    Preventing perioperative bleeding in patients with inherited bleeding disorders

    Data sources: Cochrane Cystic Fibrosis and Genetic Disorders Group's Coagulopathies Trials Register, a regularly updated database informed by trials identified within electronic databases including MEDLINE. Further defined searches were undertaken in PubMed, Embase, The Cochrane Library, ClinicalTrials.gov and WHO International Clinical Trials Registry Platform. Additional hand searching of relevant journals and books of conference proceedings was undertaken. Study selection: Randomised and quasi-randomised controlled trials in people of all ages with haemophilia or VWD undergoing oral or dental procedures using antifibrinolytic agents (tranexamic acid (TXA) or epsilon aminocaproic acid (EACA)) to prevent perioperative bleeding compared to no intervention with or without placebo. Data extraction and synthesis: Two authors independently assessed identified publications for inclusion based on defined selection criteria. The two authors performed data extraction and risk of bias assessments using standardised forms and the Cochrane risk of bias tools. A third author, deemed to have particular subject expertise, verified eligibility of inclusion. Results: One randomised, double-blinded placebo controlled trial and one quasi-randomised trial were included. A total of 59 participants with haemophilia undergoing dental extraction were involved. Both trials evidenced a notable reduction in post-operative bleeding following dental extraction when either TXA or EACA were used, in addition to routine preoperative factor replacement, when compared to placebo. The number of post-operative bleeds, amount of blood loss and the need for additional clotting factors were reduced in the groups receiving antifibrinolytic therapy. No eligible trials in people with VWD were identified. Conclusions: Low quality evidence exists to support the use of adjuvant antifibrinolytic therapy to reduce perioperative bleeding in patients with haemophilia undergoing dental extraction. The limited number of trials identified (N=2), minimal sample size (N=28, N=31) and historic nature of the studies, originating from the 1970s, in addition to study heterogeneity and subsequent selection bias results in a low quality evidence grade for recommending adjuvant antifibrinolytic therapy. There is no clear indication to alter current practice utilising antifibrinolytic therapy to manage patients with haemophilia undergoing dental surgery in accordance with internationally accepted guidelines. However, further research with standardised study deigns would be welcomed in order to enhance the evidence base in the management of people with haemophilia and VWD

    A case report: retigabine induced oral mucosal dyspigmentation of the hard palate

    Background Dyspigmentation of the oral mucosa has a multitude of aetiological causes. Retigabine, a new antiepileptic drug, has the potential side effect of inducing a blue/purple pigmentation of the oral mucosa in addition to the skin, lips, nails and retina of the eyes. This article presents a unique case of dyspigmentation present in the oral mucosa of the hard palate which has previously been unreported in the dental literature. Case presentation A 70 year old white male presented to a secondary care oral surgery department with an unusual asymptomatic pigmented lesion present in the hard palate of the oral cavity. The pigmentation was remarkable for its distinct blue/purple colouration which was associated with a similar discolouration of the nail beds of the hands. This is believed to be a side effect of the anti-epileptic medication retigabine. Conclusion The dental profession and wider healthcare community should be made fully aware of the potential side effect of oral dyspigmentation associated with the novel anti-epileptic medication retigabine. Enhanced knowledge of the causative role of retigabine in dyspigementation of the oral mucosa will allow the practitioner to make an appropriate diagnosis. As far the authors are aware this is reaction is unreported in the dental literature and should be disseminated to the wider oral health professional’s community

    Quiet Clean Short-haul Experimental Engine (QCSEE). Aerodynamic and aeromechanical performance of a 50.8 cm (20 inch) diameter 1.34 PR variable pitch fan with core flow

    The fan aerodynamic and aeromechanical performance tests of the quiet clean short haul experimental engine under the wing fan and inlet with a simulated core flow are described. Overall forward mode fan performance is presented at each rotor pitch angle setting with conventional flow pressure ratio efficiency fan maps, distinguishing the performance characteristics of the fan bypass and fan core regions. Effects of off design bypass ratio, hybrid inlet geometry, and tip radial inlet distortion on fan performance are determined. The nonaxisymmetric bypass OGV and pylon configuration is assessed relative to both total pressure loss and induced circumferential flow distortion. Reverse mode performance, obtained by resetting the rotor blades through both the stall pitch and flat pitch directions, is discussed in terms of the conventional flow pressure ratio relationship and its implications upon achievable reverse thrust. Core performance in reverse mode operation is presented in terms of overall recovery levels and radial profiles existing at the simulated core inlet plane. Observations of the starting phenomena associated with the initiation of stable rotor flow during acceleration in the reverse mode are briefly discussed. Aeromechanical response characteristics of the fan blades are presented as a separate appendix, along with a description of the vehicle instrumentation and method of data reduction

Benefits from implementation of RDA for the Library of Congress are identified and highlighted: more flexibility in cataloguing decisions, easier international sharing of cataloguing data, clearer linking among related works; closer cooperation with other libraries in the North American community, production of an online learning platform in order to deliver RDA training on a large scale in real time to catalogers

    Acute tryptophan depletion attenuates conscious appraisal of social emotional signals in healthy female volunteers

