Structure and the Physico-Mechanical Properties of the Ceramic Coatings Obtained by the Cumulative -Detonation Device
Dense, with good adhesion to the substrate, hard, wear-resistant coatings from the powder of Al2O3 were
obtained on the surface of the steel (STE255) by using the cumulative-detonation device. The results of investigations
of the structure and physico-mechanical properties of the coatings by using scanning, optical microscopy,
X-ray phase analysis, microhardness and tribological tests are presented. It was found that optimization
of plasma spraying to helps reduce the porosity of coatings of Al2O3 less than 1 % and to increase the hardness
of them to 1250 HV0.3. The tribological investigations have shown that the coatings of Al2O3 significantly increase
the wear resistance of the sample STE255 and provide a low ability to wear out the coating.

Entire solutions of hydrodynamical equations with exponential dissipation
We consider a modification of the three-dimensional Navier--Stokes equations
and other hydrodynamical evolution equations with space-periodic initial
conditions in which the usual Laplacian of the dissipation operator is replaced
by an operator whose Fourier symbol grows exponentially as \ue ^{|k|/\kd} at
high wavenumbers . Using estimates in suitable classes of analytic
functions, we show that the solutions with initially finite energy become
immediately entire in the space variables and that the Fourier coefficients
decay faster than \ue ^{-C(k/\kd) \ln (|k|/\kd)} for any . The
same result holds for the one-dimensional Burgers equation with exponential
dissipation but can be improved: heuristic arguments and very precise
simulations, analyzed by the method of asymptotic extrapolation of van der
Hoeven, indicate that the leading-order asymptotics is precisely of the above
form with . The same behavior with a universal constant
is conjectured for the Navier--Stokes equations with exponential
dissipation in any space dimension. This universality prevents the strong
growth of intermittency in the far dissipation range which is obtained for
ordinary Navier--Stokes turbulence. Possible applications to improved spectral
simulations are briefly discussed.Comment: 29 pages, 3 figures, Comm. Math. Phys., in pres
COVID-19:immunopathology, pathophysiological mechanisms, and treatment options
Coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), continues to spread globally despite the worldwide implementation of preventive measures to combat the disease. Although most COVID-19 cases are characterised by a mild, self-limiting disease course, a considerable subset of patients develop a more severe condition, varying from pneumonia and acute respiratory distress syndrome (ARDS) to multi-organ failure (MOF). Progression of COVID-19 is thought to occur as a result of a complex interplay between multiple pathophysiological mechanisms, all of which may orchestrate SARS-CoV-2 infection and contribute to organ-specific tissue damage. In this respect, dissecting currently available knowledge of COVID-19 immunopathogenesis is crucially important, not only to improve our understanding of its pathophysiology but also to fuel the rationale of both novel and repurposed treatment modalities. Various immune-mediated pathways during SARS-CoV-2 infection are relevant in this context, which relate to innate immunity, adaptive immunity, and autoimmunity. Pathological findings in tissue specimens of patients with COVID-19 provide valuable information with regard to our understanding of pathophysiology as well as the development of evidence-based treatment regimens. This review provides an updated overview of the main pathological changes observed in COVID-19 within the most commonly affected organ systems, with special emphasis on immunopathology. Current management strategies for COVID-19 include supportive care and the use of repurposed or symptomatic drugs, such as dexamethasone, remdesivir, and anticoagulants. Ultimately, prevention is key to combat COVID-19, and this requires appropriate measures to attenuate its spread and, above all, the development and implementation of effective vaccines.</p
Effects of acute nutritional ketosis during exercise in adults with glycogen storage disease typeIIIaare phenotype-specific:An investigator-initiated, randomized, crossover study
Glycogen storage disease type IIIa (GSDIIIa) is an inborn error of carbohydrate metabolism caused by a debranching enzyme deficiency. A subgroup of GSDIIIa patients develops severe myopathy. The purpose of this study was to investigate whether acute nutritional ketosis (ANK) in response to ketone-ester (KE) ingestion is effective to deliver oxidative substrate to exercising muscle in GSDIIIa patients. This was an investigator-initiated, researcher-blinded, randomized, crossover study in six adult GSDIIIa patients. Prior to exercise subjects ingested a carbohydrate drink (~66 g, CHO) or a ketone-ester (395 mg/kg, KE) + carbohydrate drink (30 g, KE + CHO). Subjects performed 15-minute cycling exercise on an upright ergometer followed by 10-minute supine cycling in a magnetic resonance (MR) scanner