    Patterns and Trends in Chlorophyll-a Concentration and Phytoplankton Phenology in the Biogeographical Regions of Southwestern Atlantic

    The Southwestern Atlantic Ocean (SWA), is considered one of the most productive areas of the world, with a high abundance of ecologically and economically important fish species. Yet, the biological responses of this complex region to climate variability are still uncertain. Here, using 24 years of satellite-derived Chl-a data, we classified the SWA into 9 spatially coherent regions based on the temporal variability of Chl-a concentration, as revealed by SOM (Self-Organizing Maps) analysis. These biogeographical regions were the basis of a regional trend analysis in phytoplankton biomass, phenological indices, and environmental forcing variations. A general positive trend in phytoplankton concentration was observed, especially in the highly productive areas of the northern shelf-break, where phytoplankton biomass has increased at a rate of up to 0.42 ± 0.04 mg m−3 per decade. Significant positive trends in sea surface temperature were observed in 4 of the 9 regions (0.08–0.26 °C decade−1) and shoaling of the mixing layer depth in 5 of the 9 regions (−1.50 to −3.36 m decade−1). In addition to the generally positive trend in Chl-a, the most conspicuous change in the phytoplankton temporal patterns in the SWA is a delay in the autumn bloom (between 15 ± 3 and 24 ± 6 days decade−1, depending on the region). The observed variations in phytoplankton phenology could be attributed to climate-induced ocean warming and extended stratification period.     Filaggrin and cytokines in respiratory samples of preterm infants at risk for respiratory viral infection

    Respiratory viral infections (RVIs) are frequent in preterm infants possibly inducing long-term impact on respiratory morbidity. Immune response and respiratory barriers are key defense elements against viral insults in premature infants admitted to Neonatal Intensive Care Units (NICUs). Our main goals were to describe the local immune response in respiratory secretions of preterm infants with RVIs during NICU admission and to evaluate the expression and synthesis of lung barrier regulators, both in respiratory samples and in vitro models. Samples from preterm infants that went on to develop RVIs had lower filaggrin gene and protein levels at a cellular level were compared to never-infected neonates (controls). Filaggrin, MIP-1α/CCL3 and MCP-1 levels were higher in pre-infection supernatants compared to controls. Filaggrin, HIF-1α, VEGF, RANTES/CCL5, IL-17A, IL-1β, MIP-1α and MIP-1β/CCL5 levels were higher during and after infection. ROC curve and logistic regression analysis shows that these molecules could be used as infection risk biomarkers. Small airway epithelial cells stimulated by poly:IC presented reduced filaggrin gene expression and increased levels in supernatant.     Health-related quality of life with palbociclib plus endocrine therapy versus capecitabine in postmenopausal patients with hormone receptor–positive metastatic breast cancer: Patient-reported outcomes in the PEARL study

    Background: The PEARL study showed that palbociclib plus endocrine therapy (palbociclib/ET) was not superior to capecitabine in improving progression-free survival in postmenopausal patients with metastatic breast cancer resistant to aromatase inhibitors, but was better tolerated. This analysis compared patient-reported outcomes. Patients and methods: The PEARL quality of life (QoL) population comprised 537 patients, 268 randomised to palbociclib/ET (exemestane or fulvestrant) and 269 to capecitabine. Patients completed the European Organisation for Research and Treatment of Cancer QLQC30 and QLQ-BR23 and EQ-5D-3L questionnaires. Changes from the baseline and time to deterioration (TTD) were analysed using linear mixed-effect and stratified Cox regression models, respectively. Results: Questionnaire completion rate was high and similar between treatment arms. Significant differences were observed in the mean change in global health status (GHS)/QoL scores from the baseline to cycle 3 (2.9 for palbociclib/ET vs.-2.1 for capecitabine (95% confidence interval [CI], 1.4-8.6; P = 0.007). The median TTD in GHS/QoL was 8.3 months for palbociclib/ET versus 5.3 months for capecitabine (adjusted hazard ratio, 0.70; 95% CI, 0.55-0.89; P = 0.003). Similar improvements for palbociclib/ET were also seen for other scales as physical, role, cognitive, social functioning, fatigue, nausea/vomiting and appetite loss. No differences were observed between the treatment arms in change from the baseline in any item of the EQ-5D-L3 questionnaire as per the overall index score and visual analogue scale. Conclusion: Patients receiving palbociclib/ET experienced a significant delay in deterioration of GHS/QoL and several functional and symptom scales compared with capecitabine, providing additional evidence that palbociclib/ET is better tolerated.     CXCL12-mediated murine neural progenitor cell movement Requires PI3Kß activation

