Spanish National Hip Fracture Registry (RNFC) : First-year results and comparison with other registries and prospective multi-centric studies from Spain
Fundamentos: El Registro Nacional de Fracturas de Cadera (RNFC) es un registro español multicéntrico, prospectivo y continuo, que comenzó en 2017. El objetivo de este artículo fue presentar los datos del primer informe anual y compararlos con los registros autonómicos y los estudios multicéntricos realizados recientemente en España. Métodos: Se incluyeron las personas de 75 años o más atendidas con el diagnóstico de fractura de cadera por fragilidad en alguno de los hospitales participantes en el RNFC, entre enero y octubre de 2017. En el análisis estadístico se utilizó la media y desviación estándar o mediana y rangos intercuartílicos para las variables numéricas y los porcentajes para las variables categóricas. Se realizó un análisis descriptivo global de la casuística y se comparó con los datos disponibles de los estudios previos mencionados. Resultados: Se registraron 7.208 personas de 54 hospitales, con una edad media de 86,7 años (DE 5,6). El 75,4% fueron mujeres y el 36,4% presentaron deterioro cognitivo previo. La demora quirúrgica media fue de 75,7 horas (DE 63,6) y la estancia media fue de 10,9 días (DE 6,7). De las personas que vivían en un domicilio antes de la fractura (75,4%), menos de la mitad (37,0%) volvieron a él tras el alta hospitalaria. Al mes, había fallecido el 7,1%. La comparación con los otros estudios mostró algunas diferencias importantes, sobre todo en la ubicación previa, en el porcentaje de pacientes institucionalizados de novo (7,7-29,4%) y en el porcentaje con tratamiento antiosteoporótico al alta (14,5-36,7%). Conclusiones: El RNFC es la mayor base de datos prospectiva que aporta datos sobre el perfil de los pacientes hospitalizados por fractura de cadera en España. Complement component C4 structural variation and quantitative traits contribute to sex-biased vulnerability in systemic sclerosis
Altres ajuts: Fondo Europeo de Desarrollo Regional (FEDER), "A way of making Europe".Copy number (CN) polymorphisms of complement C4 play distinct roles in many conditions, including immune-mediated diseases. We investigated the association of C4 CN with systemic sclerosis (SSc) risk. Imputed total C4, C4A, C4B, and HERV-K CN were analyzed in 26,633 individuals and validated in an independent cohort. Our results showed that higher C4 CN confers protection to SSc, and deviations from CN parity of C4A and C4B augmented risk. The protection contributed per copy of C4A and C4B differed by sex. Stronger protection was afforded by C4A in men and by C4B in women. C4 CN correlated well with its gene expression and serum protein levels, and less C4 was detected for both in SSc patients. Conditioned analysis suggests that C4 genetics strongly contributes to the SSc association within the major histocompatibility complex locus and highlights classical alleles and amino acid variants of HLA-DRB1 and HLA-DPB1 as C4-independent signals

Estudio de la inadaptación infanto-juvenil : estudio descriptivo de la inadaptación social infanto juvenil en la Región de Murcia : factores de riesgo y factores pronósticos
Estudio descriptivo que intenta llegar a definir y categorizar los factores de riesgo y los factores pronósticos que caracterizan a la infancia y juventud inadaptadas en la región de Murcia. 189 sujetos entre los 11 y los 18 años (110 varones y 79 mujeres) procedentes de instituciones dependientes de la Consejería de Bienestar Social de la Comunidad Autónoma, con expedientes de protección y-o tutela en el momento de comenzar el estudio (total de individuos), además de procedentes de otros centros, colegios, hogares funcionales, etc., dependientes de otras administraciones. El estudio se estructuró en dos partes. La primera consta de una exploración psicológica mediante entrevista, la segunda de un cuestionario psicobiográfico de elaboración propia que consta de 307 ítems en el que se recogen los datos de escolarización, conflictividad familiar, empleo del ocio, consumo de drogas (institucionalizadas y no), historia clínica, sexualidad, antecedentes de conductas asociales, antecedentes sociobiográficos y familiares. Los resultados del estudio reflejan los datos globales ordenados según el sexo de los sujetos entrevistados y, algunos de ellos, según el centro del que procedía el entrevistado. Además del cuestionario psicobiográfico, los rasgos de personalidad se analizaron con los siguientes instrumentos: EPI (Eysenck Personality Questionnaire, EPQ. Seisdedos, 1982), STAI (State Trait Anxiety Inventory-SelfEvaluation Questionnaire-, Spielberg y cols. Bermúdez, 1978). Test de factor g (Culture Fair Intelligence Test, Cattell y Cattell. Ey y cols., 1975). Manual Diagnóstico y Estadístico de los Transtornos Mentales (DSMIII), de la American Psychiatric Association APA, 1987. Subtest de figuras incompletas de lña Wechsler Intelligence Scale for Children (WISC) y el Matching Familiar Figures Test (MFFT, Kupietz y Richardson, 1978. Raport y cols., 1980). Prueba de atención-percepción de Toulouse-Piéron. Test del Laberinto en Espiral de Gibson (test psicomotor que mide la velocidad y precisión de la expresión muscular en respuesta a un estímulo controlado). Todos estos instrumentos miden los siguientes rasgos de personalidad: extroversión/introversión; neuroticismo/estabilidad; temperamento (Galeno, Kant,Wundt);ansiedad; inteligencia; hiperactividad; atención; impulsividad/reflexividad. El análisis de los datos obtenidos se realiza con el paquete estadístico BMDP y el proceso estadístico se compone de: análisis univariante, bivariante , multivariante y discriminante. Los sujetos estudiados muestran elevados niveles de neuroticismo, baja atención, altos niveles de impulsividad, bajas puntuaciones en la variable CI. Prevalencia alta de ansiedad, según el análisis global del total de la muestra. Son numerosos los trabajos que relacionan la inadapatación juvenil con un déficit de inteligencia, esto puede deberse a que las dificultades de desarrollo y maduración determinan unos niveles de inteligencia inferiores a los que se hubiesen alcanzado siendo el medio más favorable. Existen muchas referencias que reflejan cómo la ansiedad es un fenómeno relativamente constante en la institucionalización de los delincuentes juveniles.MurciaES
Metagene projection characterizes GEN2.2 and CAL-1 as relevant human plasmacytoid dendritic cell models.
Plasmacytoid dendritic cells (pDC) play a major role in the regulation of adaptive and innate immunity. Human pDC are difficult to isolate from peripheral blood and do not survive in culture making the study of their biology challenging. Recently, two leukemic counterparts of pDC, CAL-1 and GEN2.2, have been proposed as representative models of human pDC. Nevertheless, their relationship with pDC has been established only by means of particular functional and phenotypic similarities. With the aim of characterizing GEN2.2 and CAL-1 in the context of the main circulating immune cell populations we have performed microarray gene expression profiling of GEN2.2 and carried out an integrated analysis using publicly available gene expression datasets of CAL-1 and the main circulating primary leukocyte lineages. Our results show that GEN2.2 and CAL-1 share common gene expression programs with primary pDC, clustering apart from the rest of circulating hematopoietic lineages. We have also identified common differentially expressed genes that can be relevant in pDC biology. In addition, we have revealed the common and differential pathways activated in primary pDC and cell lines upon CpG stimulatio. R code and data are available in the supplementary material.
