Efectividad de una intervención multifactorial basada en la mejora de la dieta y la actividad física, en pacientes con Diabetes mellitus tipo 2, en Atención Primaria

Tesis por compendio de publicaciones.[ES] El estudio EMID es un ensayo clínico aleatorizado y controlado de dos grupo paralelos, en el cuál se ha pretendido valorar la efectividad de una intervención multifactorial, en pacientes con Diabetes Mellitus tipo 2, desde Atención primaria. Dicha intervención multifactorial estuvo compuesta por un taller de alimentación, cinco paseos cardiosaludables y la utilización de la aplicación para Smartphone Evident 2. Los resultados de esta Tesis Doctoral son novedosos, ya que hasta donde nosotros conocemos es el primer trabajo que muestra la efectividad de una intervención multifactorial, para incrementar la actividad física y la adherencia a la Dieta Mediterránea. Además, se ha valorado el efecto inmediato que tienen los paseos saludables, realizados en esta intervención, sobre parámetros de rigidez arterial, PA y glucemia postprandial, desglosando los resultados por género y por edad. En el primer manuscrito, se muestra que la intervención multifactorial llevada a cabo en este estudio, fue efectiva en la mejora de la adherencia a la Dieta Mediterránea. Éste se trata de un hallazgo importante en los sujetos con DMT2 ya que varios estudios han relacionado el incremento en la adherencia a la Dieta Mediterránea, con mejoras en el control glucémico y en los factores de riesgo cardiovascular. Además, en este estudio se produjo un incremento de la calidad de la dieta, la cual está relacionada, con un mejor control glucémico y una menor obesidad. El segundo manuscrito muestra que la intervención multifactorial también mejoró la actividad física en los sujetos con DMT2. En concreto, incrementó el número de pasos diarios, el número de pasos aeróbicos, los METs consumidos y disminuyó el tiempo de sedentarismo. Por último, el paseo cardiosaludable, realizado en esta intervención multifactorial, se asoció con un efecto inmediato sobre el CAVI, la PA y la PP en las EEII. Hasta donde conocemos, éste es el primer estudio en pacientes con DMT2 que valora el efecto inmediato de un ejercicio aeróbico de intensidad baja-moderada sobre parámetros de rigidez arterial y PA

Incidencia y significancia de las estructuras císticas en ovarios de gádidos

Fish regulate egg production by atresia. Sometimes, oocytes are encapsulated in cystic structures that might remain in the ovary for months, altering female reproductive potential in future spawning seasons. Ovaries of Atlantic cod (Gadus morhua, L.) from the Flemish Cap and European hake (Merluccius merluccius, L.) from the Galician Shelf (NW Spain) were analysed from 1999 to 2006. The prevalence and abundance of ovarian cysts were estimated. Cyst prevalence increased with female size and/or age for both species, and decreased with high condition factor in hake. Cyst intensity does not correlate with any analysed factor. The size/age structure of spawning stock biomass and female condition could affect the prevalence of cysts in the ovaries, though it does not seem to affect cyst intensity in the ovary. Further research is needed to determine cyst resorption time and the impact that it has on egg production and consequently on stock reproductive potential.Los peces regulan la producción de huevos a través de la atresia. En ocasiones, los ovocitos son encapsulados en estructuras quísticas que podrían permanecer en el ovario durante meses, alterando el potencial reproductivo de las hembras en estaciones de puesta posteriores. Se tomaron ovarios de bacalao (Gadus morhua, L.) de Flemish Cap y de merluza europea (Merluccius merluccius, L.) de la costa gallega (NO España) entre 1999 y 2006. Se estimó la prevalencia e intensidad de quistes en estos ovarios. En ambas especies la prevalencia de hembras con quistes aumentó con la talla y/o edad y disminuyó al aumentar el factor de condición en el caso de la merluza. La intensidad de quistes no se correlacionó con ninguna de las variables estudiadas. La estructura de talla/edades del stock reproductor y la condición de las hembras podría afectar la prevalencia de hembras con quistes. Son necesarios más estudios para determinar el tiempo de reabsorción de estos quistes y las consecuencias que esto puede tener en la producción de huevos y por tanto en el potencial reproductivo de los stocks

