    Pertussis-like syndrome and whooping cough-like illness are the terms used to refer to the indistinguishable signs and symptoms of whooping cough in the absence of laboratory tests to confirm the presence of the bacteria that causes it. Although there are no reported cases in Cuba, it is important to keep paying attention to the most representative symptoms of this disease, since there has recently been a resurgence of whooping cough in the world. Therefore, it is relevant to present the case of a patient with a two-week history of upper respiratory symptoms and dry cough.     Deciding on a university degree is undoubtedly a difficult choice. This situation brings about fear of failing and disappointing yourself and others, which is a common feeling among high school students, becoming a source of anxiety and stress. This demonstrates the need for an appropriate vocational and professional guidance. In Cuba, this subject is gaining importance, since a greater number of youngsters have the opportunity to choose from more professions; thus, deciding on one or the other is a problem of great practical and social impact. Vocational guidance and training in the Cuban educational context aims to reaffirm the inclination of youth towards medical studies, where they fully develop their potential, on the basis of consolidating values and feelings, and a high social, moral and unconditional commitment to the practice of the profession. This paper presents various issues that currently affect vocational guidance and the role of each of the major participants in the process itself

    Osteoporosis in the Aged Woman: a tracking in the nursing's office

    Objective: To describe characteristics rose in historical nursing of elderly pointing to osteoporosis and the risk of a same Outreach Program of Nursing, Fluminense Federal University. Results: It was found that 48 (12,4%) of elderly nursing historical records had problems related to osteoporosis, which stimulated the construction of an extension project for osteoporosis prevention and health promotion seeking to minimize the problem. It was used as strategy, consultations, distributing informative folder and stimulate the maintenance of healthy habits. Conclusion: This problem goes beyond individuality to reach the public space. An isolated action reaches only the intended population, but when other institutions are engaging in this care will contribute with their customers, which is reflected in the well-being of the entire community

    VALIDAÇÃO DE INSTRUMENTO PARA CUIDADO DA PESSOA IDOSA COM XEROSE CUTÂNEA

    Objective: To build and validate an instrument for nursing care to assess, prevent, and treatcutaneous xerosis in older adults.Method: A methodological study, carried out from June 2017 to August 2018 in João Pessoaand developed in three stages: integrative literature review, instrument construction, andcontent validation by a panel of judges who subject matter experts.Results: Upon completing the validation, the instrument displayed a total content validitycoefficient equal to 0.887 from the aspects of language clarity, practical relevance, theoreticalrelevance and dimension, and Kappa index equal to 0.77.Conclusion: The instrument proved to be valid, contributing to the practice of nurses as anintegral part in the process of caring for the skin of older adults.Objetivo: construir e validar um instrumento para os cuidados de Enfermagem para avaliar, prevenir e tratar a xerose cutânea em pessoas idosas.Método: estudo metodológico, realizado no período de junho de 2017 a agosto de 2018, em João Pessoa, desenvolvido em três etapas: revisão integrativa da literatura, construção do instrumento e validação de conteúdo sob um painel de juízes expertises na área.Resultados: mediante a realização da validação, o instrumento apresentou um coeficiente de validade de conteúdo total igual a 0,887 a partir dos aspectos clareza da linguagem, pertinência prática, relevância teórica e dimensão e índice de Kappa igual a 0,77.Conclusão: o instrumento mostrou-se válido, contribuindo para a prática dos enfermeiros como parte integrante no processo do cuidar da pele de pessoas idosas.Objetivo: construir y validar un instrumento para los cuidados de Enfermería con el fin deevaluar, prevenir y tratar la xerosis cutánea en adultos mayores.Método: estudio metodológico realizado entre junio de 2017 y agosto de 2018 en JoãoPessoa, y desarrollado en tres etapas: revisión integradora de la literatura, construcción delinstrumento y validación del contenido por parte de un panel jueces expertos en el área.Resultados: una vez finalizada la validación, el instrumento presentó un coeficiente de validezde contenido total igual a 0,887 a partir de los siguientes aspectos: claridad de lenguaje,pertinencia práctica, relevancia teórica y dimensión e índice de Kappa igual a 0,77.Conclusión: el instrumento demostró ser válido, lo que contribuye a la práctica de losenfermeros como componente del proceso del cuidado de la piel de los adultos mayores

    Nurses' livingness about palliative care / Vivência de enfermeiros acerca dos cuidados paliativos

    Objetivo: analisar a percepção de enfermeiros acerca da sua vivência em cuidados paliativos. Método: estudo descritivo e exploratório com abordagem qualitativa realizado em um hospital de referência em cuidados paliativos. A amostra foi constituída por 12 enfermeiros assistenciais. Para coleta de dados foi realizada uma entrevista com um roteiro semiestruturado composto por duas partes: a primeira abrangendo dados sociodemográficos e a segunda contemplava quatro questões para atender aos objetivos do estudo. Os dados oriundos da entrevista foram transcritos e em seguida avaliados mediante análise de conteúdo que permitiu a construção de três categorias: percepções acerca do que são cuidados paliativos; pontos relevantes a serem considerados para a realização dos cuidados paliativos; e o manejo da dor em cuidados paliativos. Resultados: os enfermeiros destacam que os cuidados paliativos devem contemplar não só a atenção aos pacientes, mas a família, ainda revelando sentimentos e medidas importantes como afeto, carinho e conforto. Conclusão: há um processo de efetivação acerca dos princípios que permeiam esse tipo de cuidado e cabe salientar que é um serviço novo que está em processo de formação e capacitação contínuo, o que tem contribuído para os resultados

