a doença continua a avançar nas regiões pesquisadas.Descritores:  Leishmaniose Tegumentar Americana; Epidemiologia; Leishmania.Abstract: Epidemiological study of patients with American Cutaneous Leishmaniasis in Buriticupu, pre-amazon region of Maranhão. ACL constitutes a serious public health problem; it is an endemic disease in Brazil, being distributed in all of the nation’s territory, with higher casuistic in the pre-amazon portion of Maranhão. Caused by many different protozoan species from the genus Leishmania Ross, 1903; this disease is characterized by its cutaneous compromises, and, depending on the species of Leishmania and on inherent host factors, there may occur mutilating lesions on the nasobuccopharingeal mucosae. 35 patients with ACL were assessed in the cities of São Luís and Buriticupu, state of Maranhão, from which the majority were males (85,7%), had a disease evolution time of below 2 months (51,4%) and a mean age of 33,9±15,9 years old. These characteristics indicate that, although the diagnostics made within the acute phase of the disease, it is still progressing in the locations researched.Descriptors: American Cutaneous Leishmaniasis; Epidemiology; Leishmania

    COVID-19 in Brazilian children and adolescents: findings from 21 hospitals / COVID-19 em crianças e adolescentes brasileiros: registros de 21 hospitais

    Introdução: Crianças e adolescentes com Covid-19 apresentam menor mortalidade e sintomas menos intensos quando comparados aos adultos. Os estudos no Brasil baseiam-se apenas no sistema de notificação compulsória. Objetivo: Analisar as características clínicas, laboratoriais, radiológicas e desfechos de pacientes hospitalizados com menos de 20 anos de idade com Covid-19. Métodos: Série de casos de pacientes internados com Covid-19, confirmado, com idade inferior a 20 anos, obtida em estudo de coorte em 21 hospitais de cinco estados brasileiros. Resultados: Dos 36 pacientes, 20 (55,5%) eram adolescentes, 20 (55,5%) eram do sexo masculino, 18 (50,0%) apresentavam comorbidades, 2 estavam grávidas; e em 7 (19,4%) os sintomas iniciais ocorreram durante a internação por outras causas, dos quais 3 foram possivelmente infectados no hospital. Febre (61,1%), dispneia (33,3%) e sintomas neurológicos (33,0%) foram as queixas mais comuns. A proteína C reativa estava acima de 50mg / L em 16,7% e o dímero-D estava acima do limite de referência em 22,2%. Radiografias de tórax foram realizadas em 20 (55,5%) pacientes, 9 apresentavam anormalidades; e tomografias computadorizadas de tórax em 5. O tempo de internação variou de 1-40 dias (mediana 5 [intervalo interquartil 3-10]), 16 (44,4%) necessitaram de cuidados intensivos, 6 (16,7%) necessitaram de ventilação mecânica e um paciente (2,8%) faleceu. Conclusão: Em uma amostra de pacientes menores de 20 anos, procedentes de hospitais de 5 estados do Brasil, as comorbidades foram frequentes e os sintomas mais comuns foram febre, dispneia e sintomas neurológicos. Quarenta e quatro por cento dos pacientes necessitaram de cuidados intensivos, mostrando que na amostra avaliada a doença não era tão leve quanto o esperado, e um paciente morreu. 

    SARS-CoV-2 introductions and early dynamics of the epidemic in Portugal

    Genomic surveillance of SARS-CoV-2 in Portugal was rapidly implemented by the National Institute of Health in the early stages of the COVID-19 epidemic, in collaboration with more than 50 laboratories distributed nationwide. Methods By applying recent phylodynamic models that allow integration of individual-based travel history, we reconstructed and characterized the spatio-temporal dynamics of SARSCoV-2 introductions and early dissemination in Portugal. Results We detected at least 277 independent SARS-CoV-2 introductions, mostly from European countries (namely the United Kingdom, Spain, France, Italy, and Switzerland), which were consistent with the countries with the highest connectivity with Portugal. Although most introductions were estimated to have occurred during early March 2020, it is likely that SARS-CoV-2 was silently circulating in Portugal throughout February, before the first cases were confirmed. Conclusions Here we conclude that the earlier implementation of measures could have minimized the number of introductions and subsequent virus expansion in Portugal.     Quality of life, psychological morbidity and family stress in elderly residing in the community

    Este estudo procurou investigar as relações existentes entre morbilidade psicológica, stress familiar e qualidade de vida (QV) da pessoa idosa. A amostra foi constituída por 126 idosos. Os instrumentos utilizados foram: The Lawton Instrumental Activities of Daily Living (IADL), Quality of Life (WHOQOL-Bref), Geriatric Anxiety Inventory (GSI), Geriatric Depression Scale (GDS); e Index of Family Relations (IFR). Os resultados revelaram a importância da idade, estado civil, escolaridade e número de patologias assim como o género na capacidade funcional, morbilidade, stress familiar e QV. Ao nível dos preditores, a depressão foi a variável que mais contribuiu para a QV. Não foram encontradas variáveis moderadoras no modelo. A discussão e implicações dos resultados são abordadas bem como a intervenção psicológica nesta população.This study sought to understand the relationships among psychological morbidity, family stress and quality of life (QL) of elderly. The sample consisted of 126 elderly. The following instruments were used: the Lawton Instrumental Activities of Daily Living (IADL); Quality of Life (WHOQOL-Bref), Geriatric Anxiety Inventory (GSI), Geriatric Depression Scale (GDS), and the Index of Family Relations (IFR). Results revealed the importance of age, marital status, education and number of pathologies as well as gender on functional capacity, morbidity, family stress and QV. In terms of predictors, depression was the variable that contributed the most to QL. There were no moderating variables in the model.
