    Effect of hyperbaric oxygen therapy: case report of patient with hepato-pulmonary syndrome on waiting list for liver transplantation

    INTRODUÇÃO: As doenças hepáticas apresentam índices de morbidade e mortalidade elevados e quando em estágio avançado têm o transplante do fígado como forma de tratamento potencialmente curativo e eficaz, embora este não possa ser oferecido a todos os pacientes. Isso faz com que essas doenças sejam consideradas problema de saúde pública em todo o mundo. Os cuidados clínicos para manter o paciente com condições de esperar e suportar o transplante continua um desafio. RELATO DO CASO: Mulher com 65 anos de idade, procedente do Recife, com diagnóstico de cirrose hepática secundária a vírus C apresentava dispnéia importante aos mínimos esforços tendo PaO2 de repouso de 60 mmHg e O2 de 90%, com espirometria normal. Realizou eco-Doppler que evidenciou shunt pulmonar importante. Durante a triagem em lista de transplante (MELD de 16 em agosto de 2006) foi optado pelo início de sessões de oxigenioterapia em câmara hiperbárica a fim de melhorar a sintomatologia respiratória da síndrome hepato-pulmonar. Apresentava melhora substancial da tolerância ao exercício após a terapia hiperbárica, assim como os valores do PaO2 à gasometria. Realizou 10 sessões de oxigenioterapia em câmara hiperbárica. Realizou transplante hepático em outubro de 2007 e vem em acompanhamento ambulatorial com boa evolução e melhora substancial da dispnéia. CONCLUSÃO: Constatou-se melhora da condição hepato-pulmonar após oxigenoterapia hiperbárica. Desta forma, ela surge como mais uma ferramenta para o tratamento das doenças hepáticas, devendo ser realizados outros estudos que avaliem sua utilização clínica.INTRODUCTION: Progressive impairment of liver function in hepatic cirrhosis is a major medical problem with limited therapeutic approaches, and liver transplantation is the only effective therapy. Reports of alternatives for liver function improvement on patients awaiting liver transplantation may include hyperbaric oxygen therapy. CASE REPORT: A 65-year old woman had diagnosis of hepatitis C and liver cirrhosis during treatment for breast cancer. In addition to liver failure and a MELD of 16, the patient presented hypoxemia with a PaO2 of 60 mmHg, and further evaluation with eco-Doppler revealed a hepatopulmonary syndrome. Clinical improvement was attempted with hyperbaric oxygeniotherapy, and substantial clinical and laboratorial improvement was acheived after 10 sessions, with significant improvement on cyanosis and arterial gas values. CONCLUSION: Oxygen therapy was effective in the improvement of dyspnea and hypoxemia secondary to hepatopulmonary syndrome in a patient with liver cirrhosis indicated for liver transplantation. The positive results of this treatment suggest that hyperbaric oxygen therapy may help on improvement of symptoms of hepatopulmonary syndrome

    Este estudo de caso analisou a articulação da disciplina educação física a um currículo organizado por temas transdisciplinares, a partir do pressuposto de que as limitações da organização linear disciplinar não atendem às necessidades de uma escola imersa numa sociedade onde cada vez mais os aspectos econômicos, culturais, políticos, ambientais e científicos se mostram interdependentes. A revisão de literatura discutiu o desenvolvimento dos paradigmas da ciência e da indústria, conceitos de disciplina, currículo integrado, transdisciplinaridade e exemplos de currículo integrado e métodos globalizados. As entrevistas semiestruturadas e a observação participante revelaram dificuldades na compreensão dos conceitos essenciais, na elaboração de conexões entre o conteúdo disciplinar e unidades de investigação e na articulação do planejamento com questões que emergem do cotidiano. Foram identificados avanços, como o pensamento linear disciplinar dos professores dando lugar a uma visão integrada de educação e o modelo diretivo de ensino tornou-se mais construtivo e reflexivo

    Proposal of an augmented reality tag uav positioning system for power line tower inspection

