    Clinical Characteristics, Patient-Reported Outcomes, and Previous Therapeutic Management of Patients with Uncontrolled Neuropathic Pain Referred to Pain Clinics

    Background. The aim of this report was to evaluate the clinical profile and previous management of patients with uncontrolled neuropathic pain who were referred to pain clinics. Methods. We included adult patients with uncontrolled pain who had a score of ≥4 in the DN4 questionnaire. In addition to sociodemographic and clinical data, we evaluated pain levels using a visual analog scale as well as anxiety, depression, sleep, disability, and treatment satisfaction employing validated tools. Results. A total of 755 patients were included in the study. The patients were predominantly referred to pain clinics by traumatologists (34.3%) and primary care physicians (16.7%). The most common diagnoses were radiculopathy (43%) and pain of oncological origin (14.3%). The major cause for uncontrolled pain was suboptimal treatment (88%). Fifty-three percent of the patients were depressed, 43% had clinical anxiety, 50% rated their overall health as bad or very bad, and 45% noted that their disease was severely or extremely interfering with their daily activities. Conclusions. Our results showed that uncontrolled neuropathic pain is a common phenomenon among the specialties that address these clinical entities and, regardless of its etiology, uncontrolled pain is associated with a dramatic impact on patient well-being

    Time Series on Functional Service Life of Buildings using Fuzzy Delphi Method

    The functional service life of heritage buildings, defined as the time period during which the building fulfils the requirements for which it was designed, is a complex system that has still not been fully resolved and continues to be the object of research regarding its social, economic and cultural importance. This paper presents an application for analysing time series that reflect the state of building performance over time. To this end, historical time records are used that provided data that could be interpreted by experts in the field. The latter can then evaluate the input variables (vulnerability and risk) using the expert system for predicting the service life of buildings, Fuzzy Building Service Life (FBSL), this methodology put together the fuzzy logic tools and Delphi method. This model provides output data on the state of functionality or performance of each buildings at each moment in time whenever information records are available. The Delphi Method is used to eliminate expert subjectivity, establishing an FDM-type assessment methodology that effectively quantifies the service life of buildings over time. The application is able to provide significant data when generating future preventive maintenance programmes in architectural-cultural heritage buildings. It can also be used to optimise the resources invested in the conservation of heritage buildings.     Preventive Conservation and Restoration Monitoring of Heritage Buildings Based on Fuzzy Logic

    This article discusses the usability of the Art-Risk 3.0 software for research on the conservation of heritage buildings. It is a new and free software based on fuzzy logic, which enables the assessment of preventive conservation and surveillance of the restoration of heritage buildings over a period of time. This artificial intelligence-based tool considers the vulnerability of buildings, their environ ments, and their management to evaluate the necessity of their restoration or preventive con servation. To validate the Art-Risk 3.0, 500 theoretical case studies were analyzed, and a 14th century Mudejar-Gothic-style Church in Seville, Spain was studied both before and after its restora tion to identify post-restoration changes. This proof of concept demonstrates the capability of the Art-Risk 3.0 software to analyze environmental impacts on the vulnerability, risk, and functional service life of buildings, and assess the effectiveness of restoration activities.     Triglycerides and Residual Atherosclerotic Risk.

    Even when low-density lipoprotein-cholesterol (LDL-C) levels are lower than guideline thresholds, a residual risk of atherosclerosis remains. It is unknown whether triglyceride (TG) levels are associated with subclinical atherosclerosis and vascular inflammation regardless of LDL-C. This study sought to assess the association between serum TG levels and early atherosclerosis and vascular inflammation in apparently healthy individuals. An observational, longitudinal, and prospective cohort study, including 3,754 middle-aged individuals with low to moderate cardiovascular risk from the PESA (Progression of Early Subclinical Atherosclerosis) study who were consecutively recruited between June 2010 and February 2014, was conducted. Peripheral atherosclerotic plaques were assessed by 2-dimensional vascular ultrasound, and coronary artery calcification (CAC) was assessed by noncontrast computed tomography, whereas vascular inflammation was assessed by fluorine-18 fluorodeoxyglucose uptake on positron emission tomography. Atherosclerotic plaques and CAC were observed in 58.0% and 16.8% of participants, respectively, whereas vascular inflammation was evident in 46.7% of evaluated participants. After multivariate adjustment, TG levels ≥150 mg/dl showed an association with subclinical noncoronary atherosclerosis (odds ratio [OR]: 1.35; 95% confidence interval [CI]: 1.08 to 1.68; p = 0.008). This association was significant for groups with high LDL-C (OR: 1.42; 95% CI: 1.11 to 1.80; p = 0.005) and normal LDL-C (OR: 1.85; 95% CI: 1.08 to 3.18; p = 0.008). No association was found between TG level and CAC score. TG levels ≥150 mg/dl were significantly associated with the presence of arterial inflammation (OR: 2.09; 95% CI: 1.29 to 3.40; p = 0.003). In individuals with low to moderate cardiovascular risk, hypertriglyceridemia was associated with subclinical atherosclerosis and vascular inflammation, even in participants with normal LDL-C levels.     Critical update, systematic review, and meta-analysis of oral erythroplakia as an oral potentially malignant disorder