Effectiveness of a multifactorial intervention in increasing adherence to the mediterranean diet among patients with diabetes mellitus type 2: a controlled and randomized study (EMID Study)

The Mediterranean diet (MD) is recognized as one of the healthiest dietary patterns and has benefits such as improving glycaemic control among patients with type 2 diabetes (T2DM). Our aim is to assess the effectiveness of a multifactorial intervention to improve adherence to theMD, diet quality and biomedical parameters. The EMID study is a randomized and controlled clinical trial with two parallel groups and a 12-month follow-up period. The study included 204 subjects between 25–70 years with T2DM. The participants were randomized into intervention group (IG) and control group (CG). Both groups received brief advice about healthy eating and physical activity. The IG participants additionally took part in a food workshop, five walks and received a smartphone application for three months. The population studied had a mean age of 60.6 years. At the 3-month follow-up visit, there were improvements in adherence to the MD and diet quality of 2.2 and 2.5 points, compared to the baseline visit, respectively, in favour of the IG. This tendency of the improvement was maintained, in favour of the IG, at the 12-month follow-up visit. In conclusion, the multifactorial intervention performed could improve adherence to the MD and diet quality among patients with T2DM.Regional Health Management through the 2016 grants to carry out research projects in biomedicine, health management and socio-health care (GRS 1276/B/16), the 2016 program for the professional development of nurses in their research activity (BOCYL-D-11022016-2) and the 2015 incentive program for nurses who have completed their residency (ORDER SAN / 360/2015). La imagen personal que un directivo proyecta, será fundamental a la hora de ejercer confianza y liderazgo. El objetivo de la investigación fue establecer la relación que existe entre la imagen personal y el liderazgo. La naturaleza de la investigación fue cualitativa de razonamiento inductivo; el diseño fue no experimental transeccional descriptivo. Los resultados demostraron que, desde la percepción de los entrevistados, los conocimientos de un líder dependen fuertemente de cómo habla y cómo piensa; en menor grado de cómo luce y cómo actúa; las actitudes de un líder están fuertemente ligadas a cómo se desempeña, en mediano grado a cómo luce y en menor grado en cómo habla y cómo piensa; las conductas de un líder están fuertemente ligadas a cómo habla; en menor grado en cómo actúa y cómo luce; y, el carisma de un líder está medianamente ligado a cómo habla, cómo luce y cómo actúa.   The personal image that a manager projects will be essential when it comes to exercising trust and leadership. The aim of the research was to establish the relationship between personal image and leadership. The nature of the research was qualitative with inductive reasoning; the design was non-experimental transectional descriptive. The results showed that, from the perception of the interviewees, the knowledge of a leader strongly depends on how he speaks and how he thinks; to a lesser degree how he looks and how he acts; a leader's attitudes are strongly linked to how she performs, to a medium degree to how she looks, and to a lesser degree to how she talks and thinks; a leader's behaviors are strongly linked to how she speaks; to a lesser degree how she acts and how she looks; and, the charisma of a leader is moderately linked to how she speaks, how she looks and how she acts

Revisión Teórica de los Factores que Inhiben en el Ciudadano el Uso del e-Gobierno

El objetivo de la presente investigación fue analizar la literatura relativa a los factores que inhiben el uso de los servicios electrónicos que presta una institución gubernamental mediante el método documental basado en una revisión bibliográfica de los principales autores en materia de gobierno electrónico. Los resultados destacan la importancia del conocimiento de los servicios digitales por parte de los ciudadanos, la facilidad de uso y la calidad del sistema, la influencia del perfil sociodemográfico, así como la satisfacción y confianza del usuario para  utilizarlos. Los hallazgos describen cada uno de los factores y las relaciones entre sí. La relevancia se orientó a visualizar estrategias para mejorar el gobierno electrónico en el contexto mexicano.   The aim of this research is to analyze the literature on the factors that inhibit the use of electronic services provided by a government institution through the documentary method based on a bibliographic review of the main authors in the field of electronic e-government. The results highlight the importance of citizens' knowledge of digital services, the ease of use and the quality of the system, the influence of the sociodemographic profile, as well as the satisfaction and confidence of citizens in using them. The findings attempt to describe each of the factors and whether they are related to each other. The relevance is oriented to visualize strategies to improve e-government in the Mexican context