at two submaximal workloads (30% and 60% of individual maximum, respectively). Blood metabolites, indirect calorimetry data, and in vivo 31P-MR spectra from quadriceps muscle were collected during exercise. KE + CHO induced ANK in all six subjects with median peak βHB concentration of 2.6 mmol/L (range: 1.6-3.1). Subjects remained normoglycemic in both study arms, but delta glucose concentration was 2-fold lower in the KE + CHO arm. The respiratory exchange ratio did not increase in the KE + CHO arm when workload was doubled in subjects with overt myopathy. In vivo 31P MR spectra showed a favorable change in quadriceps energetic state during exercise in the KE + CHO arm compared to CHO in subjects with overt myopathy. Effects of ANK during exercise are phenotype-specific in adult GSDIIIa patients. ANK presents a promising therapy in GSDIIIa patients with a severe myopathic phenotype. Upfront resection versus no resection of the primary tumor in patients with synchronous metastatic colorectal cancer:the randomized phase III CAIRO4 study conducted by the Dutch Colorectal Cancer Group and the Danish Colorectal Cancer Group
Background: Upfront primary tumor resection (PTR) has been associated with longer overall survival (OS) in patients with synchronous unresectable metastatic colorectal cancer (mCRC) in retrospective analyses. The aim of the CAIRO4 study was to investigate whether the addition of upfront PTR to systemic therapy resulted in a survival benefit in patients with synchronous mCRC without severe symptoms of their primary tumor. Patients and methods: This randomized phase III trial was conducted in 45 hospitals in The Netherlands and Denmark. Eligibility criteria included previously untreated mCRC, unresectable metastases, and no severe symptoms of the primary tumor. Patients were randomized (1 : 1) to upfront PTR followed by systemic therapy or systemic therapy without upfront PTR. Systemic therapy consisted of first-line fluoropyrimidine-based chemotherapy with bevacizumab in both arms. Primary endpoint was OS in the intention-to-treat population. The study was registered at ClinicalTrials.gov, NCT01606098. Results: Between August 2012 and February 2021, 206 patients were randomized. In the intention-to-treat analysis, 204 patients were included (n = 103 without upfront PTR, n = 101 with upfront PTR) of whom 116 were men (57%) with median age of 65 years (interquartile range 59-71 years). Median follow-up was 69.4 months. Median OS in the arm without upfront PTR was 18.3 months (95% confidence interval 16.0-22.2 months) compared with 20.1 months (95% confidence interval 17.0-25.1 months) in the upfront PTR arm (P = 0.32). The number of grade 3-4 events was 71 (72%) in the arm without upfront PTR and 61 (65%) in the upfront PTR arm (P = 0.33). Three deaths (3%) possibly related to treatment were reported in the arm without upfront PTR and four (4%) in the upfront PTR arm. Conclusions: Addition of upfront PTR to palliative systemic therapy in patients with synchronous mCRC without severe symptoms of the primary tumor does not result in a survival benefit. This practice should no longer be considered standard of care.</p
Specific Heat Discontinuity, deltaC, at Tc in BaFe2(As0.7P0.3)2 - Consistent with Unconventional Superconductivity
We report the specific heat discontinuity, deltaC/Tc, at Tc = 28.2 K of a
collage of single crystals of BaFe2(As0.7P0.3)2 and compare the measured value
of 38.5 mJ/molK**2 with other iron pnictide and iron chalcogenide (FePn/Ch)
superconductors. This value agrees well with the trend established by Bud'ko,
Ni and Canfield who found that deltaC/Tc ~ a*Tc**2 for 14 examples of doped
Ba1-xKxFe2As2 and BaFe2-xTMxAs2, where the transition metal TM=Co and Ni. We
extend their analysis to include all the FePn/Ch superconductors for which
deltaC/Tc is currently known and find deltaC/Tc ~ a*Tc**1.9 and a=0.083
mJ/molK**4. A comparison with the elemental superconductors with Tc>1 K and
with A-15 superconductors shows that, contrary to the FePn/Ch superconductors,
electron-phonon-coupled conventional superconductors exhibit a significantly
different dependence of deltaC on Tc, namely deltaC/Tc ~ Tc**0.9. However
deltaC/gamma*Tc appears to be comparable in all three classes (FePn/Ch,
elemental and A-15) of superconductors with, e. g., deltaC/gamma*Tc=2.4 for
BaFe2(As0.7P0.3)2. A discussion of the possible implications of these
phenomenological comparisons for the unconventional superconductivity believed
to exist in the FePn/Ch is given.Comment: some disagreement in reference and footnote numbering with the

Recovery after critical illness: putting the puzzle together-a consensus of 29.
In this review, we seek to highlight how critical illness and critical care affect longer-term outcomes, to underline the contribution of ICU delirium to cognitive dysfunction several months after ICU discharge, to give new insights into ICU acquired weakness, to emphasize the importance of value-based healthcare, and to delineate the elements of family-centered care. This consensus of 29 also provides a perspective and a research agenda about post-ICU recovery