    Antioxidant and antibacterial activity of extracts, fractions and isolated substances from the flowers of Acacia podalyriifolia A. Cunn. ex G. Don

    The extracts and fractions from the flowers of A. podalyriifolia were analyzed previously for antibacterial activity using diffusion in disk, Antioxidant properties were evaluated by determining radical scavenging power (DPPH test) and total phenol content was measured (Folin method). The present study describes the in vitro antibacterial (determining minimum inhibitory concentration) and antioxidant activities (by thiobarbituric acid reactive species - TBARS method) for the ethanol extract, dichloromethane and ethyl acetate fractions and two flavanones (naringenin and 5-β-D-glycosyl-naringenin) isolated from the flowers of Acacia podalyriifolia A. Cunn. ex G. Don. The flavanones naringenin and 5-β-D-glycosyl-naringenin had not previously been obtained from this species. The most effective antibacterial activity was observed in the ethyl acetate fraction (MIC=0.25 mg mL-1 against Staphylococcus aureus ATCC 6538, MIC = 0.125 mg mL-1 against Staphylococcus epidermidis ATCC 12229, MIC=0.5 mg mL-1 against Streptococcus pyogenes ATCC 19615, Klebsiella pneumoniae ATCC 13883 and Proteus mirabilis ATCC 43071). The evaluated samples showed antioxidant activity on the TBARS test, especially for ethanol extract (1000 ppm), which was the most active (29.43% ± 0.65) followed by ethyl acetate fraction (1000 ppm, 24.84% ± 1,28), both demonstrating higher activity than that presented by ascorbic acid (1000 ppm, 21.73% ± 1.77), although lower than the BHT (1000 ppm 35.15% ± 3.42), both reference compounds. Naringenin and 5-β-D-glycosyl-naringenin demonstrated antioxidant action, but only naringenin inhibited the growth of gram-positive and gram-negative bacteria.Os extratos e frações de Acacia podalyriifolia foram analisados previamente para a atividade antibacteriana através da difusão em disco e as propriedades antioxidantes foram verificadas pela determinação da capacidade removedora do radical livre DPPH e pela mensuração do conteúdo de fenólicos totais (Método de Folin). O presente estudo descreve as atividades antibacteriana (determinação da concentração inibitória mínima) e antioxidante (espécies reativas do ácido tiobarbitúrico - teste TBARS) para o extrato etanólico e as frações diclorometano e acetato de etila e para duas flavanonas (naringenina e 5-β-D-glicosil-naringenina) isoladas das flores de Acacia podalyriifolia A. Cunn. ex G. Don. As flavanonas naringenina e 5-β-D-glicosil-naringenina ainda não haviam sido obtidas desta espécie. A atividade antibacteriana mais efetiva foi observada com a fração acetato de etila (CIM=0,25 mg/mL contra Staphylococcus aureus ATCC 6538; CIM=0,125 mg/mL, contra Staphylococcus epidermidis ATCC 12229; CIM=0,5 mg/mL contra Streptococcus pyogenes ATCC 19615, Klebsiella pneumoniae ATCC 13883 e Proteus mirabilis ATCC 43071). As amostras avaliadas demonstraram atividade pelo teste TBARS, especialmente o extrato etanólico (1000 ppm), que foi o mais ativo (29,43% ± 0.65), seguido pela fração acetato de etila (1000 ppm, 24,84% ± 1,28), ambos demonstrando atividade mais elevada que a apresentada pelo ácido ascórbico (1000 ppm, 21,73% ± 1,77), ainda que menor que a do BHT (1000 ppm, 35,15% ± 3,42), ambas substâncias de referência. AS is supported by a doctoral grant from Fundação para a Ciência e Tecnologia (Foundation for Science and Technology) (SFRH/BD/108246/2015).OBJECTIVES: To compare definitions of high disease activity of the Ankylosing Spondylitis Disease Activity Score (ASDAS) and Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) in selecting patients for treatment with biologic disease-modifying antirheumatic drugs (bDMARDs). METHODS: Patients from Rheumatic Diseases Portuguese Register (Reuma.pt) with a clinical diagnosis of axial spondyloarthritis (axSpA) were included. Four subgroups (cross-tabulation between ASDAS (≥2.1) and BASDAI (≥4) definitions of high disease activity) were compared regarding baseline characteristics and response to bDMARDs at 3 and 6 months estimated in multivariable regression models. RESULTS: Of the 594 patients included, the majority (82%) had both BASDAI≥4 and ASDAS ≥2.1. The frequency of ASDAS ≥2.1, if BASDAI<4 was much larger than the opposite (ie, ASDAS <2.1, if BASDAI≥4): 62% vs 0.8%. Compared to patients fulfilling both definitions, those with ASDAS ≥2.1 only were more likely to be male (77% vs 51%), human leucocyte antigen B27 positive (79% vs 65%) and have a higher C reactive protein (2.9 (SD 3.5) vs 2.1 (2.9)). Among bDMARD-treated patients (n=359), responses across subgroups were globally overlapping, except for the most 'stringent' outcomes. Patients captured only by ASDAS responded better compared to patients fulfilling both definitions (eg, ASDAS inactive disease at 3 months: 61% vs 25% and at 6 months: 42% vs 25%). CONCLUSION: The ASDAS definition of high disease activity is more inclusive than the BASDAI definition in selecting patients with axSpA for bDMARD treatment. The additionally 'captured' patients respond better and have higher likelihood of predictors thereof. These results support using ASDAS≥2.1 as a criterion for treatment decisions.publishersversionpublishe
