    Quality of bladder cancer information on YouTube.

    Introduction YouTube is one of the social networks most widely used as a source of information. However, there are doubts about the scientific quality of the information available. This study aims to characterise this by analysing videos about bladder cancer posted on YouTube. Material and methods This was a cross-sectional descriptive study of the first 50 Spanish-language videos published on YouTube, leaving 38 for analysis. The videos were evaluated by three urologists using two validated questionnaires: Patient Education Materials Assessment Tool (PEMAT) and DISCERN (quality criteria for consumer health information), classifying them according to the score of the latter, in poor quality (1–2 points) and moderate/good quality (3–5 points). Results The median PEMAT score was 71.6% (16–5–100%) for understanding and 35.5% (0–100%) for action. According to DISCERN, 26 videos (66.7%) were of poor quality and 12 (30.8%) of moderate/good quality. We found significant differences in terms of PEMAT of understanding (p = 0.004) and action (p = 0.000). In total, 90.9% of those involving medical staff were of low quality, which is paradoxical, but statistically significant (p = 0.01). Furthermore, 52.4% of those describing relevant information were of moderate/good quality, and 94.1% of those not describing relevant information were of poor quality (p = 0.02). Conclusions More than 60% of the videos published on YouTube about bladder cancer in Spanish are of low quality. This represents an important risk of misinformation for the general public to whom most of them are addressed.post-print729 K

    Feasible glass-melting process assisted by microwaves

This article may be used for non-commercial purposes in accordance with Wiley Terms and Conditions for Self-Archiving.[EN] The advantages of microwave (MW) energy processing have been verified in the sintering of a ceramic frit at a pre-industrial scale. The challenge of achieving high temperature using MW energy at such dimensions was overcome and a mix of natural raw materials was heated until its fluxing point. Changes in dielectric properties of the raw materials mix were also measured in situ with the increase in temperature, being in accordance to thermal processes of a conventional heating process. The properties of the resulting ceramic frit were compared with the same frit obtained by the conventional sintering method. Both frits showed similar thermal behavior regarding DTA-TGA, heating microscopy and XRD (only glassy phase was present). A Raman study confirms the existence of the mentioned glass phase. The application of the frits as glazes were performed and their properties were studied.     Photodynamic Therapy for the Treatment of Complex Anal Fistula

    Background and objectives: To validate and analyze the results of intralesional photodynamic therapy in the treatment of complex anal fistula. Study design/materials and methods: This prospective multicentric observational study enrolled patients treated for complex anal fistula who underwent intralesional photodynamic therapy (i-PDT). The included patients were treated from January 2016 to December 2018 with a minimum follow-up of 1 year to evaluate recurrence, continence and postoperative morbidity. Intralesional 5-aminolevulinic acid (ALA) gel (2%) was injected directly into the fistula. The internal and external orifices were closed. After an incubation period of 2 hours, the fistula was irradiated using an optical fiber connected to a red laser (Multidiode 630 PDT) operating at 1 W/cm for 3 minutes (180 J). Results: In total, 49 patients were included (61.2% male). The mean age was 48 years, and the mean duration of fistula was 13 months. Of the fistulas included, 75.5% were medium transphincteric, and 24.5% were high transphincteric. The median fistula length was 4 ± 1,14 cm (range: 3-5). A total of 41 patients (83.7%) had a previous history of fistula surgery. Preoperatively, some degree of anal incontinence was found in 5 patients (10.2%). No center reported any other procedure-related complications intraoperatively. Phototoxicity was found in one patient. In the first 48 hours after the procedure, fever was reported in 2 patients (4%). At the end of follow-up, total healing was observed in 32/49 patients (65.3%). No patient reported new incontinence postoperatively. Conclusion: i-PDT could be considered a good choice in patients with complex anal fistulas to avoid surgery and its complications. Lasers Surg.     Psicosis y diferencias sociales: Comparando la prevalencia de las psicosis en dos medios urbanos diferenciados.

    Objetivos: Contribuir a la reflexión sobre la etiología y/o los factores de riesgo para las psicosis comparando la prevalencia en población general y población de riesgo de la esquizofrenia y otras psicosis en dos barrios de Barcelona (España). Método: Nuestras aportaciones en este trabajo se apoyan sobre todo en un estudio descriptivo transversal de todos los pacientes con psicopatología detectados en la USM de Sant Martí-La Mina: un territorio geodemográfica y asistencialmente delimitado formado por 5 Áreas Básicas de Salud (103.615 habitantes. Resultados: Sobre un total de 21.536 pacientes con registro de casos abierto desde el año 1982 hasta el año 2000, se halló que 838 cumplían los criterios restrictivos para ser diagnosticados como “esquizofrénicos” (N=476) o “afectados por otras psicosis” (N=362). Sin embargo, las prevalencias de esquizofrenia y otras psicosis en el barrio sujeto a más factores de riesgo psicosociales eran alrededor de 2 veces mayores que las encontradas en el barrio colindante por el mismo equipo y en el mismo período temporal. Conclusión: Es necesario tener en cuenta el gran peso de los factores de sociales y psicosociales para poderse explicar las diferencias de prevalencia de la esquizofrenia y otras psicosis entre diferentes poblaciones

    Cobertura de la vacunación antigripal en los farmacéuticos comunitarios: razones aportadas y absentismo relacionado. Predisposición a vacunar a la población

