    Health status and nutritional development of adopted Ethiopian children living in southern Spain: A prospective cohort study

    Objective: The first aim of this study was to evaluate the health status and anthropometrical development of adopted children from Ethiopia living in southern Spain. A second aim was to evaluate the association between these parameters and adherence to the Mediterranean dietary pattern. Methods: The study sample included 53 adopted children from Ethiopia and a matched sample of 54 native-born children. A physical examination of the children, including height and weight, was conducted in Ethiopia at the time of entry into the adoption process. Height and weight were re-measured at the first day of adoption and 6, 12, and 24 mo after adoption. After 2 y of follow-up, another physical examination was performed, including the KIDMED test, to measure adherence to the Mediterranean diet. Results: Skin and digestive conditions were the most prevalent disorders in Ethiopian children before adoption and at the end of follow-up. Baseline anthropometric characteristics indicated a low wasting prevalence (7.5%); however, stunted growth was more prevalent (35.8%). After 6 mo, the weight-for-age of Ethiopian children was restored (change from baseline P Conclusions: This prospective study of adopted Ethiopian children confirmed a rapid growth development that occurred from the beginning of the adoption process and continued after the 2-y of follow-up. A higher adherence to the Mediterranean diet was associated with better growth development, which reinforces the importance of a balanced and adequate diet in growing children.</br

    Reclamation of Real Urban Wastewater Using Solar Advanced Oxidation Processes: An Assessment of Microbial Pathogens and 74 Organic Microcontaminants Uptake in Lettuce and Radish

    In this study, disinfection of urban wastewater (UWW) with two solar processes (H2O2 -20 mg/L and photo-Fenton 10 mg/L-Fe2+/20 mg/L-H2O2 at natural water pH) at pilot scale using a 60 L compound parabolic collector reactor for irrigation of two raw-eaten vegetables (lettuce and radish) has been investigated. Several microbial targets (total coliforms, Escherichia coli, Salmonella spp., and Enterococcus spp.) naturally occurring in UWW and 74 organic microcontaminants (OMCs) were monitored. Disinfection results showed no significant differences between both processes, showing the following inactivation resistance order: Salmonella spp. &lt; E. coli &lt; total coliforms &lt; Enterococcus spp. Reductions of target microorganisms to concentrations below the limit of detection (LOD) was achieved in all cases with cumulative solar UV energy per volume (QUV) ranged from 12 to 40 kJ/L (90 min to 5 h). Solar photo-Fenton showed a reduction of 66% of OMCs and solar/H2O2 of 56% in 5 h treatment. Irrigation of radish and lettuce with solar treated effluents, secondary effluents, and mineral water was performed for 6 and 16 weeks, respectively. The presence of bacteria was monitored in surfaces and uptake of leaves, fruit, and also in soil. The bacterial concentrations detected were below the LOD in the 81.2% (lettuce) and the 87.5% (radish) of the total number of samples evaluated. Moreover, uptake of OMCs was reduced above 70% in crops irrigated with solar treated effluents in comparison with secondary effluents of UWW. © 2019 American Chemical Society

    Caloric Beverages Were Major Sources of Energy among Children and Adults in Mexico, 1999–2012

    Mexico, with 1 of the highest obesity prevalences in the world, instituted a 10% excise tax for any sugar-sweetened beverage (SSB) starting on 1 January 2014. Understanding the recent patterns and trends in beverage intake and sales in Mexico provides both background and baseline data for the importance of SSBs and other beverages in the Mexican diet. We analyzed a single 24-h dietary recall from 2 nationally representative surveys: the Mexican Nutrition Survey 1999 (n = 6049) and the National Health and Nutrition Survey 2012 (n = 10,343). To describe trends and patterns in beverages, we calculated the volume and energy intake per capita and per consumer and the proportion of consumers of each beverage group in each survey. A commercial sales dataset was used to describe beverage sales trends from 1999 to 2012. From 1999 to 2012, total daily energy from beverages increased among children aged 5–11 y (+45.3 kcal), females aged 12–19 y (+57.3 kcal), and adult females aged 20–49 y (+96.4 kcal) (P < 0.05). Over the same period, intake of beverages with added sugars increased, specifically flavored milk, agua fresca (fruit water made in stalls or at home, usually with added sugars), and fruit drinks among children aged 5–11 y and females aged 12–19 y and caloric coffee/tea, soda, and agua fresca among adult females aged 20–49 y. In 2012, beverages represented 17.5% (325 kcal) and 19.0% (382 kcal) of the total daily energy intake per capita in children aged 1–19 y and adults aged ≥20 y, respectively. In 2012, flavored milk beverages, caloric soda, and high-fat milk were the top 3 major contributors to total daily energy intake per capita in all children aged 1–19 y. Caloric soda, caloric coffee/tea, and agua fresca were the top 3 major energy contributors in adults aged ≥20 y. From 1999 to 2012, sales of soda, fruit-flavored drinks, and flavored waters increased. In conclusion, consumption of several beverages with added sugars increased among children and adult females in Mexico. Because caloric soda is currently 1 of the top beverages consumed, a 10% tax on SSBs might help to significantly reduce added sugars intake in Mexico