    Autonomous inspection Unmanned Aerial Vehicle systems are an essential research area, including power line distribution inspection. Considerable efforts to solve the demanding presented in the autonomous U AV inspection process are present in technical and scientific research. One of these challenges is the precise positioning and fly control of the U AV around the energy structures, which is vital to assure the security of the operation. The most common techniques to achieve precise positioning in UAV fly are Global Positioning Systems with RealTime Kinematic. This technique demands a proper satellite signal receiving to work appropriately, sometimes hard to achieve. The present work proposes a complementary position data system based on augmented reality tags (AR Tags) to increase the reliability of the UAV fly positioning system. The system application is proposed for energy power tower inspections as an example of use. The adaptation to other inspection tasks is possible whit some small changes.     Cooperative UAV–UGV autonomous power pylon inspection: an investigation of cooperative outdoor vehicle positioning architecture

    Realizing autonomous inspection, such as that of power distribution lines, through unmanned aerial vehicle (UAV) systems is a key research domain in robotics. In particular, the use of autonomous and semi-autonomous vehicles to execute the tasks of an inspection process can enhance the efficacy and safety of the operation; however, many technical problems, such as those pertaining to the precise positioning and path following of the vehicles, robust obstacle detection, and intelligent control, must be addressed. In this study, an innovative architecture involving an unmanned aircraft vehicle (UAV) and an unmanned ground vehicle (UGV) was examined for detailed inspections of power lines. In the proposed strategy, each vehicle provides its position information to the other, which ensures a safe inspection process. The results of real-world experiments indicate a satisfactory performance, thereby demonstrating the feasibility of the proposed approach.This research was funded by National Counsel of Technological and Scientific Development of Brazil (CNPq).     Artificial intelligence architecture based on planar LIDAR scan data to detect energy pylon structures in a UAV autonomous detailed inspection process

    The technological advances in Unmanned Aerial Vehicles (UAV) related to energy power structure inspection are gaining visibility in the past decade, due to the advantages of this technique compared with traditional inspection methods. In the particular case of power pylon structure and components, autonomous UAV inspection architectures are able to increase the efficacy and security of these tasks. This kind of application presents technical challenges that must be faced to build real-world solutions, especially the precise positioning and path following for the UAV during a mission. This paper aims to evaluate a novel architecture applied to a power line pylon inspection process, based on the machine learning techniques to process and identify the signal obtained from a UAV-embedded planar Light Detection and Ranging - LiDAR sensor. A simulated environment built on the GAZEBO software presents a first evaluation of the architecture. The results show an positive detection accuracy level superior to 97% using the vertical scan data and 70% using the horizontal scan data. This accuracy level indicates that the proposed architecture is proper for the development of positioning algorithms based on the LiDAR scan data of a power pylon.This work has been supported by FCT - Fundação para a Ciência e Tecnologia within the Project Scope: UIDB/05757/2020.     ANTIBIOTIC PROPHYLAXIS FOR ABDOMINAL SURGERY: WHEN TO RECOMMEND? BRAZILIAN COLLEGE OF DIGESTIVE SURGERY POSITION PAPER

    ABSTRACT BACKGROUND: Surgical antibiotic prophylaxis is an essential component of perioperative care. The use of prophylactic regimens of antibiotics is a well-established practice that is encouraged to be implemented in preoperative/perioperative protocols in order to prevent surgical site infections. AIMS: The aim of this study was to emphasize the crucial aspects of antibiotic prophylaxis in abdominal surgery. RESULTS: Antibiotic prophylaxis is defined as the administration of antibiotics before contamination occurs, given with the intention of preventing infection by achieving tissue levels of antibiotics above the minimum inhibitory concentration at the time of surgical incision. It is indicated for clean operations with prosthetic materials or in cases where severe consequences may arise in the event of an infection. It is also suitable for all clean-contaminated and contaminated operations. The spectrum of action is determined by the pathogens present at the surgical site. Ideally, a single intravenous bolus dose should be administered within 60 min before the surgical incision. An additional dose should be given in case of hemorrhage or prolonged surgery, according to the half-life of the drug. Factors such as the patient’s weight, history of allergies, and the likelihood of colonization by resistant bacteria should be considered. Compliance with institutional protocols enhances the effectiveness of antibiotic use. CONCLUSION: Surgical antibiotic prophylaxis is associated with reduced rates of surgical site infection, hospital stay, and morbimortality
