    GP perspectives on hospital discharge letters : an interview and focus group study

    Background: Written discharge communication following inpatient or outpatient clinic discharge is essential for communicating information to the GP, but GPs’ opinions on discharge communication are seldom sought. Patients are sometimes copied into this communication, but the reasons for this variation, and the resultant effects, remain unclear. Aim: To explore GP perspectives on how discharge letters can be improved in order to enhance patient outcomes. Design & setting: The study used narrative interviews with 26 GPs from 13 GP practices within the West Midlands, England. Method: Interviews were transcribed and data were analysed using corpus linguistics (CL) techniques. Results Elements pivotal to a successful letter were: diagnosis, appropriate follow-up plan, medication changes and reasons, clinical summary, investigations and/or procedures and outcomes, and what information has been given to the patient. GPs supported patients receiving discharge letters and expounded a number of benefits of this practice; for example, increased patient autonomy. Nevertheless, GPs felt that if patients are to receive direct discharge letter copies, modifications such as use of lay language and avoidance of acronyms may be required to increase patient understanding. Conclusion: GPs reported that discharge letters frequently lacked content items they assessed to be important; GPs highlighted that this can have subsequent ramifications on resources and patient experiences. Templates should be devised that put discharge letter elements assessed to be important by GPs to the forefront. Future research needs to consider other perspectives on letter content, particularly those of patients

    Designing a novel virtual collaborative environment to support collaboration in design review meetings

    Project review meetings are part of the project management process and are organised to assess progress and resolve any design conflicts to avoid delays in construction. One of the key challenges during a project review meeting is to bring the stakeholders together and use this time effectively to address design issues as quickly as possible. At present, current technology solutions based on BIM or CAD are information-centric and do not allow project teams to collectively explore the design from a range of perspectives and brainstorm ideas when design conflicts are encountered. This paper presents a system architecture that can be used to support multi-functional team collaboration more effectively during such design review meetings. The proposed architecture illustrates how information-centric BIM or CAD systems can be made human- and team-centric to enhance team communication and problem solving. An implementation of the proposed system architecture has been tested for its utility, likability and usefulness during design review meetings. The evaluation results suggest that the collaboration platform has the potential to enhance collaboration among multi-functional teams

    Design as conversation with digital materials

    This paper explores Donald Schön's concept of design as a conversation with materials, in the context of designing digital systems. It proposes material utterance as a central event in designing. A material utterance is a situated communication act that depends on the particularities of speaker, audience, material and genre. The paper argues that, if digital designing differs from other forms of designing, then accounts for such differences must be sought by understanding the material properties of digital systems and the genres of practice that surround their use. Perspectives from human-computer interaction (HCI) and the psychology of programming are used to examine how such an understanding might be constructed.</p

    Rodney Brooks' call for 'new mathematics' to revitalize the disciplines of artificial intelligence and artificial life can be answered by adaptation of what Adams has called 'the informational turn in philosophy' and by the novel perspectives that program gives into empirical studies of animal cognition and consciousness. Going backward from the necessary conditions communication theory imposes on cognition and consciousness to sufficient conditions for machine design is, however, an extraordinarily difficult engineering task. The most likely use of the first generations of conscious machines will be to model the various forms of psychopathology, since we have little or no understanding of how consciousness is stabilized in humans or other animals

    Opportunities and Constraints for Independent Digital Magazine Publishing

    Optimists have long hoped that digital communication would diversify media, but the realization of this dream is far from certain today. This study investigates the emerging opportunities available to independent magazine publishers through digital publishing methods, such as distributing their magazines through the Apple Newsstand. These publishing methods have the potential to diversify magazine publishing beyond the currently dominant offerings of major multinational magazine publishers. However, at the same time, a variety of limitations — software and design expertise, public awareness and interest, and technology companies’ constraints on publishers — may have already limited the ways these independent publishers can reach audiences. Through in-depth interviews with independent digital magazine publishers, this study illuminates the effects of these factors on these publishers’ efforts to offer varied new perspectives to the public within the digital magazine medium

    Communication technologies in the study environment: institutional and personal media as a reflection of organisational structure

    In this article, I will analyse and discuss two qualitative case studies concerning ICT in the study environment at the University of Copenhagen, Denmark. I will place special focus on the way in which organisational perspectives as well as technological affordances shape how communication technologies are integrated into organisational structures and practices on campus. This involves a comparison between course management systems on the one hand and students’ personal media (mobile phones, e-mails) on the other hand, with regard to how these are used on campus. On the basis of this analysis, I will argue that the ways in which these technologies are used reflects two different perspectives on the interplay between communication technology and organisational structure: organisational structure as an anticipation of communication patterns implied in course management system’s design and implementation as well as organisational structure as a product of the use of personal media

    Exploring the intersections: Researchers and communication professionals' perspectives on the organizational role of science communication

    Purpose: This paper reports on research exploring the intersections between researchers and communication professionals' perspectives on the objectives, funders and organizational influences on their science communication practices. Design/methodology/approach: Examining one context, the inter-organizational BCDC Energy Research project based at five different research organizations in Finland, this paper presents data from semi-structured interviews with 17 researchers and 15 communication professionals. Findings: The results suggest that performance-based funding policies that drive the proliferation of large-scale research projects can create challenges. In particular, a challenge arises in generating a shared sense of identity and purpose amongst researchers and communication professionals. This may have unintended negative impacts on the quality and cohesiveness of the science communication which occurs. Research limitations/implications: The study was exploratory in nature and focuses on one organizational and institutional environment. Further research with a wider number of projects, as well as funders, would be conducive to a greater understanding of the issues involved. Practical implications: On a practical level, this research suggests that the creation of clearer communications awareness and guidance may be helpful in some large-scale projects, particularly involving broad numbers of organizations, individual researchers and funders. Originality/value: This is one of the first studies examining the perspectives of both researchers and communication professionals working over one project, drawing together a range of different institutional and disciplinary perspectives. The results highlight the importance of the influences of funding on science communication aims, assumptions, cultures and structures. The article articulates the need for further research in this area

    Can design documentaries disrupt design for disability?

    This paper shows how design documentaries can motivate new perspectives for design and disability. We critically consider the ways in which design documentaries can foreground children's lived experiences and priorities, in cases where it is not always possible to involve children early on in the design process. By presenting a design case for supporting communication that involves children with severe speech and physical impairments and their social peers, we discuss how this narrative method can evoke designer empathy and guide new interpretations. Our findings show that design documentaries can convey to designers rich and multifaceted accounts of children's communication experiences. Although this is found to be generative, we also identify a tension with a bodily impairment understanding of disability. Drawing on reflections from our case study, we propose new methodological implications for embedding design documentaries in the design process of technologies for disability
