Colonoscopy screening rates among patients of colonoscopy-trained African American primary care physicians
Background - When performed competently, colonoscopy screening can reduce colorectal cancer rates, especially in high-risk groups such as African Americans (AAs). Training primary care physicians (PCPs) to perform colonoscopy may improve screening rates among underserved high-risk populations.
Methods - We compared colonoscopy screening rates and computed adjusted odds ratios for colonoscopy-eligible patients of trained AA PCPs (study group) vs. untrained PCPs (comparison group), before and since initiating colonoscopy training. All colonoscopies were performed at a licensed ambulatory surgery center with specialist standby support. Retrospective chart review was conducted on 200 consecutive, established outpatients aged â„50 years at each of 12 PCP offices (7 trained AA PCPs and 5 untrained PCPs, practicing in the same region), total 1,244 study group and 923 comparison group patients.
Results - Post-training colonoscopy rates in both groups were higher than pre-training rates: 48.3% vs. 9.3% in the study group, 29.6% vs. 9.8% in the comparison group (both p\u3c0.001). AA patients in the study group showed over 5-fold increase (8.9% pre-training vs. 52.8% post training), with no change among Whites (18.2% vs. 25.0%). Corresponding pre- and post-training rates among comparison patients were 10.4% and 38.7% respectively among AAs (p\u3c0.001), and 13.3% vs. 13.2% respectively among Whites. After adjusting for demographics, duration since becoming the PCP\u27s patient, and health insurance, the study group had a 66% higher likelihood of colonoscopy in the post-training period (OR=1.66; CI, 1.30, 2.13), and AAs had a five-fold increased likelihood of colonoscopy relative to Whites.
Conclusions - Colonoscopy-trained PCPs may help reduce colorectal cancer disparities
Gamma-Ray Burster Counterparts: HST Blue and Ultraviolet Data
The surest solution of the Gamma Ray Burst (GRB) mystery is to find an
unambiguous low-energy quiescent counterpart. However, to date no reasonable
candidates have been identified in the x-ray, optical, infrared, or radio
ranges. The Hubble Space Telescope (HST) has now allowed for the first deep
ultraviolet searches for quiescent counterparts. This paper reports on
multiepoch ultraviolet searches of five GRB positions with HST. We found no
sources with significant ultraviolet excesses, variability, parallax, or proper
motion in any of the burst error regions. In particular, we see no sources
similar to that proposed as a counterpart to the GRB970228. While this negative
result is disappointing, it still has good utility for its strict limits on the
no-host-galaxy problem in cosmological models of GRBs. For most cosmological
models (with peak luminosity 6X10^50 erg/s), the absolute B magnitude of any
possible host galaxy must be fainter than -15.5 to -17.4. These smallest boxes
for some of the brightest bursts provide the most critical test, and our limits

Reducing Colorectal Cancer Incidence and Disparities: Performance and Outcomes of a Screening Colonoscopy Program in South Carolina
This study evaluated the efficiency, effectiveness, and racial disparities reduction potential of Screening Colonoscopies for People Everywhere in South Carolina (SCOPE SC), a state-funded program for indigent persons aged 50â64 years (45â64 years for African American (AA)) with a medical home in community health centers. Patients were referred to existing referral network providers, and the centers were compensated for patient navigation. Data on procedures and patient demographics were analyzed. Of 782 individuals recruited (71.2% AA), 85% (665) completed the procedure (71.1% AA). The adenoma detection rate was 27.8% (males 34.6% and females 25.1%), advanced neoplasm rate 7.7% (including 3 cancers), cecum intubation rate 98.9%, inadequate bowel preparation rate 7.9%, and adverse event rate 0.9%. All indicators met the national quality benchmarks. The adenoma rate of 26.0% among AAs aged 45â49 years was similar to that of older Whites and AAs. We found that patient navigation and a medical home setting resulted in a successful and high-quality screening program. The observed high adenoma rate among younger AAs calls for more research with larger cohorts to evaluate the appropriateness of the current screening guidelines for AAs, given that they suffer 47% higher colorectal cancer mortality than Whites

Disordered clock protein interactions and charge blocks turn an hourglass into a persistent circadian oscillator
Organismal physiology is widely regulated by the molecular circadian clock, a feedback loop composed of protein complexes whose members are enriched in intrinsically disordered regions. These regions can mediate protein-protein interactions via SLiMs, but the contribution of these disordered regions to clock protein interactions had not been elucidated. To determine the functionality of these disordered regions, we applied a synthetic peptide microarray approach to the disordered clock protein FRQ in Neurospora crassa. We identified residues required for FRQ\u27s interaction with its partner protein FRH, the mutation of which demonstrated FRH is necessary for persistent clock oscillations but not repression of transcriptional activity. Additionally, the microarray demonstrated an enrichment of FRH binding to FRQ peptides with a net positive charge. We found that positively charged residues occurred in significant blocks within the amino acid sequence of FRQ and that ablation of one of these blocks affected both core clock timing and physiological clock output. Finally, we found positive charge clusters were a commonly shared molecular feature in repressive circadian clock proteins. Overall, our study suggests a mechanistic purpose for positive charge blocks and yielded insights into repressive arm protein roles in clock function
Sunyaev-Zel'dovich observations of galaxy clusters out to the virial radius with the Arcminute Microkelvin Imager
We present observations using the Small Array of the Arcminute Microkelvin
Imager (AMI; 14-18 GHz) of four Abell and three MACS clusters spanning
0.171-0.686 in redshift. We detect Sunyaev-Zel'dovich (SZ) signals in five of
these without any attempt at source subtraction, although strong source
contamination is present. With radio-source measurements from high-resolution
observations, and under the assumptions of spherical -model,
isothermality and hydrostatic equilibrium, a Bayesian analysis of the data in
the visibility plane detects extended SZ decrements in all seven clusters over
and above receiver noise, radio sources and primary CMB imprints. Bayesian
evidence ratios range from 10^{11}:1 to 10^{43}:1 for six of the clusters and
3000:1 for one with substantially less data than the others. We present
posterior probability distributions for, e.g., total mass and gas fraction
averaged over radii internal to which the mean overdensity is 1000, 500 and
200, r_200 being the virial radius. Reaching r_200 involves some extrapolation
for the nearer clusters but not for the more-distant ones. We find that our
estimates of gas fraction are low (compared with most in the literature) and
decrease with increasing radius. These results appear to be consistent with the
notion that gas temperature in fact falls with distance (away from near the