    Rationale: Acute tryptophan depletion (ATD) decreases levels of central serotonin. ATD thus enables the cognitive effects of serotonin to be studied, with implications for the understanding of psychiatric conditions, including depression. Objective: To determine the role of serotonin in conscious (explicit) and unconscious/incidental processing of emotional information. Materials and methods: A randomized, double-blind, cross-over design was used with 15 healthy female participants. Subjective mood was recorded at baseline and after 4 h, when participants performed an explicit emotional face processing task, and a task eliciting unconscious processing of emotionally aversive and neutral images presented subliminally using backward masking. Results: ATD was associated with a robust reduction in plasma tryptophan at 4 h but had no effect on mood or autonomic physiology. ATD was associated with significantly lower attractiveness ratings for happy faces and attenuation of intensity/arousal ratings of angry faces. ATD also reduced overall reaction times on the unconscious perception task, but there was no interaction with emotional content of masked stimuli. ATD did not affect breakthrough perception (accuracy in identification) of masked images. Conclusions: ATD attenuates the attractiveness of positive faces and the negative intensity of threatening faces, suggesting that serotonin contributes specifically to the appraisal of the social salience of both positive and negative salient social emotional cues. We found no evidence that serotonin affects unconscious processing of negative emotional stimuli. These novel findings implicate serotonin in conscious aspects of active social and behavioural engagement and extend knowledge regarding the effects of ATD on emotional perception

    Unsteady Flow and Whirl-Inducing Forces in Axial-Flow Compressors: Part I—Experiment

    An experimental and theoretical investigation has been conducted to evaluate the effects seen in axial-flow compressors when the centerline of the rotor is displaced from the centerline of the static structure of the engine. This creates circumferentially nonuniform rotor-tip clearances, unsteady flow, and potentially increased clearances if the rotating and stationary parts come in contact. The result not only adversely affects compressor stall margin, pressure rise capability, and efficiency, but also generates an unsteady, destabilizing, aerodynamic force, called the Thomas/Alford force, which contributes significantly to rotor whirl instabilities in turbomachinery. Determining both the direction and magnitude of this force in compressors, relative to those in turbines, is especially important for the design of mechanically stable turbomachinery components. Part I of this two-part paper addresses these issues experimentally and Part II presents analyses from relevant computational models. Our results clearly show that the Thomas/Alford force can promote significant backward rotor whirl over much of the operating range of modern compressors, although some regions of zero and forward whirl were found near the design point. This is the first time that definitive measurements, coupled with compelling analyses, have been reported in the literature to resolve the long-standing disparity in findings concerning the direction and magnitude of whirl-inducing forces important in the design of modern axial-flow compressors

    Structural brain abnormalities in postural tachycardia syndrome: A VBM-DARTEL study

    Postural tachycardia syndrome (PoTS), a form of dysautonomia, is characterized by orthostatic intolerance, and is frequently accompanied by a range of symptoms including palpitations, lightheadedness, clouding of thought, blurred vision, fatigue, anxiety, and depression. Although the estimated prevalence of PoTS is approximately 5–10 times as common as the better-known condition orthostatic hypotension, the neural substrates of the syndrome are poorly characterized. In the present study, we used magnetic resonance imaging (MRI) with voxel-based morphometry (VBM) applying the diffeomorphic anatomical registration through exponentiated lie algebra (DARTEL) procedure to examine variation in regional brain structure associated with PoTS. We recruited 11 patients with established PoTS and 23 age-matched normal controls. Group comparison of gray matter volume revealed diminished gray matter volume within the left anterior insula, right middle frontal gyrus and right cingulate gyrus in the PoTS group. We also observed lower white matter volume beneath the precentral gyrus and paracentral lobule, right pre- and post-central gyrus, paracentral lobule and superior frontal gyrus in PoTS patients. Subsequent ROI analyses revealed significant negative correlations between left insula volume and trait anxiety and depression scores. Together, these findings of structural differences, particularly within insular and cingulate components of the salience network, suggest a link between dysregulated physiological reactions arising from compromised central autonomic control (and interoceptive representation) and increased vulnerability to psychiatric symptoms in PoTS patients

    Neuroanatomical substrates for the volitional regulation of heart rate

    The control of physiological arousal can assist in the regulation of emotional state. A subset cortical and subcortical brain regions are implicated in autonomic control of bodily arousal during emotional behaviors. Here, we combined human functional neuroimaging with autonomic monitoring to identify neural mechanisms that support the volitional regulation of heart rate, a process that may be assisted by visual feedback. During functional magnetic resonance imaging (fMRI), 15 healthy adults performed an experimental task in which they were prompted voluntarily to increase or decrease cardiovascular arousal (heart rate) during true, false, or absent visual feedback. Participants achieved appropriate changes in heart rate, without significant modulation of respiratory rate, and were overall not influenced by the presence of visual feedback. Increased activity in right amygdala, striatum and brainstem occurred when participants attempted to increase heart rate. In contrast, activation of ventrolateral prefrontal and parietal cortices occurred when attempting to decrease heart rate. Biofeedback enhanced activity within occipito-temporal cortices, but there was no significant interaction with task conditions. Activity in regions including pregenual anterior cingulate and ventral striatum reflected the magnitude of successful task performance, which was negatively related to subclinical anxiety symptoms. Measured changes in respiration correlated with posterior insula activation and heart rate, at a more lenient threshold, change correlated with insula, caudate, and midbrain activity. Our findings highlight a set of brain regions, notably ventrolateral prefrontal cortex, supporting volitional control of cardiovascular arousal. These data are relevant to understanding neural substrates supporting interaction between intentional and interoceptive states related to anxiety, with implications for biofeedback interventions, e.g., real-time fMRI, that target emotional regulation