    Introducción: la vacuna antigripal está indicada entre el personal sanitario, aunque la cobertura antigripal en farmacéuticos comunitarios españoles actualmente se desconoce. Este trabajo pretende averiguar esta cobertura, las causas de los farmacéuticos para vacunarse o no de la gripe, el absentismo laboral que les ocasiona, si recomiendan la vacunación antigripal entre los grupos de riesgo y su disposición a administrarla en la farmacia. Métodos: estudio descriptivo transversal mediante un cuestionario online destinado a farmacéuticos comunitarios de toda España. Se realizó un análisis descriptivo de las variables estudiadas y de la asociación entre la vacunación y las variables cualitativas (test Chi-cuadrado y Fisher) y cuantitativas (ANOVA). Resultados: se obtuvo una tasa de respuesta del 9,4 % (n=1.436). Los farmacéuticos refirieron vacunarse en torno al 30 % en las tres temporadas estudiadas (2016-2019), siendo del 31-35,8 % (IC95 %) en la temporada 2018-2019. Ser titular/cotitular de la farmacia, la experiencia profesional, la edad, vacunarse para evitar la gripe y/o por responsabilidad, y vacunarse en temporadas anteriores son factores asociados a vacunarse (p<0,05). Ser socio de SEFAC también lo es. El absentismo por gripe en la temporada 2018-2019 fue del 9,5 %. Los farmacéuticos que más dijeron vacunarse recomiendan más la vacuna que los que no, y tendrían mayor predisposición a administrarla (p<0,05). Conclusiones: la cobertura vacunal antigripal entre los farmacéuticos comunitarios es baja a pesar del absentismo que les causa. Facilitar la vacunación a los farmacéuticos podría incrementar su cobertura. Los farmacéuticos recomiendan la vacunación contra la gripe a los pacientes de riesgo y estarían dispuestos a vacunar en farmacias autorizadas

    Exploring bovine fecal bacterial microbiota in the Mapimi Biosphere Reserve, Northern Mexico

    En México la información sobre la microbiota fecal bovina (Bos taurus) es escasa. El presente estudio describe la diversidad y abundancia de bacterias en muestras fecales de bovinos en pastizales, recolectadas en la Reserva de la Biosfera de Mapimí en la parte central del desierto chihuahuense. Las muestras fecales se analizaron mediante secuenciación masiva de siguiente generación de alto rendimiento utilizando la V3-V4 del ARNr 16S en Miseq de Illumina. Se identificaron un total de 17 filos, 24 clases, 33 órdenes, 50 familias, 281 géneros y 297 especies. Firmicutes y Verrucomicrobia fueron los filos más abundantes. Los géneros más abundantes fueron Sporobacter, PAC000748_g (géneros de la familia Ruminococcaceae) y Eubacterium_g23. Se registraron tres géneros (Clostridium, Corynebacterium y Fusobacterium) y una especie (Campylobacter fetus) de bacterias bovinas potencialmente patógenas. Esta información representa una línea base bacteriológica para monitorear el estado de salud intestinal de bovinos en pastoreo y para rastrear posibles interacciones con la microbiota fecal de la fauna nativa itinerante del área.In Mexico, information on the bovine fecal microbiota (Bos taurus) is scarce. The present study describes the diversity and abundance of bacteria in fecal samples from rangeland bovines, collected in the Mapimi Biosphere Reserve in the central part of the Chihuahuan desert. Fecal samples were analysed using high-throughput next generation massive sequencing using V3-V4 16S rRNA on Illumina Miseq. A total of 17 phyla, 24 classes, 33 orders, 50 families, 281 genera, and 297 species were identified. Firmicutes and Verrucomicrobia were the most abundant phyla. The most abundant genera were Sporobacter, PAC000748_g (genera into the Ruminococcaceae family) and Eubacterium_g23. Three genera (Clostridium, Corynebacterium and Fusobacterium) and one species (Campylobacter fetus) potentially pathogenic bovine bacteria were registered. This information represents a bacteriological baseline for monitoring the grazing bovine intestinal health status, and to trace possible interactions with the fecal microbiota of native roaming wildlife in the area

    Gluten-induced RNA methylation changes regulate intestinal inflammation via allele-specific XPO1 translation in epithelial cells

    [EN] Objectives Coeliac disease (CD) is a complex autoimmune disorder that develops in genetically susceptible individuals. Dietary gluten triggers an immune response for which the only available treatment so far is a strict, lifelong gluten free diet. Human leucocyte antigen (HLA) genes and several non-HLA regions have been associated with the genetic susceptibility to CD, but their role in the pathogenesis of the disease is still essentially unknown, making it complicated to develop much needed non-dietary treatments. Here, we describe the functional involvement of a CD-associated single-nudeotide polymorphism (SNP) located in the 5'UTR of XPO1 in the inflammatory environment characteristic of the coeliac intestinal epithelium. Design The function of the CD-associated SNP was investigated using an intestinal cell line heterozygous for the SNP, N6-methyladenosine (m(6)A)-related knock-out and HLA-DQ2 mice, and human samples from patients with CD. Results Individuals harbouring the risk allele had higher m(6)A methylation in the 5'UTR of XPO1 RNA, rendering greater XPO1 protein amounts that led to downstream nuclear factor kappa B (NFkB) activity and subsequent inflammation. Furthermore, gluten exposure increased overall m(6)A methylation in humans as well as in in vitro and in vivo models. Conclusion We identify a novel m(6)A-XPO1-NFkB pathway that is activated in CD patients.