AMI observations of Lynds Dark Nebulae: further evidence for anomalous cm-wave emission
Observations at 14.2 to 17.9 GHz made with the AMI Small Array towards
fourteen Lynds Dark Nebulae with a resolution of 2' are reported. These sources
are selected from the SCUBA observations of Visser et al. (2001) as small
angular diameter clouds well matched to the synthesized beam of the AMI Small
Array. Comparison of the AMI observations with radio observations at lower
frequencies with matched uv-plane coverage is made, in order to search for any
anomalous excess emission which can be attributed to spinning dust. Possible
emission from spinning dust is identified as a source within a 2' radius of the
Scuba position of the Lynds dark nebula, exhibiting an excess with respect to
lower frequency radio emission. We find five sources which show a possible
spinning dust component in their spectra. These sources have rising spectral
indices in the frequency range 14.2--17.9 GHz. Of these five one has already
been reported, L1111, we report one new definite detection, L675, and three new
probable detections (L944, L1103 and L1246). The relative certainty of these
detections is assessed on the basis of three criteria: the extent of the
emission, the coincidence of the emission with the Scuba position and the
likelihood of alternative explanations for the excess. Extended microwave
emission makes the likelihood of the anomalous emission arising as a
consequence of a radio counterpart to a protostar or a proto-planetary disk
unlikely. We use a 2' radius in order to be consistent with the IRAS
identifications of dark nebulae (Parker 1988), and our third criterion is used
in the case of L1103 where a high flux density at 850 microns relative to the
FIR data suggests a more complicated emission spectrum.Comment: submitted MNRA
Women's preferences for a new contraceptive under development: an exploratory study
ObjectiveCurrently available contraceptive methods do not meet the needs of all users. We sought to explore preferences of potential end-users regarding an on-demand, non-hormonal female contraceptive currently under development, using a web-based survey.Study designWe recruited respondents for an exploratory survey via web link on Amazon Mechanical Turk (MTurk). Individuals were eligible if they were 18â44 years of age, identified as cis-gender female, were English-speaking, not pregnant, and had used barrier contraception previously. Respondents provided demographic characteristics and a basic reproductive history. We then provided a brief description of the potential contraceptive. Respondents were asked about their interest in the proposed contraceptive and preferences for method attributes.ResultsA total of 500 respondents completed the survey. Three-quarters of respondents were <35 years of age and 48.2% were currently using a barrier contraceptive method. Three-fourths of respondents (73.8%) expressed interest in using the contraceptive under development. The majority wanted the method to be small (â€2 inches), rod-shaped, and low cost (<$5 per use). More than half (59.4%) said it was important to be able to use the method without partnersâ knowledge. The most reported potential concerns were vaginal irritation (51.6%) and lack of effectiveness (46.4%). Sixty percent of respondents were confident they could use the method correctly.DiscussionAvailable contraceptive methods lack attributes preferred by some users. Development of new contraceptives frequently does not involve end-user input early in the development process. Individuals in this sample displayed interest in the proposed contraceptive and expressed preferences that can inform the further development of this method
Seaweed and Soy: Companion Foods in Asian Cuisine and Their Effects on Thyroid Function in American Women
Seaweeds and soy are two commonly eaten foods in Asia. Both have been reported to affect thyroid function, seaweed because of its iodine content and soy because of its goitrogenic effect. Twenty-five healthy postmenopausal women (mean age 58 years) completed a double-blinded randomized crossover study. Ten capsules (5 g/day) of placebo or seaweed (Alaria exculenta), providing 475 ug of iodine/day, were consumed daily for 7 weeks. A powdered soy protein isolate (Solae Co., St. Louis, MO) providing 2 mg of isoflavones/kg of body weight, was given daily during the last week of each treatment arm. On average, this provided 141.3 mg of isoflavones/day and 67.5 g of protein/day. Blood samples and 48-hour urine samples were collected before and after each intervention period, and urinary I/C (ug of iodine/g of creatinine) and serum thyroxine, free thyroxine index, total triiodothyronine, and thyroidstimulating hormone (TSH) were measured. Seaweed ingestion increased I/C concentrations (P \u3c .0001) and serum TSH (P \u3c .0001) (1.69 +/- 0.22 vs. 2.19 +/- 0.22 uU/mL, mean +/- SE). Soy supplementation did not affect thyroid end points. Seven weeks of 5 g/day seaweed supplementation was associated with a small but statistically significant increase in TSH. Soy protein isolate supplementation was not associated with changes in serum thyroid hormone concentrations