    Clinical characteristics of patients with central nervous system relapse in BCR-ABL1-positive acute lymphoblastic leukemia : the importance of characterizing ABL1 mutations in cerebrospinal fluid

    We investigated the frequency, predictors, and evolution of acute lymphoblastic leukemia (ALL) in patients with CNS relapse and introduced a novel method for studying BCR-ABL1 protein variants in cDNA from bone marrow (BM) and cerebrospinal fluid (CSF) blast cells. A total of 128 patients were analyzed in two PETHEMA clinical trials. All achieved complete remission after imatinib treatment. Of these, 30 (23%) experienced a relapse after achieving complete remission, and 13 (10%) had an isolated CNS relapse or combined CNS and BM relapses. We compared the characteristics of patients with and without CNS relapse and further analyzed CSF and BM samples from two of the 13 patients with CNS relapse. In both patients, classical sequencing analysis of the kinase domain of BCR-ABL1 from the cDNA of CSF blasts revealed the pathogenic variant p.L387M. We also performed ultra-deep next-generation sequencing (NGS) in three samples from one of the relapsed patients. We did not find the mutation in the BM sample, but we did find it in CSF blasts with 45% of reads at the time of relapse. These data demonstrate the feasibility of detecting BCR-ABL1 mutations in CSF blasts by NGS and highlight the importance of monitoring clonal evolution over time

Although tyrosine-kinase inhibitors (TKI), especially imatinib, are included in; the first-line treatment in these patients, their concentration in CNS is too low; to effectively prevent CNS relapse, making CNS prophylaxis mandatory. Aims: To study the frequency, predictors and evolution of BCR-ABL positive; ALL patients with CNS relapse in two consecutive clinical trials of the PETHEMA; group using imatinib and chemotherapy. As secondary objective we proposed; the introduction of a new method for the study of variants of uncertain; significance (VUS) in kinase domain of the BCR-ABL from cDNA of cerebrospinal; fluid (CSF) blasts, in order to adapt the TKI used in relapse according; to the clonal evolution from bone marrow (BM) to CSF cells.N

    Treatment of Frail Older Adults and Elderly Patients With Philadelphia Chromosome-negative Acute Lymphoblastic Leukemia: Results of a Prospective Trial With Minimal Chemotherapy

    [Background]: The treatment of acute lymphoblastic leukemia (ALL) in older adults and elderly patients is a challenge, and modern protocols include targeted therapy and immunotherapy in combination with attenuated or minimal chemotherapy. However, frail patients are excluded from these trials, and reports on the outcome of this subgroup of patients are scarce. Our objective was to analyze the outcome of unfit older adults and elderly patients with Philadelphia chromosome-negative ALL included in a prospective trial (ALL-07FRAIL).[Patients and Methods]: Older adults and elderly patients with Charlson Comorbidity Index (CCI) ≥ 4 were included. Induction therapy consisted of vincristine and dexamethasone, and maintenance therapy with mercaptopurine and methotrexate for 2 years.[Results]: Seventy-two patients with a median age of 67 years (range, 57-89 years) and a median CCI of 5 (range, 4-12) were included. The rates of early withdrawal, early death, resistance, and complete response (CR) were 5%, 10%, 31%, and 54%, respectively. Six patients with CR abandoned the study, 5 died in CR, and 23 relapsed (cumulative relapse incidence 75%). The medians of disease-free and overall survival (OS) were 6.9 months (95% confidence interval [CI], 0.3-13.5 months) and 7.6 months (95% CI, 6.3-8.9 months), respectively. The most frequent toxic events were hematologic (neutropenia 77% and thrombocytopenia 54%, of grade III-IV in all cases). Eastern Cooperative Oncology Group score but not the CCI had significant impact on OS.[Conclusion]: Complete remission with very attenuated chemotherapy can be attained in one-half of older or elderly infirm patients with ALL.