    The migratory route of neural progenitor/precursor cells (NPC) has a central role in central nervous system development. Although the role of the chemokine CXCL12 in NPC migration has been described, the intracellular signaling cascade involved remains largely unclear. Here we studied the molecular mechanisms that promote murine NPC migration in response to CXCL12, in vitro and ex vivo. Migration was highly dependent on signaling by the CXCL12 receptor, CXCR4. Although the JAK/STAT pathway was activated following CXCL12 stimulation of NPC, JAK activity was not necessary for NPC migration in vitro. Whereas CXCL12 activated the PI3K catalytic subunits p110α and p110β in NPC, only p110β participated in CXCL12-mediated NPC migration. Ex vivo experiments using organotypic slice cultures showed that p110β blockade impaired NPC exit from the medial ganglionic eminence. In vivo experiments using in utero electroporation nonetheless showed that p110β is dispensable for radial migration of pyramidal neurons.     Skin manifestations in COVID-19: prevalence and relationship with disease severity

    Background: Data on the clinical patterns and histopathology of SARS-CoV-2 related skin lesions, as well as on their relationship with the severity of COVID-19 are limited. Methods and Materials: Retrospective analysis of a prospectively collected cohort of patients with SARS-CoV-2 infection in a teaching hospital in Barcelona, Spain, from 1 April to 1 May 2020. Clinical, microbiological and therapeutic characteristics, clinicopathological patterns of skin lesions, and direct immunofluorescence and immunohistochemical findings in skin biopsies were analyzed. Results: Fifty-eight out of the 2761 patients (2.1%) either consulting to the emergency room or admitted to the hospital for COVID-19 suspicion during the study period presented COVID-19 related skin lesions. Cutaneous lesions could be categorized into six patterns represented by the acronym "GROUCH": Generalized maculo-papular (20.7%), Grover's disease and other papulo-vesicular eruptions (13.8%), livedo Reticularis (6.9%), Other eruptions (22.4%), Urticarial (6.9%), and CHilblain-like (29.3%). Skin biopsies were performed in 72.4%, including direct immunofluorescence in 71.4% and immunohistochemistry in 28.6%. Patients with chilblain-like lesions exhibited a characteristic histology and were significantly younger and presented lower rates of systemic symptoms, radiological lung infiltrates and analytical abnormalities, and hospital and ICU admission compared to the rest of patients. Conclusion: Cutaneous lesions in patients with COVID-19 appear to be relatively rare and varied. Patients with chilblain-like lesions have a characteristic clinicopathological pattern and a less severe presentation of COVID-19

    HAZLO: Plataforma de telesalud basada en tecnologías mhealth para el despliegue de programas personalizados de rehabilitación cardiaca fase II

Madrid 4, 5 y 6 de noviembre 2015.Actualmente, los Programas de Rehabilitación Cardiaca, tanto en provisión presencial como domiciliaria, afrontan la necesidad de incrementar sus tasas de adopción y adherencia, y en estos retos, los servicios de telesalud basados en mhealth comienzan a jugar un papel relevante, aunque la evidencia es fragmentada y de baja calidad. Se ha implementado un servicio de telesalud que despliega un programa basado en actividades terapéuticas de rehabilitación física (marcha) y psicológica (relajación), contenidos educativos para la autogestión, y herramientas para la interacción virtual (mensajería, videollamada y foros). Se presenta en este trabajo la descripción del servicio y los resultados del pilotaje (41 pacientes, 5 meses), para evaluar la viabilidad en términos de operatividad-funcionalidad en cada uno de sus componentes y adherencia a los protocolos por parte de los pacientes. Se ha iniciado un ensayo aleatorizado controlado (128+128 pacientes) para estudiar la no inferioridad en resultados clínicos del modelo de provisión basado en telesalud frente al tradicional; adicionalmente, se estudiarán la mejora en calidad de vida, satisfacción y usabilidad.Este trabajo está siendo financiado por la AES 2012, PI12/00389 y PI12/00585 (coordinados), y PI12/00508, y la colaboración de REDISSEC RD12/0001/0001.N

    Development of an activity disease score in patients with uveitis (UVEDAI)

    To develop a disease activity index for patients with uveitis (UVEDAI) encompassing the relevant domains of disease activity considered important among experts in this field. The steps for designing UVEDAI were: (a) Defining the construct and establishing the domains through a formal judgment of experts, (b) A two-round Delphi study with a panel of 15 experts to determine the relevant items, (c) Selection of items: A logistic regression model was developed that set ocular inflammatory activity as the dependent variable. The construct "uveitis inflammatory activity" was defined as any intraocular inflammation that included external structures (cornea) in addition to uvea. Seven domains and 15 items were identified: best-corrected visual acuity, inflammation of the anterior chamber (anterior chamber cells, hypopyon, the presence of fibrin, active posterior keratic precipitates and iris nodules), intraocular pressure, inflammation of the vitreous cavity (vitreous haze, snowballs and snowbanks), central macular edema, inflammation of the posterior pole (the presence and number of choroidal/retinal lesions, vascular inflammation and papillitis), and global assessment from both (patient and physician). From all the variables studied in the multivariate model, anterior chamber cell grade, vitreous haze, central macular edema, inflammatory vessel sheathing, papillitis, choroidal/retinal lesions and patient evaluation were included in UVEDAI. UVEDAI is an index designed to assess the global ocular inflammatory activity in patients with uveitis. It might prove worthwhile to motorize the activity of this extraarticular manifestation of some rheumatic diseases
