    Developing Intercultural Competency among Undergraduate Students in the College of Allied Health and Nursing

    This study examined intercultural competence (ICC) among a group of university undergraduate students who were early in their studies. Mareno & Hart (2014) noted that demographic patterns have shifted toward becoming more racially and ethnically diverse. Therefore, health care providers must be equipped to provide culturally competent care to patients. This study will help universities develop curriculum that fosters student development of their ICC. For this study, ICC was defined as the capability to accurately understand and adapt behavior to cultural difference and commonality (Hammer & Bennett, 2010). The study responded to these research questions: (1) What is the starting level of cultural competence among undergraduate students in the College of Allied Health and Nursing? (2) How does the intercultural competence of undergraduate students in the College of Allied Health and Nursing change during their experiences in general education classes? Data was collected using a computer-based, online inventory. Students enrolled in an introductory course completed the Intercultural Development Inventory (IDI), developed by Hammer and Bennett (1998, 2001). The IDI was based on Bennett’s Developmental Model of Intercultural Sensitivity (1986), which identified five orientations toward cultural differences: denial, polarization, minimization, acceptance, and adaptation. Investigators expect that the study may show that students’ ICC will grow over the period of a semester-long course when it is supplemented with cultural activities such as a mentor partnership with an international student. The results of this study will provide institutions with information about the level of ICC of their students and how those levels can be improved so their students are better equipped to help others in the futur

    Investigating the Dynamics and Density Evolution of Returning Plasma Blobs from the 2011 June 7 Eruption

    This work examines infalling matter following an enormous Coronal Mass Ejection (CME) on 2011 June 7. The material formed discrete concentrations, or blobs, in the corona and fell back to the surface, appearing as dark clouds against the bright corona. In this work we examined the density and dynamic evolution of these blobs in order to formally assess the intriguing morphology displayed throughout their descent. The blobs were studied in five wavelengths (94, 131, 171, 193 and 211 \AA) using the Solar Dynamics Observatory Atmospheric Imaging Assembly (SDO/AIA), comparing background emission to attenuated emission as a function of wavelength to calculate column densities across the descent of four separate blobs. We found the material to have a column density of hydrogen of approximately 2 ×\times 1019^{19} cm−2^{-2}, which is comparable with typical pre-eruption filament column densities. Repeated splitting of the returning material is seen in a manner consistent with the Rayleigh-Taylor instability. Furthermore, the observed distribution of density and its evolution are also a signature of this instability. By approximating the three-dimensional geometry (with data from STEREO-A), volumetric densities were found to be approximately 2 ×\times 10−14^{-14} g cm−3^{-3}, and this, along with observed dominant length-scales of the instability, was used to infer a magnetic field of the order 1 G associated with the descending blobs.Comment: 9 pages, 13 figures, accepted for publication in Ap

    News Stories of Intimate Partner Violence: An Experimental Examination of Participant Sex, Perpetrator Sex, and Violence Severity on Seriousness, Sympathy, and Punishment Preferences

    This study experimentally examines the effects of participant sex, perpetrator sex, and severity of violence on perceptions of intimate partner violence (IPV) seriousness, sympathy toward the victim, and punishment preferences for the perpetrator. Participants (N = 449) were randomly assigned to a condition, exposed to a composite news story, and then completed a survey. Ratings of seriousness of IPV for stories with male perpetrators were significantly higher than ratings of seriousness for stories with female perpetrators. Men had significantly higher sympathy for female victims in any condition than for male victims in the weak or strong severity of violence conditions. Men’s sympathy for male victims in the fatal severity of violence condition did not differ from their sympathy for female victims. Women had the least sympathy for female victims in the weak severity condition and men in the weak or strong severity conditions. Women reported significantly higher sympathy for female victims in the strong and fatal severity of violence conditions. Women’s ratings of sympathy for male victims in the fatal severity of violence condition were statistically indistinguishable from any other group. Participants reported stronger punishment preferences for male perpetrators and this effect was magnified among men. Theoretical implications are presented with attention provided to practical considerations about support for public health services

    Outcome measures used to assess hand activity in amputee and intact populations : a literature review

    BACKGROUND: The human hand is critical in assisting with activities of daily living (ADL). Amputation of the hand can affect a person physically, socially and psychologically. Knowledge of outcome measures used to assess upper limb activity of intact and amputee populations may aid in guiding research to develop applicable measurement tools specific to the amputee population. Tools could aid developments in prosthetic design and prescription, which benefit both users and healthcare researchers. OBJECTIVE(S): This literature review examined outcome measurement tools used with non-amputee and amputee populations to assess hand activity. The objectives were to identify which characteristics of hand activity are captured by currently available measurement tools. METHODOLOGY: Searches were conducted using PubMed, Cochrane and ProQuest for studies investigating hand activity for amputee and non-amputee populations. A total of 15 studies were included. PRISMA guidelines were used to assist with study selection. Data extraction and narrative synthesis were carried out. FINDINGS: A total of 32 outcome measures were found. Frequently used tools were: Box and Block Test, Swedish Disabilities of the Arm Shoulder and Hand Questionnaire, and range of motion. Studies employed a combination of 2 to 12 tools. Themes extracted were: importance of function and quality of life, the need for realistic tasks, and the need for outcome measures specific of the population. CONCLUSION: There is a gap in research surrounding outcome measurement tools used to assess hand activity in the amputee population. A combination of outcome measures are required to obtain insight into the hand activities of intact and amputee populations.     Late-Stage Metastatic Melanoma Emerges through a Diversity of Evolutionary Pathways