    Background: Oral erythroplakia has been classically considered as the potentially malignant disorder with the highest rate of malignant development into squamous cell carcinoma. This critical systematic review and meta-analysis aim to estimate the malignant development rate of oral erythroplakia and identify the associated risk factors. Methods: We performed a bibliographic search in PubMed, Scopus, Web of Science, Embase, and LILACS, with keywords “erythroplakia,” “erythroplasia,” “malignant transformation,” “malignant development,” “malignization,” “carcinogenesis,” “oral cancer,” “oral squamous cell carcinoma,” “mouth neoplasm,” and “prognosis.” Meta-analysis was conducted using a random-effects model. Results: Ten observational studies with 441 patients met the inclusion criteria, whose mean malignant development rate was 12.7% and with a mean follow-up period of patients of 6.66 years. In the initial biopsy, 42.8% of oral erythroplakia were already squamous cell carcinoma. The buccal mucosa was the most frequent location of oral erythroplakia, but the floor of the mouth was the most common site of malignant development. All patients who underwent malignant development showed epithelial dysplasia on the initial diagnostic biopsy. Conclusion: Overall malignant development rate of OE in the meta-analysis was 19.9%. We could not associate any specific clinicopathological feature with the malignant development. The presence of epithelial dysplasia in the initial biopsy remains the worst prognostic factor. Further observational studies on OE are needed, with well-established diagnostic criteria and good clinical follow-up, in order to identify the true risk of malignant development of oral erythroplakia and the related risk factorsS

    Clinical and Cytokine Profile in Patients with Early and Late Onset Meniere Disease

    Background: Meniere disease (MD) is an inner ear disorder associated with comorbidities such as autoimmune diseases or migraine. This study describes clinical and cytokine profiles in MD according to the age of onset of the condition. Methods: A cross-sectional study including 83 MD patients: 44 with early-onset MD (EOMD, <35 years old), and 39 with late-onset MD (LOMD, >50 years old), 64 patients with migraine and 55 controls was carried out. Clinical variables and cytokines levels of CCL3, CCL4, CCL18, CCL22, CXCL,1 and IL-1β were compared among the different groups. Results: CCL18 levels were higher in patients with migraine or MD than in controls. Elevated levels of IL-1β were observed in 11.4% EOMD and in 10.3% LOMD patients and these levels were not dependent on the age of individuals. EOMD had a longer duration of the disease (p = 0.004) and a higher prevalence of migraine than LOMD (p = 0.045). Conclusions: Patients with EOMD have a higher prevalence of migraine than LOMD, but migraine is not associated with any cytokine profile in patients with MD.     Els temps canvien cada vegada més ràpidament, i a la universitat això encara es nota més. L’històric departament d‘Expressió Gràfica a l’Enginyeria (EGE) de la Universitat Politècnica de Catalunya, garant d’una docència de Grau, Màster i Doctorat de qualitat i adaptada a les necessitats de la societat, emprèn l’any 2020 amb una proposta de canvi de nom per adaptar-se al nous coneixements que estan esdevenint la seva matèria principal, al voltant de l’enginyeria gràfica i el disseny. Les àrees de recerca del centenar de professors que formen el departament són àmplies i variades, i sempre en col·laboració en diversos grups tant de la pròpia UPC com d’altres universitats. Una recerca avançada, de caràcter pluridisciplinari, on s'apliquen creativitat i innovació com a eines de coneixement, implicats en un territori ampli, i situats als diferents campus de la UPC. En els capítols d’aquest llibre podeu veure una petita mostra d’aquesta recerca tecnològica en camps ben variats.Postprint (published version