    Understanding the evolutionary pathways to metastasis and resistance to immune-checkpoint inhibitors (ICI) in melanoma is critical for improving outcomes. Here, we present the most comprehensive intrapatient metastatic melanoma dataset assembled to date as part of the Posthumous Evaluation of Advanced Cancer Environment (PEACE) research autopsy program, including 222 exome sequencing, 493 panel-sequenced, 161 RNA sequencing, and 22 single-cell whole-genome sequencing samples from 14 ICI-treated patients. We observed frequent whole-genome doubling and widespread loss of heterozygosity, often involving antigen-presentation machinery. We found KIT extrachromosomal DNA may have contributed to the lack of response to KIT inhibitors of a KIT-driven melanoma. At the lesion-level, MYC amplifications were enriched in ICI nonresponders. Single-cell sequencing revealed polyclonal seeding of metastases originating from clones with different ploidy in one patient. Finally, we observed that brain metastases that diverged early in molecular evolution emerge late in disease. Overall, our study illustrates the diverse evolutionary landscape of advanced melanoma.SIGNIFICANCE: Despite treatment advances, melanoma remains a deadly disease at stage IV. Through research autopsy and dense sampling of metastases combined with extensive multiomic profiling, our study elucidates the many mechanisms that melanomas use to evade treatment and the immune system, whether through mutations, widespread copy-number alterations, or extrachromosomal DNA.See related commentary by Shain, p. 1294.     European Cystic Fibrosis Society standards of care: best practice guidelines

    Specialised CF care has led to a dramatic improvement in survival in CF: in the last four decades, well above what was seen in the general population over the same period. With the implementation of newborn screening in many European countries, centres are increasingly caring for a cohort of patients who have minimal lung disease at diagnosis and therefore have the potential to enjoy an excellent quality of life and an even greater life expectancy than was seen previously. To allow high quality care to be delivered throughout Europe, a landmark document was published in 2005 that sets standards of care. Our current document builds on this work, setting standards for best practice in key aspects of CF care. The objective of our document is to give a broad overview of the standards expected for screening, diagnosis, pre-emptive treatment of lung disease, nutrition, complications, transplant/end of life care and psychological support. For comprehensive details of clinical care of CF, references to the most up to date European Consensus Statements, Guidelines or Position Papers are provided in Table 1. We hope that this best practice document will be useful to clinical teams both in countries where CF care is developing and those with established CF centres

    Functional antibody and T-cell immunity following SARS-CoV-2 infection, including by variants of concern, in patients with cancer: the CAPTURE study

    Patients with cancer have higher COVID-19 morbidity and mortality. Here we present the prospective CAPTURE study (NCT03226886) integrating longitudinal immune profiling with clinical annotation. Of 357 patients with cancer, 118 were SARS-CoV-2-positive, 94 were symptomatic and 2 patients died of COVID-19. In this cohort, 83% patients had S1-reactive antibodies, 82% had neutralizing antibodies against WT, whereas neutralizing antibody titers (NAbT) against the Alpha, Beta, and Delta variants were substantially reduced. Whereas S1-reactive antibody levels decreased in 13% of patients, NAbT remained stable up to 329 days. Patients also had detectable SARS-CoV-2-specific T cells and CD4+ responses correlating with S1-reactive antibody levels, although patients with hematological malignancies had impaired immune responses that were disease and treatment-specific, but presented compensatory cellular responses, further supported by clinical. Overall, these findings advance the understanding of the nature and duration of immune response to SARS-CoV-2 in patients with cancer

    Finishing the euchromatic sequence of the human genome

    The sequence of the human genome encodes the genetic instructions for human physiology, as well as rich information about human evolution. In 2001, the International Human Genome Sequencing Consortium reported a draft sequence of the euchromatic portion of the human genome. Since then, the international collaboration has worked to convert this draft into a genome sequence with high accuracy and nearly complete coverage. Here, we report the result of this finishing process. The current genome sequence (Build 35) contains 2.85 billion nucleotides interrupted by only 341 gaps. It covers ∼99% of the euchromatic genome and is accurate to an error rate of ∼1 event per 100,000 bases. Many of the remaining euchromatic gaps are associated with segmental duplications and will require focused work with new methods. The near-complete sequence, the first for a vertebrate, greatly improves the precision of biological analyses of the human genome including studies of gene number, birth and death. Notably, the human enome seems to encode only 20,000-25,000 protein-coding genes. The genome sequence reported here should serve as a firm foundation for biomedical research in the decades ahead
